This report summarizes research into the root causes of failures in mine power systems which impact safety and reliability. Abnormally low fault currents, due to arcing faults, self-excited systems, and fallen trolley wires are examined and quantified. The reliability of components which are critical for safety is studied, as well as reliability of components which are crucial to the mining pro cess. Failure rates are found through in-situ tests, accelerated life tests, studies of mine maintenance, and fault-tree analysis. Reliability data is used to predict fatalities and lost-time accidents through computer-aided fault-tree analysis. Analytical support tasks reported include the deriving of trailing-cable ampacities under cyclic loading, and the measurement of connected ground beds.

THE telluride of gold and silver, the " Tellurgold-silver" of Hausmann, and " Petzite" of other mineralogists, has been found in so few localities, that a late discovery of the mineral in a new locality may not be uninteresbing. So far this mineral has been found in Europe only in a few mines in Transylvania, and in Hungary; and in the United States in the Stanislaus Mine, in Calaveras County, California, in the Golden Rule Mine, Tuolumne County, California, and in a mine in Montana, the precise locality of which I have forgot, ten. The new locality referred to is at the Red Cloud Mine, Gold Hill District, Boulder County, Colorado.

The purposes of composition are various; one purpose, for instance, is to make a record for the writer's own use, as in a diary. That does not involve responsibility to others. There is also the writing meant to influence opinion-to be persuasive, and pleasing. With such writing we need not concern ourselves at this stage of our study. The prime purpose of technical writing is to be informative-to convey information; therefore it must" be clear beyond the chance of misunderstanding. Such clearness is impossible if meaningless or ill chosen, words are sprinkled through the text. Discard the trivial phrases that are constantly at your elbow. Brush aside a host of vapid superlatives. Metternich exclaimed:' "The superla¬tive is the mark of fools". It is a false emphasis, like the underlining in a school-girl's letter. For example, the little word `very' can be deleted nine times out of ten; it is an impediment to terse and perspicuous writing, as 'the multitudinous hand-baggage of the British tourist is to his travel. 'Very' supposes comparison. A mine with a 1000-ft. shaft is very deep to the scribe who writes from the Joplin district', in Missouri, but it seems a shallow hole to a man living at Calumet, Michigan. A vein that is 10 feet across may be considered very wide at Cripple Creek, Colorado, but it is only a `stringer' to the miner at the Homestake, in South Dakota. Ore assaying $20 in gold is very rich at Juneau, Alaska, where .10,000 tons of $1 ore is crushed daily, but it is relatively low-grade to the pocket-miner at Alleghany, in California. It is all a matter of comparison; unless the reader knows your standards of depth, width, or richness, your `very' has no significance. "Where erosion was very rapid or oxidation very shallow"

By means of further developed machines and techniques and with growing experiences in jet grouting, the challenge of this building method becomes more and more complex. To keep the concerning risks as low as possible, the requirements on the quality assurance continuously in­crease. The construction of a 37.0m deep seated slab for the Amsterdam metro station Vijzelgracht in marine clay represents such a complex task. This paper summarises the required measures of quality assurance. In particular a new hydraulic measuring device is introduced with which it was possible to detect the column diameter of 2.6m at a depth of 37.0m during the production process and with a very high accuracy.

The Holmes Safety Association is an offspring of the Joseph A. Holmes Safety Association which was organized in Washington, D. C., in 1916 at a meeting of representatives of twenty-four leading mining associations connected with every branch of the United States mineral industries. The purpose of the meeting was to do honor to the memory of Joseph A. Holmes, one of the founders of the United States Bureau of Mines and its first director, whose death had occurred the year before on July 12, 1915. In deference to what was believed would have been the expressed wish of this great humanitarian and benefactor of the mineral industries, it was decided to establish a national safety organization to cover the mineral indus¬tries exclusively; out of respect for his memory and service, the organization was named the Joseph A. Holmes Safety Association. On receiving requests from several mining districts, in 1922 a number of chapters ware established in several mining States.

