FOR the past seven years, all flotation at the Britannia mill has been done in Forrester Air-Lift cells. Prior to 1931, the roughing and part of the cleaning operation had been performed in four 21-cell and two 14-cell M.S. machines. The resulting economies in reagent, power, maintenance, and general operating costs, and the increased recovery, more than justified the exclusive use of the Forrester machine. About three years ago, the possibility of still further improving the efficiency of the flotation section was the subject of considerable discussion and experiment. Arising from these experiments, tests were made of the practicability of using a cell of greater depth than that of any pneumatic or mechanically-agitated machine in common use. Cells were constructed with depths of 7 .5 feet, 10 feet, and 15 feet, and, in all cases, a very definite improvement in operation was indicated. The cell now being used in the mill treats from 5,500 to 6,200 tons of dry feed per day. It is 100 ft. long by 5 ft. wide by 8 ft. deep, and has replaced a total length of over 500 feet of standard Forrester machine. The choice of a depth of 8 feet was governed by the type of blower which was available-in this case, a Roots-Connersville blower, driven by a synchronous motor and delivering 6,000 c.f.m. of free air at 4.0 pounds per square inch. At the Chelan Division of the Howe Sound Company, where a new mill was recently built, all flotation is carried on in cells 10 feet deep, with good results.

The San Miguel Mine is owned by San Miguel Electrical Cooperative, Inc. and operated by Atascosa Mining Company, a wholly-owned subsidiary of Morrison-Knudsen Company, Inc. This multiseam, single-zone lignite surface mine is located near Jourdanton, Texas, 110 kilometers (68 miles) south of San Antonio. Lignite extracted from four closely-spaced seams is used to fuel the 400-MW San Miguel Power Plant. Recovery depths range from 7 to 37 meters (25 to 120 feet). The mine began stripping operations in the spring of 1980 and now produces approximately 2.5 million tonnes (2.8 million tons) of lignite annually. Atascosa Mining Company is responsible for production, maintenance, engineering, and management services. Production responsibilities include topsoil removal, overburden removal, lignite mining, spoil pile regrading and topsoil replacement, as well as ash disposal and construction and maintenance of water retention and diversion facilities.

