As national and international health and safety management system (HSMS) standards are voluntarily accepted or regulated into practice, organizations are making an effort to modify and integrate strategic elements of a connected management system into their daily risk management practices. In high-risk industries such as mining, that effort takes on added importance. The mining industry has long recognized the importance of a more integrated approach to recognizing and responding to sitespecific risks, encouraging the adoption of a risk-based management framework. Recently, the U.S. National Mining Association led the development of an industry-specific HSMS built on the strategic frameworks of ANSI: Z10, OHSAS 18001, The American Chemistry Council’s Responsible Care, and ILO-OSH 2001. All of these standards provide strategic guidance and focus on how to incorporate a plan-do-check-act cycle into the identification, management and evaluation of worksite risks. This paper details an exploratory study into whether practices associated with executing a risk-based management framework are visible through the actions of an organization’s site-level management of health and safety risks. The results of this study show ways that site-level leaders manage day-to-day risk at their operations that can be characterized according to practices associated with a risk-based management framework. Having tangible operational examples of day-to-day risk management can serve as a starting point for evaluating field-level risk assessment efforts and their alignment to overall company efforts at effective risk mitigation through a HSMS or other processes.

For the integrity of welded structures such as pipelines, the toughness of the heat-affected zones (HAZ) is an important property. This property is often specified in terms of Charpy V-notch (CVN) impact toughness and determined by the standard test method. An experimental study has been conducted in an industrial laboratory to measure the impact toughness of the coarse grain heat-affected zones (CGHAZ) of an X80 line pipe material. The field girth welding conditions in terms of heat input were simulated, together with special edge preparation to improve the targeted testing of the CGHAZ. The effect of toughness of the neighboring regions, in particular the weld metal, on measured impact toughness of the CGHAZ was investigated. The fracture paths of the CVN tested samples were examined by both optical and scanning electron microscopy. The deviation of segments of the fracture path into the weld metal was found to have a significant effect on the measured impact toughness of the CGHAZ.

In this work, we investigated the effect of the rolling process (warm-rolled or cold-rolled) on microstructure evolution and mechanical properties of 9Mn steel. After intercritical annealing at 670°C for 10min, both cold-rolled and warm-rolled microstructures are composed of ultra-fined austenite and intercritical ferrite. The austenite in warm-rolled sample includes two types of morphology, namely equiaxed and lamellar, which exhibits a wide distribution of grain-size. The incomplete recrystallization or recovery behavior of ferrite in warm-rolled and annealed sample results in higher yield strength and shorter yield point elongation. Moreover, the warm-rolled and annealed sample extends the TRIP-related work hardening over a broad strain range, resulting in a better combination of ultimate tensile strength and total elongation.

"Potential metal supply refers to metals to be produced from as yet unknown deposits. Naturally, to measure potential supply requires, among other things, inference to the number of deposits, their tonnage, and their grade Thus, probability distributions were constructed for each 400-square mile cell in Northern Sonora, Mexico. A computer system was developed that simulated the occurrence of deposits for each cell, exploration for the deposits, and their exploitation. Evaluation was performed for each deposit by discounted cash flow analysis.An objective of this study was to explore the effect of the transportation network and required road or railroad construction upon potential supply. Consequently, the evaluation system was designed to consider the spatial location of a simulated deposit to the existing transportation network and the market destination. The construction of linkages to connect the deposit to existing transportation arteries as well as the carrying of the concentrates to the intended market were simulated. in terms of cost. The selection of the path took into account the existing terrain. This was done by digitizing topographical maps and representing the topography of the area by an orthogonal grid of elevations. The optimum route was selected by dynamic programming, meeting a prescribed gradient constraint. The potential supply of each cell was determined, allowing for the infrastructure effect."

