The Chairman opened the session by wondering why so few papers on Maritime Structures had been received for this conference. Perhaps because the marine environment is so hostile and the access so difficult that engineers tend to be more prudent about new ideas and designs. Dennis Waite noted the number of grey hairs in the room - grey hairs which could be equated with experience gained the hard way. He hoped that this experience would be passed on in the discussion period of this session. The Chairman was delighted to welcome two authors from China, Mr Sun and Mr Liang. He was most grateful on behalf of the conference for their efforts both in preparing their papers and for being able to present them. He regretted that Messrs Noce and Fedolino were not able to present their paper and then welcomed Mr Christensen from Denmark who would present the first paper.

The International Ground Control Conference in Mining has always provided an open forum for the publication, presentation, discussion, and often heated debate on roof bolting systems mostly with attention to how, when, and why they work. During the last 24 Conferences at least 170 papers have been presented by International Experts on these “simple” devices that range in length from 30-inches to 20 ft and from 5/8 to 2 inches in diameter. Roof bolts are primarily mechanically anchored, glued, cemented, or driven. Bolts are placed vertically, angled, or tied together with special fixtures (trusses and slings). They have been termed active, passive, stick-slip, and several other descriptive acronyms or mining slang expressions. This retrospective paper will present the changes in bolt types and usage, point out some of the biggest myths and hail the most significant advances (according to the authors’ opinions of course).

Canadian leadership in a global mining context will depend not only on entrepreneurship and best practice but also on technological innovation. This paper argues for more ambition and commitment to R&D for technological innovation in Canadian mining. It focuses on underground hardrock mining, where past R&D initiatives have simply not stood the test of time, and it is important to examine why. Concerns arise because industry’s commitment to R&D has not been sustained over time. Innovation through exploration, processing, and environmental technologies has been more significant. Meanwhile, R&D capacity and synergies in government institutions and universities have not been exploited. There is a need for more commitment, coordination, and collaboration by industry, governments, and universities. This paper examines some corporate models that have achieved sustainable innovation, along with some that have not. A strategy for innovation is discussed that relates to culture, commitment, coordination, and collaboration.

The most fundamental feature of fracture is that it involves separation. Such separation can arise by either separation of molecules, more or less along the line joining their centers (tension), or by d sliding separation roughly perpendicular to that line (shear). Theoretically, a perfect crystal could never be broken by the application of perfectly axial compressive stress (until some sub-molecular actions occur). In tension fields the calculated forces between molecules are extremely high. Consequently, calculations based on the forces known to exist in materials at the molecular and crystalline levels always lead to strengths which are two or three magnitudes higher than the strengths obtained in service or in the laboratory. It has been a major problem, therefore, to explain: (a) How fracture can occur in compressive stress states. (b) Why the observed strength of a material is always much lower than its calculated strength.

