Bow-tie analysis is a commonly used tool to identify and display risks that have the potential for an undesired hazardous event. The output of the analysis are bow-tie diagrams, which are an effective and versatile communication format that can be applied to any type of risk. Contrary to their simplicity, a proper bow-tie diagram that maintains its usefulness and clear communication is difficult to build. Over two decades assisting Major Hazard Facilities with bow-tie analysis for Safety Cases, several common mistakes have been identified. This paper aims to highlight those issues with examples and provide improvement strategies. A sample of the most common mistakes are: - overly complicated risk analysis - poorly scoped and defined terms - poor descriptions and justifications - identifying too many dependent controls. Overly complicated risk analysis leads to wasted time in obtaining data for little gain. The bow-tie diagram is only a model. Effort should be redirected into identifying and managing controls that will make a tangible difference to site risk. Broad scopes lead to enormous and confusing bow-tie diagrams for both the user and the workshop team. Defining the scope of the study, the consequence severity of interest and critical control characteristics creates focused workshops and targeted analysis. With more definitive scope, there may be more bow-tie diagrams, however, they are succinct and specific. Well described causes are imperative for clear communication to users, regulators and future bowtie review workshop groups. It is best to use common wording structures and explain the event stepby- step. Also, clearly document any likelihood calculations and use data references where applicable. Identifying all possible controls creates a false sense of security that there are many layers of protection. In reality, most controls are dependent on a single system or person and could all be compromised from a single failure. A definition of a ‘bow-tie’ control is required whereby it must be independent from other controls. Improving the bow-tie analysis process will assist operators in developing meaningful and ‘operational’ bow-tie diagrams to support risk management systems and practices.

The application of the law of supply and demand to the sale of crude oil is generally conceded, but the motives underlying the buyer's (refiner's) demands are not always obvious to the seller (producer). In some cases this obscurity has led to the conclusion that the refiner may be unnecessarily particular when he selects one crude and rejects another. This preference, however, is very largely the reflection of the refiner's product demands and the quality of the stocks obtainable from the selected crude. The purpose of this paper is to point out critical characteristics of the major classes of refined products and to show how crudes vary in their suitability for such products. With relatively few exceptions, available processing methods and addition agents make it theoretically possible to produce any type of refined product from any crude source, although it may not be economi-cally sound to do so. The widespread application of thermal cracking for converting higher boiling stocks into gasoline is well known. More recently cracking and refining in the presence of catalysts9 have been developed, as well as various processes for polymerizing lighter hydro-carbons into gasolines having desirable characteristics.6 For several years, selective solvents have been applied commercially to refining inferior quality kerosene, Diesel fuel and lubricating oil distillates .3 Improved methods for refining lubricating oils from crude residua,4 formerly suitable only for fuel, are now in use and a number of special solvent processes are available for dewaxing lubricating oils to low pour point.5 These improved refining methods have been developed not only to meet the demand for improved product quality but also to enable the refiner to obtain quality products from crude sources that would not yield such products by the older refining methods. In many cases, the utilization of these modern refining tools necessi-tates an appreciable expenditure for equipment as well as added manufac-turing costs, and their application therefore must be justified economically. Despite the availability of such tools, the added processing costs involved and yields obtainable may render their application entirely impractical. The White Pine Copper Division of Copper Range Company is a large copper producer operating an underground mine, mill, and smelter on Michigan's Upper Peninsula. Principal water discharges are from the tailings basin where these discharges are high in dissolved solids because of mine water pumped into the basin. Principal air emissions are from the smelter where SO2 emissions are uncontrolled. Although water discharges and air emissions have not had significant environmental impacts, environmental regulations could require additional control procedures. Because of low copper prices and poor profitability, cost of compliance with possible new environmental regulations could have serious impact on White Pine with no apparent significant beneficial environmental impacts.

In Romania, there are significant lignite and brown coal reserves hosted by different regions of the country. The most important lignite bearing zone of the country situated in the North ? West part of Oltenia region and which covers a surface area of 2,500 km2 and host a total reserve of 3.0 billion tons of lignite, of which more than 95% can be exploited by open cast method. The paper deals with the existing geological and mining conditions of the zone , the distribution by mining basins and exploitation perimeters, the open cast working technologies and the economic outcomes obtained throughout the time. Years ago, for the improvement of the economic situation as a whole and in each unit, a large production reorganization, technology update and modernization process was started up. There are described the rehabilitation measures of the technological lines and modernization of the equipments of the production flow-sheet, measures developed at present, too and which will still continue in future. There are mentioned the equipments considered, the components under modernization and the economic outcomes obtained so far. At the end of the paper there are presented the future actions for the equipment modernization and perspectives of the lignite deposit exploitation to meet the national economy requirements of solid fuels.

