Problem: It is estimated that 5 to 10 arc flash explosions occur in electric equipment every day in the United States. In the mining industry the largest single injury category of electrical injuries are caused by non-contact electrical arcs. Method: This investigation progressed in two phases: (a) 836 Mine Safety and Health Administration (MSHA) reports of electric arcing incidents that occurred over a period of 11 years were reviewed, and (b) personal interviews were conducted with 32 individuals. A theoretical Safe Job Performance Model guided the study. Results: Behavioral dimensions were identified and included the effect of worker experience, judgment and decision-making ability, behavioral and organizational controls, and safety culture. Summary: The National Institute for Occupational Safety and Health (NIOSH) conducted an investigation of behavioral components associated with arc flash incidents and developed recommendations for interventions based on findings. Impact on Industry: This study fills a vacuum in electrical training with a focus on the organizational and behavioral aspects of arc flash incidents. There search is cross-cutting in its scope, in that the results apply not only to mining and construction, but many other industries employing electricians. Although the majority of mine electrical injuries are the results of burns from electrical arcs, few miners are aware that such a hazard exists. A safety training program, which includes a video and an instructor's discussion guide, was developed for electricians based on this study's findings. “Arc Flash Awareness” was released in 2007 (DHHS NIOSH PublicationNo.2007-116D) and is available through1-800 CDC-INFO. SUMMARY The objective of this program was to develop a system to extinguish coal mine fires by remote sealing of the underground passageways. The system developed had to be versatile enough to function under a wide range of operating conditions, including large variations in passageway geometry, depth of mine, water conditions, both in the mine and at the surface, and rubble in the mine passage. The system developed had to include instrumentation for the remote probing of the passageway prior to sealing, a system for sealing the mine passages, and instrumentation for remote monitoring of the quality of the seal during and after construction. These objectives were successfully achieved during this program. The remote mine sealing capability of the system was satisfactc rely demonstrated in a field test conducted at the Eastern Associated Coal Corporation Federal No. 1 Mine in Grant Town, West Virginia. The remote sealing consists of: (1) A sonar initial probing system which can operate satisfactorily even in the presence of dense smoke. The system is capable of determining both the local passageway geometry, identifying the presence of cribs and other obstructions, and the position of the borehole in the mine passage. (2) A mine sealing material system which utilizes fly ash for making the complete seal or as a bulk filler in conjunction with a urethane, froth foam, seal topping system. The choice of using fly ash alone or fly ash with foam topping depends on a number of factors, such as quality of seal desired, time available per seal, depth of borehole, passageway geometry, and water holding require¬ment. The Bureau of Mines has developed a rapid analytical method for the determination of anions by ion chromatography (IC) in phosphate rock mining materials and cement kiln dust (CKD). The procedure employs an Na2C03 fusion of the sample, followed by a water extraction that allows direct injection of the sample into an ion chromatograph without separator column poisoning by the transition metal ions. Fluoride (F-) and phosphate (P043-) were determined in 24 phosphate rock processing and waste material samples by the IC method. The results were compared with a pyrohydrolytic distillation-titration method for F-and a colorimetric procedure for P043-. Excellent analytical agreement was obtained for F-and p043-values determined by IC and the comparable wet chemical methods. Fluoride values ranged from 0.3 wt-pct in the filter cake to 3.6 wt-pct in phosphate rock concentrate. Phosphate values ranged from 1.2 wt-pct in the filter cake to 33.8 wt-pct in the rock concentrate. Fluoride, chloride, phosphate, nitrate, and sulfate in several CKD samples were determined by the IC method and standard wet chemical techniques. Excellent agreement was obtained between the two methods for all anions. A total of 113 CKD samples from all areas of the United States was analyzed by the IC method. Fluoride values ranged from <0.01 to 0.36 wt-pct. Chloride values ranged from <0.01 to 12.5 wt-pct. Sulfate values ranged from 0.41 to 31.6 wt-pct. Phosphate and nitrate were detected in only about 25 pct of the samples at >0.02 wt-pct. Maximum phosphate and nitrate values were 1.0 and 1.6 wt-pct, respectively.

Minimize drilling time in roof bolting. Approach The Bureau of Mines conducted a survey of 34 roof bolting machines used in underground coal mines. Results of the survey indicated that drilling time, bit cost, and respirable dust concentrations and be reduced by using the appropriate bits for drilling, To evaluate bits currently used and to find optimum cutting parameters needed to minimize drilling time, the Bureau tested two bits commonly used in most underground mines on both wet and dry systems. These are the dusthog* and shank type. The dusthog has dust ports in the bit body to remove cuttings. With shank bits, holes in the drill steel do this. Cost of each type is roughly equal. Bolters with dry vacuum systems were tested in the laboratory first. The lab tests were verified by underground tests in several mines. Bolters with low-pressure wet drilling systems were also tested under-ground. Tests concentrated on the effect of rpm, water and airflow rates, heating, wear, bit design and bit type. Lab Results -Dry Drilling Holes were drilled during the laboratory tests using a production model bolter drilling 45-inch-deep holes into mixtures of lime stone and sandstone aggregate. All tests started with new shank and dusthog type bits at 225 and 450 rpm. Thrust was about 6,000 lbs in all tests. Each bit was tested at two airflows, 15 and 25 cfm, airflows which are typical on operating roof bolter bits. Eleven styles of bits were evaluated. On the average, dusthog bits drilled 25% faster than shank types and were always cooler after drilling. The effect of RPM was tested with 5 different bit types. When bit rpm was increased from 300 to 400, drilling rates doubled. With used, well-worn bits drilling time and final bit temperature were more than doubled.

