The mine is entered by a central vertical shaft, strongly timbered, and containing two compartments-one for the passenger cage, the other for the large skips bringing the ore to the surface. A man-way, for emergencies, is also provided, parallel to the shaft. The cage is a single-deck steel box, carrying about thirty persons; by means of arrangements above and below, it can also hoist and lower materials needed in the workings. Levels are run at intervals of 100 feet. In special cases, where the ore conditions warrant, sub-levels are operated, such as those at 1,113 feet and 1,123 feet (Fig. 7), which are connected by winzes with the 11 th level. One of these was pushed for 40 feet, and then abandoned because it was unprofitable to take up ore of the grade encountered. The ore between levels is removed by stoping. The drifts follow the course of the veins as much as possible, with numerous cross-cuts. Winzes, raises, connections, etc., are blasted out where needed. The main drifts are at least 8 ft. high by about 9 ft. wide. All former narrow drifts had to be widened sorne time ago, by ruling of the Ontario Department of Mines, to lessen the danger of accidents with cars in the small passages. Accurate plans of the under-ground workings are kept in the office, and extensive surveying is always being done underground to keep the maps up to date, measure the cubical contents of stopes, and so forth. Timber is extensively used in the construction of supports and chutes, and in the sinking of the shaft. Steel and concrete are too expensive to compete with timber, which 1s abundant in such sections as have escaped forest tires.

The Bureau of Mines conducted seven pioneering experiments in a 12-inch-diameter Kroll reduction reactor to examine the feasibility of coreducing the chlorides of Ti, AI, and V to produce a homogeneous titanium alloy sponge. The coreduction studies proceeded in a normal manner, and the yields of sponge were comparable with those of pure titanium sponge made in the same reactor. Vanadium concentration was very uniform throughout the sponge cake when VC14 in solution in TiCl4 was fed into the molten magnesium. More difficulty was encountered in feeding solid VCl2 and AIC13. Vanadium and aluminum concentration in the sponge was not uniform, and mechanical difficulties were experienced with the small-scale screw feeder. Although the sponge varied in composition from region to region, the ingots prepared from it were uniform in composition.

"Although the centennial of the Peirce-Smith (PS) converting process has recently been celebrated, the technology has essentially not changed much since its inception. PS converting is used extensively in the Cu, Ni and PGM industry to remove iron and sulphur from a molten matte phase. A typical PS converting process would use low pressure air supplied by a blower to bubble air through the molten bath. This bubbling injection regime is inherently inefficient from a process and energy consideration. In the late 70’s and early 80’s, Brimacombe and co-workers demonstrated at both laboratory and plant scales that sonic injection (jetting regime) into copper or nickel converters could reduce or eliminate the above hindrances characteristic of PS converting. Since then, in spite of these successes, very little industrial applications have made headway in the converting process of non-ferrous metal mattes on a commercial scale. As part of its operational improvement and energy reduction program (process efficiency), Lonmin Platinum decided to conduct a full scale industrial evaluation of sonic injection using air only to establish if punchless, high efficiency sonic injection was feasible in its small 3.05 × 4.57 m converters (10’ × 15’). A plant trial was run for four weeks in one of its three converters. This paper discusses the plant demonstration with a focus on the experience acquired by Lonmin together with the key learnings from the trials. Reference will also be made to the commercial aspects of a full scale implementation of sonic injection in all three converters in the smelter. "IntroductionThe Chairman, in his speech to this body last year in Montreal, stated that the Dominion Coal Board looked to and depended upon the Bureau of Statistics for statistical material, and the Bureau does collect and publish monthly and annual data on coal. It may therefore be asked why an officer of the Dominion Coal Board should prepare and give a talk on statistics.I would explain that it has been our experience in the Board that, since the supply and consumption of coal in Canada is subject to a seasonal ebb and flow, it can be more profitably studied on the basis of a twelve-month period ending on March 31st than of a calendar year, since the end of March does represent a fairly stable end point in this ebb and flow. The figures are not prepared or published by the Bureau on that basis and it has therefore been thought that a presentation on this fiscal-year period would be of interest. The Bureau of Statistics has agreed that this may be valuable and all the data here presented are based on the reports secured from various sources by the Mining, Metallurgical and Chemical Division, Dominion Bureau of Statistics. The only contribution by either myself or the staff of the Dominion Coal Board has been the tabulation in a different twelve month period, and the comments upon the picture presented.It should be noted that the data for the month of March, 1950, are not available in all cases and consequently the figures for that year contain an estimate for this month."

