Computer-related accidents have caused injuries and fatalities in mining as well as other industries. Normal accident theory (NAT) explains that some accidents are inevitable because of system complexity. NAT is a classic argument in organizational sociology although it has been criticized as having imprecise definitions and lacking criteria for quantifying complexity. These limitations are addressed by a unique approach that recasts this organizational theory into an engineering-based methodology to quantify NAT complexities of computer-based systems. In this approach complexity is categorized as external or internal. External complexity is defined by the external behavior of a system, and is quantified by these dependent variables: system predictability, observability, and usability. Dependent variable data contain the perceptions of 32 subjects running simulations of a system. The system?s internal complexity is characterized by modeling system-level requirements with the software cost reduction (SCR) formal method. Model attributes are quantified using 15 graph-theoretical metrics?the independent variables. Five of 15 metrics are correlated with the dependent variables as evidenced by structure correlations exceeding 0.25, with standard errors <0.10 and a 95% confidence interval. The results also show that the system predictability, observability, and usability decreased as NAT complexities increased. This research takes a step forward in operationalizing NAT for computerized systems. The research benefits mining and other industries as well.

Glass recycling in the Netherlands &apos;has grown fr.om 10.000 to 300.000 tonnes per annum. The various advantages and problems of the glass cycle with reference to the state of the art in the Netherlands is given. Special attention is given to new technologies for the automated sorting of cullet with detection systems. In Western Europe the recycling of glass has, become a success story. Because of this, the percentage of ,glass cullet used in ,glass furnaces has increased. To meet the quality demands of the glass industry, automated sorting for the removal of stones, non-ferrous metals and other impurities ,had to be developed and incorporated in glass recycling plants. ;In Holland, Germany and other countries, the amount of glass collected has reached a level that colour-sorting becomes necessary to avoid market saturation with mixed cullet. Recently, two systems for colour-sorting have been developed and tested for the separation of bottles and culletin the size .range of 20 -50 mm. With the increased capacity of the new glass recycling plants, 120,000 -200,000 tpy, the quality systems have also to be improved and automated. These quality control systems are based on tile automated sorting technology developed earlier. for the glass recycling plants. The data obtained are automatically processed and printed. The sampling system and its relation to the theory of Gy will be described. Results of both developments in glass, recycling plants will be described.

The application of the QEM*SEM image acquisition and analysis system to the measurement of the mineral matter in coal and the materials in fly ash is described. A new sample mounting technique was developed using iodinated epoxy to provide a backscattered electron contrast between the coal, the mounting medium and the mineral matter. Several comprehensive and detailed species identification programs for silicate minerals and materials were developed to analyse the 1000 count x-ray spectra used for the measurements. Case studies are presented to demonstrate the capabilities of the new technique to distinguish different coals and fly ashes by their mineral and material composition rather than by conventional chemical analysis. Applications of the new technique are aimed at demonstrating that composition and size distributions obtained from OEM*SEM measurements can be used: 1. to characterize coal with respect to combustion problems in power stations, 2. to characterize coal for fine coal cleaning, 3. to characterize fly ashes in relation to problems such as erosion in combustion systems and the ease of precipitability in dust collection facilities, 4. to refine the classification systems used in marketing coal for pulverized fuel firing and fly ashes as a component of cement and building materials.

The U.S. Bureau of Mines conducted research to develop an accurate, real-time, position monitoring and warning system for the vehicles used in surface mining. The product of this research will be technology to reduce accidents and injuries associated with the operation of surface mining haulage equipment. The position monitoring system should reduce accidents related to vehicle position and also increase the efficiency of haulage operations. This research was conducted in preparation for development of an accurate, real-time position monitoring and warning system, which notifies equipment operators when they deviate from a known safe course and are approaching a fixed hazard. A radar positioning system designed for marine applications was evaluated and a series of tests was run to determine the accuracy of the radar positioning system when used in a land vehicle. The radar position determination was compared to surveyed values. Both static and dynamic (moving vehicle) tests were conducted. The static test results were marginal and the dynamic test results were not accurate enough for the position monitoring and warning system. Although a promising technology, the system tested needs to be modified to meet the accuracy requirements of mobile mine equipment.

