"In June, 2018 Lalor Mine experienced a catastrophic blade failure of its No. 2 main exhaust fan. Over the following weeks several key changes were made to the auxiliary ventilation system to allow for continued production while the exhaust fans were repaired. Issues encountered during the period of reduced ventilation, and how they were addressed, are analyzed. The suspected causes of the exhaust fan blade failure and how risk was mitigated during the reconstruction are discussed. Key findings and lessons learned are summarized.INTRODUCTIONLalor Mine, owned and operated by Hudbay is an underground zinc, gold, and copper mine located in northern Manitoba. The mine workings extend to a depth of 1090 metres below surface and is ventilated using a push-pull system. The mine exhausts through the main exhaust raise; equipped with two, 2,500 HP fans and upcasts through a portal located at the neighboring mine, Chisel North. Air is supplied to Lalor Mine through four intake fans: The Production Shaft Downcast (two, 250 HP), Lalor Downcast (one, 400 HP), and Chisel Downcast (one, 600 HP) fans (Figure 1). In June 2018, Lalor was scheduled to produce approximately 4,000 tpd.On June 20, 2018 at 2:52 pm Lalor Mine experienced a fault on the Variable Frequency Drive (VFD) for Exhaust Fan No. 1. Time would be needed to complete the necessary repairs. The No. 2 exhaust fan was ramped up from 525 rpm to 800 rpm, 90% of its capacity, in order to maintain underground ventilation. At 4:46 pm, approximately two hours after the rpm was increased, exhaust fan No. 2 experienced a catastrophic blade failure. The forces from the blade failure resulted in the top half of the fan housing being thrown 50 metres from where it was installed; with several large sections of fan blade being thrown further (Figure 2)."

The Deep Mixing Method (DMM) was employed to raise and support the LPV (Lake Pontchartrain and Vicinity) -111 Levee in New Orleans. Even using innovative technologies, the tight schedule to achieve the 100 year elevation by June 1, 2011 was a daunting endeavor. The size and scope of DMM proposed for LPV-111 was on a scale never attempted outside of Japan. Over 1.65 million cubic yards of DMM was installed. LPV-111 is adjacent to the Bayou Sauvage Wildlife Refuge, which is the largest urban National Wildlife refuge in the United States. By utilizing DMM to control settlement and improve stability of the taller levee, side slopes were steepened from the conventional 1V:4H to 1V:2.85H, thus avoiding encroachment into the Bayou Sauvage and reducing the amount of clay needed for the completed levee. The COE utilized Early Contractor Involvement (ECI) for this contract. The ECI process allowed for input from the DMM contractor, TREVIICOS South, in order to optimize equipment resources and material requirements. This paper gives an overview of the extensive DMM requirements for LPV-111 and explains the benefits of the ECI process.

Refractory gold ore contains sulfide minerals like pyrite, chalcopyrite and sphalerite that are difficult to be directly leached by cyanidation or by lixiviants such as thiourea and halides. In such cases, it is necessary to perform oxidative pretreatment of the ore to oxidize the sulfide matrix, and gold particles can be exposed to the solution during the leaching process [1]. In recent years, hypochlorite and chloride solutions have been introduced as alternative materials for hydrometallurgical methods of gold extraction instead of cyanide solution [2]. A previous study by Hasab et al. [3] reported that the gold dissolution rate in hypochlorite solution was higher than in the cyanidation process, due to the presence of hypochlorous acid (HOCl) as a strong oxidizing agent. In a hypochlorite leaching system, the change in pH can influence the presence of oxidant species in the solution: the hypochlorite ion (OCl−) is stable in pH greater than 7.5, HOCl can be found in pH between 4.0 and 7.5, and aqueous chlorine (Cl2 (aq)) is the predominant oxidant when pH is below 4.0. As HOCl is the most effective oxidizing agent for gold extraction, pH should be maintained in the range of HOCl stability [4].

INTRODUCTION The primary tool which is available to improve the stability of an open pit mine at a given slope angle is control of groundwater pressure. This paper sets out the methods by which water pressure control can be achieved in large mines in different materials. The emphasis of this paper is on large mine slopes because. these slopes exhibit the greatest need for groundwater control, the greatest economic benefit from groundwater control, and the greatest technical challenge in achieving groundwater control. STABILIZATION USING MTER PRESSURE CONTROL Groundwater pressures reduce the stability of slopes in two fundamental ways, as shown schematically in Figure 1. In the example, a horizontal failure surface intersects a vertical joint or weak zone. Water pressure acts: o On the base plane to reduce the normal force on the sliding surface and hence reduce frictional resistance to sliding, and o On the vertical back plane, to create a driving force for the system.