The sulfur dioxide emitted in low concentrations in flue gases from the combustion of sulfur-containing coals and fuel oils is a significant factor in contamination of the atmosphere with sulfur dioxide. A review of methods for treating gases containing high concentrations of sulfur dioxide shows that they are seldom satisfactory for purifying flue gases. Processes developed specifically for treating flue gases using liquid absorbents are costly and may cause localized pollution, because the cooled flue gases can settle in the vicinity of the stack. Bench-scale research is being conducted to investigate the use of dry absorbents for sulfur dioxide removal at temperature levels of about 250°- 300° F. and 600°-650° F. Alkalized alumina has been found effective for treating simulated flue gas and Is regenerable by hydrogen, carbon monoxide, or producer gas. Other metallic oxides have been tested as absorbents also. Chemically impregnated activated carbons removed sulfur dioxide from the gas at 265° F. Vanadium pentoxide is effective as a catalyst for oxidizing of sulfur dioxide in the simulated flue gas at temperatures lower than have been used commercially for gases with higher concentrations of sulfur dioxide. Conversions of sulfur dioxide to trioxide wore studied as a function of temp- erature over a range of about 570° to 750° F.

The listed title of my talk to-day is "MODERN ON STRAND SINTER COOLING". Perhaps it should he ve been "COMPLETION OF SINTERING ON THE STRAND" as the two are synonymous. By the very nature of the DWIGHT-LLOYD SINTERING PROCESS partial cooling on the strand is always present, but not completion of sintering and cooling on the strand. I have mentioned the Dwight-Lloyd process just to be certain the type of sintering under discussion is fully identified because some of the basic mechanisms of sintering occur in various pyro-processing of fines lying in close quiescent contact, such as powdered metals practices, some types of pellet induration, firing of briquets, or other compacts, where the entire bed is brought to, and held at, the high reaction temperatures of sintering. Figure 1, The continuous sintering process as originally conceived consisted of intimately mixing carefully proportioned burdens, placing them in a comparatively thin, draft permeable layer, on a moving grate and igniting the surface while drawing sir through the bedded burden. The moving grate was advanced slowly as the line of burning fuel travelled from the surface to the bottom of the bed and no more fire zone could be seen.

THIRTY years ago, when I stood on the cliff of red fossil iron-ore, on Red mountain, Jefferson county, Ala., I asked what were the geological relations of this remarkable deposit. In reply I was told that it was a limestone colored and saturated by some unknown but wholly external influences, and that at a reasonable distance from the surface the color would disappear. I could find no satisfactory explanation either of the nature of this wonderful external permeating influence, or of the cause of such a ferrous impregnation selecting this one particular bed of limestone, to the exclusion of the higher and lower beds of the same material which are not even stained. I. THE PRESENT THEORY. Referring to this subject, Dana says:' "The clays, clayey sandstones and limestones of the Clinton epoch, through New York and the Appalachians, show that the mud-flats and sand-banks, and hence the shallow seas of the coast region, still continued. . . The beds of argillaceous iron-ore, which spread so widely through New York and some of the other States, south and west, could not have formed in an open sea, for clayey iron-deposits do not form under such circumstances. They are proof of extensive marshes, and, therefore, of land near the sea-level. The fragments of crinoids and shells found in these beds are evidence that they were, in part at least, salt-water marshes, and that the tides sometimes reached them." O processo de injeção sob pressão de ligas de alumínio é empregado para a produção de peças diversas, destinadas principalmente à indústria automobilística, com a possibilidade de produzir peças complexas, contendo seções delgadas, e a alta velocidade de produção. O reduzido tempo de ciclo e as elevadas temperaturas envolvidas promovem danos por fadiga térmica na superfície de trabalho das matrizes, que crescem com o aumento da produção até atingirem condições que inviabilizam o uso da matriz. O presente trabalho visou estudar o efeito da nitretação na superfície do material, relacionando-o aos aspectos microestruturais e tensões residuais inseridas durante nitretação. O aço AISI H13, foi tratado para dureza de 45 HRC e posteriormente nitretado sob plasma em diversas condições de temperatura, tempo e composição da atmosfera gasosa. Os resultados obtidos mostraram que as variações dos parâmetros de nitretação afetam os perfis de dureza e de tensões residuais presentes nas camadas nitretadas, bem como os microconstituintes das mesmas. O material nitretado, após ser submetido a temperaturas da ordem de 600°C, apresentou redução no nível de tensões residuais de compressão, ocorrendo processo de alívio de tensões. O mesmo efeito não foi tão fortemente verificado no perfil de dureza do material.

