The management of safety has undergone a number of radical changes in the past 100 years. The most important paradigm shifts occurred through the work of Heinrich, followed by the advent of the safety systems era and, more recently, the behavioural safety approaches that have become increasingly popular around the world. Each of these æapproachesÆ to safety has left an indelible mark on the way we target safety improvements, helping decide which interventions are deployed and how they will be measured. They have also resulted in a number of æmythsÆ about safety management, each of which will be discussed, analysed and challenged in this paper. This paper highlights the complex problem of æcomplacencyÆ, that has arisen out of the increased levels of regulation and consequent perceived protection that workers enjoy in the work place. It also looks at a phenomenon called æaccident migrationÆ which results from a superficial treatment of accidents with changes to rules and procedures. The most recent approach to safety is generally known as behaviour-based safety, which flowed from the human sciences and quality management era. This approach has had a dramatic impact on the way safety is managed around the world, but still falls short in a number of key aspects, most notably the area of risk awareness and risk understanding. Many workplace accidents occur simply because the risk is unidentified, underestimated, not understood or ignored. This paper proposes a new direction for safety, called competency-based safety. It is based on cognitive psychology aimed at combating the consequence of modern protection. A case study is presented detailing the deployment of this approach throughout Placer Dome.

Reserves of high-grade quartz gravel, lignite, and timber suitable for the production of silicon metal and ferrosilicon on large scale occur within 40 miles of Bluff Harbour. Undeveloped hydroelectric power resources on the Clutha River are located within 50 miles of the most promising quartz gravel deposits while opencastable lignite, potentially suitable for electric power production, occurs in the same locality as the quartz gravels. A preliminary study of production economics indicates that the commercial feasibility of silicon/ferrosilicon production would depend mainly on the cost of electric power, total capital cost of plant, and the market value of the products. The proximity of Bluff Harbour is advantageous for the export of products.

This paper describes several sources of external dilution affecting the stope walls during underground sub-level and bench stoping. Factors which control ground behaviour such as the geometry of the openings, the influence of geological discontinuities, stress re-distribution and blasting practices are discussed. Crown stope instability due to stress concentrations induced by changes in the excavation geometry, is discussed and the most efficient ways to sequence blasting within a stope are analysed. Effects of undercutting by blasting, blast damage in the near field and cut-off slot blast designs are analysed. The influence of dynamic loading on the exposed walls as well as the practice of blasting fill passes within stope crowns are discussed.

Production scheduling is often reliant on an estimation of a mineÆs trucking capacity. While this mechanistic approach is relatively simple, it often over-estimates actual throughput resulting in under-utilisation of downstream process and a shortfall in revenue. An alternate approach has been developed that recognises the interdependency between processes in the mine production system including materials loading, haulage and crushing. This approach incorporates a heuristic model based on queuing theory that accounts for the variability in performance of individual processes and that this paper presents the results of a study comparing the conventional approach to mine scheduling with a capacity constrained model at a large open cut mine having a truck and shovel fleet. The capacity constrained model consistently predicted a lower material throughput compared to the conventional approach. Differences in throughput between the two approaches were found to increase with loading time, for example when loading time exceeded 3.9 minutes there was a shortfall of up to 40 per cent in estimated throughput. Longer travel times tended to ameliorate any differences. When applied to a mine schedule that extended over a two and a half year period, the difference was equivalent to a shortfall of nearly eight per cent in total material movement.

The granulation behavior of specularite fines which possess poor ballability was studied with limonite fines as a function of nuclei particles and adhering fines. The results show that the permeability of granules deteriorated as the ratio of specularite fines increased in the mixture, and the optimal granulation moisture content also decreased. The granulation kinetics results demonstrate that the adhering load of specularite fines increases rapidly within three minutes and then levels off. With increasing specularite fines ratio, the adhering speed is accelerated but the granulation duration needs to be longer to reach adhering balance. The microstructure of granule quasi-particles indicates that the fine limonite particles act as binders to bond together the specularite particles which are dispersed in the adhering layers. This can improve the adhering strength of the granules. The operation of a one tonne submerged combustion pilot plant for the recovery of copper from anode furnace and converter slags is described and the effects of variables discussed. Using coal as reductant, copper levels of anode furnace slag were lowered to 1% or less, with batch times of approximately 120 minutes. Fluxing with limestone was required to produce a fluid slag at temperatures of 1200 - 1300¦C. For converter slags, reduction to 0.5% Cu or less was achieved in batch times of 35 minutes. The chemistry of the pyrometallurg- ical recovery of copper is briefly reviewed in relation.to copper recovery methods.