The Approach: A miniature sampler absorbs NO2 which can be measured using a simple method adaptable to both multiple automatic analyses or individual on-site analysis. How It Works: The NO2 sampler is encased in a plastic tube approximately half the length of an ordinary ball point pen and clipped to an underground worker's hard hat. Removal of the end cap activates the personal sampler. Air from the working place enters the open end and diffuses at a controlled rate down the tube interior. The diffusion rate is dependent on tube dimensions (ratio of cross-sectional area to length). Today?s steel industry is in a vibrant and resurgent mode. This transformation has been in the making for some time now. Over the last few decades, the industry was steadily transforming itself into becoming a high tech industry. New technology was adopted, efficiencies were enhanced, lean manufacturing concepts were brought in and a strong customer focus was developed. Today?s mills employ sophisticated instrumentation, computerized monitoring, on-line smart systems etc. driving processes that are producing at rates of many tons/minute. All this is not possible without judiciously using the latest available technologies in various fields and developing a more sophisticated work force. Prof. Sohn has been a pioneer in driving the concept of process engineering in process metallurgy. In this paper the focus is on some aspects of process engineering ? primarily process modeling - as applied to iron and steelmaking processes. Examples are shown to demonstrate how problems have been addressed. Some thoughts on future directions are presented.

The zinc-lead blast furnace process has been installed by two smelters in Japan. In 1966 a unit was commissioned at Harima in Hyogo prefecture by Sumitomo Metal Mining Co. Ltd. and in 1969 a consortium of other Japanese zinc and lead smelters completed their unit at Hachinohe in Aomori prefecture, managed by Mitsui Mining & Smelting Go. Ltd. , the major shareholders. The special characteristics of these two plants are described with particular emphasis on their location and operation. Reference is also made to the processing of a wide range of raw materials, including residues from other processes. Both plants pay scrupulous attention to plant and environmental hygiene to meet the high standards set by the local prefectural governments; the ways in which this has been achieved are discussed. General performance data on each furnace is included, showing that this pyrometallurgical process is supplying 16% of Japan's zinc and refined lead consumption. Impacts of mining on surface water and groundwater occur frequently not only in active mines but in abandoned mines, then the establishment of effective legal mechanisms for the regulation and control of mine waters is a necessary step in order to avoid pollution from mine waters and in consequence to contribute for a sustainable development of the mining industry. Currently there is a lack of a specific European legislation about mine waters and this absence of legislation occurs too at national level in many European countries. This absence of an European specific regulatory framework for mine water pollution questions the effectiveness and the applicability of the current legislation and this fact has led the EC to put forward proposals legislation to adequately deal with environmental and social consequences of mining, and among those proposals is a draft for a directive on waste from the extractive industry that includes provisions for mine water pollution.

Geological hazards such as faulting, basement ridges, and zones of "thin" (< 6 m) coal have a major impact on mining economics of underground operations in Huntly Coalfield. Experience has shown that drillhole-based investigations do not yield sufficiently detailed models of the coal seam to allow management of the planning risks associated with these hazards. Consequently, operational performance is affected by unplanned costs associated with lower productivity, loss of coal reserves, and expensive strata control remedies in problem ground conditions. Huntly Coalfield is a challenging environment for acquisition of good quality seismic reflection data due to the very thick (10-85m) weathering layer. Since 1994, successful acquisition of 2D and 3D data has been achieved through a combination of careful testing of technical parameters and experimental trials prior to committing to production recording. High resolution seismic reflection (HRSR) is proving to be an investigations technique which generates accurate and reliable models of the coal seam and associated structures, such as normal faults and topography of the surface on which the coal seam rests. The HRSR technique has been applied and developed in the Okowhao Sector where Huntly East Mine is developing and extracting coal reserves. Results of a recent 3D survey demonstrate that this technique is capable of revolutionising risk management in mine planning for underground mines in structurally complicated coal deposits.