THE scope of this paper is the description of two districts in Peru in which deposits of vanadium have been found, and the consideration of much laboratory-work that I and others have done to determine the nature of these deposits. It is with regret that I present some of the data in a state of evident incompletion, particularly the analyses of a few of the minerals, which leaves some doubt as to whether the true nature of the minerals is understood. I make no apology, however, since in this case, as in many others referring to ore-deposits, conclusive opinions cannot be stated until a great amount of work has been done, and the deposit has been thoroughly exploited. I believe that the data and the tentative conclusions here presented indicate a unique and interesting condition which may assist others in work along similar lines; and further, what is of more practical importance, stimulate a search for a metal which, though long considered rare, is probably so merely because proper care has not been taken to recognize it. An excellent bibliography on the occurrence of vanadium in nature is given by F. W. Clarke in his paper, Data of Geo-chemistry;1 and in the preparation of this paper I have consulted many of the references contained in the Bibliography of the Chemistry of Vanadium, by M. Moissan.2 A brief review of the subject having special reference to the metallurgy of vanadium is contained in the small book by P.. The steady-state and dynamic behaviour of semi-autogenous grinding (SAG) mills are arguably best described using a population model that caters for the breakage kinetics and the material trans-port of rock and pulp inside the mill. Breakage kinetics are usually expressed in terms of a specific breakage rate function and a separate primary breakage distribution function. Material transport usually invokes the concept of a specific discharge rate function. Unfortunately, the breakage rate and breakage distribution functions for SAG mills are very difficult to estimate uniquely from plant data alone. Simplifying assumptions must be made to over-come this problem, which leads to many unresolved dilemmas with regard to whether or not the de-rived functions are accurate, precise, and reflect reality. For example, it is sometimes assumed that the breakage distribution function for a SAG mill treating run-of-mine ore can be derived from laboratory dropweight and tumbling tests on small rocks in narrow size ranges. Such tests hardly reflect the environmental conditions likely to be encountered in a pilot or production SAG mill where there is a wide spectrum of particle sizes. It is shown that the dilemmas associated with standard population balance models (models that utilise separate breakage rate and breakage distribution functions) can be overcome by expressing breakage behaviour inside a SAG mill in terms of a single function. This function is an energy-based cumulative breakage rate function defined as the fractional rate at which particles and rocks coarser than a given size break to below that size per unit specific energy input. A model for SAG milling is formulated, which invokes the concept of a cumulative breakage rate function. Comparisons are made between the mathematical structure of this ?simple model? and standard population balance models. Perhaps the most attractive feature of the ?simple model? is that the cumulative breakage rate function can be uniquely estimated from measurements of size distributions and mass flows of the feed and discharge streams together with the size distribution and holdup mass of the mill charge. Such measurement can easily be made at pilot scale. It is concluded that this simplified approach to SAG milling may turn out to be the quickest way to fully describe and quantify the performance of these very complex mills.

