At the time of mine accidents one frequently hears the expression "there will be accidents as long as there are mines", or it may be varied to state that the accident in question was unavoidable, or something similar indicating inability to prevent accidents. However, it is the opinion of the writer, after over twenty-one years experience in both coal and metal mining, seven years being spent wholly in safety work, that by far the greater number of all accidents is due to carelessness or negligence. This carelessness or negligence may be on part of the victim of the accident or of one of his companions, or on part of his employer or possibly even of the inspector or safety man, and in some cases all of these agencies are involved. It manifests itself in various guises, one of the most serious being violation of state mining laws or regulations or of mine rules, and frequently the rules of ordinary common-sense. The cause very frequently is that trait in human nature to "take a chance" and it has been the writer's observation that the worst offender is the old-time miner who has come safely through many years experience, who was mining when the "safety man was a baby", and who clings to old-time ideas and practices and has "no use" for "newfangled" present-day "notions". Unquestionably, there are accidents, wholly unavoidable and many are caused by ignorance and by the new-comer in mining, yet as previously indicated, the writer's observation is that the old-timer is by far the worst offender, bringing grief to himself, companions and employer. Of the numerous manifestations of carelessness and negligence in connection with mine accidents, which have come to the notice of the writer, only one will be treated in this paper, viz: Violation of State Mining Law, and mention will be made only of explosions and major accidents in coal' mines with which the writer came in personal contact as investigator. In most cases, in addition to mine inspection after the accident, the writer heard the evidence given at Coroner's inquest and thus was enabled to for¬mulate fairly definite impressions as to events and conditions leading up to the occurrence.

"The Central Bayside System Improvement Project (CBSIP) is a critical element of the Sewer System Im-provement Program (SSIP), a 20-year, $6.9-billion investment by the San Francisco Public Utilities Commis-sion (SFPUC) to upgrade San Francisco’s 100+ years old seismically vulnerable combined sewer system. The CBSIP’s centerpiece, a proposed 10,000-foot-long (3,048-meter-long) Channel Tunnel, will provide reliable and redundant gravity conveyance and storage of combined wastewater flows from the northern and central parts of San Francisco to the Southeast Water Pollution Control Plant (SEP) while ensuring compliance with regulatory requirements for combined sewer discharges to the Bay. A tunnel with a diameter of 23 to 30 feet (7 to 9 meter) is being planned using pressurized-face tunnel boring machine technology. Project challenges include tunneling through Franciscan Bedrock, mixed ground conditions in close proximity to the Bay, connector tunnels and shafts, construction in congested urban environment, and challenging real estate issues for shaft access. BACKGROUND AND NEED FOR TUNNEL The City of San Francisco is one of the most beautiful cities in the world. About 850,000 people live in the city and many more come to work, for tourism, and for attending conferences. The SFPUC is a department of the City and County of San Francisco that provides energy, drinking water and wastewater services to San Francisco, wholesale water to three Bay Area counties, and green hydro-electrical and solar power to San Francisco municipal departments. The SFPUC wastewater system includes tunnels, large storage/transport boxes, 1000 miles of collection sewers, 28 pump stations, and three wastewater treatment plants. San Francisco’s combined sewer system collects both wastewater and stormwater in the same sewers, which is then pumped, treated to stringent state standards and discharged into the San Francisco Bay and Pacific Ocean. After decades of service, much of the sewer system is nearing end of its working life. During the 1989 Loma Prieta earthquake, damage to SFPUC water and wastewater infrastructure was extensive. The 11,200-foot-long (3,400-meter-long), 66-inch-diameter (1,676-mm-diameter) Channel Force Main, built in 1976, and a critical component of the SF wastewater system was also damaged. This force main carries 70 percent of the bayside dry weather sewage flow from the northern and central part of the city to the SEP. The force main is at the end of its life and was not constructed to the current seismic code; in addition it has no redundancy. Figure 1 shows the project background and the force main that will be provided redundancy by the new gravity tunnel."