Physical modelling of liquid-phase oxygen mass transfer in a water model of part of a proposed new polymetallic smelter has been achieved. A study of solute mass transfer caused by multiple, subsonic air jets contacting superoxygenated water flowing in a channel model of the smelter converting hearth has been carried out. For a wide range of channel flow rates the rate of mass transfer was found to be independent of water velocity. The water model has been used to compare mass transfer for both open-packed and close-packed multiple-lance arrays with circular nozzles of diameters 2.26, 4.95, 10.95 and 24.40 mm. It was found that with fewer of the larger-diameter nozzles a reduction of up to 58% in liquid-phase mass transfer occurred, especially for the close-packed lance configurations. This restriction of liquid-phase mass transfer would result in reduced metal losses in the analogous smelting situation. The larger nozzles, however, exhibited more jet-jet and jet-channel wall interference effects than their smaller counterparts, resulting in an effective reduction in the momentum fluxes of the impinging jets

Spark ignition of combustible-gas mixtures is a relatively old subject of investigation and has been treated both experimentally and analytically by many people. Elements of static and current electricity are almost always present in mining and industrial operations, and the spark can be an extremely efficient source of ignition. Thus the practical interest of such problems as the "intrinsic safety" of electrical apparatus is obvious. Ignition and quenching phenomena are the primary laboratory representatives of combustion phenomena that are dynamically nonsteady, and such nonsteady phenomena playa considerable part both in industrial processes and in safety matters, although they are of less immediate practical interest. For several years, or from about 1932 to 1935, inclusive, the coal industry of the United States had an excellent record as to coal-mine explosions. In 1937 and in 1938 to date there has been a definite increase in the number of explosions attended by considerable loss of life and property damage. In most of these explosions one important underlying and contributing cause has been lack of sufficient (or at least efficient) ventilation to properly dilute and remove explosive gas from the mine. This increase in explosions indicates that. the lesson taught through bitter experience, many times in the not far-distant past, of the need for good ventilation is frequently being overlooked, partly because the relatively good experience of the industry in the past several years has made many mining people forgetful. Moreover mining practices are rapidly changing, and in some instances the changes are being made with little if any regard for safety. It appears, therefore, that a review of some ventilation principles and practices should be of value in preventing further disasters; this paper has been prepared with that object in mind. In coal mining the need for thorough ventilation of all underground working places is recognized by both mining practice and law; such ventilation is generally obtained by the use of fans that circulate a quantity of air much in excess of the quantity of coal produced, by weight as well as by volume. For example, in a relatively small mine producing 1,000 tons of coal per day and circulating 100,000 cubic feet of air, which is by no means abnormal, 2,000,000 pounds of coal and approximately 10,900,000 pounds of air would be moved out of the mine every 24 hours. AT the end of September, ' the metal-producing industries were almost prostrate, the production of fuels was largely curtailed, there was a fair degree of activity in general manufacturing, while retail trade was moderately good. Looking at industry on the whole, there was a large percentage of unemployment, the degree of which has increased, progressively, month by month since the beginning of 1921. There is the prospect of much destitution and misery this winter; yet while' in some, quarters, idle workers are already being fed from, commissaries; in other quarters profligacy; in "spending continues, unabated. Amid this confusion in conditions there is;' much that appears paradoxical, but in fact it is not so and indeed everything is running pretty true to form: Such activity in production as there is to be seen is mainly in textiles; - leather goods, foodstuffs, etc., i.e., consumption goods for immediate use. Even in hard times 105,000,000 people must buy a good deal of clothing and food. Besides this there is indispensable repair work and- maintenances. Thus, the good business of the year in white lead for painting. The chief uses for the metals, however, are in the production of capital goods, and except for occasional and special reasons nobody in his senses is capitalizing anything at present prices for labor. Hence several millions of men are idle. The impoverishment of Europe and the exhaustion of her credits adds to this by curtailing our export trade.

Two exploratory gasification tests of limited duration were made at Dorsten, Germany, with strongly coking Pittsburgh-bed coal in a fixed-bed pressure gasifier. To reduce the coking potential of the Arkwright coal, ash refuse from the gasification of Dorsten (Leopold) coal was added to pre-pare coal-ash mixtures which contained nominal 20 and 30 percent ash. Major adjustments in principal process variables at the beginning of the tests resulted in wide fluctuations in the rate of gas production, gas offtake temperature, and carbon dioxide concentration in the gas. Because a higher steam-to-oxygen ratio was used in the tests than was customarily employed for routine operation, gas production was lower, and steam and oxygen consumption was higher. After operating conditions had been improved, the results of a 12~-hour-long test with addition of sufficient ash refuse to obtain a nominal 30 percent ash concentration indicated that operation is feasible with this coal mixture using a fixed-bed pressure gasifier equipped with a rotating coal distributor and stirrer.