Ferrochrome (FeCr) is a vital ingredient in stainless steel production and is commonly produced by smelting chromite ores in submerged arc furnaces. Silicon (Si) is a componrnt of the FeCr alloy from the smelting process. Being both a contaminant and an indicator of the state of the process, its content needs to be kept within a narrow range. The complex chemistry of the smelting process and interactions between various factors make Si prediction by fundamental models infeasible. A data-driven approach offers an alternative by formulating the model based on historical data. This paper presents a systematic development of a data-driven model for predicting Si content. The model includes dimensionality reduction, regularized linear regression, and a boosting method to reduce the variability of the linear model residuals. It shows a good performance on testing data (R2 = 0.63). The most significant predictors, as determined by linear model analysis and permutation testing, are previous Si content, carbon and titanium in the alloy, calcium oxide in the slag, resistance between electrodes, and electrode slips. Further analysis using thermodynamic data and models, links these predictors to electrode control and slag chemistry. This analysis lays the foundation for implementing Si content control on a ferrochrome smelter.

The accurate estimation of peak particle velocity (PPV) is crucial during the design of bench blasting operations in open pit mines, since the vibrations caused by blasting can significantly affect the integrity of nearby buildings and other structures. Conventional models used to predict blast-induced vibrations are not capable of capturing nonlinear relationships between the different blasting-related parameters. Soft computing techniques, i.e., techniques that are founded on the principles of artificial intelligence, effectively model these complexities. In this paper, we use the random forest (RF) algorithm to develop a model to predict blast-induced ground vibrations from bench blasting using 48 data records. The model was trained and tested using WEKA datamining software. To build this model, a feature selection process using several combinations of Attribute Evaluators and Search Methods under the WEKA Select Attributes tab was performed. The correlation coefficient of the actual data and RF model-predicted data was 0.95, and the weighted average of the relative absolute error (RAE) was 10.9%. The RF model performance was also compared to the equivalent-path-based (EPB) equation on the testing data-set, and it was seen that the RF model can effectively be used to predict PPV. The study also demonstrates that the EPB equation is a suitable empirical method for predicting PPV.

It has been almost a decade since the release of the International Council of Mining and Metals (ICMM) Health and Safety Critical Control Management: Good Practice Guide (2015a). During this time the ICMM member companies, along with many others across industry, have been busy designing, deploying and enhancing their own critical control management (CCM) programs. So, what have we learnt? This paper discusses the scope, application and learnings from the implementation of CCM within the mining industry. What’s going well? What are the common challenges? What are the opportunities to strengthen the approach? What are the threats to successful implementation? The paper draws on the author’s experience as one of the key contributors to the development of the CCM Good Practice Guide and an adviser to many mining companies, including six of the ICMM members. The paper focuses on the practical (operational) application of CCM and the cycle of identifying, defining, implementing, verifying and improving critical controls at the frontline. The paper also includes a discussion on the safety performance of the ICMM member companies over the past decade and provides a recommendation for how CCM can be incorporated into a more proactive model to measure safety performance. The paper concludes with practical recommendations that mining companies, regardless of their CCM maturity, can consider as they look ahead. They are supplemented by three additional recommendations for consideration at the industry level.

Health and safety indicators help mine sites predict the likelihood of an event, advance initiatives to control risks, and track progress. Although useful to encourage individuals within the mining companies to work together to identify such indicators, executing risk assessments comes with challenges. Specifically, varying or inaccurate perceptions of risk, in addition to trust and buy-in of a risk management system, contribute to inconsistent levels of participation in risk programs. This paper focuses on one trona mine’s experience in the development and implementation of a field-level risk assessment program to help its organization understand and manage risk to an acceptable level. Through a transformational process of ongoing leadership development, support and communication, Solvay Green River fostered a culture grounded in risk assessment, safety interactions and hazard correction. The application of consistent risk assessment tools was critical to create a participatory workforce that not only talks about safety but actively identifies factors that contribute to hazards and potential incidents. In this paper, reflecting on the mine’s previous process of risk-assessment implementation provides examples of likely barriers that sites may encounter when trying to document and manage risks, as well as a variety of mini case examples that showcase how the organization worked through these barriers to facilitate the identification of leading indicators to ultimately reduce incidents.