"11.1. INTRODUCTIONA royalty is the most common form of interest retained by an owner who is disposing of his property or by a joint venturer who ceases, voluntarily or otherwise, to participate in the joint venture.1 Rarely does a non-participating party have a direct interest in the property and/or assets of a project, except during the period when a non-participating joint venturer may be having his interest diluted.2 In fact, it is most unwise to permit a non-participating party to have such an interest. It may give rise to significant problems that could prevent or substantially delay the development of an orebody and require a considerable amount of time, effort, and money to correct.3In negotiating a royalty, it is important to be aware that there is no law or convention that says, “You just don’t do that in a royalty agreement.” As with any agreement, inventiveness and flexibility on the part of the negotiator are not just assets, but virtual necessities. It is also for this reason that it is essential to read the royalty provisions, particularly the definitions, fully and carefully, especially if you are the recipient. On the other hand, if your inventiveness and flexibility cause your accountant to question the advisability of one of your ideas, pay close attention to him.4 He will, no doubt, have had experience with many royalties, and no competent professional adviser disparages a client’s ideas without good reason. Besides, he will probably be the person who will be determining the royalty or certifying the determination of it. At the very least, he will be auditing your general financial statements, which must, of course, reflect the royalty determination. Heeding his advice not only may make the determination easier but also will probably save you money in professional fees later on. Nevertheless, do not simply accept his comments or recommendations without understanding what they mean, why they have been made with respect to your royalty agreement (sometimes professionals have comments that they make as a matter of course and, even though they may be valid, you should require them to be justified to you), and how they will affect your royalty determination.It is surprising how many corporate payors insist that their standard royalty provisions5 be used as a matter of policy, thereby effectively tying the negotiator’s hands. It is even more surprising, and to professional advisers frightening, how many of the senior employees who insist that such a policy be adhered to, virtually without variation, do not know what is in the standard provisions and would be hard-pressed to explain them to a property owner. If standard provisions are appropriate, by all means use them, but do allow experienced and sensible negotiators some flexibility and make sure that everyone within the corporation who may have to use such provisions – negotiator and supervisor alike – knows what they contain and why they are as they are. For example, there are often good and convincing reasons to describe a deduction in a certain way or to include a certain specific type of deduction. On the other hand, quite often the answer to the question “why?” is that “it has always been there”; this is, or should be, an unacceptable answer. In any event, if you, as the negotiator, appear to know what you are talking about, are familiar and comfortable with your royalty provisions, and are backed up by your boss with the same words or reasoning, a property owner cannot help but be satisfied that you are knowledgeable, even if he still does not agree with your provisions. Finally, if you do use standard provisions, make sure that they are reviewed and updated regularly."

"Many mentorship programs are available to Canadian mining industry employees. Some are company-led initiatives, whereas others are provided through educational institutions or nonprofit organizations. In 2016, a national survey was launched to gain insight into perceptions of what mentorship is, learn about available programs, and collate experiences with formal and informal mentorship opportunities. This paper outlines survey findings and delivers recommendations to industry for consideration in employee mentorship programs. In addition, mining industry leaders share their mentor/mentee perspectives. The intent of this paper is to bring awareness of best practices that can help employees grow and develop in their profession.RÉSUMÉ Les employés de l’industrie minière canadienne ont accès à de nombreux programmes d’encadrement. Certains sont des initiatives menées par les sociétés, d’autres sont proposées par le biais d’établissements d’enseignement ou d’organismes à but non lucratif. En 2016, une enquête à l’échelle nationale a été lancée en vue d’obtenir des informations quant à la perception de ce qu’est réellement l’encadrement, de découvrir les programmes proposés et de comparer les expériences aux possibilités d’encadrement, officielles ou non. Cet article présente les résultats de cette enquête et fournit des recommandations à l’intention de l’industrie afin qu’elle les prenne en compte dans ses programmes d’encadrement des employés. En outre, des chefs de file de l’industrie minière partagent leurs points de vue conseillers/protégés. In 2016, a link to an online mentorship survey was circulated to males and females in the mining industry with the belief that mentorship programs should be available to everyone. The link was delivered via the authors’ personal networks in mining, the CIM Reporter, Women in Mining (WIM) email blasts, LinkedIn, and Facebook. This paper summarizes the answers to the survey questions. The paper is a follow up to one by the same authors (Nyabeze, Espley, & Beneteau, 2010), which was written to gain insight into why women join and stay in the mining industry, and to learn why the overall representation of women is low."