This paper describes the slope design case history for Fleming NO.3 mine and illustrates the application of modern techniques. The new design has resulted in significant financial savings due to a reduction in required waste stripping.

The recovery of gold by ore leaching is influenced by the size of the particles and the chemical environment. Within the experimental error associated to the testing of gold ore, results of a series of leaching tests conducted with natural ores rather than monosized samples confirm that gold dissolution increases with decreasing ore particle size. The tests also indicate that the dissolution of gold in the ore within a size interval is weakly affected by the intensity of the size reduction method.

Geochemical and mineralogical analyses of 46 samples from six drill cores in the Mayday mine pile near Silverton, Colorado, demonstrate wide variability in trace metal contents with spatial location. This variability results from the original geochemical composition of mined rocks, mixing of ore and non- ore rocks during emplacement of the pile, and weathering since rocks were placed in the pile. Weathering reactions are controlled by factors such as pile thickness (depth), water availability, microbial activity, and mineral dissolution and formation kinetics. These factors result in variable redistributions of trace metals from primary (ore) to secondary (weathering products) minerals within each core.

Appalachian kimberlites appear to be spatially distributed along two structure parallel trends running from Tennessee to New York. It is hypothesized that the kimberlites were emplaced at the intersection of structure parallel basement faults and cross- structural lineaments. The latter are expressed in potential field and remotely sensed data. The western trend follows the Rome Trough from eastern Kentucky to south central New York. The eastern trend parallels the edge of the Ridge and Valley in Virginia and Maryland.

One of the major problems facing today's mining industry is how to manage people and control costs in the underground mine. It is a problem with a solution. Because of the concerted effort to find and mine minerals near the surface, open-pit mine opportunities for certain minerals are becoming more limited; the mining industry will eventually be forced to produce more and more from underground mines in order to obtain those resources necessary to sustain our civilization. Although much technical progress has been made towards solving problems associated with the underground mine, such as - mine design, ventilation, rock support, equipment, and blasting techniques, little has been written on managing mining personnel. Much of what is being taught and practiced today about human relations, motivation, incentives, etc., applies to the underground miner, but the uniqueness of his group dictates a different treatment from that associated with the typical construction worker or factory employee. The difference lies with the kind of man attracted to underground mining and his working environment. Anyone who has had the responsibility of managing an underground mine would agree it is a unique problem.

The requirements of today's and tomorrow's markets led to a new generation of facilities which handles large tonnages annually, loading them into larger vessels, at higher rates, efficiently and reliably, with minimum labor input, and significantly greater attention to the environment. A great variety of products are handled, including various grades of metallurgical and steaming coal, coke, iron ore in its many forms, shapes and moisture content, phosphates and fertilizer products, bauxite and alumina ores, limestone, sugar and grains, cement and many other products. When we examine the annual tonnages involved in these movements the economic value of handling these products on a bulk basis can be readily seen. Although nearly all these products have been handled by continuous bulk conveyors for many years, recent governmental response to environmental concerns require a re-evaluation of previous procedures. Most air pollution and emission control regulations and ordinances have recognized several specific sources of pollution. Detailed regulations and requirements have been developed concerning smoke emissions, emissions of visible air contaminants and particulate matter from power plants, heating plants, refuse burning equipment, coke ovens, steel furnaces, kilns, and other industrial installations. Although fugitive dust is also an environmental pollutant it is governed by regulations that are more general and therefore, more difficult to apply. A general definition of a "fugitive particle" is any matter which escapes or becomes airborne from an operation or which is emitted into the atmosphere without passing or being conducted through a flue, pipe, stack or other structure designed for the purpose of emitting air pollutants into the atmosphere. This paper deals with the aspects of fugitive dust emissions as they apply to shiploading. Fugitive dust emissions control is important not only because of environmental regulations but also to prevent the loss of valuable product, reduce housekeeping, and reduce breakdown and maintenance problems to land and vessel equipment. Remote sensing methods have been used to evaluate vegetative growth patterns for many applications, though relatively little work has focused on tracking mine reclamation progress. For coal mines in Central Appalachia, reclamation approaches that include production of biofuel feedstocks are increasingly attractive, as these may yield significant post-mining land values and contribute to carbon-neutral energy supplies. To optimize biomass productivity, the influence of reclamation practices on biomass production must be well understood, which necessitates tracking biomass production over long time periods. Satellite-based estimations may offer low-cost alternatives to conventional biomass estimations and, also, the potential to provide critical input for carbon accounting at varied spatial scales. In this paper, experimental biomass production plots on reclaimed mine sites established in Wise County, VA, are used for a comparative study of satellite spectral derivatives, which are evaluated against groundbased biomass estimates. Metrics based on four spectral derivatives, band ratios and spectral transforms were regressed against biomass measured in situ for different species. The results demonstrate that greenness and principal component 2 (PC2) were more efficient than the other spectral derivatives for monitoring the biomass of this reclaimed area. Also, it is shown that Landsat-5 TM has provided promising results for monitoring of the biomass changes occurring from 2008 to 2010 over the study area.

