Fortunately, few mining professionals have dealt with what all mining professionals fear---a mine disaster. Unfortunately, many have dealt with ?accidents? which may or may not claim lives but nevertheless rise to the level of a ?disaster? given the fallout of liability and litigation in today?s society.1 The undersigned authors published an article some ten years ago regarding crisis management2 based upon their collective first-hand experiences in actual disasters to that point in history. Unfortunately, disasters have occurred since that time, prompting legislation, and a fresh analysis and expansion of those basic principles is certainly in order. Without question, no two disasters are ever alike, yet general guidelines for forming an effective plan of action can be formulated?and if a mining operation is to have any chance of survival after a disaster, such a plan is vital. The primary concern as any disaster evolves remains as stated a 2003 SME article3--the protection of human life. From that point, the focus moves to any assistance that may be needed given the magnitude of the situation. Permeating throughout this process is "damage control." The first step in dealing with a disaster, however, begins long before any threat of danger arises?formulation and implementation of an Emergency Response Plan.

"A locked cycle flotation test was done on a sample from the ovoid portion of the Voisey 's Bay copper-nickel sulphide ore. The chemical conditions for this test were generated by prior independent testing. In this test work the time in each of the five separation stages was optimized prior to executing the locked cycle test. A numerical simulation of the continuous circuit material balance was done prior to the locked cycle test. The test results were not fully satisfactory because the flotation of pyrrhotite in the bulk rougher and cleaner stages was not consistent. The preliminary indication was that the samples had oxidized to varying degrees during storage.INTRODUCTIONINCO has performed batch laboratory flotation testing on ore from Voisey's Bay which is a mixture of chalcopyrite, pentlandite and pyrrhotite. Earlier, Lakefield Research was contracted by Diamond Field Resources to test the ""flotability"" of the ore, which included a pilot plant trial. INCO's J. Roy Gordon Research Laboratory has repeated many of the batch tests that Lakefield did to verify their results. Based on their own work, INCO has modified the circuit proposed by Lakefield Research. The modified circuit is based on batch test work, but no locked cycle test (LCT) was done. It is believed that this circuit will produce a copper concentrate at 29-30% copper and a primary nickel concentrate at 15-20% with recoveries of 80% or above.For several years the University of Toronto has been working with INCO to provide ""hands on"" experience for undergraduate students as an Industrial Research Project (IRP). This year, the objective for IRP was to optimize the time for each of the five separation stages for the Voisey's Bay ore then to numerically and experimentally simulate the material balance for a continuous circuit."

Abstract. The Surface Mining Control and Reclamation Act of 1977 (PL-95-87), Section 522, establishes requirements for state regulatory authorities to designate lands unsuitable for all or certain types of surface coal mining. Any party having an interest in an area that may be affected can petition for it to be designated unsuitable for mining. Section 522 could present major obstacles to the mining industry by typing up an area in an endless round of petitions. The program under development for Kentucky is examined and analyzed as an example for determining the hidden impacts of Section 522.

Significant cost and construction benefits were obtained from performing a bi-directional static load test prior to construction on the Russell Street Bridge Replacement project in Missoula, Montana. The project utilized 24 drilled shafts to support two three-span bridges across the Clark Fork River. A bi-directional static load test (“BDSLT”) was performed on a large-diameter production shaft to demonstrate construction methods and confirm design assumptions. Subsurface conditions consisted of gravel with sand to a depth of 20 feet (6.1 m), underlain by gravel with sand, silt, cobbles, and occasional boulders. The test shaft had a nominal diameter of 5.9 feet (1.8 m), and an embedded length of 75.5 feet (23.0 m). The geotechnical engineer of record and the BDSLT test engineer collaborated to optimize the BDSLT jack assembly location within the test shaft, determine appropriate instrumentation locations, and plan the test protocol. The maximum applied test load was 10,707 kips (47.6 kN). Significant benefits were obtained from performing the BDSLT test, including 1) reducing each production shaft’s length by using a higher LRFD resistance factor allowed by having performed the testing, 2) using test-determined unit shaft resistances to evaluate and accept two production shafts that may have otherwise required remediation, and 3) providing the Montana Department of Transportation (and other designers) with increased presumptive unit resistance values in the gravel/cobble/boulder deposits. PROJECT DESCRIPTION The project was constructed in Missoula, Montana for the Montana Department of Transportation (“MDT”). The original bridge was approximately 60 years old, and consisted of a two-lane, 420-foot-long (128-m), four-span, riveted steel plate girder design spanning the Clark Fork River. The bridge was due for replacement owing to its deteriorated state, and the need for increased traffic volume capability afforded by a four-lane structure. HDR was the structural engineer. The preferred bridge alternate was a 459-foot- long (140-m), three-span, welded steel plate girder design. The new bridge was designed to meet current standards in accordance with MDT bridge design and AASHTO LRFD criteria. The new bridge was constructed in phases to keep traffic open during construction.

