"Risk management in tunneling is more important today than it was a few years ago, considering increasing demands for safety and environmental and socio-economic sustainability, which are now required by citizens, owners, lenders, and insurers. Moreover, our current risk management philosophy should not only address risk avoidance and mitigation, but should also be used as a means of value creation, benefiting the overall project. Given the strategic implications of risk management, the authors intend to present, in this paper, a comprehensive overview of references and experiences from current practice—e.g., what is missing or weak, as well as possible or necessary developments that can have a key role in implementing effective risk management, particularly for a project’s design and construction phases. Therefore, as God has set some things in broad daylight; as he has given us some certain knowledge, though limited to a few things so, in the greatest part of our concernments, he has afforded us only the twilight, as I may so say, of probability (John Locke) PREFACE Many large-scale engineering and construction projects suffer millions of dollars in cost overruns, delays in design and construction, and operability troubles once finally completed (Edward W. Merrow, 2011). The undesired effects of these problems can become disastrous for large-scale megaprojects (those that typically cost US$1 billion or more), particularly considering that these are increasingly used as the preferred delivery model for infrastructure (Bent Flyvbjerg, 2014), often in public-private partnerships (PPPs). McKinsey G.I. (2015) has estimated that bridges and tunnels average 35% in cost overruns, and roads 20%. Some of the values published by Flyvbjerg (ibidem, 2014) for projects with relevant underground elements are the highest, with cost overruns from 60% to 300%. The analysis of 32 other case histories (from 1994 to 2010) given by Hartmut, R. (2011, IMIA Annual Conference, Amsterdam) indicates an average increase of costs around $55 million, with a 20-month average delay."

Information to assist them in making the decision to retreat mine and in selecting the best retreat mining technique for specific mining conditions. 2. Information on mine planning that will enable them to design mine layouts for safe and efficient retreat mining. 3. Information that will enable them to develop a section foreman's handbook. The manual has been organized to gradually increase in level of detail as the reader progresses from beginning to end. Individuals interested in an overview of retreat mining can confine themselves to the first few chapters. Those interested in mine planning, should read the middle chapters as well. Those interested in the development of a foreman's handbook should read the entire manual.

"The use of load and resistance factor design (LRFD) is well documented since the 1960s for structural applications when it was first adopted into the American Concrete Institute Building Code in 1963 and was established for geotechnical applications by the Danish Geotechnical Institute about the same time under the name of Limit State Design (LSD).The Federal Highway Administration required all federally funded projects to use the LRFD method by 2007. Due to this funding requirement, recent LRFD research for geotechnical applications has focused on common bridge foundations (i.e., driven or drilled shafts), resulting in good documentation for design using these methods. Unfortunately, the practicing geotechnical engineer does not intuitively know how to create resistance factors for deep foundations systems that have design methodologies and installation techniques significantly different from driven or drilled shaft foundation systems.Helical anchors typically use semiempirical methods for preliminary capacity design and a torque correlation methodology during installation to verify required capacity. Here we provide basic information on the LRFD methodology for helical anchor design. Additionally, the First- Order Second Moment (FOSM) method is applied to a previous helical anchor study (Hoyt and Clemence 1989) to determine resistance factors for an ultimate tension capacity strength limit state.MethodologyThe concept of the LRFD method is to establish factors for both the load and resistance that account for uncertainties using probability theory. The factors increase the nominal load and/or decrease the nominal resistance such that the probability of failure is an acceptable value. LRFD also accounts for the effects of load combinations (dead, live, seismic, wind, etc.) by determining the probability of the combined occurrence. In this paper it is shown that when the tensile strength of a given steel in various states of heat-treatment is plotted against its elongation, a straight line results. The equation of this straight line can be computed with considerable accuracy from its chemical composition. The appropriate equations are given. The logarithm of the slope of the tensile-strength locus is shown to be proportional to the cumulative effect of the total alloy addition and the vosition of the line to be directly proportional to the cumulative effect of the alloy content. It is further pointed out that heat-treatment is more effective than composition in determining yield ratio. The latter enters mainly by its effect on hardenability, which is briefly discussed. Reduction of area and Brinell number are given some incidental attention. The data are applicable only to steels that have been normalized and/or quenched. The steels, if normalized, may or may not have been tempered. If quenched they must have been tempered at or above 4oo°C. (75o°F.). Introduction Cast steels vary in composition and frequently are used in various states of heat-treatment. It is obviously important to be able to predict the compositions that might accompany specific engineering properties by suitable selection of heat-treatments. For cast steels, which are of necessity produced heat by heat in accordance with the particular requirements of the castings to be poured, there has been, happily, much less tendency to standardization than for rolled products, whose destination is not necessarily known when particular ingots are made or rolled. It is of distinct advantage to the steel foundry-men to be able to make steel of given properties, using alloys that can be recovered from scrap, or most conveniently or cheaply bought, rather than a particular formula adopted in the interest of standardization. The heat-treatment appropriate to securing a given result in a given steel is well known to be closely related to composition. For example, molybdenum steels are not much softened by tempering except at rather high temperatures. Highly alloyed steels are "air-hardening," and on cooling in air behave like carbon steels that have been quenched. With these matters, we do not concern ourselves in this paper, although we recognize their importance. The entire picture evidently will be very complex and can be brought within accurate understanding only if our knowledge of test results can be coordinated in terms of general principles.