Coal mining in the Bulli seam in the Southern Coalfield in New South Wales has experienced the dangers of outbursts of coal and gas. These incidents present a major risk to the safety of mine workers and consequently a threat to the overall viability of the mining operations. Since the occurrence of a major outburst incident in the South Bulli Colliery in 1991 involving three fatalities, the primary contributory factors associated with coal/gas outbursts in the Bulli coal seam have been identified and a strategy for the management of the outburst risk has been formulated by the cooperation of mine operators, mine workers and the Department of Mineral Resources. Utilisation of risk management techniques in combination with modern gas drainage technologies and geological control methods have led to the formation of this effective outburst management plan. Clause 15 of the Coal Mines (Underground) Regulation 1999 in NSW has made it mandatory in an area of outburst to carry out mining in accordance with a code or guidelines developed for mining in outburst prone areas (DMR, 1999). In this paper an outburst management system is discussed which was developed for collieries in the Southern Coalfield that brings together all the available information including past experiences and risk management techniques in a documented form in order to maximise safety of mine workers. It is concluded that since the introduction of the outburst risk management systems in the Southern Coalfield in 1994 considerable reduction in outburst incidences have been experienced.

Remote sensing, in general, is the recording of light and heat from a planet's surface within a certain range of the electromagnetic spectrum. For the Earth, systems capable of this recording are the MultiSpectral Scanner (MSS) and SLAR. The former is the recording system used in satellite and will be discussed in the next section. SLAR stands for Side Looking Airborne Radar. This device sends invisible radar waves which illuminate the Earth's surface. The reflected waves are subsequently recorded as images. The system can operate through darkness and dense cloud cover, and is particularly suited for the study of geological structures at both large and small scale (Wayland 1976). Remote sensing techniques are now widely being made use of in mineral exploration, especially for those last remote 'sanctuaries' where geological data are meagre or non-existent. For places like Borneo, West Irian, the Amazon Basin etc. remote sensing, in the form of both satellite imagery and SLAR, is of considerable help, in fact more often than not the only help, to delineate geological provinces, geotectonic domains and meaningful lineaments. In the case of New Zealand, well made geological maps at scale 1:250 000 by the Geological Survey are available to the public and these provide useful preliminary grounds for the selection of target areas. However, even in this case LANDSAT imagery can be of aid as will be shown later.

"Increased mechanisation in underground stoping operations results in higher waste percentages in the run of mine. A new method of reducing waste is optical sorting. Optical sorters have low maintenance costs and can be installed underground. Rejected waste can be backfilled directly without having to be hoisted, therefore increasing the shaft capacity for a more valuable product.Pre-Concentration by Optical SortingA series of tests with an optical sorter at the laboratories of the Mineral Processing Department of the RWTH Aachen have shown that it is technically possible to sort almost any kind of ROM which has even slight differences in colour (Platinum Reef, Gold Reef, Kimberlites, Industrial Minerals, Aggregates, Coal, etc.).By employing optical sorters prior to conventional sorting plants, it is possible to reduce the amount of waste to be processed. Figure one shows test results of ore which is quite similar in colour to the waste (grey). With increasing waste in the feed, the recovery decreases according to the graph. In figure two, the colour ratio parameter of the sorter was changed with a similar waste percentage in the feed. With increasing loss of ore, a cleaner product can be achieved. Before designing a sorting plant, throughput tests are carried out. Throughputs vary depending on the size range. Sorters have a limit of about 250mm top size. In this upper size range, throughputs of 150t/h can be achieved."