Theoretical flotation models suggest that there is a positive relationship between bubble-particle collision rates and turbulent kinetic energy dissipation. Fine particle flotation performance is generally enhanced by increased collision frequency and hence higher energy dissipation. Contrarily, increased turbulence in the rotor-stator region is related to higher detachment frequency of the coarser size range. Therefore, the optimal modes of recovery for the ?fine? and ?coarse? size classes are diametrically opposed. Industrial applications have previously confirmed that applying greater power to flotation slurries yields significant improvements in fine particle recovery. However, recovery of the coarser size class favors a different flotation environment. An improvement in the kinetics of the fine and coarse size classes, provided there is no adverse metallurgical influence on the intermediate size ranges, is obviously beneficial to the overall recovery response. Managing the local turbulent kinetic energy dissipation, and hence the power imparted to the slurry, offers the benefit of targeting the particle size ranges exhibiting slower kinetics. FLSmidth recently introduced the practical implementation of this concept. In principle, it decouples flotation regimes where fine and coarse particles exhibit preferential recovery. In the case of naturally aspirated machines (Wemco®), it is referred to as Hybrid Energy Flotation? and incorporates at least three phases: ? Standard flotation machines (standard energy input, rpm, rotor size/type) at the beginning of the row, where flotation is typically froth phase limited and operational, and set-up parameters have a limited influence on the recovery. ? Higher-powered flotation machines (high rpm, high power rotor size/type) at the end of the row to improve fine particle recovery. ? Lower-powered flotation machines (low rpm, low power rotor size/type) to enhance coarse particle recovery. In evaluating the effect of turbulent kinetic energy on flotation performance, the spatial distribution and its effect on size denominated recovery needs to be evaluated. The mean energy dissipation rate is a measure of the bulk energy input, but CFD-based models offer a more introspective view of flotation sub-processes. A CFD-based flotation model is used to highlight the effect of turbulent dissipation energy on attachment and detachment rates. Preferential collection zones for ?fine? and ?coarse? particles are predicted for both forced air and naturally aspirated machines. The greater predominance of UG2 ore types, coupled with the skewed feed distribution of platinum group metals (PGMs) to the finer size fractions, suggests that PGM flotation circuits are not designed for optimal recovery across the size distribution. The application of the hybrid energy concept to PGM flotation offers a possible shift towards a more efficient flotation circuit solution through a managed distribution of energy.

Erosion control during the mining cycle can be more effectively planned using Erosion and Sedimentation Control Plans (ESCPs). This raises the issue of who should develop an Erosion and Sedimentation Control Plan Guidance document (ESCPG) to standardize the content of ESCPs. A search for ESCPG documents in Canada revealed no examples for mine sites. In the U.S. there are a number of regulatory authorities who developed such documents for mine sites, including EPA. An ESCPG is expected to result in: (a) plans will be more standardized (b) more clarity and efficiency for those generating ESCPs; (c) assisting regulators tasked with reviewing/approving ESCPs; (d) providing guidance for the mine effluent permitting process relative to sediment control; (e) providing a basis for developing and including ESCPs in the Environmental Assessment (EA) phase and the mine economic assessment; (f) applying and optimizing ESCPs above the ?watersheds? of sediment ponds; (g) utilizing the ESCPG document for ?exploration? sites; (h) lowering the risk of adversely impacting water quality downstream of mining activities; (i) providing a methodology for determining the need for sedimentation pond(s) during the construction and production phases; and (j) providing a clear environmental sustainability message relative to sediment control if the mining industry takes the lead in developing an ESCPG. ESCPs will proactively create increased administrative (regulatory) efficiency in the EA and sub-EA reviews relative to erosion control and sediment release from mine sites. This also assists the new initiative in BC, One Project, One Process (OPOP) and synchronous permitting (a variation of concurrent permitting): as an example, enhanced erosion control planning at the EA phase then allows the synchronous permitting initiative to proceed efficiently, particularly for the mine permit (Ministry of Energy, Mines and Natural Gas) and effluent permit (Ministry of Environment). Approximately twenty more management plans (e.g. ARD/ML, explosive management, emergency response, hazardous waste, sediment pond operation, environmental effects, environmental sampling, etc.) may also be entered into the EA process to create increased efficiency, and then feed into the synchronous permitting effort. The BC OPOP initiative also adds support to the Canadian Federal ?One Project, One Review Process? approach. The ESCP and a sediment pond management plan will also address the Canadian Metal Mining Effluent Regulations requirement for all discharges from mine sites to meet a grab sample of 30 mg/L Total Suspended Solids.