Value added blasting is a process methodology that links tasks and processes, with the aim to improve/enhance the technical application of explosives technology and initiating systems, resulting in improved output. The phases and steps of a typical value added blasting intervention are discussed in this paper. Each step having distinct purposes, principles and methodology. The underlying philosophies and process steps of the programme are described for each step. This paper gives an account of projects that have been completed in two Anglo American Gold Mines. This will include a description of the projects that were conducted at Vaal Reefs 8 Shaft and Western Deep Levels South Mine covering: ? The need for blasting improvements ? The approach that was followed ? The result of the programme.

This report details a study which was designed to assess the effect of retroreflective material on detection and form recognition of workers in the mining environment. Further, the study attempted to determine an optimal configuration of retroreflective material with respect to amount and placement on the body to facilitate detection and form recognition. The study involved a one-fifth scale simulation of the perceptual task found in the underground low seam coal mine. Four different configurations of reflective material were presented in various body positions, common to low coal miners, at three different locations in the visual field, i.e., 10, 25, and 45 degrees from focal line-of-sight. All reflective configurations significantly improve detection and form recognition over configuration (cap only) worn by miners. As the amount of reflective material increased, accuracy also tended to increase. Based on the configurations investigated by this study, the Armband configuration (i.e., refectorized armbands, belt, self-rescuer and battery pack) was determined to be optimum. It is recommended that an actual field evaluation be made using the Armband configuration to establish if simulator results are generalizable to the underground mining environment.

During the past few years the combination of continuous billet casting with in-line reduction has been studied and several machines have been built with this capability included in the design parameters. (1,2,3,4,5) In-line reduction and continuous casting in combination are obvious and attractive. As pinch rolls are already in place for the continuous casting operation, it can be readily seen that a redesign of the mechanical and electrical equipment of the pinch rolls is all that is necessary to have in-line rolling. There have been numerous in-line reduction experiments conducted by simply increasing the pinch roll pressure on existing machines. (6,7,8,9) These generally failed to produce quality billets and often resulted in internal metallurgical defects (Figure 1).

The toughness and fracture mechanisms of ores with significantly different microstructures from Yandi, Koolan and Mount Whaleback were studied with a view to understanding their degradation behaviour. Bulk fracture toughness was determined by the 3-point single-edge notch beam test and the results interpreted using Weibull statistical analysis; indentation fracture mechanics was also used to measure fracture toughness of the different mineral forms and to understand their response to contact fracture

Hydrolysis of metallic ions depends on pH and the products of hydrolysis either activate flotation or can lead to inadvertent activation of gangue particles, but can also depress flotation of valuable components. One of the most recently studied topics is the use of seawater in flotation. Seawater is a concentrated NaCl solution (about 0.6 mol/L) that also contains considerable amount of Mg2+ and Ca2+ ions. The hydrolysis products of Mg2+ strongly depress flotation of molybdenite over the pH ranges over which they form in the process water. This effect can be eliminated by removal of hydrolysing ions from the process water or by carrying out the flotation at the pH over which such a hydrolysis can be avoided. The former can be exemplified by prior precipitation of the harmful hydrolysing ions as, for example, in the University of Concepcion Patent. The latter can be exemplified by the use of metabisulfite to depress pyrite in the flotation of Cu-Mo sulfide ores in seawater, the process that is carried out at much lower pH, which avoids formation of Mg2+ hydrolysis products.

During the past several years, Raymond Kaiser Engineers has prepared numerous studies concerning the technical and economic feasibility of coal exporting facilities in the western United States. Perhaps the most striking conclusion to be drawn from these efforts is that many people underestimate the complexity of the highly competitive, dynamic coal marketing system which includes coal loading terminals as an essential but relatively low-cost link in the energy production and delivery chain. The title of this paper, "The Status of Western United States Coal Ports - An Economic Assessment," should not delude anyone into thinking that the future of western U.S. coal- exporting ports has now congealed into a well- defined economic plan. As this paper will demonstrate, the situation is complex and more dynamic than ever and promises to remain so into the late 1980's.