Introduction Autogenous, semi-autogenous, and conventional grinding circuits are used to process ores. Current emphasis, though, is towards the use of the first two systems. The reason is the generally lower initial investment required for an autogenous or semi-autogenous circuit as compared to a conventional comminution circuit. However, the effect of operating costs that will significantly affect overall comminution economics is not considered in this evaluation. About 70% of the energy in a mineral processing plant is used in the comminution circuit. Until the 1974 oil embargo, energy was relatively inexpensive. Since that time, the cost of energy has escalated 40-fold. As a result, energy cost now forms a significant portion of operating costs in comminution. According to a survey conducted by General Electric Co. (Powers, 1982), the price of electricity will continue to rise 15% to 20% per year, in spite of the current oil glut. Both autogenous and semi-autogenous grinding circuits consume more energy than the conventional comminution circuit. Higher consumption ranging from 16% to 35% has been reported in the literature (Bassarear, 1982; Gjerdrum. 1979; Betts, 1982; and Christie, 1979). Consequently, even a small increase in energy costs is likely to have a significant impact on operating costs. Therefore, the economic decision of selecting an optimal comminution circuit is important. More than ever before, it is now critical to include in the evaluation of communition circuits not only the capital costs, but also the operating costs; in particular, the expected energy cost over the life of the operation. Failure to do this may lead to a less than optimal comminution circuit. Design A comminution system should be designed only after bench-scale or pilot plant testing of a representative sample. Conventional crushing and grinding circuits can be designed on the basis of the Bond Work Index (Rowland and Kjos, 1980). Over the years, sizing of full-scale rod and ball mills from Bond Work Index data has become well established. Autogenous and semi-autogenous mills cannot be sized based on bench-scale tests. Pilot plant tests are necessary to determine the amenability of the ore, critical size buildup, and power requirements. For this study, conventional and semi-autogenous grinding circuits were designed for ore throughput of 12, 24, 36, 48, 72, and 90 kt/d (13,200, 26,400, 39,600, 52,000, 79,200, and 99,000 stpd). Rod and ball mills were scaled up on the basis of the Bond Work Index. A two-stage semi-autogenous grinding circuit was designed on the basis of pilot plant test results. A week-long selective overburden handling dragline op ration was conducted at a strip mine near Colstrip, Montana. The objective of the demonstration was to hvdrologically isolate a saline overburden zone during placement in the spoil piles. The saline zone had concentrations considered inimical to successful reclamation and was potentially dangerous to future water quality in the spoils aquifer. The zone was identified after review of chemical data for overburden samples collected from the intense 60 m grid drilling pattern. The saline zone in the overburden was: 1) handled in a normal manner; 2) segregated and buried; and 3) segregated, buried, and covered with a relatively impermeable 1.0 m thick clay cap. The method used to formulate this demonstration, the demonstration itself, and the monitoring and evaluation techniques are described in this report. Observation wells were installed to monitor ground water in the overburden, Rosebud coal, and McKay coal. Data from these wells were, used to estimate post reclamation water levels and to evaluate recharge and water movement characteristics in the vicinity of the demonstrations. The inimical zone was successfully handled in a selective manner with a minimum of delays. Economic analysis indicated cost of these selective burial operations to be about 1.1 to 1.5 times the cost of the normal operation at this particular mine site. Long term monitoring will be required to evaluate the effectiveness of these handling techniques in hydrologically isolating undesirable overburden materials.

As exploration for new mineral deposits advances farther into the Canadian North, the more the design engineer is faced with the problems of cold weather conditions. This is especially true when designing belt conveyor systems for open-pit operations where the temperature in the winter may go to -40°F or below. Problems with the mechanical components and also with the belting will arise, which in turn may reduce the availability of the system. This paper outlines the problems encountered during cold weather operation and what precautionary measures must be taken into consideration in the design of equipment, as well as in the selection of materials, in order to ensure a system which will maintain production. THE conveying of ore and waste in the pit or to the processing plant poses problems during cold weather, when temperatures may drop to -4o?F or be-low. Troubles with the mechanical components, the belting or the supporting structure may arise, and this, in turn, will reduce the availability of the conveying system.

Few efforts have been undertaken to date to assess the quality of mandated coal mine health and safety training for new miners and for experienced miners. This study was concerned with the quality and effectiveness of existing classroom health and safety training programs. The study involved field data collection through pre- and posttesting of trainees, audio-taping class sessions, and administering short feedback forms with trainers and trainees at 14 sites in West Virginia, Maryland, Kentucky, and Pennsylvania. Results indicate that goals, methods, materials, and outcomes of existing training programs vary widely among trainers and sites. The report provides less by way of solution to problems, than it highlights the breadth of issues that relate to the question of training effectiveness and quality. The report concludes that additional work by industry, labor, government, and academia is necessary in order to improve health and safety training programs. Recommended steps are: (a) to develop performance standards and measurable criteria in each of the types of mandated training, (b) to develop better training of trainers, (c) to develop expanded training support services. and (d) to undertake further research and development in health and safety training in the mining industry

