The objectives of this study were to examine a representative cross section of formal safety analysis techniques developed for the nuclear and aerospace industries, to recommend those methods that would be suitable for application to the mining industry, and to include those methods in a user's manual. Volume XI of this report, the user's manual, describes several methods that were determined to be immediately transferrable to the mining industry. The handbook gives detailed instructions on appropriate application and utilization of the methods. Other techniques will require further development before they can be effectively transferred to the mining industry.

A case study is presented that details the safety journey at a North American project involving two major contracts: a 1000 m shaft recovery and rehabilitation, and the development of a drainage gallery in a large open pit operation. In the early stages of the contracts, there were a number of incidents that created tension between the owner and contractor, which resulted in the contractor being placed on a final warning with almost imminent potential termination. As a result of this, it was agreed to initiate a focused safety recovery plan as a ‘last chance’.Over a period of two years, the engagement and consequent performance of the contractor was turned around, steadily working to set it up for success such that in one of the contract work areas, a record of ‘one year LTI (lost time injury) free’ was achieved. The journey involved the owner and the contractor teams working cohesively to educate all parties in the safety requirements and, most importantly, providing the context and understanding for certain requirements being put in place. Early on in the process, it was recognised that the owner team unknowingly was an impediment in that they did not fully understand the safety requirements and so was not able to lead the way for the contractor effectively. The journey to rectify the situation was supported and followed up by focused engagement with the project team, which generated a progressive improvement in a very challenging environment.In particular, the contractor turnaround was significant and of enough value for the contractor to ultimately export some of the safety systems from this site to other contract sites. Arsenopyrite is often associated with sulphides like pyrite, chalcopyrite and galena in refrac-tory gold ores. Selective flotation of arsenopyrite from associated sulphides is desirable for effective gold extraction when the gold is preferentially associated with the arsenopyrite matrix. By contrast, presence of arsenopyrite in flotation tailings poses a serious environmental threat due to arsenic dissolution. To prevent arsenic contamination from the mine wastes, it would be more economical and environmentally beneficial to remove this mineral during the froth flotation step. Use of Acidithiobacillus thiooxidans in the detoxification of arsenic containing sulphides is outlined in this pa-per. Biomodulation using Acidithiobacillus thiooxidans resulted in the preferential depression of pyrite from arsenopyrite. By suitable conditioning with cells and collector effective separation of arsenopyrite from pyrite was achieved. Biomodulation with wild cells did not yield separation of arsenopyrite from chalcopyrite. However, bacterial affinity could be induced to a desired sulfide mineral through environmentally controlled adaptation techniques. Through the use of arsenic-adapted bacterial cells, selective removal of arsenopyrite from chalcopyrite was readily achieved.

Virtually all industries use sulphur in some form. About half of the world's production is used in the production of fertilizers. Sulphur is produced in one form or another in over sixty countries in the world. The major producing countries, in order of importance, are: U.S.A., Canada, U.S.S.R., Poland, Japan, France, Mexico, and Spain. Canada is the largest exporter of sulphur presently supplying about 30% of the world elemental sulphur export trade. It has held this position since 1968. World sulphur resources are virtually unlimited. Future world competition will come from Iraq and other Middle East countries as well as Central and South America.

Attractive mining workplaces are good, safe and healthy environments. They are also workplaces that the workforce of the future wants to work in. As skilled miners and engineers are already scarce, this is important both for today and for the future. However, the future of mining will also be subject to increased international competition. In attempting to become more competitive, many mining companies turn to rationalisation, looking to both rationalise their production and organisation as well as optimise their processes. Work organisation is important in both rationalisation and providing attractive mining work. As a result, this paper investigates a ‘lean production’ (LP) philosophy for the organisation of future mines. The aim of the paper is to investigate to what extent LP can be used to organise for attractive mining. It first defines what attractive mining work is and to what degree it can be influenced by work organisation. A short insight into the LP concept will be given as well as a review of how it is discussed and utilised in mining today. High blast temperature is one of the important technical characteristics of modern blast furnace (BF), which is also an important technical approach for the green development of ironmaking. Increase blast temperature can improve and promote the BF operation smooth and stable, reduce coke rate, fuel consumption and CO2 emission. Top combustion hot blast stove technology has been applied in Shougang Jingtang’s 5500 m3 BF for 8 years. Under the condition of burning single BF gas, high efficiency energy conversion and over 1250 °C high blast temperature have achieved. The combustion and heat transformation process were researched by numerical simulation technology to optimize the structure design. The high efficiency annular ceramic burner and checker brick were developed and applied, the flue gas waste heat was recovered and reused to preheat the combustion air and gas. Prominent achievement of high blast temperature under the condition single BF gas burning has been realized.