This paper describes a laboratory study of causes of external casing corrosion and the test work that led to the use of oxygen scavengers to prevent this attack. External casing failures are classified as water-line, casing-casing, collar and body failures. A corrosion mechanism based on principles of differential oxygen availability is developed that is consistent with facts known about each kind of failure. The field use of oxygen scavengers is depicted as a direct result of the laboratory study. A part of the paper is devoted to reporting on the field use of hydra-zine to control external casing corrosion. Results of field measurements made over a period of several years are presented as evidence of the efectiveness of the hydrazine treatment. The first conclusion reached is that the use of hydrazine materially reduces the cathodic protection requirements for treated wells. This result is interpreted to mean that a reduction is taking place in the amount of corrosion on the casing. Results indicate also that hydrazine shows its greatest usefulness within the first 12 to 18 months after a well is completed when pitting corrosion is likely to be most active. INTRODUCTION According to surveys sponsored by the National Association of Corrosion Engineers,' the cost of repairing casing leaks caused by external corrosion may exceed $4 million per year. In addition, well damage and lost production resulting from casing leaks probably costs the petroleum industry an additional $5 to $6 million per year. Concern about the cost of external casing corrosion led to an extensive laboratory study of factors causing this external corrosion and to the development of a new approach to its prevention. This paper presents a discussion of various causes of external casing corrosion, details of laboratory studies and the results of the field use of an oxygen scavenger in well cementing fluids to prevent the external corrosion of oil-string casing. Measurements on test wells over a period of several years show that cathodic-protection current requirements are greatly reduced when hydrazine is used in cementing mud. Reduction of current requirements can be interpreted to mean that removal of oxygen by hydrazine has greatly suppressed corrosion cells on the external surface of the casing and thereby, has reduced corrosion. To date, hydrazine has been used by the Standard Oil Co. of California in more than 200 well completions. KINDS OF CASING FAILURES A survey of a large number of casing leaks disclosed four types of external casing failures — water-line, casing-casing, collar and body failures. These types are identified largely by their location on the casing. Water-line failures are found just below the surface of water or mud in the casing annulus. Casing-casing failures occur on the oil string just below the shoe of the surface string. Collar failures are found in the threaded ends of casing joints where they are screwed into casing collars. Body failures may occur at any point on the body of a casing joint. Ex- amples of each kind of failure have some of the general characteristics that are shown in Fig. 1. Water-line failures usually result in the circumferential severance of an oil-string casing. The corrosive action causing a water-line failure usually is sharply defined and is limited to a short length of the casing. Casing-casing failures usually are accompanied by pitting corrosion distributed around the oil-string casing for distances up to 100-ft below the shoe of the surface string. Casing-casing failures may also sever the casing. Collar failures seem to start on the first thread at the bottom of recesses between collar and casing joint. Corrosion proceeds across the threads by what appears to be a normal pitting mechanism. Both casing and collar are severely attacked. Body failures are the result of highly localized pitting at any point on a casing wall. Besides the pit that perforates a casing, a large number of other pits usually are found along one side of the casing joint. The pits occasionally are filled with corrosion products consisting largely of oxides and sulfides.' Frequently, the mill scale is largely intact on the rest of the casing. Examination of a casing failure does not always reveal the cause of the failure. Frequently, the necessary details are destroyed when the failure occurs. For example, formation water flowing through a perforation at high velocity may enlarge the hole and destroy any remaining evidence of the cause of the failure. One way to obtain undistorted information about a failure is to study the nature of other pits on the casing in the vicinity of the failure. A study of such pits frequently suggests that they are characteristic of an attack resulting from the differential availability of molecular oxygen.

"Thin Spray-on Liners resist relative small deformations (< 1 mm) between rock blocks or fragments in terms of a combination of shear, adhesive and tensile-bond strengths. Shear-bond strength addresses the ability of a thin liner to resist stresses that act parallel to the rock-liner interface. Therefore quantification of shear-bond strength is essential in the formulation of design standards and requirements for Thin Spray-on Liners. In this paper Thin Spray-on Liner support mechanisms are reviewed. In addition previous methods of Thin Spray-on Liner shear-bond testing are discussed. The paper also explains the research being carried out at the University of Cape Town to investigate the effect of different parameters on the shear-bond strength.INTRODUCTIONRock surface support is widely used to combat rockfalls and resulting injuries and fatalities in the vicinity of active faces (Potvin, 2002), where workers spend most of their time (Adams & Baker, 2002; Potvin, 2002; Lacerda, 2004). The conventional (traditional) surface support systems in underground rock support include passive methods such as mesh and lacing and active methods such as shotcrete. However, rockfall sources between 1990 and 2000 in Nevada gold mines showed a steady increase in rockfall injury associated with mesh support as shown in Figure 1 (Lacerda, 2004). The conventional methods also negatively impact the mining operations with regards to costs, logistics and mining cycle times due to large material volumes (Tannant, 2001; Ozturk & Tannant, 2010; Ferreira, 2012). Therefore, these systems need to be substituted, with enhanced methods, in order to realize the business target of the mining industry of providing the planned return on investment made without harm and within budget."