"Recently, small sized micro-needles for medical are required more frequently by patients. The biggest reason is that it presents great burden on the patients, due to the size of the needle injection. People need to use a very small needle to lower the pain, but the downfall is that it makes them more expensive due to their complicated manufacturing method. Therefore, we need to fabricate a needle by wire-drawing to decrease its price. Tube-drawing is beneficial for its production value; however, in conventional tube-drawing it is difficult of control the wall thickness of the tube and makes it 1mmor lower in diameter. That is why, we propose fluid-mandrel drawing for new tube drawing. This method allows fabrication of a fine tube by using the pressure of the fluid in the tube. In this research, the goal is the fabrication of a fine tube by fluid-mandrel drawing and its evaluation. Finally, the fabrication of a fine tube by fluid-mandrel drawing was conducted and in result a tube with a diameter of 0.2mm, high strength and thin wall and fine inner surface was drawn, which can be used as a lesser pain micro-needle.INTRODUCTIONThe main manufacturing method of the current medical needles is roll forming. With this method, it is difficult to make a tube with a diameter lower than 1mm. Deep drawing has been proposed and used as method to further reduce the diameter, but this method is costly and the injection needle obtained by this method is several times more expensive.Therefore, we need to obtain a fine medical needle by drawing process that can be mass-produced and inexpensively manufactured. However, with the conventional drawing process, it is not possible to obtain a syringe needle with a reduced diameter easily, that is why, a fluid-mandrel drawing has been proposed which can reduce the diameter without damaging the inner surface. The methodology of the method is, a fluid is filled in the sealed tube and the pressure inside the tube during drawing is responsible for the good inner surface and wall thickness. It is also expected that the fluid can be easily discarded and that the tube can be manufactured on a long scale."

The importance of the grinding environment and its effect on sulphide minerals flotation behavior has been known for many years. The grinding environment may play a role in controlling fine and coarse particle flotation along with dissociation of different chemical species in grinding environment. Dissociation of different chemical species and ions within the process water may result from changing the grinding environment. The mining and mineral processing industry undertakes activities that may impact on water resources. These include use and storage of chemical storm-water management, process water management, mine dewatering and management of tailings. In mineral flotation understanding and controlling water quality is paramount important not only because of its effect on flotation but also with respect to control the quality of the water which is released to the environment. Releasing of different anionic and/or cationic chemical species in the process water results from the interactions among different mineral within the ore, process water, and the milling environment. In fact grinding mill acts as an electrochemical reactor which affects the flotation performances of different value minerals as well as the quality of the tailing products. An increasing in antimony content in process/discharge water at Boliden?s Tara processing plant in Ireland to above limitation level was reported in April 2010 which needed to be controlled. An increase up to 6 times of the permitted level was reported which needed an immediate action to control due to environmental considerations. Therefore attempts were made to detect why this higher amount of Sb is released and how to control and minimize its release to the process/reclaim water. This paper discusses how/why Sb content in process/reclaim water was increased by changing the mill electrochemistry and the strategies were taken into considerations to control it. Different strategies, including electrochemical controlling of the milling section, Fenton process, using Bauxsol in treating the process water, etc., have been considered and tested which are described herein. All strategies taken into consideration for solving the problem were successful which indicated good understanding of the issue and good decision making. Furthermore, for each solution the capital and operational costs were roughly estimated. Accordingly, the Sb content in process/discharge water was controlled to a value lower than allowed limitation, i.e., 1 ppm. This paper considers certain aspects of the acid bessemer process, particularly in its relations to the duplex process—that combination in which the pig iron is first desiliconized and decarburized in acid bessemer converters and then dephosphorized in basic open-hearth furnaces. The acid bessemer process employs an acid-lined converter and produces an acid slag. The blow is usually thought to eliminate the silicon and manganese as oxides and then to burn off the carbon as carbon monoxide On account, however, of the relatively large amount of metallic iron present in the converter, compared with the relatively small amount of the impurities it is desired to oxidize, the first reaction in the converter probably is the formation of iron oxide, which dissolves in the bath and acts as an oxygen carrier for the silicon and carbon. As soon as iron oxide is dissolved throughout the bath, the oxidation of silicon commences; but in the early stages of the blow the mass law would indicate that iron oxide must form first, and this oxide probably increases up to some saturation point. The molten metal in the converter from the early stages of the blow right through to its completion remains