This report, sponsored by the U.S. Bureau of Mines, presents the results of an examination of the safety of the impactor shaft sinking system in comparison to conventional shaft sinking technology. Detailed study of the impactor system was also performed to identify and recommend design changes for optimum safety conditions. A comparison of the impactor machine and its support systems with conventional shaft sinking was made on the basis of relative risk levels. Risk incorporates measures of both the frequency and the potential consequence of accidents. A safety evaluation of the component operations of the impactor shaft sinking system was performed using a Failure Modes and Effects Analysis (FMEA) methodology. The results of this analysis show generally that the impactor shaft sinking system is safer than conventional shaft sinking. The potential risk of conventional shaft sinking operations could be reduced by an average of over 35 percent with use of the impactor shaft sinking system. Design change recommendations to further improve the safety of the impactor system include a revised fire suppression system, improved access to and from the operator cabs, reliable noise attenuation measures, and improved communication systems. An operational safety manual was also developed for the impactor shaft sinking system.

To the question, "Do the Pennsylvania anthracites contain phosphorus ?" asked at the last meeting of the Institute during the discussion on the metallurgical value of Western lignites, I can now give a definite answer. Through the kindness of a gentleman connected with the Philadelphia and Reading Railroad, and another connected with one of the principal ironworks on the Lehigh, there was sent to the Iron-masters' Laboratory a number of samples of the coals now largely shipped from the Schuylkill and Lehigh regions. Seventeen of these I examined for their contents of ash and phosphorus, and obtained results as follows

The current insurance market has been brought about by a number of pressures, including: collapsing investment returns; growing losses, both normal and catastrophic; and reserve strengthening. This is impacting most classes of insurance coverage, with the traditional characteristics of higher pricing and greater levels of retention, however it is now translating into a lack of capacity and reduced coverage. This presentation will explain how the market has arrived at this situation and will challenge whether this is the traditional insurance cycle or a structural correction. Driven piles are an effective and cost-efficient deep foundation method; however, when piles are driven near existing buildings and improvements, vibrations must be controlled to non-damaging levels. Monitoring of adjacent structures is important for loss prevention, specification compliance, and public relations. The fundamentals of ground vibrations, including particle velocity, acceleration, displacement and frequency will be discussed. Types of pile driving and the ground vibrations that result will be summarized. Industry standards for safe vibration levels for various structures (residential, commercial, industrial, historic, utilities) will be reviewed. The paper/presentation will include a discussion of soil settlement from pile driving vibrations and case histories of observed settlements. Methods for calculating vibration amplitudes and frequencies will be presented. Best practices for monitoring and controlling ground vibrations, including available state-of-the art equipment, will be outlined. The paper/presentation will conclude with steps for designing and implementing a monitoring and control program.

The results of a laboratory investigation of pullout tests conducted on grouted, plain cable bolts are reported. The main purpose of the study is to determine experimentally the shear bond stiffiess of cement grouted and resin grouted cable bolt systems, which is an important input parameter for numerical modeling of cable bolt support. Three nominal embedment lengths of 152mm (6 inch), 203mm (8 inch) and 254mm (10 inch) were used and two types of grout namely cement and Anchortite resin were used. Two water- to-cement ratios of 0.3 and 0.4 were employed for the cement grout. The concepts of effective shear stress and effective shear stiffiess are explained and adopted to calculate the shear bond strength and the elastic shear bond stiffiess per unit length. A comparison of results with previous research is presented to reveal the effect of water-to-cement ratio and embedment length on the shear bond characteristics of grouted cable bolts.

The treatment meIhods selected for the concentrator at Cobar Mines Pty. Ltd. called for a high degree of automatic control of both the grinding and flotation sections.The resulting concentrator layout includes a central control room overlooking both grinding and flotation sections. The control room houses process recorders, controllers, and indicators. together with motor stopstart stations and alarms.Plant instrumentation includes load cells and variable speed hydraulic drive feeders for feed-rate control of autogenous mills, together with nuclear density gauges, differential flow transmitters, and pneumatic control valves for water and pulp density control.Flotation cell pulp levels are controlled by floats and pneumatic actuators; flotation reagent controls include magnetic flowmeters and time-operated solenoid valves.

The effect of grinding media composition, mineral type, and slurry environment on the wear rate of various grinding media (balls) are discussed. Sufficient wear data have been obtained and correlated. Modes of wear were determined from examination of the ball surfaces using scanning electron microscopy. Five modes of wear have been identified to be operative under certain slurry conditions for the various ball compositions studied.

The final results of the operation of the Cananea test dumps are discussed. Scale-up factors between shake-flask tests, column tests and the large test dumps are established for the Cananea ore. The formation of clay layers, which formed inside the dumps and markedly affected extraction, as well as secondary enrichment, are discussed.

Industrial relations on Australian and New Zealand resource projects can be volatile. Therefore investors have become increasingly aware over the years that sound industrial relations during construction is vital to the overall success of their project. There has been a wide range of actual industrial relations performance, project by project, and the degree. of success often directly correlates with the degree and thoroughness of pre-project planning. However, although many projects have been crippled by delays and cost overruns because of labour disputes, good results are achievable. Total commitment to the project by all parties involved is essential and it is up to the employers to seek out this commitment and not just trust mother luck. Industrial relations must be taken very seriously as the price of failure can render a resource project unviable.

The purpose of this paper is to present the results of 102 mu1tiaxial tests conducted on cubical specimens of Indiana limestone using the mu1tiaxial apparatus available at the University of Colorado at Boulder. These tests include (1) uni-axia1 compression and tension, (2) bi-axial and tri-axial compression, (3) biaxial compression- tension and (4) tri-axial compression-compression- tension. The applicabi1ity of existing failure criteria to predict the strength of the tested materia1 is discussed.