"A self-tramming remotely controlled mobile Dry Scrubber (DS) was developed by J. H. Fletcher and Co. under a contract with the National Institute for Occupational Safety and Health’s Office of Mine Safety and Health Research (NIOSH OMSHR) to reduce exposure of miners to airborne dust. NIOSH OMSHR laboratory testing showed that the DS averaged greater than 95% dust removal efficiency with the disposable filters and averaged 88% and 90%, respectively, for the optional washable filters in their pre-washed and post-washed test conditions. Although the washable filters could be reused, washing them generated personal and downstream respirable dust concentrations of 1.2 and 8.3 mg/m3, respectively, for a 10-minute washing period. The DS’s velocity pressure regulated variable frequency drive (VFD) fan maintained a relatively consistent airflow near the targeted 1.43 and 4.25 m3/s (3000 and 9000 ft3/min) airflow rates during most of the laboratory dust testing until reaching its maximum 60 Hz fan motor frequency or horsepower rating at 2610 Pa (10.5 in. w.g.) of filter differential pressure and 3.97 m3/s (8420 ft3/min) of scrubber airflow quantity. Laboratory sound level measurements of the DS also showed that the outlet side of the scrubber was noisier and the loaded filters increased sound levels as compared to clean filters at the same airflow quantities. With loaded filters the scrubber reached a 90 dB(A) sound level at 2.83 m3/s (6000 ft3/min) of scrubber airflow, indicating that miners should not be overexposed in relation to MSHA’s permissible exposure level (PEL) of 90 dB(A) at or below this airflow quantity (1). The DS’s washable filters were not used during field testing because of their lower respirable dust removal efficiency and the airborne dust generated from filter washing. Field testing the DS with the disposable filters at two underground coal mine sections showed that it can clean a portion of the section return air and provide about a 50% dust reduction at the face area downstream of the continuous miner (CM) operation. INTRODUCTION Coal mine worker overexposure to coal and quartz dust continues to be a problem at underground coal mining operations in the U.S. The U.S. Mine Safety and Health Administration (MSHA) recently changed the standard for coal mine worker respirable dust exposure from 2.0 mg/m3 average during an eight-hour shift to 1.5 mg/m3 average over the actual working shift, effective as of August 1, 2016 (1). If more than 0.1 mg/m3 quartz mass is found in the coal mine worker dust sample, the applicable respirable dust standard is further reduced to the quotient of 10 divided by the percentage of the quartz in the sample (1). Over 90% of the mechanized mining units operating in U.S. underground coal mines are continuous mining machines (2). MSHA inspector dust samples from 2009 to 2012 showed that 3.7% of continuous miner operators exceeded the 2.0 mg/m3 dust standard, while 8.8% of these dust samples exceeded the new 1.5 mg/m3 dust standard and 9.7% of these samples exceeded the reduced quartz standard (3). Additionally, roof bolter dust samples at these mechanized mining units during this same period have exceeded the 2.0 mg/m3, 1.5 mg/m3, and reduced quartz levels at 1.1%, 3.7%, and 10.6%, respectively (3)."

The history of the flotation process, as applied to the practical field of ore dressing, dates back somewhat less than twenty years; yet this fact may be overlooked in view of the present universal dependence on this process in the concentration of almost all classes of ore. It is therefore interesting to recall that Britannia was the scene of some of the earliest experiments with flotation on a commercial scale. In 1911, sixteen years ago, an Elmore vacuum machine was operated for several months with encouraging results. In 1913, some time before flotation was very generally adopted elsewhere, a Minerals Separation plant, capable of treating 600 tons daily, was operating successfully at Britannia. Considering how little was known of the theory and practice of flotation at that time, the recovery was remarkably good, but it was not until the present mill was completed in 1923 that any attempt was made to raise the concentrate grade by differential flotation, or, in other words, to float the chalcopyrite while rejecting the then worthless pyrite with the tailings. The first ore passed through the present concentrator in March, 1923. As originally designed, the rated capacity was for 2,000-2,500 tons, but this has been increased year by year, until at the present time 4,000 tons daily can be treated economically. During the first few months of operation the concentrate grade averaged only 10-11 per cent Cu, but by the end of 1923 this had been raised to over 20 per cent, due to the elimination of the pyrite by differential flotation. In 1923 the tailings averaged approximately 0.25 per cent Cu, but also in the matter of recovery a considerable improvement has been effected, for during the year 1926 an average tailing of 0.15 per cent Cu was maintained.