"With the development of industrial production, more and more wastewater containing high concentration ammonia nitrogen was discharge into the environmemt, which caused serious harm to the environmemt. Treating the wastewater has great significance to protect water resources. Using air-stripping combined with oxidation to uptake high concentration of ammonia nitrogen wastewater. The influence of operating parameters was investigated. Experimental results revealed that pH of solution, aeration time and aeration strength showed heavy influence on ammonia nitrogen removal. Air-stripping combined with oxidation can be considered as an alternative approach for the removal of high concentration of ammonia nitrogen from practical metallurgical wastewater. When pH was 11, aeration time 20 min, aeration strength 1040 Lh-1, and oxidation with 10 % NaClO solution 10 min, the initial ammonia nitrogen 2130 mgr1 could reduce to 12 mgr1. Other items could also be removed to meet the ""Integrated wastewater discharge standard (GB8978-88)"".1. IntroductionThe rapid development of human activities has caused a worldwide increase of nitrogen in rivers, lakes and coastal waters. It leads to considerable eutrophication and an increase in the frequency of toxic algae blooms [1,2]. The concentrations of nitrogen in the water of China are pervading high [3]. The discharge of wastewater containing high concentration of ammonia nitrogen is one of the most important factors. Great efforts have been devoted to the removal of ammonia nitrogen from wastewater. Traditional methods include biological denitrication [ 4 ], chemical precipitation with magnesium ammonium phosphate [5], electrochemical conversion [6], air-stripping [7] and so on. Biological denitrication is the most common process in the treatment of ammonia nitrogen wastewater. But the process is only suitable for the removal of relatively low ammonia concentration due to the requirement of appropriate C/N ratio [8]. Chemical precipitation needs additional reagents, which may introduce new pollutants to water body. Electrochemical method often uses expensive metal or metal oxide as electrodes, and also consumes large quantity of energy [6].Although air-stripping makes use of stripping tower and consumes energy, it is very suitable for the removal of relatively high ammonia concentration. Different air-stripping conditions would appear different removal efficiency. And only air-stripping the effluent can not be attained the ""Integrated wastewater discharge standard (GB8978-88)"". In this study, we use air-stripping and oxidation to removal high concentration of ammonia nitrogen from simulated and real ammonia nitrogen wastewater."

"Roof bolting has long been used in underground mines across the world to provide ground support. Modern roof bolts are cheap and easy to install with the use of specialized machines as a part of the production cycle. Lhoist North America’s Crab Orchard Mine is an underground room and pillar limestone mine that uses mechanically anchored roof bolts for ground support. The mine currently employs two different roof bolting patterns: a standard 5 × 5 foot pattern, and a 2.5 × 2.5 foot pattern for use in areas with particularly hazardous roof conditions. The purpose of this study is to evaluate the relative effectiveness of each bolting pattern.A series of numerical models were created using RocScience’s RS2. The models were based on a symmetrical section of the mine at its deepest point, and were modeled using Generalized Hoek-Brown failure criterion along with a discrete fracture network. A series of sensitivity analyses were performed on the models by varying parameters such as joint friction angle, crack persistence, joint randomization, and tensile strength of the limestone. Based on the results of the original models and sensitivity analyses, it appears that the standard bolting pattern provides sufficient roof support capacity under almost all the expected conditions at the mine, since factors of safety below the design value of 1.5 were only found for individual bolts in a few of the worst test cases considered. These results can help improve the Mine’s productivity and reduce operating costs without compromising safety.INTRODUCTION AND CASE STUDY DESCRIPTIONLhoist North America’s Crab Orchard Mine is an underground limestone mine located in Crab Orchard, Tennessee. The mine is located under an 800-foot-high mountain, and outcrops on three sides. The mine is accessed through three separate portals and has two main levels, along with a partially developed third level. Extraction is accomplished by the room and pillar method using 50 × 50 feet pillars and 50 × 50 feet rooms. The mining height is limited to 50 feet to keep the pillar width to height ratio around 1.0. Each heading is excavated in a two-stage process that consists of a 30-foot-tall top heading and a 20-foot-tall bench. A distance of at least 30 feet is maintained between the two levels at all times (Crab Orchard Mine Technical Staff, 2017)."