Equilibrium relations in the system FeO-Fe2O3 Cr2O3 have been determined at 1300°C at oxygen pressures ranging from that of air (0.21 atm) to 1.5 x 10-11 atm. The following oxide phases have stable equilibrium existence under these conditions : a sesquioxide solid solution with corundum-type structure (approximate composition Fe2O3-Cr2O3); a ternary solid solution with spinel-type structure (approximate composition FeO Fe2O3-FeO Cr2O3) and a ternary wüstite solid solution with periclase-type structure and compositions approaching FeO. The metal phase occurring in equilibrium with oxide phase(s) at the lowest oxygen pressures used in the present investigation is almost pure iron. The extent of solid-solution areas and the location of oxygen isobars have been determined. ThE system Fe-Cr-O has attracted a great deal of interest among metallurgists as well as ceramists and geochemists. Metallurgists have studied the system because of its importance in deoxidation equilibria, ceramists because of its importance in basic brick technology, and geochemists because of its importance for an understanding of natural chromite deposits. Chen and chipman1 investigated the Cr-O equilibrium in liquid iron at 1595°C in atmospheres of known oxygen pressures (controlled H2O/H2 ratios). The main purpose of their work was to determine the stability range of the iron-chromite phase. Hilty et al.2 studied oxide phases in equilibrium with liquid Fe-Cr alloys at 1550°, 1600°, and 1650°C. They reported the existence of two previously unknown oxide phases, one a distorted spinel with composition intermediate between FeO Cr203 and Cr3O4, the other Cr3O4 with tetragonal structure. They also sketched diagrams showing the inferred liqui-dus surface and the inferred 1600°C isothermal section for the system Fe-Cr-O. Koch et al3 studied oxide inclusions in Fe-Cr alloys and also observed the distorted spinel phase reported by Hilty et al. Richards and white4 as well as Woodhouse and White5 investigated spinel-sesquioxide equilibria in the system Fe-Cr-O in air in the temperature range of 1420" to 1650°C, and Muan and Somiya6 delineated approximate phase relations in the system in air from 1400" to 2050°C. The present study was carried out at a constant temperature of 1300° C and at oxygen pressures ranging from 0.21 atm (air) to 1.5 x 10-11 atm. The chosen temperature is high enough to permit equilibrium to be attained within a reasonable period of time within most composition areas of the system, and still low enough to permit use of experimental methods which give highly accurate and reliable results. These methods are described in detail in the following. I) EXPERIMENTAL METHODS 1) General Procedures. Two different experimental methods were used in the present investigation: quenching and thermogravimetry. In the quenching method, oxide samples were heated at chosen temperature and chosen oxygen pressure until equilibrium was attained among gas and condensed phases. The samples were then quenched rapidly to room temperature and the phases present determined by X-ray and microscopic examination. Total compositions were determined by chemical analysis after quenching. In the thermogravimetric method, pellets of oxide mixtures were suspended by a thin platinum wire from one beam of an analytical balance, and the weight changes were recorded as a function of oxygen pressure at constant temperature. The data thus obtained were used to locate oxygen isobars. The courses of the latter curves reflect changes in phase assemblages and serve to supplement the observations made by the quenching technique. 2) Materials. Analytical-grade Fe2O3 and Cr2O3 were used as starting materials. Each oxide was first heated separately in air at 1000°C for several hours. Mixtures of desired ratios of the two oxides were then prepared. Each mixture was finely ground and mixed, and heated at 1250" to 1300°C in air for 2 hr, ground and mixed again and heated at the same temperature for 5 to 24 hr, depending on the Cr2O3 content of the mixture. A homogeneous sesquioxide solid solution between the two end members resulted from this treatment. "A coal outburst is a severe safety hazard in room-and-pillar mining under deep cover. It is more likely to occur during pillar retreating. Multi-seam mining dramatically increases the risk of coal outburst within the influence zones created by remnant pillars and gob-solid boundaries. Though coal outburst is generally associated with heavy loading of coal pillars, its occurrence is difficult to predict. Risk management provides a proactive tool to minimize coal outburst in room-and-pillar mining under deep cover. Risk assessment is the first step in identifying and quantifying outburst risk factors. The primary risk factors for coal outburst are overburden depth, roof and floor strength, geological anomalies, mining type, multi-seam mining, and panel width. A risk assessment chart can be used to proactively screen out mining sections with high risk of coal outburst for further analysis. Gob-solid boundaries and remnant pillars are critical factors in evaluation of the coal outburst risk of multi-seam mining. Risk identification, risk assessment, geologic influence mapping, geotechnical evaluation, risk analysis, risk mitigation, and monitoring are essential elements of coal outburst risk management process. Training is an integral part of risk management for risk identification and communication between all the stakeholders including management, technical and safety personnel, and miners.INTRODUCTIONA coal outburst is a sudden violent burst of coal from a pillar with broken coal or blocks of coal forcibly ejected into open entries. A coal outburst is a severe safety hazard as the mining crew is highly exposed at the site when the event occurs. Though deep cover and strong roof and floor are underlying geologic conditions of a potential burst incident, its real occurrence is also the result of additional mining factors. In room-and-pillar mining, a coal outburst could occur during both development and pillar retreating, but the latter greatly increases the risk of outburst. The other risk factors of coal outburst also include mining layout, multi-seam mining, presence of adjacent gob, cutting sequence, and local abnormal geologic conditions. Over the years, the cases of coal outbursts have been studied by many researchers and mining practitioners(Peng, 2008; Newman, 2008; Iannacchione and Tadolini, 2008; Gauna and Phillipson, 2008; Hoelle, 2008; and Newman, 2002). It is commonly believed that the coal outburst is the result of a sudden release of elastic strain energy stored in coal pillars and is highly associated with cutting into heavily loaded coal pillars, but its occurrence is a rare event and is difficult to predict. A number of engineering controls have been recommended to mitigate outburst potential. For room-and-pillar mining, sufficient pillar sizes have been the primary control for coal outburst prevention. The pillar design tools such as ARMPS and AMSS developed by NIOSH have played an important role in the design of stable pillars to prevent pillar collapse and squeezing as well as coal outbursts. In fact, with the implementation of pillar design using proper stability factors, pillar collapse and squeezing have been almost eliminated, and the number of coal outbursts has been greatly reduced in the US over the past decade. However, after a few outbursts occurred during pillar retreating in the US over the past a few years (MSHA, 1996; MSHA, 2014; MSHA, 2013), it has been realized that sufficient pillar size is still not enough to prevent coal outbursts. The investigations of the incidents showed that other factors such as multi-seam mining, panel layout, cutting sequence, and local geologic factors also seemed critical in causing the events. Therefore, it has become imperative that additional proactive measures beyond proper pillar design be implemented to prevent coal outbursts."