A Method to Calculate Hydrodynamic Loads on Underground Bulkheads Bulkheads are structures built in openings in underground mines to impound water, preventing the water from entering the active mine workings. These structures not only have to be designed to withstand the static, hydraulic pressure, but also the dynamic or seismic pressure, which would be generated by an earthquake. Bulkheads can be considered underground dams. Unlike dams on the surface, bulkheads are often subjected to hydraulic heads that exceed the heights of the bulkheads. Theodor von Karman in 1933, and Westergaard earlier, developed the equation by which the hydrodynamic pressures on surface dams could be calculated. Civil engineering handbooks of today contain this equation. In this paper, using the methodology developed by von Karman and Westergaard, the authors have developed an equation by which the hydrodynamic forces on bulkheads could be calculated when the hydraulic head exceeds the height of the bulkhead.

The recently completed major modifications and additions to Reserve Mining Company's taconite concentrating process incorporate a totally new control system. Design of the new control scheme is centered on reliability, versatility, centralization, and optimization. The design objectives were achieved through a distributed control system based on a network of independent field located micro-processor based control systems connected to a central control room. Operation of each micro-processor can be interrogated and modified from the central control room by the operator or by either of two computer systems. The two computer systems handle logging, alarming, specialized sequenced controls, and complex process optimization routines.

Introduction Under our present civilization, industrial developments will be greatest in those areas in which energy .can be most economically produced and efficiently used, where labour is in adequate supply, and where markets for the products of industry are available. A knowledge of fuels- the present major sources of energy- and of their utilization is therefore of vital importance. With that in mind, the aim in this paper will be to outline the known reserves of fuels in Alberta, to give their characteristics, to indicate the markets served by each, and to offer some suggestions for the future development of the coal industry. The paper should be regarded as a compilation of published data rather than as an original treatise. Alberta is richly blessed by having large reserves of mineral fuels. The estimated reserves of coal are 46,562 million tons. Alberta is the oil and gas province of Canada. The world's largest deposit of bituminous sand occurs along the Athabaska river in Alberta. In addition, there are large areas of forested land in the northern and western sections of the Province, and the rivers serve for the development of hydro-power.

THE Montana Phosphate Products Co., subsidiary of the Consolidated Mining and Smelting Company of Canada, operates three phosphate properties north and northeast of Garrison, Powell County, Mont. Production of all three is from the same phosphate bed, a 3 to 4-ft. member of the Phosphoria formation, whose outcrop is traceable across the mountains of the Garnet Range in a series of southeasterly pitching folds. Because of topographical differences and of variations in the attitude of the bed, each property has its own mining methods and problems. Most of the phosphate rock produced since I930 has been shipped to the plant of the Consolidated Mining and Smelting Co. at Trail, B.C., where it is treated with sulphuric acid for conversion to phosphoric acid, which in turn is used to produce various fertilizers. HISTORY The occurrence of rock phosphate in Montana has been known. since 1910, and the Garrison area was mapped by the United States Geological Survey in 1913In the nineteen twenties William Anderson, owner of a ranch 8 miles north of Garrison, discovered phosphate on his land, and the Montana Phosphate Co., which later became the Montana Phosphate Products Co., was formed to work the deposit. Montana Phosphate Products Co. later obtained United States leases on two adjoining parcels of ground, and this combined property, known as the Anderson mine, has been the main source of Consolidated's phosphate rock. The mine now includes an adjoining parcel leased from Anaconda and mined from the Anderson workings. In 1940, the Graveley mine, a small privately owned property 6 miles east of the Anderson, was leased from the owner by Montana Phosphate Products Co., and a U. S. lease held by Graveley on some adjoining ground was subleased. In 1943 the Luke mine, on the Mineral Hill property, 1 ¾ miles south-southwest of the Graveley, a U. S. lease, which previously had produced a few thousand tons, was optioned from the lessees by Montana Phosphate Products Co., and is now being developed. At both these mines the rock is appreciably higher grade than at the Anderson mine. GEOLOGY The Phosphoria formation is the Permian member of an extensive sequence of Paleozoic and Mesozoic rocks. It is underlain by Pennsylvanian (Quadrant) quartzite and Mississippian limestone, and overlain without any marked unconformity by Jurassic (Ellis) shale and Cretaceous shale and sandstone. Many deep well waste disposal. operations have adopted the technology of the petroleum industry but fail to make full use of the knowledge and experience upon which this technology is based. Inexperience in subsurface exploration commonly leads industrial personnel to an over simplified understanding of the complex subsurface rock/fluid environment and overly optimistic "degree of exploration /adequacy of knowledge" ratios. Several fundamental differences between disposal and petroleum production are discussed. In particular, the evolution of understanding of the subsurface in relation to degree of exploration is demonstrated by summarized case histories of such other forms of subsurface fluid exploitation as oil pro¬duction and underground gas storage, tracing subsurface interpretations from the initial exploratory concept to that of fullest development. Evolutionary changes in interpretation and their significance as applied to deep well disposal are stressed.