Sindesar Khurd Mine is situated in Rajasthan, India, and operated by Hindustan Zinc Ltd, under Vedanta Group. The base metal mine is high in reserves of Lead, Zinc and silver and has been producing since 2007 with initial production of 0.3 mtpa. Recently the mine has gained prominence as a base metal underground mine in introduction of latest mining techniques and achieving high production at lowest costs. The promising Lead-Zinc reserves of Sindesar Khurd Mine (SKM) and high-tech exploration adopted by Hindustan Zinc Ltd (HZL), (Vedanta Group), raising the reserves and resources from a meager 9 Mt (1997) to 81 Mt (2012), has prompted HZL management to ramp up the mine production from 0.3 mtpa (2006-07) to 2 mtpa by 2013-14 with ambitious plans to reach 3.5mtpa subsequently (presently largest underground metal mine in India). The complete trackless system underground mine, recently equipped with latest leaky feeder communication system (unique in India), utilize modern drilling, mucking and hauling equipment aided with various utility/service vehicles. Excavation optimization and quality recovery maximization from the open and post filled stoping methods, continual expert analysis and feedback from International consultants like AMC (Australia) and SRK (UK), and involvement of renowned bodies in management excellence, have lead the mine towards highest productivity and lowest mine operating cost, earning the mine a prominent place in the global base metal industry. The paper deals with how the various challenges in mine planning and designing, mine development, operator skill, equipment introduction and maintenance, expertise availability, provide training and exposure to officials, prompt and effective communication/service, control of ground vibrations, maintain ventilation standards etc are faced in attaining high operational excellence and thus attain a record mine output/man shift (OMS) of 14 ton/man shift with targets to reach 20 ton/man /shift. Scientific designing using 2D/3D numerical mine-modeling techniques, faster stope sequencing with introduction of paste filling ( first time in India),latest leaky feeder communication system, control ground vibration to stringent standards using electronic delay system, faster raising techniques using raise borers to enhance development and ventilation, cavity scanners to monitor large stope voids, fast shaft sinking at 100 m/month(Chinese expertise) and maintaining adequate ventilation with close software monitoring are some of the highlights discussed in the paper towards achieving the targets.

Introduction A brief account of the East Malartic fire is essential if this paper is to serve the purpose implied by its title. The fire originated in the timbers of No. 4 shaft at the 10th, or 1,570-foot, level; this was the top or collar of the underground sinking operations. At the time, sinking was in progress and the shaft bottom was 242 feet below the 12th station horizon, or 618 feet 'below the l0th level. Above the l0th level, the shaft was not in use as the surface plant was not yet completed. The shaft crews descended from surface in No. 3 vertical shaft to the l0th level, where they transferred to No. 4 shaft, a distance of 1,450 feet. Of the sixteen shaftmen mucking at the shaft bottom, four were able to reach safety. One shaftman was overcome by fumes midway between the two shafts on the l0th level, while attempting his escape. The remaining eleven men climbed as far as the 11th station and there succumbed to carbon monoxide. No. 4 shaft, inclined at 61 degrees, bad been raised and timbered from the 10th level to surface; this bad been followed by sinking operations below the l0th. Two air hoists had been set up in the footwall of the shaft, 110 feet above the 10th level, and directly behind compartments No. 2 and No. 4. The waste from the shaft bottom was hoisted in 28-cubicfoot buckets on mono rails and dumped over standard shaft sinking doors into waste chutes built into No. 1 and No. 3 compartments. The five-compartment shaft, 7 ft. 8 in. by 34 ft. 6 in. overall size, was heavily timbered. In addition to the standard shaft timbers of 8 in. by 8 in. and 8 in. by 10 in. B.C. fir, the hanging-wall and ends of the shaft were laced with 4-inch spruce lagging. This represented approximately 300 board feet of timber in placee for every foot of shaft. The cause of the fire bas never been determined, despite every effort to do so. The inquest held for those who lost their lives in the fire was an extremely thorough one, but only the location of the origin, and not the cause, was established.

The G-Tunnel heated block experiment is being conducted on the Nevada Test Site (NTS) as part of the Nevada Nuclear Waste Storage Investigations project (NNWSI). The purpose of the ambient temperature testing ph se is to evaluate rock-mass mechanical properties of a block (- 8 m3) under biaxial stress changes up to 7.5 MPa above an initialization in situ value of 3.1 MPa. Results indicate that the modulus of deformation ranges from 9.7 to 17.0 GPa and Poisson's ratio ranges from 0.21 to 0.33. In general, the higher values of the modulus and Poisson' s ratio were influenced by fracture propagations parallel to the compressive stress field. Other measurements indicated that cross-hole compression (p) wave velocities and single fracture permeability values were relatively insensitive to stress changes above the in situ value.