Currently, it is very difficult to obtain qualitative and quantitative information about the status and production process of a construction site. As a rule, companies depend on reports from on-site experts. There is no end-to-end digitalization due to the mobility of equipment and ever-changing locations. Equipment from different manufacturers, especially in special foundation engineering, is highly specialized. Any data collected by this equipment is recorded in proprietary formats and made available on the manufacturers' platforms. This exacerbates the situation, making simple analysis of performance indicators impossible. Most functions such as construction diaries, equipment utilization analyses, etc. are implemented only as standalone solutions. This does not allow for prompt visualization and intervention in critical process sequences because the construction site process is usually managed using a cost-based method. Furthermore, special foundation engineering requires a high level of expertise in an extremely wide range of construction processes. In this article, the results from a set of non-proprietary performance analysis tools are presented using numerous practical examples. Once a construction element is completed, the equipment data is immediately transferred to the platform using the latest cloud technology. Because the data is normalized, construction progress can be documented immediately, even for mixed equipment fleets. Bottlenecks can be immediately detected by visualizing the current performance of the production process, which is broken down into individual steps. Modern analyses of this equipment data improve quality, thereby providing better information about current performance. These sets of tools are essential for changing from the current monetary or cost-based method of process management to time- and bottleneck-oriented management. This makes it much easier to identify the critical path during projects. The article shows that processes can be documented and managed.

Bow-tie analysis is a commonly used tool to identify and display risks that have the potential for an undesired hazardous event. The output of the analysis are bow-tie diagrams, which are an effective and versatile communication format that can be applied to any type of risk. Contrary to their simplicity, a proper bow-tie diagram that maintains its usefulness and clear communication is difficult to build. Over two decades assisting Major Hazard Facilities with bow-tie analysis for Safety Cases, several common mistakes have been identified. This paper aims to highlight those issues with examples and provide improvement strategies. A sample of the most common mistakes are: - overly complicated risk analysis - poorly scoped and defined terms - poor descriptions and justifications - identifying too many dependent controls. Overly complicated risk analysis leads to wasted time in obtaining data for little gain. The bow-tie diagram is only a model. Effort should be redirected into identifying and managing controls that will make a tangible difference to site risk. Broad scopes lead to enormous and confusing bow-tie diagrams for both the user and the workshop team. Defining the scope of the study, the consequence severity of interest and critical control characteristics creates focused workshops and targeted analysis. With more definitive scope, there may be more bow-tie diagrams, however, they are succinct and specific. Well described causes are imperative for clear communication to users, regulators and future bowtie review workshop groups. It is best to use common wording structures and explain the event stepby- step. Also, clearly document any likelihood calculations and use data references where applicable. Identifying all possible controls creates a false sense of security that there are many layers of protection. In reality, most controls are dependent on a single system or person and could all be compromised from a single failure. A definition of a ‘bow-tie’ control is required whereby it must be independent from other controls. Improving the bow-tie analysis process will assist operators in developing meaningful and ‘operational’ bow-tie diagrams to support risk management systems and practices.

"The Jaguar concentrator, 60 km north of Leonora, Western Australia (WA), consists of a standard closed circuit semi-autogenous grinding (SAG) and ball mill producing a float feed of approximately 60 micron at 70 t/h. Following comminution, separate copper and zinc concentrates are produced by differential flotation. These concentrates are subsequently dewatered by thickening and then filtration.Separate audits of the Jaguar Operation’s copper and zinc concentrate thickeners were conducted to evaluate their performance, following occasions where site personnel had identified unnecessary loss of copper and zinc concentrate to the thickener overflow. The feedwells were identified as the source of the observed problems due to the following issues with their design/duty:lack of retention time in the feedwell as current design is completely opennot conducive to effective mixing required for efficient flocculationdiameter of the zinc concentrate thickener feedwell was undersize for its duty.To improve the performance of the thickeners, it is planned to replace both concentrate thickener feedwells with a high rate design for their respective duties in February 2016. The audits also identified that the feed system requires modifications with particular importance placed on minimising air entrainment of slurry into the thickener feedwell. Minimisation of air entrainment is hoped to be achieved by providing constant head pressure through installation of weir arrangements in the feedboxes and redirecting each feed pipe so that it feeds tangentially into the feedwell. In addition, improvements to the control philosophy of both thickeners will be made with the installation of an ultrasonic bed level probe and bed mass sensor, in order to make real-time adjustments to the underflow pump speed to maintain steady state operation.Following the feedwell replacement and modifications to both thickeners it is expected that losses of recovered copper and zinc will decrease to undetectable levels. Nomographs and examples are presented to assist in the determination of proper barrier pillar sizing. Additionally, performance evaluation techniques and criteria are included to assist in determining the effectiveness of selected barrier pillar configurations.