This paper outlines the evolution of managing high consequence risk scenarios in BHP Coal, with particular focus on managing the stability of an endwall that is adjacent to a public infrastructure corridor running through a Central Queensland coalmine. BHP Coal developed a Geotechnical Consequence Register after industry failures identified shortcomings in traditional risk assessment practices. Traditional risk assessments consider the consequence and likelihood of a geotechnical event occurring under normal operational conditions. As such, the risk rating for a high consequence, low likelihood failure may fall within the acceptable range, even where a lack of meaningful geological data exists due to the area being off lease or outside of mining reserves. The Geotechnical Consequence Register lists all geotechnical failure scenarios of high consequence, identifies if current controls are adequate, and highlights if specific Geotechnical Management Plans are required. This case study reviews a failure scenario that was identified through the geotechnical consequence register. The scenario had previously been considered low likelihood and subsequently driving a low risk; however, the likelihood ranking was impacted by a lack of geological data outside of the mining lease and predominately informed by an acceptable historical slope behaviour. Additional exploration identified previously unmapped faulting which increased the risk rating. The requirement to develop a specific management plan resulted in an increase in the required factor of safety, design changes, the implementation of additional monitoring and a corresponding monitoring Trigger Action Response Plans (TARP). In the case study area, the initiation of a geotechnical instability was identified early through the increased monitoring program. The instability was successfully managed and any impact to the infrastructure corridor off lease was prevented. BHP Coal has used the Geotechnical Consequence Register for several years to identify and assess high consequence geotechnical failure scenarios. In some areas this has justified additional exploration, more stringent design acceptance criteria and an increased level of geotechnical monitoring. The process has been successful in ensuring that geotechnical risk is controlled in areas that often receive less attention than active production areas.

The Bureau of Mines evaluated potential availability of domestic nickel from 23 deposits, Evaluations included an estimation of in situ and recoverable resources, applicable mining and processing technologies, and capital and operating costs. Discounted cash flow analysis with a 15-pct rate of return was used to determine the average total cost of production of each operation, Identified in situ resources contain 9.0 million metric tons (t) of nickel; 5.5 million t is recoverable as nickel metal or ferronickel. Demonstrated in situ resources contain 8.2 million t of nickel; 4.8 million t is recoverable as nickel metal or ferronickel. No nickel is available at a total cost of production equal to the January 1983 market price of $3.24 per pound. Analyses indicate that potential annual production can only meet current consumption levels at a total cost of production of $7.60 per pound of nickel, Nickel's availability is contingent on (1) successful adaptability of nickel processing methods to domestic resources, (2) location of a smelting facility, and (3) marketability of byproducts and coproducts. Analyses were performed on the effects of byproduct and coproduct revenues, energy, labor, and transportation costs, and metallurgical recoveries on nickel's availability.

"The Frog's Leg project is located in the Kundana mining centre 22 kms West of Kalgoorlie, in Western Australia. The Mines and Resources Australia (MRA) Dioro Exploration Joint Venture has had considerable exploration success centred on the high grade Frog's Leg discovery. A gold resource of 930,000 ounces has been defined. At Frog's Leg there have been three discrete resource domains recognised - Supergene, Contact, and Quartz Lodes. This paper describes the metallurgical test work undertaken.The selection of the drill core was a joint effort by the MRA Senior Geologist and Mineral Engineering Technical Services (METS) to ensure the samples were representative of the ore body. Optimisation of gold recovery was essential as both the Contact and Quartz Lodes have exceptionally high percentages of free gold as well as significantly higher grades than the typical open pit ores found in Western Australia. The metallurgical scope of work included documenting all of the metallurgical test results, establishing the comminution characteristics, determining the percentage of gravity recoverable gold, evaluating the cyanide leach characteristics and determining the tailings characteristics. These studies were used as the basis of flow sheet design to treat both ores and to discuss the advantages and disadvantages of alternative flowsheets. Investigations also included toll milling options at various plants as well as second hand and transportable plants. Considerable technical effort has been put into ore characterisation by way of test work aimed at managing the process risk at each stage of project development, including mitigation. MRA also carried out detailed mineralogical analysis of the ores."