"The current paper presents the results of a study designed to explore the types of practices that are fundamentally important to developing and maintaining comprehensive risk-based health and safety management systems (HSMS) in mining organizations. Eighteen practicing health and safety (H&S) executives, managers, and professionals from a variety of mining commodities were asked to assess the fundamental importance of 20 distinct elements and 133 different practices assembled from a review of several HSMS consensus standards. The research approach assumes that developing and maintaining a robust HSMS requires unwavering attention to activities designed to either: 1) directly contribute to a risk management system’s plan-do-check-act cycle; or 2) build an organization’s internal infrastructure without which the cycle cannot be sustained. The results of this study suggest that the following areas require consistent focus and attention: Leadership Development; Accountability; Knowledge, Skills, and Abilities Development; System Coordination; Culture Enhancement; Behavior Optimization; and Risk Management Studies. Each element and its corresponding top-ranked and top-rated practices are discussed.INTRODUCTION Since the time of the Industrial Revolution, occupational health and safety (H&S) management has been a topic of practitioner thought and academic inquiry (1). In the U.S., decades of regulation and research have resulted in a surplus of prescriptive information (techniques, tools, approaches, standards, programs, practices, policies, etc.). With an abundance of such information, a simple H&S solution to address a problem in the U.S. mining industry may seem straightforward. Counter to this assertion, however, some evidence suggests that the abundance of information may create complexity and cause confusion in the minds of organizational leaders and H&S managers, ultimately discouraging the voluntary pursuit of a sound approach (2). In line with this perspective, in 2006 the U.S. National Mining Association (NMA) formed an independent commission to examine H&S management issues within the mining industry. The findings of the commission called for the mining industry to adopt a risk-based management paradigm. In the presentation of its findings, the commission stated that “…every mine should employ a sound risk-analysis process, should conduct a risk analysis, and should develop a management plan to address significant hazards identified by the analysis” (3). Grounded in the well-known plan-do-check-act cycle, the recommended risk management system allows for mines to ground strategic management choices in their mine-specific hazards and risk profiles"

Location and History God's lake is in the northeastern part of Manitoba, about 400 miles northeast of Winnipeg, or about 250 miles due north of Red lake, Ontario. The lake is quite large, having an overall length of 54 miles and a maximum width of 18 miles. There are numerous islands in the lake, the largest of these, known as Elk island, being about 15 miles long and 3 miles wide in the widest part. The accompanying map (Figure 1) shows the location of God's lake and of the mine. The God's Lake area has long been regarded as promising country for the prospector, but, due to its inaccessibility, practically no prospecting was done prior to 1927. In the years 1927-1931, inclusive, a few prospectors visited the area, but no discoveries were made. In 1932, Bob Jowsey and Archie McDonald entered the area from Island lake. They first prospected in the vicinity of Knife lake, which is immediately south of the main part of God's lake, and then moved on to the latter lake, heading for the north shore of Elk island. The first afternoon on God's Lake, when looking for a place to camp on the north shore of what is now known as Jowsey island, Jowsey discovered a quartz porphyry dyke which was cut by a number of gold-bearing bluish quartz stringers. On the strength of this discovery, he and McDonald began at once to stake the Reno group of claims. Rumours of the discovery brought in other prospectors, and other groups of claims were staked to the east, along the north shore of Elk island. A second discovery was soon made by Dan McCrea on his Gold Standard claims, three and a half miles to the east of the original discovery. Staking still continued to the east, along the north shore of Elk island, and Bob Brown and Harvey McKenzie staked the Akers group on a discovery approximately two miles east of McCrea's discovery. Through the efforts of Jowsey, the Reno, Rand, Elk, and Cona groups, belonging to the original syndicate for which Jowsey and McDonald staked, were combined with the Gold Standard, Greenstone, and Akers groups, and the Company known as God's Lake Gold Mines, Limited, was formed. Several additional groups of claims were later taken over by the Company and a considerable area of adjoining ground was staked to round-out the property.