The procedure suggested in this report is intended for the use of engineers, esters, accountants, etc., in estimating the cost of chemical-type operations. It is intended to over operating costs, not investment costs, as the latter have been covered numerous other publications. However, as capital investment costs are reflected to a high degree in operating costs, it is important to point out that accuracy of investment costs is highly essential. Therefore, all factors that might be included under plant -capital costs should be considered, such as requirements of shops, employees' housing, etc. The procedure is designed primarily for use in types of chemical oper¬ations, such as the production of synthetic liquid fuels, anhydrous ammonia, and others of like or similar nature. Yet, no attempt has been made to cover the entire chemical industry.

INTRODUCTION Traditionally mineral processing equipment has been procured based on (equipment) specifications prepared by the engineers. As newer materials of construction are being used for equipment manufacture and plant control systems become more sophisticated, the need for thorough and detailed specifications is greater than ever. For example, it is not uncommon to find plants looking at grinding circuits consisting of semiautogenous mills with variable speed, gearless, drives of large horsepower. Technical specifications for the mill, wrap- around motor and control system must, therefore, be carefully prepared to ensure that the respective equipment supplied by vendors meets overall grinding needs. Various engineering and design companies have expended considerable time and effort in developing the specification format. Equipment manufacturers have also given significant effort to developing data sheets aimed at collecting information regarding not only the duty of the equipment, but also the conditions under which it will be operated. As engineers moved from one company to another, the specification format became cross-fertilized and further developed. As a result, specifications on file with various engineers have improved considerably over a period of time. Equipment specifications from earlier projects are commonly used as a basis for tailoring new ones. To keep equipment specifications short, general technical considerations are often compiled as separate specifications. For example, general considerations applicable to the design, manufacture and supply of the mechanical equipment are consolidated under "Standard Requirements for Mechanical Equipment” and included along with that specified for purchase. As the economic feasibility of the project has been established during earlier studies, purchase-related activities can be initiated following completion of the specifications. Procurement itself is a major activity, however, in this chapter only those activities directly related to the purchase of equipment will be discussed. "The objective of this work is to validate a methodology for spatially predicting the fracture intensity, defined as the average fracture area per unit volume. First, using the observations from drill holes, the fracture intensity is directly inferred at the support of drill-hole composites based on a Terzaghi correction. Subsequently, the composite values are interpolated to larger blocks. The methodology is tested on sets of simulated fracture networks, in which each fracture consists of a disc with a random diameter, random position and random orientation. For each of these simulated networks, one can calculate the actual fracture intensity within any region of space, based on geometric considerations. A regular sampling mesh is then defined to emulate a set of vertical drill holes and the proposed methodology is applied to predict the fracture intensity, both globally and locally. The actual and predicted fracture intensities are finally compared, showing that predictions are unbiased, insofar as, on average, the prediction error is always close to zero. Also, the error dispersion increases when the support of the prediction decreases, which is consistent with the well-known support effect. In practice, the proposed methodology allows predicting fracture intensity based exclusively on drill-hole information, without the need for simulating fracture networks in space, hence its interest. INTRODUCTION The characterization of fractures in rock masses is an important part in the evaluation of mechanical and hydraulic properties of rocks. Fractures have a large impact in disciplines such as reservoir engineering, safety of underground storage of CO2 or of nuclear waste, assessment of high-enthalpy geothermal installations, safety of mining exploitations, efficiency of in-situ leaching operations or of extraction by block-caving. As a rock mass property, fractures must be characterized in the three-dimensional space, whereas observations are limited to 1D (drill holes) and 2D (outcrops, drift walls), moreover very often to rather short stations, so that the available data are subject to geometric bias, truncation and censoring. Fracture characteristics includes the number of fracture sets and, for each set, the number, orientation, spacing, location, shape and size of the fractures must be inferred from data sampling. Some important parameters that are considered nearly in all models are fracture intensity and fracture size."