The Mitsubishi Higashitani limestone mine is a ten million tonne per annum operation located near the city of Kitakyushu on JapanÆs western island of Kyushu. Higashitani is one of the largest limestone mines in Japan and accounts for about five per cent of national production. Blasting practices at the Higashitani mine involve the use of bagged ANFO, dynamite primers and non-electric detonators. Regulations in Japan prohibit the use of site mixed bulk explosives. Blast size is usually around 17 000 tonnes with 20 holes in a single row, two or three blasts are fired per day at a powder factor of between 0.25 and 0.32 kg/m3. As part of a program looking at oversize reduction Mitsubishi, Asahi and Orica agreed to conduct a joint project at the Higashitani mine to investigate the relationship between the powder factor and fragmentation. The first stage of the project was to benchmark the current blasting performance. Five blasts were monitored in each of the 210 (hard rock) and 220 (soft rock) areas of the mine. OricaÆs Powersieve« software was used to measure fragmentation and total rock breaker hours were also recorded for each blast. OricaÆs computer blasting simulation model Sabrex« was used to prepare revised blast designs with a higher powder factor and predict their performance. The performance of these blasts was monitored using the same methods.

The current methodology for delineating and subsequently evaluating coal reserves is based on the use of vertical drill holes and surface based geophysics/geology. These techniques have three major problems, these being respectively:-a) Vertical drill holes can only provide a small sample of the coal seam and stratigraphy. Therefore the planar variation in coal reserve and environment parameters can only be approximated.b) Although surface based geophysics has had its fair share of success in recent years, there are still environments which have masked the finding of major structures.c) Access to drill sites is difficult at some locations due to mountaineous terrain, standing water and environmental impact.

The Ocean Drilling Program is the largest international geoscience project in the world, and the state-of-the-art drill ship, the JOIDES Resolution, is permanently at sea. Each year there are six scientific cruises (Legs) which each last approximately two months. These legs are designed by geoscientists from the 19 countries that subscribe to ODP. Australia and Canada share ODP membership under the banner of the AUSCAN Consortium. The Australian Secretariat of the ODP is housed at the Department of Earth Sciences at James Cook University, along with the AUSCAN Consortium office. The Canadian Secretariat is presently based at the University of Toronto. Both secretariats have major research interests in the ore deposits, both ancient and modem, and so there is currently a major drive by the AUSCAN ODP Consortium to make available data and technological information to the minerals industry. This type of information includes data from drilling hydrothermal sites such as the hydrothermally active TAG black smoker complex on the Mid-Atlantic Ridge, drilled during Leg 158, and the Middle Valley sulphide deposit, on the northern Juan de Fuca Ridge, which was drilled during Leg 139.

The ore mined from the Lead Lode of The Zinc Corporation leases consists of a fairly granular mixture of galena, marmatite, chalcopyrite, tetrahedrite, pyrrhotite, pyrite, and lollingite with a gangue association of calcite, quartz, bustamite, rhodonite, mangan-hedenbergite, garnet, fluorite, mangano-calcite and apatite. The silver content of the ore occurs in three minerals tetrahedrite, dyscrasite, and pyrargyrite, the last two mentioned occurring in microscopic association only. Other metallic and gangue minerals are also present in this complex mineralogical association, but do not influence the milling operations.Fortunately, the silver minerals favour the galena and chalcopyrite as their host minerals, thus facilitating the recovery of both copper and silver in the lead concentrate. The ore from the Lead Lode assays from 14 - 18 per cent lead; 3.0 - 4.5 oz silver per tonne, and 9.5 - 12.0 per cent zinc.A parallel lode, known as the Zinc Lode, is well developed in The Zinc Corporation leases. This lode assays from six to eight per cent lead, 1.5 oz silver per ton, 18 - 25 per cent zinc with a siliceous gangue. The common Lead Lode gangue minerals, calcite, rhodonite, bustamite - are very rare in present underground exposures. No metallurgical work was done on this ore during the pilot plant operations.The aim in the milling process is to produce:1. A lead concentrate with the maximum economic recovery of lead, silver, gold, copper and antimony, and minimum zinc content.2. A zinc concentrate with the maximum economic recovery of zinc, cadmium, and cobalt, and minimum iron, arsenic and fluorine content.3. A waste residue containing sufftcient granular material to be utilised as underground fill.In considering the economics of the milling practice the above three requirements plus consideration of the tariffs submitted by the purchasers of concentrates were the deciding factors in the evolution of the present milling practice.Towards the end of 1924 The Zinc Corporation Limited added to and adapted the old zinc tailings plant for the treatment of 5000 tons of crude ore per week. This plant, which operated until January 1936, was in general principles similar to the standard Broken Hill methods of ore concentration, whereby the galena was recovered by the gravity concentration methods of jigging and tabling with selective flotation concentration of the residual galena and marmatite. One important difference from other Broken Hill plants was that the primary grinding of the jig tailing was performed with flint-loaded mills operating in closed circuit with revolving screens. Two primary grinding sections were installed, each consisting of two 5 ft diameter by 10ft long tube mills loaded with flint pebbles working in closed circuit with four 3 ft diameter by 3 ft wide revolving screens covered with 22mesh Toncap woven wire screen with 0.57 mm opening. Each section carried a circulating load of 28.3 tons per hour for a jig.