Mass spectrographic studies have been conducted on the positive ions and neutral species of glow discharges containing 1.0 pct S02 and 1.0 pct CO in helium. Measurements were conducted for gas pressures from 3.5 to 10.1 torr using both dc and 60-Hz ac excitation. The ion species that were observed included S+, SO+, SO+, 0+, O2+, C+, CO+, and CO2+ with lesser amounts of SO3+, (CO)2+, SCO+, and (SO?CO)+ or CS+2. Interpretation of the functional dependence of the various ionic and neutral species on pressure and applied power showed that CO reacted with the SO formed in the discharges to produce CO2 and sulfur. Deposition of elemental sulfur was observed in the plasma container.

Since the 1960’s toroidal conductivity has been used to determine the Alumina to Caustic ratio of the digester discharge liquor stream. In recent years the application of ceramic materials have allowed the sensors to moved upstream to provide tighter control of the process. In the Bayer process, dissolving the aluminum contained in bauxite ore in caustic soda and converting it to aluminum oxide produces alumina. An important parameter of the process is measuring the alumina to caustic ratio in the digester discharge stream. The A/C ratio at a given caustic concentration is a measure of the degree of saturation of the alumina in the plant liquor. The aim in the process is to achieve a high A/C ratio. There is however a limit where precipitation will occur if the A/C ratio is too high. For many years conductivity has been used to determine the A/C ratio. Conductivity of discharge liquor is related to the caustic concentration. The change in the caustic concentration is in the same direction of the change of solubility of the alumina. Controlling the conductivity through adding additional bauxite ore and varying the dilution flow can increase plant yield.

Upon recently, most offshore foundations were based on a number of driven piles that were installed with an impact hammer. With the increase in offshore windfarms two major developments have occurred: the shift from jackets to monopiles, leading to increase of the diameter of the foundation piles, and a shift from impact hammers to vibro hammers. As the use of vibratory hammers is becoming more and more common practice, the need for accurate vibro-driving simulation software has increased, which requires that the soil modelling is enhanced to address soil fatigue during pile driving and to predict reliably the soil behavior and resistance during pile driving. In addition pile driving monitoring, which is routine for piles driven with an impact hammer, needs to become common practice. This paper addresses advances in soil modelling that allows more accurate pile driving simulation as well as the application of Vibro Driving Analysis (VDA) or Monitoring (VDM) to validate the simulation results. This is illustrated by a case study of the test pile for the Delft Offshore Turbine project, a 28 m long monopile with a diameter of 4 m that was driven 15 m into dense sand layers using a vibro hammer. After some 6 months the pile was extracted, and pile driving simulations and VDA were done both for the installation and extraction phase. 1. INTRODUCTION At the end of 2017, the total worldwide offshore wind power capacity was 18.8 GW [1]. The vast majority of offshore wind farms are currently in Northern Europe, which together account for over two thirds of the total offshore wind power installed worldwide, followed by East Asia, while developments in North America are still in very early stages.

Electrical trailing cable used to power coal mining machinery is frequently damaged by tensile strain or crushing by mining machines running over the cable. A prototype electrostatic/electromagnetic/ TDR transmitter and an electrostatic/electro-magnetic receiver sot that: o Locates cable faults by time domain reflectometry o Locates opens by detecting electrostatic signals o Locates shorts by electromagnetic coupling. 2. A prototype infrared detector and an infrared detector power supply that heats a shorted area by applying high current (at low voltage) and locating a short by infrared output. 3. Twelve production infrared detectors. 4. Detail drawings for all of the developed instruments.

