THE production and inspection of the materials of war involves thousands of observations. Logical action is generally based on the interpretation of many observations. The success of such action depends upon the accuracy of the observations and the soundness of the interpretation placed upon them. There are countless instances where the observer 'sees but perceives not'; that is, the significance of a group of observations is lost, due either to faulty interpretation or to no interpretation at all being placed on the results. This condition is being partly corrected in the United Kingdom and the United States (1) by the promotion of certain principles by Ordnance officials. The term Quality Control has been adopted to describe a system of studying and controlling industrial products by methods of handling observations. In these pages an attempt is made to outline in a non-technical manner some of the ideas used in the handling of observations. Of course the authenticity of the observations determines the worth of any decisions based on them. No matter how brilliant is the theory built on unreliable sampling, it is usually of little practical value. n some fields it is almost impossible to obtain observations free from bias. Let us suppose that observers of speed in miles per hour were available for judgment: (a) a bystander, (b) a motorcycle policeman, and (c) a motorist accused of speeding. The bystander might be unbiased but probably would be very inaccurate. The motorist is usually biased on the low side. The policeman is the only qualified observer and, if over-zealous, he might be biased on the high side. Conclusions drawn from large numbers of biased observations naturally would be of little value.

The term Engineer of Record (EoR) is commonly used in North America and South Africa to define the responsible person for the design, construction and operational phases of a tailings storage facility (TSF). Recently, this role has been elevated in prominence within Australia, but what does it really mean? Recent updates to international TSF guidelines suggests appointing an EoR is good practice, but where does the responsibilities of the EoR start and finish. Interestingly, both Samarco and Mount Polley failures had active EoR’s in place. This suggests that nominating an EoR alone, is not the full answer to mitigating the potential for a TSF failure. Appropriate construction and operational practices are key to the success of the design of TSFs, where all too often, operators ignore the operating plans developed by the designer. To be successful, a key consideration of the EoR role, is involvement in the operational phase of the TSF. This requires ‘buy-in’ from the mine managers and a change in the way TSFs are perceived by site. This paper explores how the roles and responsibilities of the EoR can assist in improving governance of TSFs if implemented effectively and what role the client has to ensure the success of the EoR.

South32 (S32), a globally diversified mining and metals company, is listed on securities exchanges around the world (ASX, JSE, LSE), with its primary listing being on the Australian Securities Exchange (ASX). Chapter 5 of the ASX Listing Rules (Australian Securities Exchange (ASX), 2019b) requires a mining entity to provide a summary of governance arrangements and internal controls with respect to its estimation processes, estimates and reporting of Mineral Resources and Ore Reserves annually. As a global company, it is also important to design and implement all governance processes and internal controls which can be used consistently across the organisation. The three lines model (previously known as the ‘three lines of defense’ model) is employed across South32 to manage risk where the first line (Practitioners) designs, implements and executes processes and controls to manage risk; the second line (Stewardship) identifies material gaps and improvement opportunities in the first line by monitoring conformance with the group requirements, legislative and regulatory obligations. Second line also assists with the development and improvement of core fundamentals based on industry good practise. The third line (Group Assurance) provides independent and objective assurance over the Group’s system of risk management and control. The estimation of resources and reserves for most of the operations within South32 is managed by the Planning function. The estimation process is part of the annual Life of Operation Plan (LoOP). The LoOP identifies and assesses the strategic alternatives for the development of an operation and recommends the preferred pathway based on value and risk. The LoOP starts with opportunity framing and the updated resources as an input and ends with the short-term plan and annual budget (the outputs). Ore Reserves estimates are one of the outputs from the LoOP. All the inputs and outputs follow internal standards and procedures which includes peer review and performance reconciliation. Stewardship (Second line) which is a centralised function has standards in place to ensure compliance to internal controls and external regulatory requirements. This includes assessment of the competence of Competent Persons, technical reviews, independent audits and training in the relevant chapters of the ASX Listing Rules, Guidance Notes and Appendices. Group Assurance (Third line) which reports to the Risk and Audit Committee of the Board of Directors ensures the design and operating effectiveness of the processes, standards and procedures associated with the estimation and reporting of resources and reserves. While the level of scrutiny has a different intent for each line in the three lines model, the overall purpose is to achieve a consistent outcome across the organisation