basic, while the slag produced is siliceous. This fact is shown by the corrosion of the acid bottom and the tuyeres, which is greatest at the tuyere orifice where oxidation is most active and where there is the most iron oxide. Why does this acid bottom corrode? Because it is attacked by a base. The thought, therefore, suggests itself, why not. make the bottoms of basic or neutral material?' As the converter slag is acid, those parts of the converter coming in contact with the slag should be acid; but those parts of the converter that are in contact with the molten metal saturated with metallic oxides should be basic. or neutral. Bessemer operators have for some time tacitly admitted this condition, for the blast pressures have steadily increased with the object of keeping the metal in suspension, and so preventing bottom corrosion; if the molten metal were not basic, it would not attack an acid bottom. Most of those involved in the mining industry are aware of the negative feelings that the well-informed citizens of this country, also known as the American public, have for the industry. As noted in an issue of Mining World News, “The general public perceives mining as dirty, noisy, destructive and belonging somewhere else.” The same theme was raised at a conference in Scotland in July 1991. It was noted there that mining has an unattractive public image around the world “as dirty, dangerous work that pollutes and degrades the environment.” Why is this? Why does mining have such a bad rap? I’m afraid that some of the blame rests with the mining industry itself - both in the past and in the present. Hopefully, we can do something about this in the future. And hopefully, the information here will help the reader to spread the word to offset the negative messages being issued almost daily by industry foes. I hope this will become more than just the words of another miner talking to miners and their friends. In the past, belching smokestacks and polluted streams and waterways were common sights. Today, the negative results of mining operations can also be seen in the form of waste dumps, leaked acid mine waste water and dust blowing from tailings piles. Some will argue that mining did nothing illegal in the past, and that all the regulations in force at that time were carried out to the letter. That may be so. The fact remains, however, that mining did these things, and the evidence is very clear in many parts of the country. It is visible to the well-informed citizens, in spite of the fact that many mine sites have been cleaned up and new technologies, such as flash smelting, dump leaching, pressure leaching and bioleaching, have greatly overcome many of the pollution problems. What is not so visible is the importance of the mining industry in our daily lives. Not long ago, knowledge about our non-renewable mineral resources ranked low on the list of qualifications for being a well-informed citizen. Events during the past two decades have changed this scenario. Politicians, writers, newscasters, economists and administrators, as well as the ‘public,’ are expressing opinions and forming judgments on oil company profits, desecration of landscapes, waste disposal sites and pollution by the extractive industries.

WHAT IS AI? It's very hard to say. Most researchers and practitioners disagree with nearly any attempted definition, but most would agree that A1 does deal with the development of computer-related methods o f problem-solving. Artificial intelligence is a discipline which seeks to emulate human anatomy and/or thought processes by applying computer science, electronics, and mechanical engineering to the development of natural language processing, machine vision, robotics, and knowledge-based expert systems. Integration of these four fields will someday produce the robots of today's science fiction. A1 addresses problems of structuring large amounts of knowledge and representing relationships with rules and semantic frameworks rather than with mathematical equations. A1 is convergent rather than divergent - - conventional conputer applications use relatively small quantities of input to generate large output, but A1 generally reduces large amounts of information to establish a few major conclusions. The development of A1 has prompted greatly increased efforts on the part of scientists and engineers to understand just how humans think, organize information, approach problem-solving, and form conclusions. Some of the more exciting and possibly far-reaching advances in A1 are coming in this more-or-less soft-science area. Thirty years of A1 research (dating from a 1956 conference at Dartmouth College at which the term A1 was coined) has already spawned many widespread, almost ordinary, products: window graphics, hand-held controllers (mouse). function menus, etc., all in an attempt to make computers and people more compatible. WHAT IS AN ES? A1 can be used to develop EXPERT SYSTEMS, which are computer programs or systems which contain knowledge and rules by which problems can be solved. or at least by which possible solutions can be suggested for very complex problems. A1 can be used to better define problems so that solutions can be found. An ES is designed not to process numbers, but to process information according to rules and logic derived from actual practice and experience. WHY AI? WHY ES? Companies are increasingly aware of the value of accumulated knowledge and experience in designing, building, operating, and maintaining the increasingly complex or expensive physical plants in any business. As knowledge branches out and becomes more specialized, the accumulated learning of a veteran operator or manager or engineer becomes a scarce resource well worth preserving. EXPERT SYSTEMS offer a means of doing that, reinforcing, not replacing, human capabilities.