Introduction As the Sherritt Gordon property is still in the development stage, . it is rather difficult to write a proper account of the mining methods and milling practice that will be used. While the general scheme of operation will remain the same, there will undoubtedly be many minor changes to meet special conditions which we cannot forsee at the present time. Most of the work accomplished to date has followed the usual routine of preliminary development in the Canadian North. However, we have learned a few things which might be of use to others operating under similar conditions. The most important of these is the saving in power cost made possible by the use of Diesels as against steam power when using wood fuel, and the movement of freight by tractors when rail transportation is not available. Location The property i6 located about 90 miles north of The Pas, or 40 miles northeast of the Flin Flon mine. A branch line from Cranberry Portage, on the?C. N. railway, between Flin Flon and The Pas, connects the property with the outside.

This U.S. Bureau of Mines report describes a system to determine the position and heading of a mining machine during maneuvers in the face area of an operating mine section. The system is the first step in the development of a guidance system for automated mining machines. The position and heading algorithm is described, and a preliminary error analysis is performed. The sensors employed were linear position transducers mounted on a mining machine with their cables attached to points on a stationary reference. Four linear position transducers were used to provide a data redundancy that increased the reliability of the guidance system. The linear position information obtained from the transducers was processed mathematically via the position and heading algorithm to provide the desired position and heading information. The algorithm takes advantage of sensor redundancy to continually test the accuracy of the sensor data. The major sources of error were determined to be the linear position transducers, the analog-to-digital (A/D) converter used to interpret the data, and the sampling frequency of the measurement system. These sources of error were evaluated to determine their inaccuracies for use in calculating the overall system accuracy.

Introduction Asbestos, a mineral known and used in ancient times, has become an article of commerce only during the life of the older asbestos miners in Quebec. The diversified uses of this adaptable substance are now numbered by the hundreds. Wherever insulation or resistance to heat may be required, asbestos, either by itself or mixed with some other material, may be employed to advantage. Regarded as a curiosity for centuries, it is now in such common use that it is familiar to all. The demand for long fibre for use in the manufacture of textiles and brake-lining, and for shorter fibre for making shingles, paper, and insulation materials, serves to support the industry of mining asbestos-bearing rock and milling the fibre from it in Canada, South Africa, Russia, Cyprus, and the United States. The commercial use of asbestos dates only from 1876, in which year large deposits were discovered in the Province of Quebec, Canada. Prior to that year a small amount of long fibre amphibole asbestos was produced in Italy. Occurrence In Canada In the Eastern Townships of Quebec, along the line of the Quebec Central railway, is an area of serpentine rock. It extends westerly into Vermont and easterly to Gaspe. A part of it, the section from Danville to East Broughton, contains areas in which occur deposits of commercial asbestos fibre of the chrysotile variety. Pits have been opened on some of these deposits at Danville, Coleraine, Black Lake, Thetford Mines, Robertson, and East Broughton. Thetford is distant 68 miles from Quebec city, East Broughton lies 25 miles to the northeast of Thetford, while Danville is 45 miles to the southwest of Thetford.

Formulation and commercial utilization of solid-waste incinerator slag by-product is the focus of this paper. Particular attention is given to iron aluminosilicate melts having compositions similar to that of nature's basalt The earth?s crust can be viewed as an environmental laboratory for predicting stability of various silicates; small-scale laboratory experiments are necessary to confirm findings there from. An ideal slag consumes flux additives at acceptable cost, is compatible with furnace refractory, flows readily from the furnace, and has physical properties suitable for its commercial utilization -all within the critical constraint that the slag is environmentally stable. Examples of successful solid waste slagging practice, including tonnage, utilization and pricing, are provided from metallurgical and waste treatment operations of North America. THE application of artificial refrigeration, or air-conditioning, to the ventilation of deep, hot mines has long been a subject of interest to the operators of such properties. Artificial cooling of the air in theaters, railroad coaches, and various industrial plants has been so successful within recent years that it has been natural to assume that the adoption of similar methods would afford a ready solution to the difficult problem of mine ventilation. It should however, be obvious, that although there may be nothing technically impossible in the application of this process to mine ventilation there is a great economic distinction between the maintenance of a high standard of comfort for a large number of people in a relatively small space such as a theater and the cooling of air to improve working conditions for miners scattered throughout the extensive workings of a large mine. Many methods of air-conditioning have been considered by the management of the Anaconda Copper Mining Co. at various times, but until the development of the process to be described in this paper none has seemed economically practicable.