"U.S. Mine Safety and Health Administration (MSHA) regulations require underground coal mines to use refuge alternatives (RAs) to provide a breathable air environment for 96 hours. One of the main concerns with the use of mobile RAs is heat and humidity buildup inside the RA. The accumulation of heat and humidity can result in miners suffering heat stress or even death. MSHA regulations require that the apparent temperature in an occupied RA not exceed 95 °F. To investigate this, the U.S. National Institute for Occupational Safety and Health (NIOSH) conducted testing on a 23-person tent-type RA in its Experimental Mine in a test area that was isolated from the mine ventilation system. The test results showed that the average measured air temperature within the RA increased by 9.4 °C (17 °F) and the relative humidity approached 94 percent at the end of a 96-hour test. The test results were used to benchmark a thermal simulation model of the tested RA. The validated thermal simulation model predicted the volume-weighted average air temperature inside the RA tent at the end of 96 hours to within 0.06 °C (0.1 °F) of the average measured air temperature. IntroductionIf an accident occurs in an underground coal mine, miners who fail to escape from the mine can enter a refuge alternative (RA) for protection from adverse conditions, such as high carbon monoxide levels. One of the main concerns with the use of mobile RAs is the potentially adverse thermal environment inside an RA from the metabolic heat of the occupants and the heat released by the carbon dioxide (CO2) scrubbing system. Moreover, the humidity within the RA will increase through occupants’ respiration and perspiration, and from the chemical reaction within the CO2 scrubbing system. The accumulation of heat and humidity can result in miners suffering heat stress, heat stroke or even death."