The stresses in the simplest structures are often those we find most difficult to analyze. The most complex condition in mine stresses is found in simple tunnels where the roof, the sides, and the floor are a monolith. The functions of the parts are like the parts themselves not distinct and spccialized, and the problems to be solved are like those in a metal structure with riveted joints or a redundancy of bars. This difficulty explains perhaps why the condition has not been treated. But just because it cannot be discussed in its entirety is no reason why it should be treated as an action of parts with spccialized functions as a roof beam with supports and a foundation. The problern cannot be ignored on the ground that it is not of sufficient importance to warrant careful consideration, because conditions of complete monolithism, of which the tunnel is the type, are found materially unchanged in room-and-pillar and in longwall work. This unity between roof, sides, and floor, which to the coal miner is a difficult conception, really deserves a scientific appellation, and perhaps holoid (from holos, whole, and eidos, form) will serve the purpose as well as any other. In a simple tunnel the roof, the sides, and the floor form integral parts of one and the same structure, and the distortion of one cannot be conceived without a consequent strain in the others. Thus when the roof of the tunnel droops by reason of its weight, the upper parts of the sides are drawn in becausc they are integrally connected with the roof and must approach each other whenever, by the sagging of the roof, the distance between any two points in it is diminished. (See Fig. 1.) Thc sides in their turn operate on the floor of the holoid structure, producing a tensile stress. The writer has always been impressed with the value of soap as a means of illustrating the action of mine stresses. With that idea in mind' a cake of naphtha soap measuring 43/8 by 2¼ by 19i in. was taken and a tunnel was made through it 13/4 in. long, 21/2 in. wide and 1¼ in. high. (See Fig. 2.) Rickard, Denver, Colorado: The observations made by the author concerning the treatment of gold-bearing ores, deal with the subject only briefly and in a general way; but, coming from an authority, they invite comment. In stating the fact that a " finely-divided condition " is a necessary characteristic of ores containing free gold, the author must be refer ring to those mining districts only with which he himself is ac quainted. The statement would not be true of certain gold-mining regions in America and Australia. The ores of some Californian mines and the mill-stuff treated at certain of the mills of Clunes and Ballarat in Australia, and of Otago in New Zealand, contain the gold in a free but in a coarse state. Moreover, "reduction to fine sand and flour" is not required by certain ores, even though the gold be in a finely-divided condition. In such cases the gold becomes de tached from the quartz before the gangue has been pulverized into particles as small as those in which the gold occurs. The dry-crushing of free-milling gold-ores is onknown in the new mining regions of the world. It is not clear whether the author has in mind any European district in which such a process is in actual use. If he has, a description of the method and the results obtained would be very valuable. Dry-crushing is, other things being equal, twice as slow and twice as expensive as wet-crushing. In the case of free-milling gold-quartz it serves no purpose, since such ore does not carry sulphides which contain any large percentage of value, and which, by being slimed, may carry away part of that value. The dry-crushing of a free-milling gold-quartz would be considered an absurdity by an American metallurgist. If the ore contain "free gold only," why submit it to a further treatment, such as concentration, after its passage over the amalga mating-plate ? What does the author propose to concentrate ? Again, why anticipate the treatment of tailings by the McArthur Forrest, or some other process ? The satisfaction felt by a mill-man in saving gold by a subsequent treatment of the tailings from his mill appears to me to be like that of the man who shoots at a rabbit,