Graphite with layers structure is easily to form graphite intercalation compounds (GICs) by means of intercalation reactions due to the weak cohesion force among carbon layers, van der Waal?s interactions. By controlling the oxidation-intercalation, high quality exfoliated graphite with low residual sulfur content and MClx-GICs (M=Fe, Co, Ni, Cu, Zn) powder for electromagnetic wave absorbing and shielding materials have been invented. The expansion volume of expandable graphite can be larger than 160 mL/g while the residual sulfur content is less than 800 ppm. The MClx-GICs powder can shield infrared ray and laser completely up to 15 min. We have also in-vented exfoliated graphite for heavy oil adsorption and mild-expansion exfoliated graphite for anode materials in lithium ion battery by controlling the intercalation/de-intercalation. The exfoliated graphite can adsorb heavy oil up to 80 g/g, it also exhibits better performance than commercial active carbon in sewage treatment. The mild-expansion exfoliated graphite as anode material shows high capacity of 370 mAh/g and good cycle performance.

INTRODUCTION In the fall of 1983, the fourth new Rapid Transit System constructed in this country in the last forty years will go into revenue service. The Baltimore Metro will bring to this major Mid-Atlantic seaport the first phase of a planned intermodal transit system, serving a metropolitan area with a population in excess of four million people. Originally, the Mass Transit Administration (MTA) considered construction of a twenty-eight mile system known as Phase 1. Its two lines would run from the center of Baltimore City, one going northwest to Owings Mills and the other south to Anne Arundel County. Legislative approval was granted in April, 1976; however, funding was limited to constructing a segment fewer than eight miles in length, along the northwest corridor from Charles Center to the Baltimore City county line. This segment is known as Section A. The project is 80% federally funded and 20% state funded, with a capital cost estimate of $797,000,000 to complete. This report addresses the development and implementation of a Coordinated Insurance Program for the construction phase of Section A, and includes a description of the associated risk management process. GENERAL DESCRIPTION OF SECTION A Section A of the Baltimore Metro is composed of a rapid transit line eight miles in length which serves the northwest corridor between Charles Center in downtown Baltimore and Reisterstown Plaza at the Baltimore City county boundary. Who would believe that young elementary school children could understand something as complex as the mining industry? The Challenge The challenge of accomplishing this is tremendous. An examination of Canadian school children indicated that they knew very little about the world of work in general, and hardly anything about the mineral resource industry in particular. It calls to mind the story about two youngsters who decided to play house. The little girl played her role well as she went about the house doing things she had watched her mother do every day. When it was the little boy's turn, he grabbed his lunch, waved goodbye to his "wife," jumped on his bicycle and pedalled furiously down to the end of the driveway-where he stopped. The youngster explained that he saw his dad go out of the driveway each morning, but he didn't know where he went, or what he did on the job. When you stop to consider the number of families today with two working parents, you can begin to appreciate the importance of the opportunities for role identity provided by this project. The Approach The challenge of teaching the school children of Canada about the world of work and, specifically, the mining industry is the challenge which the Canadian Institute of Mining and Metallurgy met with the Curriculum Development Research project. It started when a small, dedicated group of Vancouver members met Dr. Doreen Binnington. After this meeting, Dr. Binnington, an expert in curriculum development at the University of British Columbia, set herself to develop a curriculum program for the elementary shcools which accomplishes two main goals: It uses the "Identity Approach," developed by Dr. Binnington, an expert in curriculum development at the enable students to understand and appreciate the contribution of people, in this case, working with mineral resources. It has pupils enthusiastically exploring the importance of rocks, minerals, and the mining industry in their lives [(Fig. 1)]. It is well accepted that the blast furnace will continue as the main route for iron-making for many more years. However, its limitations have motivated researchers to look for alternative routes, particularly, those where non-coking coal is used for liquid iron production. Smelting reduction (SR), where coal and oxygen are used to produce liquid iron and power is now receiving serious attention. Several smelting reduction reactors are now operating in pilot plant scale and one or two have reached commercial status.