Rail access to Ontario International Airport (ONT) has long been a goal of San Bernardino County Transportation Authority (SBCTA) to increase public transit options to ONT and provide incoming ONT passengers improved access to San Bernardino County and the greater Southern California region. This paper describes the planning for the Design-Build-Operate-Maintain (DBOM) delivery of an emerging technology tunnel connection between the Rancho Cucamonga Metrolink station and ONT. The paper discusses key technical considerations, challenges, and opportunities; conceptual configuration development; and construction planning and establishment of the footprint for the underground connection.

The Waihi Gold Mine is situated in a high rainfall area (annual average of over 2 m) on the North Island of New Zealand. Water treatment and discharge at the mine has been ongoing for the last 25 years. The first water treatment plant consisted of a æstandardÆ water treatment facility (ferric chloride precipitation) and cyanide detoxification. With time the project has expanded and the volumes of water increased requiring additional treatment methods (reverse osmosis) to meet the water quality for discharge. To compete in Today's Market, Mine Managers have two, often conflicting tasks. They are to improve productivity, allowing the mines to compete in our world markets and to improve the environment in which our miners must work. To Maintain and improve the environment to ever increasing acceptable levels requires monitoring. In years passed this required considerable labor, which was provided by the Ventilation Department. They continuously took manual samples using hand-held instruments, sampling gas level such as CO for fire and methane for explosive risks. In addition they took continuous air velocity readings with hand-held instruments which allow them to calculate the ventilation air movement in each area of the Mine. This manual gathering of information was not only expensive and slow but marginal at best. It was marginal because the gas sampling was sporadic rather than continuous, as sample reading taken in one part of the mine would likely not be repeated for another 8 hours. If problems did occur, they likely went unobserved until they reached crisis proportion.

That the governor is hereby authorized to appoint a commission of seven persons, to be known as the industrial accidents commission-two of whom shall be employers of labor, two of whom shall be employees in either mines or industrial establishments of this Commonwealth, or duly accredited representatives thereof, two of whom shall be learned in the law, and one of whom shall be a person skilled and experienced in making investigations-to inquire into the causes and results of industrial accidents in the mines, mills, factories, stores, and upon the railroads, street railways, ships, wharves, and in all industrial establishments, and in all other places where men, women, and children are employed in manual labor in this Commonwealth; to study the most advanced methods for safe-guarding against these accidents; to inquire into the subject of fair compensation for those who are injured in these accidents, and for the families of those who shall be killed as a result thereof; and to study the operation and effect of the Workmen's Compensation Law of 1913. SEC. 2. The chairman of said co~mission shall be designated by the governor, and the person named on said commission as a skilled and experienced investigator shall be the secretary of the commission, The commission shall have power to employ such legal counsel and other officers and employees as it may deem necessary to properly perform its duties. SEC. 3. The secretary of said commission shall receive an annual salary of $2,400 and his actual necessary expenses; and the other members of the commission shall receive no compensation for their services, but shall be allowed their actual traveling and other necessary expenses. The salaries of any other persons employed by the commission shall be fixed by it.

Civil engineering education relies on traditional methods of teaching, comprising mostly of deductive instruction by presenting the theory, followed by applications. This paper presents an alternative way for teaching foundation design to senior students, introducing a more inductive approach. The students are presented with a project/game adapted to a real-world problem at the beginning of the course. They are asked to identify the correct steps and methods throughout the lectures, for reaching an appropriate solution that they document and present at the end of the course. To make the project more enjoyable, the project has the format of a game where the students compete with each other on completing the task faster and more efficiently. The goal of this teaching approach is to enhance the ability of the students for problem solving and the engineering way of thinking, while playing a game. In the end, the students were requested to respond to a survey regarding the efficiency of this project/game on their learning. The results showed that students prefer more inductive methods for a design course, feeling that this project made them think more like engineers, while the ‘game’ twist of it made the whole course more fun. INTRODUCTION Most civil engineering courses are taught in 2019 the same way they used to be taught 40 years ago in the 80s. Classic methods are mostly deductive, where the instructor presents the theory first and then applications of that theory follow. Little or no attention is given initially to why the theory was developed or what are the problems it solves. The applications of the theory, and thus the purpose it serves, come only in the end and after methodologies have been presented, leading the students to show low interest in the material. As Albanese and Mitchell (1993) have reported, people are mostly motivated to learn things when they clearly see the need to know them. This explains the students’ little motivation that, if any, comes mainly from the fact that the material may be useful either in other courses or in their future careers. On the other hand, there are alternative inductive methods of teaching, where the role of the students is more active. A lot of researchers have highlighted the benefits of inductive methods in teaching engineering courses. Based on Prince and Fedler (2006), inductive methods include; a) inquiry-learning, b) discovery learning, c) problem and project-based learning (PBL), d) case-based teaching, and e) just-in-time learning. Inquiry-learning is maybe the most traditional method of inductive teaching/learning methods, where the instructor asks questions or poses problems and the students need to apply the theory they have learnt in order to find the solution (Bateman 1990; Prince 2004; Lee 2012). Similar to inquire-learning is the discovery learning, where students have to find the solution to a problem or answer to a question, while the instructor simply provides feedback to the students without guiding their efforts (Leonard 1988; Westbrook and Rogers 1994).