THE following is a description of the methods used in rescuing men and extinguishing the underground fire at the Pennsylvania mine, Butte, Mont. This fire, which cost the lives of 21 men, began about 9 p.m., Feb. 14, 1916, at or near a ventilating fan on the 1,200 air-shaft station. This shaft at that time was downcast, while the main hoisting shaft was upcast. Owing to the proximity of the fire to the air shaft, the air in the latter soon became heated, and as the fan on the level was not running¬the power having been cut off shortly after the outbreak' of the fire-the aim' shaft became upcast to the 300 level. At this point a blanket of cold air from the surface caused the smoke and gases to he drawn in on that level and carried down again through the workings to the 1,200. A short time after the discovery of the fire, water was turned down the air shaft through a series of pipes around the collar of the shaft, which caused the smoke to recede, below the 300, so that within 2 hr. it was possible to search that level for the missing men. From the above it will be apparent why the air shaft was upcast instead of downcast as under normal conditions, and why men were suffocated at considerable distances from the seat of the fire. The smoke and gases that came up the main hoisting shaft did not diffuse through the workings to any extent. There are doors on all levels in the connections to the main hoisting and air shafts, but-in the confusion and running around many of these doors were left open, and this undoubtedly caused the smoke and gases to spread more rapidly than they would otherwise have done. It was necessary for most of the men on the various levels to pass close to the air shaft in going to the hoisting shaft, and when they encountered the smoke and gases in that vicinity a number turned hack and climbed down with the direction of the air currents and were then suffocated, while all those who passed through the relatively short pall of smoke to the main shaft were saved.

In the case of earthquakes, there are rather well-developed analysis procedures and acceptability criteria. The analysis procedures relate mainly to selection of the dynamic loading either through design earthquakes and/or pseudo-static seismic coefficients. The acceptance criteria are typically expressed in terms of a minimum safety factor in pseudo-static analyses. The acceptance criteria are often promulgated by governments despite the fact that no open pit slope has ever been adversely affected by an earthquake. This paper explains why open pit slopes are seemingly more resistant to dynamic loads than natural landforms, which can experience catastrophic landslides.Extreme rainfall events are much more likely to cause open pit slope problems than earthquakes. Two types of problem are common – slope erosion and slope instability. Slope erosion is often mitigated by appropriate surface water controls. Slope instability due to elevated transient water pressures is more difficult to mitigate. Analysis procedures and acceptability criteria are rare. This paper will discuss the mechanisms for rainfall-induced slope instability, as well as analysis methods. Examples will be discussed and analysis methods are proposed. The idea of making permanent magnets by sintering is not a new one, but until the introduction of the iron-nickel-aluminum permanent-magnet alloys there was little need for such a process. Permanent-magnet alloys in use up to the time of this development could be forged and machined to the exact size or shape required without too much difficulty. These new Fe-Ni-A1 alloys, which have become known as Alnico in this country, soon became very popular because of their superior magnetic properties, but when they were made by casting they did have some disadvantages when compared with the older type of permanent-magnet steels. Among these disadvantages were the following: 1. It was almost essential that they be cast to size, since they could not be readily forged or machined. The material could be ground but this was quite expensive, particularly for small magnets in large quantities. 2. The material had a large grain size and was mechanically weak. 3. Close tolerances could not be held and on small magnets this meant grinding almost the entire magnet. Because of the brittle nature of the material, there was considerable loss due to chipping and cracking during these grinding operations. 4. Because of its high aluminum content, the molten alloy had a high surface tension. It was difficult to prevent loss of aluminum by oxidation during the melting operation. In view of these difficulties it is not hard to understand why a process for sintering alnico was developed, or why sintered alnico has come into such widespread use in recent years. It might be well to qualify this by saying that it has come into widespread use for small magnets weighing 2 oz. and less, of the type shown on Fig. I. In this size range, sintered alnico has been able to compete in price with cast alnico. Magnets larger in size, of the type shown on Fig. 2, can be made but they do not compare favorably in price with the cast material. An idea of the range of sizes that have been produced is given by Fig. 3, which shows a small magnet weighing 0.006 oz. and a large magnet with a steel insert weighing 4 lb., both made