The state-of-the-art in cave seismic monitoring is reviewed, with the latest developments in active seismic monitoring included. Seismic monitoring systems are routinely used by most of the world’s block and panel caving mines to track the cave initiation and progression. The current best practice is to use the cloud of seismic event locations as a proxy for cave growth somewhere below. Problems with event location reliability due to seismic ray bending around the (unknown) cave are side-stepped by using sensors installed above the cave. However, the aseismic gap – the (unknown) distance between actual cave back and seismic event cloud – currently hinders reliable estimation of cave geometry.Velocity inversions using the passive seismic data are inexpensive and should be undertaken more routinely. The straight-ray methods should be used with caution, as the underlying assumptions are invalid in caving environments. The double-difference method is more correct for the caving situation, and there have been some promising results published. Constrained geometric inversions make a lot of sense, but have yet to be demonstrated successfully.Two active seismic source have recently been developed for use in the caving environment: a piezoelectric source for ranges up to a few hundred metres and a pneumatically-powered source for ranges up to a kilometre. These sources produce signals on standard mine seismic sensors that can be used to measure seismic velocity variations down to 0.01 per cent resolution. This allows the cave progression to be monitored by measuring tiny changes to seismic velocities as the rock above the cave is fractured and destressed.CITATION:Lynch, R A, 2016. Flame safety lamps have been used in gaseous mines since the Davy lamp was introduced, more than 100 years ago. During the last decade the flame safety lamp has been rapidly replaced by the electric lamp as a device for illuminating the miner's working place; in the United States the electric cap lamp has been widely adopted. For example, in 1911 approximately 45,000 flame lamps and no electric lamps were being used in the Pennsylvania bituminous mines, which were then producing 35 per cent of all the soft coal mined in this country; by 1918 the flame lamps had decreased to 17,000, whereas electric lamps-mostly of one maketotaled nearly 48,000. In Great Britain during the same years the total of 723,934 flame lamps decreased to 590,185 and the electric lamps gained rapidly from 4,298 to 156,521. No later reliable figures are at hand. However, before the flame lamp becomes obsolete, it is fitting that the Bureau of Mines compile a permanent record of a device which has served the mining industry for more than three generations, and not only made possible the development of entire districts that otherwise could not have been worked but undoubtedly saved the lives of thousands of underground workers. Flame safety lamps are capable of doing more than the name indicates. First, under normal conditions, they can be used in a mine atmosphere that contains an explosive mixture of gas and air without great danger of exploding such a mixture, though such use of a flame lamp should not be permitted. Second, to an experienced man they indicate percentages of gas much lower than the explosive limit, and thus can be used to indicate to workers the approach of an unsafe condition in the mine atmosphere. Third, flame lamps will not burn in an atmosphere greatly deficient in oxygen, and they therefore warn the users of such deficiency in time to withdraw to a place of safety. Flame safety lamps are still used rather widely in the United States for general illumination and to a still greater extent for detecting gas. Many coal-producing States require fire bosses to use these lamps in gaseous mines, and the regulations of several States specify that a gIven number of such lamps be kept available for emergency service.

An inducer-type pump, capable of injecting dry coal into a hydraulic transport pipeline has been developed and installed in a specially designed and fabricated slurry injector vehicle. The full scale prototype injector system can deliver up to 11 ton/min of coal into an 8-in pipeline t pressures up to 95 lb/in2. The Helical Inducer Injector operates on centrifugal principles with flow passing axially through helical pitched conical rotor. Water is delivered to the pump through an annular slot at the atmospheric pressure coal inlet. The spinning rotor maintains an air-core vortex of water which pressurizes the discharge housing. A screw feeds coal through the open inlet into this vortex. The inner radius of the vortex contracts as pressure is increased, giving a limiting maximum pressure which is proportional to the square of the rotor speed. Below this limit, flow rate, discharge pressure, und rotor speed re independent variables. The self-powered, 4 -in high slurry injector vehicle follows and receives ROM coal from a continuous miner, reduces the top size to 3-in, and delivers the sized coal to the injector feed screw for slurry formation and pumps it through a mobile hose attached to the vehicle&apos;s outby end.

Two major grounding topics are covered: mine ground systems and ground-check monitors. Performance characteristics of ground-check monitors for high-voltage surface mine distribution cables and low-voltage underground mine trailing cables are evaluated. Over 20 commercially available monitors are studied to determine their utility. Several monitoring concepts are incorporated into an unusual monitor design, the prototype of which is described in detail. A comparative study of the effectiveness of several alternate methods of grounding surface mine equipment supports the system currently proposed by MSHA. An instrument design to measure earth voltage gradients is described, as well as an instrument design for measuring the effectiveness of dc haulage current return systems. An extreme analysis is made of techniques for measuring earth resistivity and resistance of safety ground beds. Electrolytic corrosion studies of earth electrodes are reported extensively also.