Industrial Engineering in the industrial world is a relatively new field, having been conceived about the turn' of the century by Frederick W. Taylor, who was not satisfied with the hit-or-miss methods of evaluation and estimation. Through crude Industrial Engineering techniques never before used, he completely changed the bricklaying industry so that work accomplished was more than doubled. To help us better understand what we mean by Industrial Engineering, I think a very simple definition Is in order. Industrial Engineering is a scientific approach to problems in cost, production and operations: and is designed to provide a means of increasing producitivity and reducing costs through standards measurement and standardization of the best methods, practices and equipment, There are many complicated definitions, but pure and simply, it all boils down to providing the means for increasing productivity and reducing the cost. Industrial Engineering has been applied in manufacturing since Frederick W. Taylor?s time and in the last 20 years has grown in stature by leaps and bounds. It is a basic function of successful manufacturing companies and is an accepted fool firmly entrenched in manufacturing organizations. You may know of some successful manufacturing companies that do not employ Industrial Engineers as such, but someone in that organization is doing Industrial Engineering work. They have standards and controls whether they call it Industrial Engineering or not. The manufacturing industry has benefited from this tool; other wise, it would have been discontinued long ago.

THE real and permanent advances which take place in any industry are for the most part slow evolutions which frequently develop and grow almost imperceptibly from clay to clay. A meritorious idea may and generally does require long and patient development and commercial application under a variety of conditions and in a number of localities before it emerges as standard and generally accepted practice. There must be not only technical perfection but also economic advantage. Thus a cross-section taken at any particular time of an industry may reveal little that is distinctly new and yet there may have been considerable progress. During the past year there has been little that is new in milling and yet there has been real progress; some problems have been solved and some of what we may term the newer developments have become more firmly, established.