from sintered alnico. Many of the gases and vapors used in anesthesia form explosive mixtures with oxygen or air. Sources of ignition for these mixtures always have existed, in operating and anesthetizing areas. When ether was first introduced, candles and gas-lights and coal stoves caused fires and mild explosions of the vapor-air mixtures. Today the chief causes of ignition are electrical. Fires and explosions are still much too frequent; and, because many of the mixtures are rich in oxygen, the explosions sometimes are very violent. Obvious sources of ignition, such as electric-power sparks and arcs in open motors, switches, and receptacles, as well as exposed incandescent wires and hot plates, usually are recognized by hospital personnel and corrected. A source of ignition that is not so obvious to many is static electricity. Until quite recently the presence of this hazard in a large majority of anesthetizing areas was not even suspected. However, numerous explosions -many of them fatal to patients -have been attributed, with good reason, to sparks from static electricity. In this paper the authors present facts to show the prevalence of static electricity in the average hospital anesthetizing area, and the degree to which this easily developed electrical form of energy constitutes an explosion hazard. An effort is made throughout to explain how static electricity behaves, why it is a hazard, and why certain safety measures are needed and recommended. Following the introduction and a short general discussion of static electricity, which incorporates a glossary of pertinent electrical terms, an account is given of inspections and tests made in a number of hospitals. This account includes many suggestions that should be useful to any person who plans to make similar inspections and tests. However, a comparison of materials and equipment used with those described in the report will reveal, in many cases, the potential dangers and obviate the necessity for tests. The hospital work was supplemented by considerable laboratory research designed to determine the spark energies required to ignite anesthetic-gas mixtures and the exact amount of time involved in developing the usual static charges. The results of this research are reported and the implications examined. The paper concludes with a general discussion of static and related hazards in anesthetizing areas and of the most urgent preventative measures.

Because of the lack of a universally accepted definition or standard for RCA, we are left to our own conventions in the marketplace as to what is most ?effective? or a ?Best Practice? for our own facilities or organization. This lack of uniformity in the application of RCA produces an equal lack of uniformity in the measureable outcomes of RCA on the bottom-line from site to site and industry to industry. What is the definition of success for an RCA? Is it regulatory compliance? Is it some quantifiable measure of bottom-line performance? Is compliance and bottom-line performance the same thing? Can we measure the ROI we receive by doing compliant RCA? These questions must be adequately addressed if any definition of RCA is to be effective on the bottom-line. Like our various Corporate Vision and Mission Statements, what would be our Vision and Mission for RCA? When pondering the answer to this question, we should consider: ? What Key Performance Indicators will the Success of RCA be Linked to? This provides a direct correlation to the corporate dashboard and therefore garners support for the RCA effort when implemented at the sharp end. Not until we know what KPI?s will effectively measure our maintenance and reliability objectives can we begin to establish which opportunities will afford the greatest returns1. ? What Bottom-Line Metrics will be Tracked to Measure the Effectiveness of Our RCA Effort? Will it be increased production, profitability, regulatory compliance, etc.? Will it be decreased injuries/fatalities, claims, customer complaints, risks, etc.? We should be specific about these metrics and their correlation to the metrics on our corporate dashboard. ? What Will Trigger an RCA Investigation? Normally companies will establish quantifiable thresholds to be surpassed before an RCA will be conducted. Examples may be an OSHA recordable injury, a failure event in which production losses exceed $250,000, an equipment failure in excess of $50,000 or a regulatory violation. This is the norm. However, we should realize this is using our RCA skills totally on a reactive basis. High Reliability Organizations (HRO) strive for proaction and they also use RCA proactively. Why can?t we use RCA to analyze why unacceptable risks from an FMEA are so high? It is perfectly applicable yet rarely implemented in this fashion.