In the screening process, used to prepare particles for many applications, shape is known to have a significant effect. The degree of flakiness of construction aggregates has been considered particularly important. The objective of this research was to find values of operating parameters that optimize the separation of flaky from non-flaky particles by slotted screens. Thus an investigation was conducted to experimentally determine the influence of the particle size and thickness on the screening process. The Response surface method was employed for modelling and analysis of the data. Experiments employing a central composite design were performed on a pilot-scale, rectangular-aperture, vibrating screen to examine the effects of the screening variables the mass recovery of flaky and non-flaky particles to the oversize product. The SAS statistics package was used to fit a second-order model to each response and to analyze the fitted surface to determine the optimum levels. The optimization associated with a dual response system was then considered. The results of this study should provide a basis for optimizing screening conditions to obtain maximum recovery of non-flaky particles to the screen oversize while minimizing the recovery of flaky particles, which should be of interest to the aggregate industries.

Real output expanded at a 3.0% rate worldwide in 1987, a slightly lower pace than the 3.2% rate in 1986. The decline was accounted for largely by slower growth in the U.S.S.R. and the East European countries, as well as in the developing countries (table 1). According to the International Monetary Fund, output of industrial countries output grew 3.1% and rose most strongly in Japan, the United Kingdom, and Canada (table 2). U.S. economic growth remained at 2.9%, the same as in 1986. Consumer and government spending declined in industrial countries, while gross fixed investment: rent increased. This increase in fixed investment expenditures could be attributed partly to lagged results of some 1986 developments: the fall in oil prices, declining interest rates, and exchange rate changes. The U.S. budget deficit, as well as the large external deficits and surpluses among major industrial countries, hindered further growth .2 A report from the Organization for Economic Cooperation and Development (OECD) indicates that the impact of the October stock market crash was short-lived because it hit economies that were growing strongly, anyway.3 The growth of output in the developing countries slowed in 1987; however, output declined in the fuel-exporting developing countries. These countries have been adversely affected by weakness in oil prices and the depreciation of the U.S. dollar. The weighted average of inflation rates in industrial countries--as measured by the [ ] GNP deflator, was 2.9% in 1987, down from 3.4% in 1986. Prices rose 3.0% in the United States, up from 2.6% the previous year. Inflation for 1988 is expected to remain slightly higher in the United States than in Japan and the Federal Republic of Germany because of recent exchange rates changes and the elimination of slack productive capacity and labor in the U.S. economy. Cyanide is used by the mining and chemical industries in tremendous quantities without attention being given to its hazardous properties. The use of cyanide compounds in mining is one of the most important environmental issues for the mining industry due to the acutely toxic properties of many cyanide compounds to humans and aquatic life. Cyanide tends to react readily with most other chemical elements, producing a wide variety of toxic, cyanide-related compounds and because cyanide is carbon based an organic compound it reacts readily with other carbon-based matter, including living organisms. This research is aimed at investigating a feasible and economical technique for the removal of cyanide from tailing wastewater of Muteh gold mine. In this research, removal from Muteh factory's waste water, was achieved at Tarbiat Modares University's mineral processing laboratory through the oxidation of cyanide by calcium hypochlorite. Cyanide oxidizes and changes to cyanide (CNO-) which is 1000 times less environmentally hazards than cyanide. Cyanide titration was accomplished using Rhodanine solution and silver Nitrate. Cyanide concentration in the wastewater, was 270 mg/L. Cyanide removal was achieved at an optimum pH of 12.3. However, at higher temperatures, cyanide removal was complete at this pH level. Optimum Dosage for complete removal of cyanide using calcium hypochlorite was 1.43g/L.