THE high tension belt conveyor is introducing a new and tremendously expanded era of low cost bulk material handling. High tension belt conveyors are generally those installations involving very long centers, high lifts, or drops, in which the belts are stressed up to their maximum tension values, and further, where the belt construction provides tension capacity far beyond what is possible with conventional belt constructions. With these high tension installations, the magnitude of the forces involved demands careful refinement of accepted design practice in order to achieve optimum balance of all factors. No attempt will be made to evaluate the relative merits of belt conveyor haulage with other means of transportation. For present purposes, it is assumed this has already been done in favor of belt conveyor. Neither will any attempt be made to evaluate the various conveyor belt constructions now available or to balance the advantages of various types of mechanical equipment. It is also assumed that the basic haulage information on which the conveyor design is based is accurate and complete. A sustained maximum, uniform load on the belt at all times must be achieved through proper feed control and the use of adequate surge storage to level the peaks and valleys of any varying demand for the material being handled. General Belt Capacity Considerations The belt conveyor capacity tables published by various belting and conveyor equipment manufacturers vary to a considerable degree, and the ratings given are quite conservative. Of necessity, these published ratings are based on the handling of average materials under average conditions. In applying a high tension belt, all possible capacity from the belt must be obtained in order to hold its width to a minimum and thereby limit the initial cost. Two factors are involved, loading to maximum cross section area and traveling at a maximum practical speed. Belt Loading: Proper treatment of the loading of the belt will result in maximum cross section to the load, and published capacity ratings can be exceeded, sometimes by appreciable margins. On the 10-mile conveyor haul used in the construction of Shasta Dam, California, although the rated capacity of the belt line was 1100 tons per hr, at times the system handled peak loads of 1400 tons per hr, almost 25 pct better than the rated capacity. One of the large coal companies has been able to exceed rated capacity by as much as 50 pct. Loading conditions which must be controlled are: 1. Large lumps must be scalped off and rejected or the load must be primary crushed before being placed on the belt. 2. The material weight per cubic foot must be accurate, must be known for all the materials being handled, and must be known for the complete range of conditions of the individual material being handled. Long centers and high lifts magnify small differences into serious proportions. 3. Uniform feeding to the belt is most important. Various types of feeders are available, which can be used to place a constant predetermined volume of material on the belt, or, where an appreciable range of material weight exists, through electrical control actuated by current demand, to place a predetermined uniform tonnage on the belt. One long slope belt in a coal mine in Pennsylvania is being fed at three separate stations with the controls so arranged that whenever the maximum load is going onto the belt from the first station, the other two stations automatically cut out. Whenever the load from the first station drops back, the other two stations again automatically cut in. 4. Careful design of the chutes and skirts is necessary to get the load centered on the belt with a minimum of free margin along each edge. Some free margin at the edge of the belt is necessary to prevent spillage, but if the load can be kept accurately centered, this free margin area can be reduced, and more material can be carried on the belt. What can be accomplished in this respect will vary widely, depending on the nature of the material being hauled. The chute and skirt design must also protect the belt. 5. The design of chutes and skirts should also get the load traveling in the same direction and close to belt speed, so that the load comes to rest on the belt as quickly as possible. The design of the chutes and skirts is worthy of careful study, and after a system is put into operation it should be experimented with to get the best results. Belt Speed: High belt speeds should be used in high tension work. Obviously, high belt speeds enable haulage on a narrower belt, reducing initial cost. INTRODUCTION Tunneling machines were introduced into civil con¬struction in the 1800s, but their practical applications were not realized fully until the 1950s. As of 1977, tunneling machines were being used on most tunneling projects of substantial size. As in civil construction, the mining applications of tunneling machines have included long drives such as tunneling for long ore-body develop¬ment drifts, long haulage tunnels, longwall access drifts, and long crosscut developments. Tunneling machines ranging from 1.75-m (5.7-ft) to more than I1.0-m (36-ft) diam are available from several manufacturers; machines for mining applications generally have ranged between 1.75 and 6.0-m (5.7¬and 20.0-ft) diam. Tunneling machines usually operate at or near horizontal grades and are supported by a rail-haulage backup system. However, declines and inclines have been made using alternative backup equip¬ment. Either circular or shaped tunnels may be bored, depending upon the particular equipment used. Using tunneling machines to bore openings through rock offers several advantages over the use of the drilling and blasting technique. These advantages include a rapid advance, continuous operation, uniformity of the opening size, smooth walls, and safe working conditions. Compared to drilling and blasting, boring reduces the disturbance of adjacent rock, thereby reducing the ground-support requirements and the exposure to water inflows. The smooth walls provide natural arching and present minimum resistance to the flow of air. A tunneling machine should be considered a spe¬cialized mining tool that is well-suited to certain mining applications. Properly applied and sustained, a tunnel¬ing machine contributes to realizing substantial econo¬mies in the development of underground mines. All costs herein are expressed in 1977 US dollars, unless otherwise noted. The mention of trade names and manufacturers is not intended to imply an endorse¬ment of any specific equipment. TYPES OF TUNNELING MACHINES As listed in Table 1, five principal types of tunneling machines were being manufactured in 1977. Each type of machine offers a special solution to a specific mining problem. Although each of these types of machines is described herein, most of this chapter is devoted to the full-face rotary rock-tunneling machine that has demon¬strated the greatest production in both civil construction and mining applications. Full-Face Rotary Rock-Tunneling Machines As shown in Fig. 1, a full-face rotary rock-tunneling machine consists of a rotating head that is fitted with rock-cutting tools and is forced into the tunnel face. This head is supported by a bearing on a structural support member that, in turn, is held in place by a hydraulically positioned wall-gripping mechanism. Both torque and thrust are reacted from the cutterhead, through the structural support and gripper mechanism, and to the tunnel wall. The rock cuttings fall to the invert at the tunnel face, and they are removed by means of cutterhead buckets or scoops that transfer the cuttings to a conveyor belt. After advancing the cutter¬head through a preset boring stroke, the tunneling machine is advanced by hydraulically pulling the gripper mechanism in from the tunnel walls, stroking forward, and resetting the gripper to the new forward position on the wall; the machine then is set for the next advance stroke. Full-face machines cut only circular openings. Ma¬chines with diameters as small as 1.8 m (6 ft) and as large as 6.1 m (20 ft) have been used in mining applica¬tions, excavating rock with unconfined compressive strengths in excess of 276 000 kPa (40,000 psi). Although special machines have been designed to turn on a radius of 30 m (100 ft) in both horizontal and vertical directions, the conveyors and backup sup¬port equipment generally limit tunneling machines to turns with a radius of more than 60 m (200 ft). The full-face rotary rock-tunneling machines have provided the fastest and most reliable production of any mining method or machine. These machines can be equipped to place any of a variety of ground-support systems, and they can be provided as "open" machines, as shown in Fig. 1, or as "shielded" machines, as shown in Fig. 2. The open machines have the advantage of allowing the ground to be supported as near to the tunnel face as possible. The shielded machines have been designed principally to allow placing precast con¬crete segments. The shielded machines completely pro¬tect the equipment and personnel at the heading, but they are economically limited to permanent long-life haulageways where full lining is required. Partial-Face Rotary Rock-Tunneling Machines As shown in Fig. 3, the partial-face rotary tunneling machine is equipped with a cutterhead that rotates slowly at about 0.21 rad/s (2.0 rpm). The single-disk cutterhead produces the tunnel section shown in Fig. 4 by means of an oscillating motion. These machines work on the undercutting principle, and they are avail¬able in spring-line dimensional increments of 0.1 m (0.32 ft) from 1.5 to 2.1 m (5.0 to 7.0 ft); the vertical dimensions are 2.4 to 3.2 m (8.0 to 10.5 ft), respec¬tively. Depending upon the tunnel width, the cutterhead disk may be fitted with from 8 to 16 drag-bit cutters. Penetration is achieved by means of hydraulic rams that force the tungsten-carbide bits into the rock mass. The operation is cyclic, with cutting occurring only on the upstroke. On the downstroke, the rotating cutter¬head draws the fallen muck from the invert into the panzer-type conveyor for rearward transfer. This feature allows time to either reposition ore cars or to exit the tunnel with muck trains while the machine is cutting, as shown in Fig. 5. Although it is impossible to give definite final figures at this time on Nation-wide accident occurrence in 1933 , enough data are at hand to indicate that the progress in safety in the mining industry, which first became apparent in 1930 and was accentuated in 1931 and 1932 , was continued and much improved in 1933. This improvement was of considerable magnitude in mining both bituminous coal and anthracite but was not so great in other branches of the mining industry . While the volume of work done in all phases of mining increased in 1933 over that in 1932 , the exact amount of the increase is not yet known . Usually increased activity in mining is accompanied by even greater increase in the rate of occurrence of accidents to the workers , but happily it appears that this " rule " was not operating in 1933 --at least in the coal -mining industry .