A short account is given of the theory and practice of determining the true U3O8 content of ore samples by the determination of the beta and gamma activities separately, using the method of Eichholz and his associates.It is shown that, despite the variation of sample activity without change in U3O8 content, the true U3O8 value can still be found.Thorium can be determined simultaneously if this element is also present.

In this report,the results of the large diameter drilling tests in coal seams assisted with water jet are described. The stress relief in coal seams by this new method is as effective as by the conventional method and the penetration speed by this method is 1.5 times as fast as by the other method. 1 Introduction The average mining depth of Japanese coal mines has already attained about 700m from the surface. Almost all coal mines are suffered from occurrence of coal and gas outbursts. To prevent outbursts, some countermeasures are taken generally. In particular, large diameter stress relief borings have been carried out quite often at Akabira coal mine located in the northern part of the Ishikari coal field in Hokkaido,JAPAN. ABSTRACT' The high gas emissions associated with coal extraction at some-Australian collieries have provided a need to investigate the methods used to predict these gas levels and the vent- ilation requirements for gas dilution. It is generally accepted that the introduction of longwall extraction in particular is associated with complex ventilation planning problems. These problems are made much more severe when the colliery introducing longwalls is already facing high gas emissions even during the development stages. A forecast of likely gas emissions during the longwall mining process indicated possible ventilation problems in maintaining the necessary ventilation levels to satisfy the statutory gas limitations for various production rates. Although there are sound principles used in world recognised methods of gas emission prediction a new approach developed from the findings of the Barbara Experimental Mine in Poland appears most suitable for Australian conditions. The recovery of molybdenum at Kennecott Utah Copper (KUC) has become increasingly important as its demand and price have risen dramatically in the last few years. Consequently, the flotation behaviour of the molybdenum bearing mineral, molybdenite, has received a significant amount of attention. The flotation performance of molybdenite generally lags that of the copper sulfide minerals and is much more variable. There are many factors including particle size, liberation, surface slimes and oxidation that contribute to this and also much speculation as to the relative contribution of each of these factors. This paper discusses the morphology of the molybdenite found at KUC, the mineral associations in the different lithogical ores types and the effect that each of these has on flotation performance. Quantitative mineralogical analysis and an assessment of particle shapes are used to explain that flotation behaviour does not necessarily follow expected trends and that some behaviour is counter-intuitive. This unique behaviour can be explained by the morphology and anisotropic nature of molybdenite.

The Whitsunday Volcanics occupy most of the offshore islands from the Whitsunday Group, southeastwards through the Cumberland and Northumberland Groups, possibly extending to the mainland adjacent to Shoalwater Bay, a distance of 370 km. Some of the islands of this group are granitic, but preliminary Rb-Sr data indicate these to represent Permo-Triassic basement. Cretaceous volcanism is interpreted to have occurred during initial rifting prior to the continental breakup of northeastern Australia and subsequent development of the Tasman Basin.The volcanics comprise mainly lithic ignimbrites, with local development of inter-bedded surge and bedded tuff, accretionary lapilli tuffs, lithic concentration zones, and lag deposits. Isolated fluidual rhyolite and dacite domes occur. The deposits are considered subaerially emplaced. A Rb-Sr isochron age for volcanism of 110 Ma is determined from new data. The total thickness of the volcanics is unknown, but a minimum thickness of 4-5 km on Whitsunday Is. is suggested. Eruptive centres are inferred to be distributed throughout the volcanic zone. The volcanics are cut by numerous, dominantly near N-S, dykes ranging from dolerite through to rhyolite in composition.Chemically, the volcanics and dykes exhibit a continuous range of compositions from basaltic andesite, through andesite, dacite, to rhyolite, with minor more alkalic compositions. Phenocryst mineralogy in the silicic rocks is mainly plagioclase, augite, and Fe-Ti oxides, with less frequent quartz and hornblende occurrence. Sr and Nd isoptic compositions for volcanics and dykes are close to mantle values. These data are tentatively interpreted in terms of a lower crustal magma arc source separated from the mantle only a geologically short time prior to Cretaceous magmatism.