A challenge on many industrial flotation circuits is controlling concentrate mineral grades using measurements that are provided infrequently. The mineral grades in concentrate streams are a critical performance indicator in any industrial flotation circuit. Failure to achieve concentrate mineral grade targets has significant financial consequences on a flotation operation. A violation of a gangue mineral grade limits by the concentrator may also result in significant penalties. Online grade analysers which can provide reliable real-time grade measurements are essential for effective grade optimisation. The Blue Cube MQi analyser (by Blue Cube Systems) has recently come to prominence for its ability to rapidly determine mineral concentrate grades online and in-stream. This has provided excellent insight into the actual process dynamics in the flotation circuit, and hence enabled real-time control and optimisation of concentrate mineral grades. MintekÆs Measurement and Control division, specialising in advanced control solutions for mineral and metallurgical industries, has had the opportunity to link the FloatStar Grade-Recovery Optimiser to the measurements provided by the Blue Cube system at a nickel concentrator plant. The project initially focused on stabilising the final concentrate grade without directly accounting for nickel recovery. The site later procured additional Blue Cube MQi analysers, resulting in the expansion of the FloatStar Grade-Recovery Optimiser to incorporate recovery component in the performance. This site already had the FloatStar Grade-Recovery Optimiser for several years, but the original grade measurement device was unreliable and ultimately failed. Washington State has a well developed set of blasting regulations which have been promulgated by the Department of Labor using extensive stakeholder input, not only from citizens but also from manufacturers and representatives of IME, blasters in charge, blasting engineers, blasting companies, local and federal law enforcement agencies, fire officials, and local and state government representatives. WAC 296-52 therefore provides a foundation or platform for developing city ordinances for Washington State cities should a City wish to do so. On the other hand, Oregon’s code is based on OSHA 1926, Subdivision U, Blasting and the Use of Explosives which provides scant direction regarding the modern use of explosives. Reference is also made to ODOT Section 00335 - Blasting Methods and Protection of Excavation Backslopes. This paper examines two separate programs that have been developed by two Cities, one in Washington State and one in Oregon to further regulate blasting inside City limits and discusses why the lack of a regulatory foundation made one approach more difficult than the other. One of the authors was retained by the City of Kennewick, WA to develop their ordinance and has more recently followed and reacted to a pending ordinance for the City of Redmond, Oregon. The paper will compare and contrast the different approaches and comment on the reaction from both Blasters and the public. We also present a case study that involved blasting in an urban area of Oregon to prepare a foundation for a water tank. Controlled Blasting was carried out within 70-ft of residencies and within a community that had expressed reservations regarding blasting even though the work would make a significant contribution to the betterment of the community. Pre-construction surveys of residencies located in a large area surrounding the site were performed and a well-planned blasting program was carried out. Residencies were subject to vibration levels that were monitored using six seismographs that surrounded the site and these units measured vibration levels that were significantly lower than allowed by statute. There were complaints triggered by individuals reactions to blast vibrations which validated commonly held models of human perception but no damage was caused by the Blasting.

"5.1. IntroductionThe sample preparation requirements for bulk analyses of platinum-group elements (PGE) are different from the requirements for ore microscopy and in-situ analyses. It is necessary that the initial sample collection results in as representative a sample as possible for both types of analyses. Ingamells (1980) believes, however, that sampling is always imperfect and that there is no such thing as a truly representative sample. Once collected, the usual low content and irregularly dispersed nature of the PGE require that samples for bulk analyses be made as uniform as possible, whereas in samples for ore microscopy and in-situ analyses the PGE must be concentrated. Because of the generally high specific gravity of platinum-group minerals (PGM), certain precautions must be taken, such as mixing the sample material in the bags prior to opening.5.2. Preparation forBulk Analytical MethodsThe principal factors that determine the size of the sample required for a bulk analysis are grade, heterogeneity and grainsize. Initially, the grade and sample heterogeneity can only be estimated and a sampling methodology is developed with experience. It is essential at all times, however, that the assayers have information about the source of the sample and the purpose for the analysis. Information on the grain-size, which can only be obtained through mineralogical analysis, is also very important for PGE analyses, but, unfortunately, is often ignored.5.2.1. Samples fromUnconsolidated DepositsConcentrations of PGE may occur in eluvial deposits as well as in a large variety of alluvial deposits, including those of fluvial and glaciofluvial origin. These deposits are often difficult to map and sampling is further complicated by the wide range, from sand-size to boulders, of the non-POE-bearing constituents, and by the irregular distribution and shape of the ""pay streaks"". PGE concentrations in unconsolidated deposits are largely composed of resistant Pt-Fe and Os-Ir-Ru alloys (cf. Cabri and Harris, 1975 and Raicevic and Cabri, 1976), thus making comminution to the usual-150 to -200 mesh somewhat unreliable, due to the malleability of these alloys."