Approach A cab which raises and lowers by pivoting about d horizontal axis was Installed on two S&S cleanup tractors and tested in two underground working mines. How It Works Hydraulic cylinders actuated by a lever inside the cab cause the cab to pivot about a horizontal axis. This action raises or lowers the cab height. The total cab excursion is 14 inches. A release valve in the hydraulic system allows the cab to lower automatically if struck by a roof load or overhead obstruction. The pivoting action of the cab is engineered so that the operator maintains comfortable use of all controls at all cab heights. The movement is such that the angular relationship of the operator's ankles With the tram and brake foot controls IS consistently within 90 to 100 degrees. Figure 1 outlines the approximate position of the operator at three cab elevations.

IN OFFERING a few comments and suggestions on trends in mining practice, and the methods and tools of tomorrow's mining, perhaps it will be appropriate to start with the subject of education. Charles Kettering, one of the best-known scientists and experimentalists in the automotive industry once said, in effect, that tech- 111ca1 schools have to be carefid not to teach a lot of hard and fast rules that may have been true once, but aren't so any more, or won't be presently. Possibly the mining school of the future will concern itself less with what has happened and more with what is going to happen. It will be built about or around problems. Such a school will be judged, not by the number of its students but by the quality of its improvements. To such a school the industry will send for men to solve particular problems. The industry will doubtless at some time sup- port such a school financially though a lot of hard work will be required to organize miners to undertake such financing, primarily because the mining industry, other than that concerned with iron and steel, is too loosely knit. Each company tends to its own knitting too closely for its own good. Wouldn't it be splendid if the mining industry would finance a "problem school" as suggested by Mr. Kettering and others.