This report describes the preparation of tritium-labeled organic compounds to be used by the Federal Bureau of Mines in studying the role of certain gasoline constituents in gum-forming mechanisms. The gasoline constituents that have been investigated boil within a range of approximately 90 to 230° C. and can be classified as paraffins. naphthenes, olefins, diolefins, monocyclic aromatics, polycyclic aromatics, and organic sulfur, nitrogen,: and oxygen compounds. The technique of Wilzbach has been applied to more than 50 samples. Purification techniques have been developed that accomplish rapid and effective purification as well as removal of labile tritium in a one-step operation. Data are presented to show the rate of exchange between tritium and hydrogen that can be expected for a variety of compounds common to a full-boiling-range gasoline. These values should be useful to other investigators using tracer programs involving tritium labeled compounds.

Steelmaking slags are usually used as roadbed and civil engineering materials. However, the expansion phenomenon, which is caused by the volume expansion during hydration of free CaO and free MgO contained in steelmaking slag, is indispensable. Since the hydration of free MgO in steelmaking slag is much slower than that of free CaO, it is qualitatively considered that free MgO contributes to the hydration expansion of steelmaking slag over a long period of time. Similar to free CaO, free MgO consists of undissolved MgO and crystallised MgO in steelmaking slag. The latter precipitates during slag cooling. When divalent metal oxides (FeO, MnO, CaO) are dissolved in the crystallised MgO to form a MgO-based solid solution, it is expected to inhibit the hydration reaction similar to precipitated CaO. From the previous hydration test of Magnesioferrite, which was calcined after mixing MgO and Fe2O3 reagents, it was found that the higher the Fe2O3 concentration, the slower the hydration reaction progressed. In this study, by clarifying the influence of calcination conditions of MgO-FeO and MgO-FeO-MnO solid solutions, which were prepared by the calcination of mixture of MgO, FeO and MnO reagents, on their hydration reactivity, the effect of composition and thermal treatment on the formation of free MgO that is not hydrated (or easily hydrated) were discussed. To investigate the influence of MgO grain size, coarse MgO particles, which were obtained by crushing MgO crucible, were also used for heat treatment. With increasing the amount of FeO and MnO in the MgO-FeO-MnO solid solution, the hydration reaction became more suppressed. The expansion suppression effect was higher with solid-solved FeO than with solidsolved MnO. When the calcination temperature of the MgO-FeO solid solution was increased, the Fe3+/Fe2+ concentration ratio increased, resulting in a higher hydration rate.

Remote control for mining machines ordinarily means the operator is not seated on the machine, but is located within direct view of the machine and work area Thus, remote control operators are exposed to the hazards of roof/rib falls and dust/methane explosions. The U.S. Bureau of Mines has developed a method to disassociate remote operators from these hazards. Off-board, computer-based "tele-operational" systems are utilized that station the operators one-hundred meters or more distant from the working faces. Details are provided for tele-operated systems for thin-seam extraction and haulage equipment in highwall and deep, room-and-pillar coal mines.

THE present truce in litigation between Western smelting and ranching interests gives opportunity for a summary of the results achieved by metallurgists in condensing fume and de-acidifying furnace-gases. In the absence of complete records of past experiments, there has been a good deal of unnecessary duplication of attempts and&apos; failures. Yet, especially under the recent policy of our large corporations, looking to the economic recovery of the values of by- and waste products, original and, in some cases, effective measures have been developed by American metallurgists. In the case of Bliss vs. the Anaconda Copper Mining Co., the Montana Supreme Court, recognizing the progressive efforts made to render the flue-gases innocuous, and the value to the community of the smelting company&apos;s operations, refused to hamper its work, and returned a verdict in favor of the defendant. In the trial, the experimental firms of the company in the so-called "affected region" were proved to be thriving perfectly, showing that immunity was guaranteed to vegetation and stock by the wide distribution of the sulphurous acid gas in the dry air of Montana. Indeed, L. S. Austin has proved that by the time the fume is 4,000 ft.. distant from the stack, the percentage of sulphurous acid has been diminished from 2.28 to 0.00045; and Angus Smith has shown that rain must contain 40 parts per million of free acid before vegetation is affected. The arsenic in the stack-gases is but a very small part of the original content of the ore, of which 80 per cent. and over is rejected in the concentrator and furnace waste-products or recovered in the flues.