"The combination of a predictive reliability assessment model and discrete-event simulation can be an effective methodology to study the impact of equipment failures on the capacity of production systems in mines. In our case, a predictive reliability assessment model using genetic algorithms is combined with a discrete-event simulation model to analyze mine equipment systems.Discrete-event simulation involves modelling of real-life systems based on variables and events that occur or change at distinct points in time. The Simul8 simulation software, by Visual Thinking, is used in this research. It is a two-dimensional (2D), discrete-event simulation/ animation software tool for creating realistic depictions of manufacturing and materials handling systems.The simulation model developed for the purpose of this research represents a typical two-level, sub-level stoping section of an underground mine in the Sudbury region in Ontario, Canada. This 2D model was created to assess the impact of equipment failures to a mine’s production throughput. In more detail, using mean time between failures (MTBF) data of scooptrams derived from a reliability assessment model based on genetic algorithms, LHD failures are incorporated into the simulation model. These failures cause production interruptions to the development cycle and consequently loss of hauling capacity.Another purpose of this research work is to demonstrate how low-cost simulation tools such as Simul8 can be successfully applied to model underground hard rock mining operations. The East West Link road tunnels are proposed to provide a connection between the CityLink toll road and the Eastern Freeway north of the Melbourne CBD. The alignment passes from Moonee Ponds valley beneath the Upfield Line railway cutting, Royal Park, the historic Melbourne Cemetery and beneath Alexander Parade rising to an eastern portal at Hoddle Street. The western (Royal Park) part of the alignment passes through the folded and faulted Silurian Melbourne Formation, which has been subjected to deep weathering since the Triassic.The Melbourne Formation in the Royal Park area is mainly characterised by interbedded sequences of siltstone and fine-grained sandstone. The beds are often graded and a clear boundary between the siltstone and fine-grained sandstone rock can be difficult to distinguish. Variation in lithology and the presence of structural geological features such as folds and faults have resulted in an irregular weathering profile within the Silurian rock; nonetheless the formation typically becomes less weathered with depth.Due to difficulties in obtaining reliable laboratory strength and stiffness relationships with weathering grade, in situ testing methods such as downhole shear wave velocity and high-quality pressuremeter testing are a useful aid to assessing appropriate geotechnical parameters for design.This paper presents an analysis of the variation in shear modulus of the Melbourne Formation, determined at small-strain (from downhole seismic data) and at ‘engineering scale’ strain (from pressuremeter tests) for extremely weathered (XW), highly weathered (HW) and moderately weathered (MW) rock. Estimates of small strain shear modulus (Go) obtained a range between 400 to 3400 MPa for these weathering grades. By comparison against the pressuremeter test data, reduction factors to be applied to these Go values to estimate shear moduli at engineering strain levels typically range between 0.10 and 0.35.The downhole geophysics undertaken in the Royal Park boreholes also suggests that there is an inversion in velocity between approximately 15 and 27?m depth below the top of the formation. This is thought to be related to the effects deep Plio-Pleistocene weathering, iron cementation of joints and increasing frequency of ‘non-healed’ joints in the transition from MW to slightly weathered (SW) rock.CITATION:Macklin, S, Schult, M, Terzic, Z and Chu, D, 2014. The U.S. Bureau of Mines (BOM) constructed the Hydraulic Transport Research Facility (HTRF) to investigate the hydraulic parameters relating to the transport of coarse coal. To achieve these goals in a timely and efficient manner, the Bureau contracted to Boeing Services International (BSI) the responsibility of operating the HTRF and providing technical support. This data report is a summary of the testing and data results for the period 22 November to 10 December 1982. The testing covered herein is for coarse 2-in by 0 (minus 50 mm) Pittsburgh Seam coal in a 6-in (150 mm) pipeline, at velocities ranging from 5 to 20 ft/s (1.5 to 6.1 m/s) and at concentrations ranging from 15 to 45 pct by weight.

"This paper presents a technical description of an experimental procedure developed for the geotechnical characterization of granular soils conditioned for EPB tunneling using a modified direct shear box. As the main concerning parameters of the conditioned soil mass were their undrained properties, in order to preserve its pseudo-fluid characteristics, a watertight direct shear apparatus was constructed. A comparison between the results obtained under dry, saturated and conditioned conditions is presented. INTRODUCTION EPB shield machines have been used for tunneling excavation in many different types of soils thanks to the use of soil conditioning agents. These modify the properties of the soil into that of a plastic paste, thus permitting the homogeneous flow of the excavated soil from the tunnel face through the bulk chamber, the screw conveyor and finally the conveyor belt. Moreover this technique allows controlling the face pressure and its regulation in the bulk chamber, preventing water inflow when tunneling below the water table. Further advantages are a drop of the cutter head and screw conveyor torque and reduction of the friction between metallic parts and the soil, consequently reducing wear phenomena (Merritt & Mair, 2006; Vinai et al., 2007; Borio et al., 2010; Thewes & Budach, 2010; Herrenknecht et al., 2011; Barbero et al., 2012; Peila, 2014). Several studies have investigated the possibility of conditioning and the chemical materials to be used in different types of soils, mainly subdivided into three groups: cohesionless soils (sand and gravel), silts and clays, and rock masses. This various researches have set up several laboratory procedures for these achievements before and after soil conditioning, as stated in Thewes & Budach (2010) and Peila (2014) (for cohesionless soils), in Thewes & Burger (2005), Hollmann & Thewes (2013), Zumsteg et al.(2012) (for clay), in Peila et al. (2013) and Martinelli et al. (2015a) (for rock mass). Such tests should be simple to execute, reliable and easy to interpret at the in-situ excavation step. The reduction of shear strength is necessary in EPB operations, since it allows an effective transmission of the pressure from the material itself to the front. In some cases the slump is not sufficient to return a reliable result on the shear strength reduction, as in some cases some materials are behaving differently. Therefore this research is aimed to study this aspect more in detail, in order to show the effectiveness of the soil conditioning process on the material to be excavated."