The causes and impact of stability problems associated with longhole open stoping (LHOS) at The Zinc Corporation, Limited (ZC) and New Broken Hill Consolidated Limited (NBHC) are discussed and LHOS stability objectives presented. The development of appropriate rock mechanics techniques is discussed and the reasons for using contract research are explained. Reference is made to the rock mechanics techniques developed and in use. The requirements to effectively use these techniques are discussed. The possibility of removing the phos phorus and sulphur contained in steelmaking slag has been studied by observing directly through the microscope melts heated on Pt- filaments. After a CaO crystal was placed in the molten slag, distributions of P, S and other elements formed around the CaO crystal were determined with the aid of EPMA. The results indicated that a layer of 2CaO SiO2 is formed on solid CaO in CaO-Si02 melts. Then between CaO and the newly formed compound the molten slag tends to migrate through cracks. For higher Fe 2 03 contents, the layers of the compounds 2CaO.Si02 and 3CaO SiO2 formed around the CaO crystal are not continuous but look more like particles, which promotes the dissolution of CaO. Phosphorus in slag was found to be enriched in the 2CaOòSi02 layer. Phosphorus in the 2CaO SiO2 phase is not related to the primary slag composition, being about 5-7% P205.. Sulphur in slag,as well as the phos- phorus,was concentrated in the 2CaO.Si02 phase. Disintegration due to weathering observed for slags produced in these investigations appeared to be due principally to hydration of lime.

The Henty Gold Mine, located in Western lasiuania use, uuiusatise solutions to effectively manage a mining operation in an environmentally sensitive setting and has been presented with several environmental awards. Fill is required as part of the mining method to provide passive ground support, minimise rock exposure and ensure maximum recovery of the small but high-grade orebody. The use of the whole portion of leach residue in the backfill reduces the surface tailing disposal requirements. Therefore, High Density Paste Fill (HDPF) has been selected as the most appropriate fill method to meet these objectives. Additional benefits include the minimisation of excess water from fill and the subsequent need for the collection and treatment of water and slimes. There are minimal equipment requirements during placement, thereby optimising mine resources for production. The initial paste fill design was based on the Bad Grund and Ranunelsberg Mines paste fill systems of Preussag in Germany and comprised pumping filtered tailings paste some 1.2 kin uphill from the mill to the mine portal, where cement was to be added before being distributed underground. The system was designed following initial testwork that was undertaken on a bulk sample of ore mined from the scar surface Sill Zone. Subsequently, during the mining of the Zone 96 orehody, it was found that there were significant mineralogical variations in the composition of the ore mined. Following processing, the resultant Icach residue caused significant changes in the paste rheology and therefore the pump and flow characteristics. An alternative solution was implemented that involved the use of agitator trucks to transport the cemented paste to the portal. This required modifications to the fill preparation system and also the underground distribution system. Laboratory strength tests of the HDPF using actual gold plant leach residue showed low early strengths were attained with HDPF containing only filtered leach residue. Therefore, high binder addition rates or an increase in the fill cycle time would be required to allow the fill to gain sufficient strength. A considerable amount of potentially acid generating waste rock also requires underground disposal. By blending the waste rock with the HDPF, the strengths were improved and the required cement addition rate reduced by about one-half. The potentially acid funning waste rock is encapsulated in the highly alkaline cemented paste, minimising the potential for acid generation.

Geonechanical modelling of island arc sys- tems and their analogs is investigated. Slow seismotectonic movements along the inclined deep fault zone under the compressive horizontal stress conditions are proposed to be the princi- pal mechanism controlling the structures and processes in the active continental margins. A model of shearing surface crack in an elastic half-plane is used for simulation. The main ele- ment of the model is viscous interaction bet- ween the crack sides, the viscosity varying with depth. To describe the stress-strain state of the medium an integro-differential singular equation is derived and solved numerically. The results of simulation are in good agreement with geological and geophysical data on the ac- tive continental margins.

A continuous casting machine, of the Concast curved mould design casting billets in four strands, was commissioned at the Newcastle Works of The Broken Hill Proprietary Co. Ltd., in November, 1967. Quality assessment of early casts from this plant indicated that steel cleanliness was sometimes inferior to that of equival- ent ingot cast steel. Extensive trials to develop a modified deoxidation technique were not successful in improving clean- liness to the desired level. A "contra-flow" gas shroud, to cover the steel stream between the tundish and the mould, was developed, and proved successful in achieving the required levels of product cleanliness. The inclusion content of six grades of steel ranging in carbon content from 0.10 to 0.70 per cent is examined, and an attempt is made to explain theoretically some of the results obtained.