"In mineral resource and ore reserve estimation, several sources of information are generally available, such as diamond drillholes, reversed-circulation drillholes, and blastholes, and originate from one or more sampling campaigns and are analyzed by one or more laboratories. In such a situation, the grade measurements may not have the same precision and one should distinguish between hard and imprecise data.This work aims at studying the impact of data imprecision in resource/reserve evaluation through two case studies on simulated deposits: a porphyry copper deposit conditioned to real exploration and production data, and a gold deposit simulated without any conditioning data. Several geostatistical techniques are used to estimate the grades of selective mining units from exploration samples, including kriging, cokriging, lognormal and indicator kriging. Two types of imprecision are examined— measurements with multiplicative errors and measurements defined as intervals. The results are compared to the simulated grades of the selective mining units from a statistical and economic viewpoint.The study concludes that imprecise measurements should never be discarded because they help to improve the estimates, and that simple methods such as kriging are as efficient as more sophisticated ones for resource/reserve assessment. Finally, a few indications on how to diagnose and model the data imprecision are given."

The diffusion barrier coating system (DBC system) was designed to suppress interdiffusion between alloy substrate and coatings in addition to ensuring slow oxide growth at high temperatures. In this report the concept of the DBC system will be introduced, and the results obtained for various heat-resistant alloys are presented. In this DBC system the s-phase of the Re-Cr-Ni system was selected as the barrier and is formed by electroplating Re-Ni and Ni films from aqueous solutions, followed by Cr and/or Al pack cementations. A typical DBC system consists of Ni-base superalloy / triple s-Re based alloy layer / ?’-Ni3Al and/or ß-NiAl (with or without Pt addition) / a-Al2O3. It was found that the DBC system maintained its structure for long time exposures because the s phase has tie-lines with the ? and ?’ alloy substrate and the ?’ and/or ß coating. The s-Re based alloy can suppress both the inward Al diffusion and the outward diffusion of alloying elements. The DBC system was successfully applied to various heat-resistant alloys such as Ni-Mo and Fe-Cr-Al based alloys, and these alloys with the DBC system were creep-deformed slowly under external applied stresses. A simultaneous diffusion coating process for Al and Hf was developed using Hf+NH4Cl+Al2O3 pack cementation. To obtain reliable and long-life coatings the combination of alloy substrate, coating, and external scale should be considered as a total system, to optimize oxidation resistance, structural stability, and mechanical properties.

"Petro-Canada's project investigative and experimental underground work in the Galt coal seam at Kipp, Alberta included the construction of an emergency egress and ventilation raise from surface.The Galt coal seam is 206 m below the surface and covered by 8 m of shale, sandstone and clay beds of the Oldman Formation, 128 m of shale, bentonite beds and sandstone of the Bearpaw Formation, and 70 m of glacial till and basal gravel. Free water was encountered from sand lenses in the glacial till and the basal gravel.Excavation for the ventilation raise was a nominal 2 m diameter and it was lined with a welded, continuous, steel liner of 1.2 m 1.D.Excavation in the glacial till was carried out with a surface bucket drill rig combined with hand mining. Ground support in the glacial till consisted of liner plates and backfill grout. The Bearpaw and Oldman formations were excavated by employing standard raise boring techniques. The individual liner sections of the continuous steel liner were assembled on surface and the entire liner column was lowered into the raise by employing hydraulic lift climbers. The liner column was back-filled with concrete.IntroductionThe Lethbridge Coal Field comprises an area of approximately 1 165 square km in the vicinity of Lethbridge in southern Alberta. Pacific Petroleums Ltd. proposed in a Preliminary Disclosure in January 1979 ""to open an experimental mine prior to development of a producing underground mine with an initial rate of 540,000 tonnes per year"". A 2 900-hectare group of leases in the Kipp-Coalhurst area was selected as the primary target for the Kipp experimental mine. The scope of work for the experimental mine included the construction of an access shaft with production capabilities, the construction of a ventilation and emergency exit raise, drive-ages in coal to yield a bulk sample, gathering of geotechnical information and the testing of experimental mining methods."

The Barrick Bullfrog underground mine was established as an afterthought to the main open pit. As open pit ore dwindles, the underground mining becomes increasingly more important as an ore source, and to the overall economics of the project. This situation is similar to many other mines, and there is a strong move towards underground mining in the United States today. There are inherent problems in starting and operating underground mines in the shadow of any open pit operation, and at the Bullfrog there were several attempts at productive increases that were both successful and unsuccessful.