Dolomite is an essential raw material for the refractory industry, a flux in the iron and steel industry and also for the extraction of magnesium metal. Generally, particle size reduction, screening and classification are used for the preparation of the high-grade sized material for different applications, whereas low-grade fine fractions are not utilised. These fines or low-grade overburdens are dumped at the mine site for future utilisation, which creates environmental issues along with inadequate space management. In the present investigation, a detailed study was carried out to decrease the silica content of the low-grade dolomite fines from Gomardih , India, containing 28.6% CaO and 20.1% MgO along with 8.3% silica. Different process flow sheet options were evaluated with a combination of dry classifier, wet classifier, gravity concentrators as well as flotation. Different process variables were assessed along with mineralogical characterisation to optimise the process for efficient rejection of siliceous gangue minerals. Results indicated that dolomite, calcite and quartz are the major mineral phases present in the ore along with other minor minerals. It was concluded that two-stage flotation was an effective process for the reduction of silica up to below 1% after grinding to a particle size below 212 μm. A process flow sheet with a combination of gravitation air classifier and flotation can produce a concentrate with silica content below 4.5%. WASHINGTON CAMP, in Santa Cruz county, Arizona, is a small and little known mining district situated on the lower, eastern slope of the Patagonia mountains, about 20 miles east of Nogales and a like distance south of Patagonia Station on the Sonora railroad, which connects it Benson, 83 miles from Nogales, with the main line of the Southern Pacific. This district is also about 4 miles north of the international boundary, 35 miles west of Bisbee and 40 miles north of Cananea, in Sonora. The topography of the district, which lies at a general elevation of 5,500 ft. above the sea, is only very moderately rugged, and presents no obstacles to mining operations. Although some of the claims of the camp, including the Pride of the West, Belmont and Holland, were extensively worked for silver 25 years ago, they passed, at moderate depths, as in so many other districts, out of the argentiferous lead carbonate and sulphide into zinc, copper and iron sulphides; and for two decades the camp was abandoned and almost forgotten. In recent years, however, its apparent possi¬bilities as a copper producer have led to a revival of mining activity; but not, as yet, with conspicuous success. Although economically relatively unimportant, the deposits of Washington Camp are, mineralogically and structurally, nevertheless, typical examples of ore-bodies developed in connection with the garnet ledge now recognized as a normal feature of granite-limestone contacts; and, in the opinion of the writer, this is an instance where a minor example is yet essentially normal and capable of throwing important light upon the origin and genetic relations of the type.

Mine closure planning integrated into life-of-mine (LOM) planning during the operational phase consists of many subcategories. One of these subcategories that needs to be fully integrated into the LOM planning is that of water management planning. Issues arise when mine water management infrastructure, which is required post-mine closure, is only considered closer to the end of LOM. Once the mine is closed monitoring is focused on water quality as an indicator of closure performance, hence inadequate upfront post-mining water management planning or integration into operations will result in environmental harm. The lack of considering post-mining water management is not uncommon in LOM planning. Poor mine site water management and planning are a wide reaching problem for the mining industry. Numerous active mines have been issued with environmental notices and penalties for contaminated water discharge; including issues with abandoned mines now being cared for by governments all over the world.Mitigating sources of water contamination caused by existing and closed mine sites that are leaching pollutants might be impractical, due to inaccessibility to the contaminants or final landform topography. The opportunity to consider and introduce post-closure required mine water management infrastructure is lost once the waste facilities are constructed. Mitigating contamination is made more problematic with mines located in remote areas, serviced by dedicated roads, water supply, electrical supply, etc. It is expected that these services will be decommissioned or drastically reduced, once closure is reached making remedial work much harder. Inadequate planning, to ensure access and easily maintainable water management infrastructure, could lead to erosion and contamination going unnoticed. There are numerous examples of poor mine water management practices in Africa, Asia, Australia, South and North America. In the United States more than 23 000 km of streams are affected by pollution sourced from some 50 000 abandoned or mature mine sites (DeGraff, 2007).CITATION:Boshoff, H J J, ?2014. The consumption of aggregates in civil construction has increased considerably in Brazil for the last years. The extraction of sand and gravel generates significant environmental impacts. As the quarries and sand deposits are getting exhausted near the big cities, the price of aggregates is increasing. Meanwhile, the extraction of quartzite (dimensional stone) in the state of Minas Gerais has been done mostly in small quarries, with very low recovery, generating approximately six million tons of wastes a year, which causes pollution, increases final prices of products, decreases the life time of quarries and ends up in the closure of enterprises. The government agencies and the quarries’ owners understand that only by finding applications to wastes there will be development to the sector. The aim of this investigation was to study the application to the quartzite wastes as aggregate in the civil construction. Samples of quartzite wastes collected in a quarry were characterized by petrographic analyses. Laboratory tests of crushing and grinding were done to analyze the viability of reducing the size of coarse material to a finer one to be used as raw material in industrialized mortars and concrete pre-molded components.