"The potential use of Black Dross (BD) as a raw material for the production of refractories, fluxing agents and glasses is the main motivation of the present study. Heat treatment experiments were carried out in Ar, and under reduced pressure (20 Pa), to evaluate the salt removal efficiency. The chemical composition of the BD after heat treatment was investigated by SEM-EDS and XRD analyses. Based on the present results, it is established that the salt starts to evaporate at ˜1273 K in Ar, and under reduced pressure. The salt removal efficiency in a 20 g sample was found to increase in both cases as a function of time and temperature. Moreover, in Ar the chlorine concentration was lowered to 0.3 wt% after heat treatment at 1523 K for 10 hours. Under reduced pressure, however, 0.2 wt% residual chlorine was obtained after 8 hours at 1473 K.IntroductionIt is believed that aluminum recycling in the near future will become the main route of aluminum production [1]. Black dross (BD) that is produced during this process contains, among other things, high amount of salts like NaCl and KCl [2, 3]. Almost 800,000 tons of BD is annually land filled in United States and this number continues to grow with the increase in aluminum recycling [2]. The BD should be dumped in controlled land field and it is costly procedure [4, 5].The water-soluble compounds in BD react with water or environmental humidity and release chlorides into the water and/or generate toxic gases such as H2, CH4, NH3 and H2 S [6-8]. In the Europe, BD is considered as toxic waste and its disposal as landfill is forbidden [8, 9]. After salt removal, the BD is considered to be non-hazardous and can be buried under ground [10].On the other hand, BD do also contains many relatively valuable compounds that may be changed in to useful materials as a result of further treatment [11, 12]"

This study relates to continuous face haulage systems in underground coal mines, and was performed by Joy Manufacturing Company for the U. S. Bureau of Mines under the Bureau of Mines Contract No. HO 242025. The effective starting date for this work was June 14, 1974. There were two main reasons for making this study, the first being to find a safer system to move coal away from the face than the currently predominant shuttle car system, and the second being to seek ways to advance the state-of-the-art of continuous face haulage as a means of increasing coal production.

The first introduction of fuzzy theory in the nineteenth century created room for continuous research and application in various fields. Fuzzy set theory has been globally applied in geotechnical engineering, and research in this field continues to date. The fuzzy inference system is considered to be one of the most popular techniques adopted to resolve some of the geomechanical challenges faced in both surface and underground excavations. This paper unpacks fuzzy theory-based approaches in mine geomechanics with the aim of expanding the innovative application of the same approach specifically in tunnel geomechanics. This aim was achieved by conducting a review of recent successful and unsuccessful applications of fuzzy inference systems in underground excavations/tunnelling geomechanics. Indeed, this review has outlined some cardinal points associated with the ability of the technique to solve complex geomechanics problems. However, the success of the technique was accompanied by a few limitations associated with the methodology. Finally, a future outlook associated with the technique has been established.

The methods of reinforcement to be discussed in this paper are all insitu methods, ones that can be used on the soil structure as it exists in the ground, and do not rely on excavation and replacement of the soil. Current practice in the United States, as well as new ideas and methods, will be discussed as they relate to slope stability, retaining walls, and to other uses such as control of mine or cavity related subsidence and to liquefaction control under existing structures. The method is based on the concept of "nailing" or reinforcing the soil in place, and thereby causing a mass of soil to act as a coherent unit. When performed properly, a true gravity structure can be constructed, one that will remain intact under both horizontal and vertical loadings and that will also resist vibrational and seismic loadings.

In Si and FeSi production, the main Si source is SiO2, in the form of quartz. Reactions with SiO2 generate SiO-gas that further reacts with SiC to Si. During heating, quartz will transform to other SiO2 modifications with cristobalite as the stable high temperature phase. Transformation to cristobalite has been investigated and shown to be a slow process where the rate varies between the investigated quartz types. The effect of quartz versus cristobalite as SiO2 source has been studied for reaction (1) 2SiO2 + SiC = 3SiO + CO, by heating of pellets with SiO2 + SiC in Ar-gas. The reaction was faster with cristobalite than with quartz. Initial experiments indicates that this is the case only in Ar and not in CO-atmosphere The higher specific surface area of cristobalite is believed to contribute to the higher rate. The industrial consequences of the observed difference between quartz types are discussed.