Kazakhstan’s first commercial copper-leach solvent extraction-electrowinning plant, Central Asia Metals’ Kounrad project, was commissioned in April 2012. Exploiting low grade waste stockpiles produced during Soviet era open pit mining, a 10,000 tpa copper cathode plant was designed, constructed and commissioned within 18 months. Overcoming initial challenges with a workforce unfamiliar with the technology and with extensive periods of sub-zero conditions during winter the project has consistently met targets and produced high quality copper cathodes at cash costs of less than US$1/lb. Challenges encountered during the first two years of operation are described and future plans briefly discussed.

The development of metallurgical processes may be divided into periods : First , the discovery of a basic reaction which brings about the production of the desired product from the available raw material ; second , a long stage of mechanical development during which little or no attention is paid to the reaction itself ; and third , a careful study of the reaction . Most metallurgical processes are to -day approaching the end of the second period . This we may illustrate by the steel process . The reaction of carbon with iron ore to produce pig iron and the subsequent refinement of the pig iron by an oxidation reaction is very old . Development in modern times has been almost entirely mechanical , the development of the blast furnace , furnace , the Bessemer process , the Siemens process , and even the Aston process for wrought iron being mechanical improvements almost entirely . Metallurgy is now entering a phase where the reactions are being considered . The question of how far can they be made to proceed is being repeatedly asked . As mechanical processes are improved , we approach a limit to which our basic reactions can be pushed which is determined by the nature of the reactions themselves . We want to know : How far can iron be decarbonized by iron oxide ? How low can the oxygen of copper be reduced by any given reaction? How efficiently can the fuel in a zinc reduction process be used ? The demand for purer metals and lower production costs has forced these and similar questions upon us . How are we to answer them ? The determination of these equilibria or end points of metallurgical reactions by direct means is fraught with difficulty because here we must overcome mechanical difficulties which too often become the limiting factors even in the laboratory and we do nottherefore get the actual limit to which the reaction itself may be carried . Further , there are so many reactions of metallurgical materials that to survey each separately is a job to give pause to any experimenter or group of experimenters .

Placing a detonating explosive inside a volume of rock is an efficient method of rock fragmentation. The explosion generates internal tensile stresses in the rock, and because rock is a brittle material, this brings about crack propagation and fragmentation far more efficiently than does application of explosive on the surface. To get the explosive inside, we drill a pattern of boreholes and charge the explosive into the holes. This is really the one and only purpose of making blastholes in mining: to give access for the explosive to the interior of the rock. No matter how big and expensive a drilling jumbo we stand up in front of a rock face, and no matter how many boreholes we let it drill, the rock does not fall apart by itself without blasting. It is the action of the explosive in the boreholes that does it. When we consider that we need a pound or two of explosive for every cubic yard of rock to be fragmented, and a pint of borehole volume for each pound of explosive, we see that the process of blasthole charging is an important element in the whole process of rock blasting.

Fire hazards can cause severe and irrecoverable damage to reinforced concrete (RC) tunnel linings, threatening the serviceability and resilience of transportation networks. This paper first examines the existing methodologies that are used to classify damage to tunnel linings from fire. These approaches are then supplemented with the results of advanced modeling and inputs from industry experts to propose new fire damage/ repair classifications with the associated thresholds. The results of the presented research can be used to guide fire risk assessment as well as the performance-based design of tunnel fire protection.

IN 1936, the Department of Mines and Resources was formed by the amalgamation of the former Departments of Mines, Interior, Indian Affairs, and Immigration. In the amalgamation, the Geological Survey and the Topographical Survey became the major divisions of the Bureau of Geology and Topography, which is a part of the Mines and Geology Branch which, in turn, is a part of the Department of Mines and Resources. The Topographical Survey was formed by combining the former Topographical Division of the Geological Survey and the air and ground mapping sections of the former Topographical and Air Survey Bureau of the Department of the Interior. The latter organization, previous to 1933, had been known as the Topographical Surveys Branch of the Department of the Interior. The amalgamation of 1936 definitely placed on the Topographical Survey the responsibility for the original mapping required for the development of our natural resources. The war has brought upon us the situation where all work has had to be in the furtherance of the war effort. With decreased staff and limited appropriations, the mapping on those scales which are directly concerned with the natural resources has been confined to the search for oil and strategic minerals. The reconstruction period will produce a different situation, with different problems. The past is behind us and it is necessary to prepare for the future. First, we should consider briefly a few general principles in the making and using of maps, then take stock of what we have, see what the needs are, and consider how we are to meet these needs.