"The Australian mining industries approach to life-of-mine planning has improved considerably in recent decades. It now needs to be matched by, and embedded in, mining governance systems that utilise a comprehensive whole-of-mine-life approach within a jurisdictional, industry and regional regime rather than just focusing on specific impacts in isolation. The need for a more comprehensive approach is supported by the many mining legacies, from historic, recent and some operating mine sites around Australia. There are sites that are leaving enduring environmental, community and public health impacts that are yet to be accurately assessed. While a number of these sites in Australia are estimated to be more than 50 000, this is probably an underestimation, with a lack of data and different state-based approaches complicating attempts to quantify mining legacies as a national issue. Qualitative assessments about the extent and nature of mining legacy impacts on nature and communities across Australia are also required if we are to understand and avoid ongoing and future mining legacies.This paper commences with an exploration of mining legacies as an umbrella term for previously mined, abandoned, orphan, derelict or neglected sites. This is followed by a discussion of the current status of mining legacies as an Australia-wide issue, contrasting the Australian response with overseas examples. Common themes from past workshops are explored recognising that mining legacies are a growing public policy issue and identifying key ingredients for a successful response. Supporting this, and based on national data which re-enforces the need for action, is the changing scale and intensity of mining in Australia that, while lowering costs for mine operators, increases the liability that may eventually fall to the state if mine sites are not rehabilitated effectively. Though a national issue, mining is a state and territory responsibility, so the current approach to mining legacies is then examined state-by-state. Given the widespread application and recent changes to bonds and levies in Western Australia (WA) and the Northern Territory (NT) the merits of both are examined with reference to specific case studies. Despite the current division of responsibility and diversity of approaches, however, mining legacies remain a significant and growing problem with a recognised need and repeated call for cooperation and coordination at a national and international level. The recent spate of catastrophic events at U.S. mines raises the question as to whether or not the mine safety and health system in the U.S. is broken. The prescriptive regulatory regime in the U.S. is not a cure-all for a broken system. Without totally dismissing the contribution of a prescriptive regulatory approach, this paper argues that safety in the U.S. mining industry would be enhanced if the nation adopted a scheme that provides a balance between specification standards and two alternatives: performance and systems-based standards. This would allow effective and flexible means to continuous health and safety performance improvement. Mine operators, regulators and politicians could then go beyond thinking that compliance alone equals strong safety by creating a proactive system that emphasizes continuous improvement. By adopting performance and systems-based standards, exemplary safety and health performers can be recognized, allowing regulators to focus their efforts on those without sound occupational safety systems and who have substandard safety performance. Entrapments are a huge risk in underground mines. This happens when miners (or any other persons who have entered underground) survive the initial event of an accident, become trapped or lost in unknown and life-threatening locations after evacuation, and remain unaccounted for. At this point, every second counts, and emergency response is highly desired for any chances of saving survivors. The survival chances of the missing persons depend on how quickly they can be located. Rescue teams are often deployed to search for the missing persons underground. However, the rescue teams are forced to search in random and presumed areas of the mine. This is due to the unavailability of information pertaining to the whereabouts of the missing persons. Not only the provision of accurate locations, but various other types of information such as post-accident two-way communication can enhance the survival chances of the missing persons. This information can be provided by a fit-for-purpose missing person locator system. A wide variety of missing person locator systems are already commercially available, some are in testing phases and others in prototype and development phases. This includes tracking, tracing, location detectors and post-accident two-way communication systems. These systems have unique functionalities, capabilities and limitations; making it difficult for mines to select the most effective, applicable and suitable system specific to their mine. In this study, the user requirements for a fit-for-purpose missing person locator system were developed. The purpose of the user requirements was to provide a guideline to facilitate the system selection process. The user requirements can also be used as a tool to measure and evaluate the performance of the selected missing person locator system. The user requirements were also used to predict the success of the selected system under different missing person incident scenarios. The development of the user requirements showed a potential for many other benefits for missing person locator systems.

This project took a watershed approach in the evaluation of water-quality impacts from past mining activities and current restoration projects in the Bear Butte Creek basin, a small and well defined drainage in a heavily mineralized area of the northern Black Hills. The basin contains numerous inactive and abandoned mines as well as a currently active heap-leach surface gold mine. Bear Butte Creek normally loses all of its discharge to sinkholes in the Madison Limestone aquifer and thus can affect the water quality of this regionally important water resource. Water-quality data were analyzed, stream surveys were conducted, and two abandoned mine restoration projects were monitored to evaluate the effectiveness of the watershed approach and to identify practical remediation techniques that could be applied to other watersheds. A geographic information system (GIS) was developed to organize information on stream quality, abandoned mine locations, and other geologic and hydrologic data. The watershed approach toward assessing and reclaiming abandoned mines appears to be effective when compared to a point-source approach that might improve water quality at specific sites but not result in significant overall improvement to the watershed because of adjacent sources.