Many mining companies aim to integrate sustainability principles into their new projects. There is an intention for improvement, but frequently this results in little more than 'business as usual' projects that retrospectively categorise controls for environmental and social risks as sustainability initiatives as part of a public relations exercise. The successful implementation of sustainability requires a change of mind-set and behaviours at an organisational and personal level. In addition, sustainability incorporates a very wide range of interests and technical disciplines. One of the key challenges is in understanding how and when to pull these diverse threads together within a project or organisational process. This paper will explore a number of themes: What does it mean to implement a project in accordance with sustainability principles? How far beyond 'business as usual' is it reasonable to go, and what is the business case for this? Why do the intentions for sustainability sometimes fail and how can we prevent this happening in future? People are instrumental in delivering sustainability; how do we embed sustainability thinking into their behaviours and decision-making? The approaches that have been taken to integrating sustainability in design; what has and has not worked and some of the barriers to integrating sustainability into design? The authors will discuss these themes drawing on case studies from URS' experience in integrating sustainability into large projects. The authors will discuss the need for: a strategic approach which provides a simple framework for tactical decision-making and the consequences of taking tactical decisions in the absence of this framework, alignment of the project team and promotion of a culture of innovation, a shift from a risk/compliance mentality to an opportunity identification mindset, and a systematic approach to integrating sustainability into project design processes.

As the 2010 president of the Society for Mining, Metallurgy, and Exploration, Inc., I am pleased to present the 2010 issue of Transactions of the Society for Mining, Metallurgy, and Exploration, Vol. 328. A significant portion of the contribution that we as a professional society make is the publication of professional and technical papers that accelerate the advancement of scientific knowledge and enable transfer of technology. Communication within the mining community is the key to a successful society. Through books, magazines, meetings and electronic media, we accomplish the technology exchange that is fundamental to the existence of the Society. This technology exchange continues a tradition that dates back to the 1871 meeting of 22 mining engineers in Wilkes-Barre, Pennsylvania, that resulted in the founding of AIME; those individuals envisioned that these transaction volumes “would form a most valuable and greatly needed addition to our professional literature.” This volume certainly fulfills that pledge. The 2010 Transactions is the first Transactions to take advantage of new publication technologies. It appears solely as an e-book. The decision to go to an all-digital format for this longstanding publication was a difficult one, but one which will certainly better serve the mining community. I am excited about this development and the opportunities it presents. The 2010 Transactions also contain some of the most exciting research from the past year, including new developments in mineral processing, safety and exploration. Be sure not to miss the two Minerals & Metallurgical Processing issues on the subject of gold hydrometallurgy. This 2010 Transactions comprise of a total of 78 technical papers (582 pages including index), including all 21 papers (136 pages) that were published in 2010 in the Technical Papers Section of Mining Engineering magazine (Vol. 62) and all 34 papers (231 pages) that were published in 2010 in Minerals & Metallurgical Processing (Vol. 27). Addition, this volume contains 23 papers (194 pages) that are being published for the first time in this issue. The papers presented in this volume were written by some of the most distinguished professionals in the industry worldwide. All 78 technical papers were peer-reviewed. The process of review and publication requires much hard work by authors, reviewers and our publications staff. As the 2010 president of SME and on behalf of all our members, I would like to extend our gratitude to all whose volunteer efforts contributed to the publication of this volume.