Water is one of the most important ressources of our planet. Unlike many other natural resources, water cannot be substituted by anything and must therefore be handled with the utmost care. Depending on the region and the possible shortage of water, the importance of water increases even further. Water consumption is not the only factor. In the construction industry, water pollution from auxiliary and operating materials and processes as well as other contaminants and their resulting load on wastewater play an important role. The sources for wastewater on tunneling construction sites are various: Washing water, seepage water accumulating in construction pits, tunnel water, water used in the slurry circuit in fluid-assisted tunneling methods or water from concrete work and stone cutting all these waters are contaminated with a range of pollutants like solids, hydrocarbons, heavy metals, etc. These contaminants can be removed or treated/reduced with the help of specifically designed wastewater treatment plants (WTPs). After the treatment the cleaned water can be either, in best case, reused on the construction site itself or discharged into water utilities like the available sewer or river pin respecting their discharge requirements. Due to dwindling water resources globally, the continuously rising discharge limits and the ever more important aspect of sustainability the use of WTPs on tunneling construction site will be indispensable. In this paper, it will be presented how these systems are designed and work, what advantages they offer environmentally as well as financially and what to look out for when planning and implementing such a system onto jobsite.

A multiple use storage facility building is being developed in the Philippines on a peninsula that was reclaimed in the 1990s without any soil treatment. The subsoil now consists of loose to medium dense silty sand underlain by a soft to medium stiff clay. A liquefaction assessment by analytical methods have shown that the loose to medium dense silty sand yields a high potential for liquefaction and lateral spreading during pthe design level seismic event. To mitigate the risk of liquefaction and lateral spreading, as well as assure the slope stability of the shoreline, a solution utilizing a combination of ground improvement techniques by means of a Mixed-In-Place (MIPTM) Lattice Grid Wall and Stone Columns was developed and successfully executed. For the detailed seismic design, a Finite Element 3D analysis was undertaken to simulate static and dynamic actions on the buttress wall. The liquefaction assessment and settlement calculation were carried out based on analytical design approaches. For the execution of the Lattice Grid Wall, the Mixed-In-Place (MIPTM) method developed by BAUER Spezialtiefbau GmbH was applied. The stone columns were installed using the Wet Top Feed method.

Northern ecosystems are dominated by a harsh physical environment which dictates a need for biological adaptation. As yet, it is unknown just how these adapted plants and animals will tolerate industrial development. Our work, sponsored by university, government and industry, is part of an effort to evaluate this developmental impact. Our present studies and the results to date will be discussed. Other problems to which the environmental student may address himself in the future to ensure against environ-mental degradation will be suggested.

The Atlanta Plane Train project at Hartsfield-Jackson Atlanta International Airport employed the progressive design-build approach. This provided opportunities for collaboration between the design-builder, design engineer, and owner and the implementation of innovative design and construction approaches. The innovations have reduced risks, allowed for adjustments during design to accommodate new challenges, and provided open-book cost and schedule evaluations. The progressive design-build approach has also allowed for pricing of work to occur in phases—capturing the design/construction challenges as the project evolves and when they are more clearly defined. While this has allowed for pricing to be performed with better project understanding and therefore less contingency, the associated time frame for approvals has required careful consideration to avoid impacts to the schedule. This paper discusses the benefits and challenges of the progressive design-build approach.

The mines of the future can be envisioned as complex, large-scale technological systems being operated remotely from few centrally located (major cities) operating centres and skeletal crew stationed at the site. These systems will require remotely located human operators to constantly adapt to new and unforeseen system and environmental demands. In addition, the remote operators will need to match system properties to the changing demands and operational conditions, ie must be able to handle the ‘non-design’ emergencies, because the system designers would not have been able to foresee all possible scenarios of failures and provide automatic safety devices for every contingency. Therefore, it is highly important that the operators’ job, which involves problem solving and decision making at the workstation level, is facilitated by the proper interface designs. This paper argues that decision support systems and thus human-system interfaces for the mines of the future should be developed based on the principles of human-system integration that take into account human information processing and cognitive abilities. The information presented to the operator must support the operator in taking appropriate action through a human-system interface designed according to scientific principles of User Centred Design. A case study is presented where an iterative design process was used by CSIRO researchers as an evaluation method involving users (control room operators) at several stages of the user interface design in order to correct any design inadequacies as they emerged. This process allowed the designers to identify any usability issues before the user interface was fully implemented, which subsequently enabled the control room operators to easily adapt to the new interface and carry out all designated tasks in a timely and effective manner.