State mining regulations in Queensland stipulate that a mine be provided with an earth installed on surface and that all underground equipment that needs to be earthed should be connected via a copper conductor to this surface grounding point. The regulations further require that all exposed metal surfaces in underground substations be connected to an earth. In a study of earthing conditions at Mount Isa mine it was found that the physical anchoring of metal work at substations (with rockbolts and associated steel mesh) also provides an excellent electrical connection to the country rock. It was established that not only is the mine's earthing system grounded at numerous underground locations but also that such anchor points offer, in general, a path to ground with lower resistance than does the legally required grounding point on the surface. It is recommended that metalliferous mines should utilise a multiple-earth system and not a run earth connected to a single, above-ground earth point

Infrared spectroscopy, employing the internal reflection method in conjunction with solvent extraction techniques, was used to study surface reactions between aqueous potassium ethyl xanthate and both sphalerite and precipitated zinc sulfide each activated with aqueous copper sulfate. It was determined that reactions occurring on the sphalerite and zinc sulfide surfaces produce the reaction products ethyl dixanthogen and cuprous ethyl xanthate, some of which are ad¬sorbed on the mineral surfaces. According to infrared spectra showing the reaction to form these compounds, the ratio of cuprous ethyl xanthate to ethyl dixanthogen adsorbed is directly related to pH of the flotation solution.

The purpose of this paper is to present some data and comparisons regarding three types of Rheolaveur plants washing bituminous coal from the Pittsburgh seam. It deals only with the sections that handle coarse coal. The Rheolaveur system consists of launders or troughs in which coal and its associated impurities are conveyed by water. The impurities. being heavier than the coal, settle toward the bottoms of the launders and are drawn off through specially designed openings called Rheo boxes. Fundamentally, each plant consists of straight-sided, flat-bottomed launders of varying width and length, each launder equipped with one or more Rheo boxes for the removal of high-grauity fractions of the feed that are undesirable in the cleaned coal. Each launder has at least two longitudinal sections, the first with a slope of from 11/2 to 4 in. per foot of length, called the classification section. The single-box launder has a classification section followed by a washing section ahead of the box. A two-box launder is a single-box launder followed by a washing section and a second box. The slope of the washing section between the first and second boxes is determined to some extent by tonnage. At the end of the classification section, which may. vary from 8 to 15 ft. in length, the slope abruptly flattens out to a slope of from 92' to 1 in. per foot of length. This flatter section of the launder, called the washing section, may vary from a few feet, in a single-box launder, to 15 ft., in a two-box launder. Raw coal entering the head end of what we choose to call the "primary" launder or launders is pushed down the classification section by a stream of water entering the head end of the launder with the coal. As it strikes the washing section, of lower pitch, the velocity of flow is retarded and the heavier particles of slate, pyrite, bone, etc., take their places on the bottom and are drawn off as they pass over the Rheo boxes. As development in underground coal mines progresses away from the critical components of the ventilation system, including the main fans and ventilation shafts, resistance within the system is markedly increased. This results in higher pressure and flow requirements from the main fan(s) to maintain adequate face ventilation. Upgrades to the primary ventilation system are often required. The use of booster fans underground is a potentially practical alternative to the addition of shafts and surface fans. This study presents simulations of the use of booster fans in two underground ventilation scenarios, one longwall and one room-and-pillar. The booster fan effects on the power requirements of the ventilation systems are evaluated.

A series of full scale laboratory linear rock cutting tests was carried out with a long bladed single disc cutter (292 mm diameter, 11mm wide constant cross-section blade) on dry and water saturated sandstone blocks using a linear rock cutting machine (LRCM). The muck collected from the cutting tests has been analyzed to establish a link with the specific energy (SE) of the operation. Coarseness index (CI), a non dimensional parameter, has been found to be a useful indicator of the relative efficiency of the cutting operation. An inverse relationship of the form SE = k CI-n is observed between SE and CI with very high CI usually corresponding with the lowest values of SE. The power n of the relationship for both dry and saturated rock tests is much less than the values found in the previous studies using constant cross section disc cutters and is believed to be dependent on the strength of the rock and the cutter type used. Although the average CI is found to be lower for saturated rock than for dry rock, at the same time saturated rock cutting is found to be less energy intensive (in terms of SE) than dry rock cutting. Moderate correlation exists between the production rate or yield and the SE of the cutting operation for both dry and saturated rock, which can provide some useful information about the field production rate discounting the effect of discontinuities in the rock mass.