This information circular is offered to the coal-mining industry for use as a guide in selecting, placing, and caring for fire-fighting equipment in coal mines. Its purpose is to suggest minimum requirements for fire protection underground, and it is presented with the hope that it will aid in establishing adequate standards that will provide reasonable protection against underground fires in coal mines. Potentially, a coal mine is a hazardous fire area. Coal itself is a combustible substance, easily ignited. In addition, large quantities of other combustible materials must be present in the area of operations; these include materials for roof support, track ties, doors, ventilation stoppings, lubricants, etc.

Several studies of the fluorite/oleate flotation system have shown that under certain conditions calcium dioleate is formed at or transported to the fluorite surface. Adsorption of significant amounts of calcium dioleate is generally observed above oleate concentrations of -6 -5 between 4 x 10 and 4 x 10 M, yet little explanation of the onset of calcium dioleate adsorption is provided. In this study in-situ and ex-situ FTIR/IRS experiments have been performed in connection with thermodynamic solution phase analysis to better understand oleate adsorption as well as the onset of calcium dioleate formation. Results from this study are analyzed and compared to industrial practice.

The study investigates the effectiveness of the canopy air curtain (CAC) in reducing respirable dust exposure for shuttle car and ramcar operators in underground coal mining. Field testing showed that the CAC reduced dust exposure by 11-34% overall and by 57-65% during ramcar loading by the continuous miner. These results demonstrate the CAC’s potential as a significant dust control device.

"It is difficult to quantify comparatively the gassiness of a property. The most common index to gassiness, cubic feet of gas per ton of coal mined, is very arbitrary and does not take into account the geological setting nor the mining operation. The gassiness of a virgin coal in situ can be assessed, but no method is precise and the most accurate methods involve prolonged field and laboratory experiments. In general, gassiness increases with depth. Whereas 'gas' normally implies methane, other gases such as nitrogen, carbon dioxide, etc. may be present in seam gas also. The inflammability of methane is a problem, particularly the explosibility of a methane-air mixture; also oxygen dilution is involved in high gas concentrations and some gases are poisonous even in low concentrations. Cross measuring into a virgin seam is usually not as troublesome as development in the seam, where narrow openings may require strong ventilation to dilute gases exuding from the face and sides. Total extraction presents problems of gas from the working seam, or from adjacent seams, particularly at the return side, or of gas-laden air from the goaf streaming out or sometimes surging out intermittently, again on the return side. In pillar extraction or longwall retreating special roadways may be required to allow the side-to-side and end-to-end flushing of the goaf which is somewhat inherent in longwall advancing. Layering may occur, particularly where ventilation conditions are not brisk. Outbursts of gas from adjoining seams may be induced by sudden rupture of roof or floor in caving workings. Outbursts of coal and gas from the working seam may occur, mostly in development work, under special environmental circumstances. Gas problems may be reduced or eliminated by predrainage of gas ....:. rarely successful from the working seam - usually undertaken from adjoining seams prior to escape of gas released by caving. The ideal solution - general predrainage of gas to the surface from measures prior to mining - has yet to be accomplished. • In the planning of gassy mines it is possible to make an assessment of gas contained in the seams and to estimate what proportion will be released into the mine air, but emission of gas into mine workings is never steady and at present there is no better planning criterion than experience at the mine next door!"

Blast violence has always been a criteria that has been analyzed without a quantitative approach relying on opinions of those viewing the blast and oftentimes does not transfer between open-pit mines, quarries, and construction projects. Added to this confusion is that on many regulate projects, such as U.S. Army Corp of Engineers construction projects, the definition of "flyrock" or violence can vary, and contractors must be able to meet this requirement. This project analyzed hundreds of blasts from various construction projects to develop a new metric to determine blast violence - the Violence Factor.