The United States is rapidly depleting many of its most important minerals and fuels. The first two Annual Reports by the Secretary of the Interior, under the Mining and Minerals Policy Act of 1970, made us acutely aware that the natural resources that enabled this Nation to attain an unprecedented growth in terms of economic well-being, and to set an example of independent strength through the democratic process, are finite. Once they are depleted, large imports will be required. Simultaneously, competition for available supplies, especially of petroleum, is increasing in the expanding European and Japanese industrial communities and in the underdeveloped nations of the world. Even before the crisis of late 1973, the oil-producing nations of the world, principally in the Middle East, but including Canada and Latin America, were recognizing that the worldwide need for crude oil, petroleum products, and natural gas could be translated into an unprecedented economic bargaining position. Thus, the members of the Organization of Petroleum Exporting Countries (OPEC) raised the price of their crude twice during the first 6 months of 1973. By yearend the energy situation had be-come acute, and it seemed probable that regardless of current political developments, the era of cheap energy was over.

This report establishes a methodology for the evaluation of gas detection devices and is the result of a critical investigation into the basic principles defining gas detector performance. A scheme is presented to evaluate the performance of a test sample of gas detectors based on the detector type and intended application. Test procedures, worksheets, test apparatus, glossary and statistical methods for handling test data arc given. In addition, evaluation standards and test parameters are identified. It is intended that this report serve as a comprehensive framework for the eventual certification of all gas detectors for mine applications. A plan is proposed and recommended for the quantification and implementation of evaluation standards for gas detection device certification.

The Federal Bureau of Mines investigated combustion of lignite in domes-tic heating equipment to obtain information on equipment performance and to evaluate testing procedures. Three types of domestic heating equipment were tested, ranging from a hand-tilled, magazine-type space heater to a ram-type overfeed stoker. Lignite was readily burned in each of the three units under various test conditions. Burnoff rates determined by adding weighed charges to the magazine were found to be in error because of shrinkage of the lignite when only a portion of the magazine capacity was consumed during a test. More reliable burnoff rates could be determined by carbon-nitrogen balances. A hand-fired magazine-type space heater (Booker heater) was operated without the furnace attachment at efficiencies to 75 pct. One test series was run with minimum supervision under conditions encountered in actual household operation. No mechanical trouble was encountered.

Zirconium has possible use in industry where severe corrosion conditions are encountered because it resists attack from a variety of corrosive environments. However, very little experimental testing has been done to evaluate the corrosion behavior of zirconium when simultaneously subjected to high tensile stresses and exposed to corrosive environments. This investigation was initiated for an evaluation test, but was limited to environments consisting of ferric chloride solutions which were known to attack zirconium and were shown to promote stress-corrosion cracking in preliminary tests. Tests for cracking susceptibility were conducted by the Bureau of Mines to show the effect of stress, solution concentration, air aeration of solution, and the method of mechanical processing. Tensile stress was applied to the specimens by a constant-load method which proved more reliable than a constant-strain method.

This is the first of a series of reports on the relation between microscopic composition and chemical and physical properties of banded bituminous coals. It describes the relation between microscopic composition and behavior during thermal decomposition of a heterogeneous, high-volatile A bituminous coal from the Chilton bed in West Virginia. The standard free-swelling test was used as a simple means of studying the effect of heat on the coal. Although swelling indexes are not precise indicators of coal behavior in the coke oven and have limited value in caking investigations, they are reliable for predicting expansion of coal in a fuel bed and have practical value in the study of fuels. The swelling test also supplements other useful laboratory tests for determining the plastic and expansion properties of coal.

Premier Mine is a large underground diamond mine, situated at Cullinan in the Gauteng Province of South Africa. The mine hoists over 3 millions tons of diamond bearing kimberlite per year, from a modern, mechanized panel retreat block cave mining operation. Troughs sited at the base of the cave are developed into the overlying caved ore, allowing loading access to the ore through draw points. Trough opening is therefore a vital aspect of the mining operation. Most of the production is dependent on fast, reliable and safe trough opening. Successful trough openings are achieved under difficult mining conditions due to restricted void, long up-holes and the inherent properties of kimberlite?soft, elastic rock with a high swell factor. Early in 1998, Premier Mine undertook a joint development project (with an electronic initiation system supplier) to determine whether the use of a fully programmable electronic detonator system would enhance the mining cycle in the trough opening process. Blasting commenced in February 1998, initially with blasts of a similar size and duration to those that had been previously blasted with shock-tube systems, to confirm the operational capabilities of the blasting system. The accuracy, reliability and flexibility of the electronic delay detonators (EDDs) minimized associated risks for re-engineering of the trough opening process. Further, as blast timing was optimized, larger blasts were successfully initiated. To date over 20 troughs have been opened successfully with the use of EDDs. Not only did it take less time to open troughs, but also drilling and blasting operations have been optimized, as well as an overall improvement in safety. This paper details the benefits that EDDs have delivered to Premier Mine in this area.