As Minister of Mines and Technical Surveys in the federal government and as Member of Parliament for Edmonton West, I have been asked to share the honour of extending greetings and a cordial welcome to all.It is a happy coincidence that this representative national gathering, indicative of the growing strength and significance of a great basic Canadian industry, is taking place in the Macdonald Hotel, the new addition of which marks another important step forward in the progress of this City and district.This happens to be the third national convention of the C.I.M. that I •have had the pleasure of attending and I trust that I am not presumptuous in expressing the hope that I will have the opportunity of attending many more conventions of the Institute in my present capacity.Dr. Kidd has bestowed on me a high honour by asking me to be Co-Chairman of this Inaugural Session of the Convention and thus to share with him the responsibility of presiding over the proceedings of this afternoon."

The importance of mine ventilation to health and safety in both coal and metalliferous mines has been long recognised. The primary and secondary ventilation systems in most metalliferous mines incur 50 per cent or more of the total mine electrical power consumption and frequently about 10 per cent of total underground mine operating costs. The requirements for ventilation-related horizontal and vertical development, fans and cooling have a significant impact on mine capital costs and also on mine schedules and start-up times and hence cash flows. There are increasing pressures on regulators and miners to reduce airborne contaminants such as diesel particulatematter, nitrogen dioxide and heat (wet bulb temperatures), and significant pressures to reducegreenhouse gases such as fugitive methane emissions from coalmines and the carbon dioxide footprint of both coal and metalliferous mines. Many previously Australian miners are now globaloperators and have extensive overseas mining operations where local ventilation standards and expertise may not be consistent with Australian practices. The role of the site ventilation officer is also changing with strict new requirements for the statutory position being introduced into Queensland metalliferous mines and the potential for this to become more widespread. Recent regulator and parliamentary review committees have identified systematic shortcomings in mine ventilation compliance, and statutory certificates in both Queensland and NSW now require continuing professional development to maintain currency. Public and investor support for mining are also increasingly contingent on safe, healthy and environmentally responsible practices and the standards for these are frequently tightening. Good ventilation design and operating practices that are consistently well executed therefore contribute significantly not only to a safe and healthy mine but also to a productive and cost-effective operation and the ongoing social licence to operate. There are a variety of ways of providing technical design and oversight into a mine’s ventilation systems; this paper explores some current trends in ventilation standards and statutory requirements, as well as ways to support and develop mine ventilation staff for both domestic and international operations and also the emerging senior technical roles of Group/Principal/Corporate mine ventilation engineer.

The International Tunnel and Underground Space Association (ITA) has a number of working groups (WG) that meet at least once a year. One of these is WG12, ?Use of Shotcrete?, of which the author is a member. The purpose of this paper is to draw attention to a number of topics that were discussed at the WG12 meetings held in April and September 2008; and also to a number of other matters that arose from the presentation of thirty-four papers at the 5th International Symposium on Sprayed Concrete held in Lillehammer, Norway between 21 and 24 April 2008.

Bureau of Mines research has addressed improved operator protection for thin-seam shuttle cars. Because of working height restrictions in thin-seam mines (1.22 m or 48 in), MSHA regulations require that-canopy protection on shuttle cars be provided only in mines 1.07 m (42 in) and greater; however, variances are given to mines 1.22 m (48 in) and below. This paper reviews past Bureau efforts to develop protective structures for thin-seam shuttle car operators. COPPER operations in the Congo mean the operations of the Union Miniere du Haut Katanga, because there are no other copper industries there. There is a mine at Bwana M'Kubwa, a little way to the South, in Rhodesia, which has been operated on a small scale, and-another farther south, the Falcon mine, but in quite a different section of the country. This Bwana M'Kubwa mine has recently been reorganized financially and the Minerals Separation interests have taken an extensive share in it, so that it may come into operation. The Union Miniere's operations are in Katanga, the most southeastern province of the Belgian Congo, and the company's concession is in the southeastern part of that province. The usual route for going into the country is by way of Cape Town, thence over the South African Railways through Kimberly and Mafe- king to Bulawayo, then by the Beira, Mashonland and Rhodesia Railways to the Rhodesia-Congo border, passing Victoria Falls on the way, and finally by the Belgian road, the Chemin de Fer du Katanga, to Elisa- bethville, the African headquarters of the company. From Cape Town to the border it is about 2150 mi. and from the border to Elisabethville about 158 mi., making a little over 2300 mi. from Cape Town, by rail.