"Replacing the aging roadway access to the iconic Golden Gate Bridge, thereby ensuring traffic safety and robustness in case of seismic occurrences, provided an opportunity for major design improvements as well as the development of a parkway that would include the construction of four new tunnels. With an eye to the future, the Doyle Drive replacement project aimed to meet the highest standards for environmental stewardship beyond regulatory requirements. Understanding that lighting equipment is the tunnel‘s subsystem that uses the most energy, the operator was looking for a sustainable solution to light the tunnels. An Intelligent Lighting Control System (ILCS), designed specifically for road tunnels, was acquired to individually control the almost 2000 luminaires lighting the four tunnels. This paper will show the results in terms of energy and operational savings generated over the nine months of operation between the commissioning and the conference in April 2016. INTRODUCTION Electrical & mechanical (E&M) equipment represents an average of 10 % of a tunnel total construction cost. Once the civil infrastructure completed, E&M equipment drives the main part of the expense during a tunnel useful life. Table 1 below describes the impact of various types of energy consumption and their cost impact in tunnels of less than 500 m. It is clear that lighting is the sub-system that requires the most energy. A good lighting strategy is the key to reducing energy consumption due to lighting and a viable solution that will bring important savings in tunnel operations.With a price of $0.14/kWh in California and a planned increase between 3% and 6% per year during the period 2013-2040, the tunnel operators could not afford to overlit the tunnels—particularly when they have to operate it for the next 30 years. However, they also need to maintain the right lighting level to ensure driver safety all the time. The Presidio Parkway project includes the Battery tunnels, which are 240 m (790 feet) and 230 m (750 feet) in length, and the Main Post tunnels, which are 280 m (920 feet) and 315 m (1030 feet) in length."

Introduction The sliding-form method of erecting concrete structures was first devised and used in the early part of this century by the Sante Fe Railroad Company for the erection of a grain elevator at Santa Fe, New Mexico. A somewhat related principle has been employed for a number of years in shaft lining work and, to our knowledge, was perfected in Butte, Montana, but until the building of the concrete headframe at the Jeffrey mine of the Canadian Johns-Manville Company, Ltd., at Asbestos, Que., the method, as applied to surface structures, was quite foreign to the mining industry in Canada. The current shortage and cost of structural steel shapes has no doubt contributed more than any other single factor to the investigations that have led to the use of reinforced concrete as a headframe building material. Mining men in western Quebec and northern Ontario have shown a great deal of interest in the design and construction of the headframe at the East Sullivan mine in Bourlamaque township, Que., and we have had many queries regarding the methods used and, in particular, for an explanation of the sliding-form principle. This article is written in an attempt to set forth the conditions which were to be met; to explain the construction methods used, and to discuss the economy of concrete headframes as compared with the conventional steel structures with which the mining fraternity is familiar.

Incident investigation is of utmost importance in most high-risk industries. This study was undertaken to study the content of past investigation reports to determine the effectiveness of the incident investigations. The study uses a semi-quantitative method to assess the effectiveness of incident investigations in the Ghanaian mining industry by evaluating the quality of past investigation reports. The assessment tool consists of 5 elements with several indicators and rating scales for assessing the quality of an investigation report as a measure of the effectiveness of the investigation. The method was applied to 304 investigation reports of 3 Ghanaian large-scale gold mines, and the results correlated with incidence rates of the mines to determine if any relationship existed. The results showed that the mines differ significantly in the quality of their investigation reports, suggesting differences in the effectiveness of their investigations. In addition, the incidence rates of the mines negatively correlated with some elements of the assessment tool. In general, the method was found useful and revealed areas where improvement is needed. INTRODUCTION The importance of learning from incidents is well documented in the literature. For instance, some scholars regard learning from incidents as important for improving safety [1,2]. Thus, many organisations spend considerable resources in identifying lessons from incidents and subsequently on the process of improvement that follows the identification of lessons. It is therefore important for such organisations to determine the effectiveness of their learning from incidents processes, in order to identify specific areas where improvements are needed, so that interventions can be as specific as possible. The learning from incidents process models the plan-act-check-do approach of management systems, often consisting of four major stages, namely; data collection and incident analysis, planning interventions, implementing interventions and follow-ups and evaluation [3,2]. The investigating and analysis of incidents have been regarded as one of the vital stages of the learning process since it is the stage where lessons are identified [3,2,4]. It has also been noted elsewhere in the literature that results of the previous stage of the learning process are important for the next stage [3]. For example, ineffective incident analyses could result in the planning of ineffective remedial measures. One of the primary aims of incident investigations is to identify lessons from previous incidents which when implemented can prevent future recurrences or minimise the effects of future incidents. There is thus the need for organisations to assess the quality of their investigations periodically since it is one of the means they adopt to improve safety and the quality of remedial measures to some extent depends on the effectiveness of the investigations. Although previous studies [5,2] have assessed the effectiveness of the learning process, and some aspects of the learning process, such as incidents reporting, there is less focus on the effectiveness of incident investigations. The research described in this paper was therefore conducted to contribute to this gap, focusing more closely on incident investigation reports.