Certain deposits of incoherent materials which are near the surface and exploitable by open-pit mining may be characterized in real time before they are mined through an analysis of ground-surface imagery. If the slice to be stripped is not overly thick, in fact, surface texture and color properties may be extrapolated to its entire thickness. Image analysis techniques to process ground surface images acquired in situ (or in the laboratory shortly after their acquisition) which yield pattern vectors representative of RGB color component distributions and HSB texture parameters have been developed. The techniques were applied to a mineral sandy ore deposit containing three different lithotypes. Geostatistical analyses showed that the data used to characterize the lithotypes were reliable. The correct recognition of the lithotypes was carried out using a multi-barycenter classification algorithm. Such in situ image-analysis procedures open the way to the proper selection of the ore to be mined, or the proper way for run of mine (r.o.m). processing, or the appropriate blend to be processed before or at the same time as the mining process itself is enacted.

In the fall of 1993, construction was completed on a state of the art industrial waste water treatment plant at Asarco's East Helena Lead Smelter. Purpose of the plant was to remove arsenic and metals using hydroxide precipitation. Completing of the plant culminated a lengthy process of laboratory bench-scale test work, engineering, construction, and Environmental Protection Agency (EPA) oversight. Unfortunately, the effiuent requirements for the plant were made significantly more stringent as construction was rearing completion. As a result, optimization activities were initiated to determine and implement measures to substantially improve effiuent quality so the new discharge limits could be met. These activities consisted of acquiring a thorough understanding of process chemistry and plant operations, and developing simple, practical methods of automating unit operations associated with the treatment plant. Implementation of these modifications in the second half of 1996 will not only significantly improve the constancy and reliability of treatment plant operations, but also result in operating cost savings and reduced need for increased operator attention at capacity flows, plus allow the treatment plant to meet the new discharge limits.

This paper will address this question from the point of view of a drill and blast engineer or researcher working in open cut and underground mining operations. The paper will establish a common set of reference standards for high-speed imaging and data capture, which engineers and researchers can apply when deciding which type of imaging technology is appropriate. It will present an overview of current image analysis technologies and practices in use across a range of appropriate scientific fields and draw lines of commonality of approach between these fields and the field of mining and blasting applications analysis and research. In addition, the paper will examine the different technologies from an image quality and data stream reference point, ease of operation, cost, and reliability parameters. Finally, this paper will present an argument for the hybridisation of established and new technologies which could provide a more beneficial approach to the use of imaging as a tool of quality control, research and analysis in the mining based industries. This will include a novel approach to digital high-speed imaging and data analysis in the field, which should deliver high-quality images and data analysis in a time frame that is more appropriate to the need for immediate action, analysis and problem solving.

Underground coal mines in Australia have embraced the use of various inertisation techniques both proactively and reactively. The use of such techniques can have a significant influence on ratios and trends used to monitor the status of the underground environment. Whilst control strategies should never be ignored in favour of preserving the integrity of ratios, the influence and significance of the changes likely to be seen should be anticipated and understood for reliable interpretation of the situation. Edward Stafford, Chester, Pa.:—Doring all my business life, I have been engaged in the manufacture of Bessemer and open-hearth steels, but, during my long connection with the Shoenberger Steel Co. of Pittsburg, I had some experience in the manufacture of high-grade puddled iron for horseshoes, and in making, by Dr. Otto Wuth's process, extremely pure, low-phosphorus puddled blooms (analyzing from 0.005 to 0.01 per cent. of phosphorus and sulphur), which were melted with low-phosphorus pig-iron in acid open-hearth furnaces, to make extra locomotive fire-box steel. I have also seen the Danks puddlers in operation at different times and places, notably where they were most successful, at the Otis Steel Works, Cleveland, Ohio; then under the management of our friend, Mr. S. T. Wellman. These Danks blooms were made solely for open-hearth melting-stock, to which they were well adapted. (The unavoidable presence of bits of ore-fix imbedded in the mass of the metal always prevents the profitable manufacture of merchant iron from Danks' blooms.) With the experience just described, I was to some degree in position to value fairly the means, methods and results of Mr. Roe's mechanical puddling from the metallurgical or technical and economic points of view, when given an opportunity by Mr. Roe to see his puddling-furnace or machine in operation in Pottstown, Pa.