"The Mine Improvement and New Emergency Response Act of 2006 (MINER Act Public Law 109-236) was passed by Congress in response to three major underground coal mine accidents in the United States that claimed the lives of 19 miners. The Act resulted in substantial changes in the underground coal industry relative to the use of technology for mine escape, rescue, disaster response and other areas where the lack of these technologies contributed to the fatalities caused by these tragedies. This year marks the 10th anniversary of the MINER Act, which makes it an opportune time to reflect on the events that led to its passage, the changes that have been made since then, and the work that remains to be done. As discussed below, the last decade has demonstrated tremendous cooperative efforts among mining operators, technology innovators, labor, The National Institute for Occupational Safety and Health (NIOSH), the U.S. Mine Safety and Health Administration (MSHA) and state regulators, resulting in substantial improvement of the technologies used for escape and rescue and noteworthy progress toward meeting many of the goals of the MINER Act. While the MINER Act and this article pertain primarily to coal mining, many of the technologies are equally applicable to any underground mining and have already found some acceptance outside of coal. TECHNOLOGY GAPS TARGETED BY THE MINER ACT The MINER Act addressed several technology challenges: • Problems with donning and switching between self-contained self-rescuers (SCSRs). • Poor visibility in smoke. • No wayfinding assistance for escape from the mine. • Limited breathable air supply. • Inability to locate trapped miners. • Inability to communicate with trapped miners. • Failure of mine seals. The MINER Act had several provisions that prescribed specific safety technologies intended to address these issues. Many of these were directed at post-accident technology requirements — an area in which Congress mandated additional technology investment. Specifically, the Act required mine operators to submit emergency response plans (ERPs) that contained provisions for implementing new or improved post-accident communications, tracking, lifelines, refuge alternatives and breathable air supplies along with training for emergency procedures and local coordination of emergency response. Most of the requirements were to be carried out within three years of the ratification of the MINER Act. In addition to these requirements for mine operators, the Act clarified and expanded NIOSH’s role and authority in enhancing the development of new mine safety technologies and expediting their commercial availability. The MINER Act also required NIOSH to develop research on refuge chambers, mine seals and the use of belt air and fire-retardant belts. Mine seals had to be upgraded and MSHA turned to NIOSH for applied research to establish the new explosion-resistance requirements. Progress toward closing the gaps."

The Mary Arnold Mining Co. in 194.3 owned 292 acre s of mineral land in Christian County, Mo., and was one of two companies with milling facilities in southern-central Missouri. Production from this property between 19.37 and 1941 amounted to 1,718 tons of zinc in the form of zinc concentrate. Because of the urgent war requirements for additional zinc and the existing facilities for treatment of zinc-lead ores at these mines, in the winter of 1943-44, the Bureau of Mines investigated the deposits by churn drilling in an endeavor to outline enough ore to maintain production of zinc from this district. Five holes aggregating 1,043 feet were bored.

Loudoun Water is currently implementing a quarry to reservoir conversion that will improve water supply reliability in periods of drought and flexibility in water supply operations when turbidity in its Potomac River supplies are high. The Milestone Reservoir Project consists of an inter-tie to the existing Potomac River Pipeline; new pipeline and tunnel; and a pumped intake and outlet works. Planned construction involves significant risks due to underground conditions which includes hard, igneous rock and high groundwater at the creek crossing. In addition, the site is constrained by Goose Creek, an adjacent residence, active mining in a quarry next to the new reservoir, public use of adjacent park space, and the Washington and Old Dominion Trail. Working with these constraints will require close coordination with active mining operations and public and private property owners. Safety of personnel involved with construction also represents a concern with quarry high walls over 30 meters in height near the planned construction. In consideration of key projects risks, Loudoun Water determined that an innovative CMAR delivery approach provided the best opportunity to achieve its goals in quality, innovation, front-end cost and schedule certainty, and operational flexibility and reliability. What matters most to Loudoun Water is the value in having a builder participate in the design process where key decisions can be made based on consideration of risks, cost, schedule, quality, and operability and maintainability of the system. While a progressive design-build delivery model may have produced similar outcomes, Loudoun Water had already invested in substantial development of the design for the work, and ultimately, CMAR provided Loudoun Water the opportunity to retain the partnership with the design team while gaining valuable input from a contractor. Among the challenges with CMAR in the Commonwealth of Virginia is a substantial limitation on self-performance corresponding to 10 percent of the construction value of the work. A significant industry outreach program was initiated by Loudoun Water to encourage participation with incentives in contract terms that allowed for the CMAR Contractor to make a reasonable profit. This paper reviews the CMAR procurement process and discusses contract incentives that were adopted to promote the work within the constraints of contract law in Virginia.