The Bureau of Mines recorded and analyzed overpressure levels for three production blasts at the White Pine copper mine, Upper Michigan, to obtain representative data from single-heading blasts in a long, straight haulageway and to estimate the damage potential inherent in production blasts fired in close proximity to underground structures. Commercial pressure gages with a flat response to 1 kHz were used with an FM tape recorder to record the overpressures from one dynamite and two AN-FO (ammonium nitrate-fuel oil), 500-msec production blasts having zero-delay charge weights of about 75 lb (34 kg). The recorded waveforms from the underground blasts (1) were more complex than the typical free-air wave, (2) were of longer duration, and (3) had numerous secondary arrivals due to reflections from the ribs and roof. The waveforms showed that the zero-delay charges produced the only high overpressures. First pulse peak overpressures from the blasts ranged from 0.10 lb/in2 (6.9 X 102 N/m2) at a distance of 471.1 ft (144 m) to a maximum of 1.76 lb/in2 (121.3 X 102 N/m2) at 191,7 ft (58.4m) from the blasts. An analysis of variance of the first pulse overpressures from the three White Pine blasts showed that no statistically significant difference existed among the overpressure levels from the dynamite and AN-FO blasts and that a single equation P = 4.9 x 103 (D /W1 / 3) 2 .15, where P = overpressure in pounds/inch2, D = d-stance from the blast in feet, and W = zero-delay charge weight in pounds, could be used to describe the falloff of overpressure with distance.

"The belief is now fairly general among the coal-mining people of the United States that one of the most effective methods of preventing wide-spread coalmine .explosions is that of introducing rock-dust into coal mines to reduce the inflammability or explosibility of dusts found on mine surfaces. Notwithstanding this belief it is improbable that even a small fraction of 1 per cent of the coal mines of the United States are at all adequately protected by rock-dust; in a survey made in the summer, and fall of 1927 it was found that but 463 out of more than 7,000 operating bituminous and lignitic coal mines were using rock-dust at all, and that few if any of the 463 were fully or adequately protected by rock-dusting.Inspections of a large number of mines through the United States by Bureau of Mines safety division field men have failed to reveal a single adequately rock-dusted mine, Yet there is no doubt that many of the owners and operators believed their properties to be fully protected by rock-dust against the much-feared hazard of widespread explosion. Two of the most disastrous explosions of recent years occurred at properties where the owners were definitely of the belief that the rock-dusting done had made the mines immune to widespread explosion involving coal-dust. In both instances it was found that although some rock-dusting had been done, the mines were more nearly 1 per cent than 100 per cent rock-dusted.This condition as to inefficiency or insufficiency of rock-dusting in the coal mines of the United States is almost universal, and unless our mining men awaken to its, dangers, the disaster record of the coal-mining industry is much more likely to become worse than better. During the fiscal year ended June 30, 1928, the attention of the safety division of the United States Bureau of Mines was called to. 22 explosions With 320 fatalities in bituminous coal mines of the United States; and of the 22 explosions there were 10 with 258 fatalities in mines that were listed as having used rock-dust; upon investigation it was found that as usual the rock-dusting had been anything but comprehensive or adequate. It is probable that in a very few instances last year the relatively small amount of rock-dusting stopped the .extension of the explosion and thus prevented further expansion of the death list. In general, however, the explosions occurred in a part of the mine that had not been rock-dusted - and right here is the crux of the failure or probable failure of present-day rock-dusting in the United States, There should be no part of any bituminous or coal mine in which all of the exposed surfaces (roof, ribs, and floor) have not been thoroughly rock-dusted and kept thoroughly rock-dusted. In other words, all exposed surfaces of all accessible places in bituminous and lignitic coal mines should be kept covered with rock-dust so that the combined rock-dust coal-dust mixture at all times and in all places has less than 35 or 40 per cent combustible matter, The only exception to this rule is that surfaces which are wet (and this does not mean merely damp) need not be rock-dusted. However, a mine opening may have several inches of water on the floor and yet have much explosive dust on the ribs and roof above the water; in such places the ribs and roof should be kept rock-dusted, irrespective of the wetness of the floor."

"Abstract. This paper attempts to characterize coal breakage due to the action of a continuous miner. Coal blocks with an approximate dimension of 45.7 cm x 38.1 cm x 15.2 cm (18 in. x 15 in. x 6 in.) were first subjected to confining pressures equivalent to in-situ conditions. Then the blocks were cut along the face/butt cleat direction by the cutting head of a specially designed unique automated rotary coal cutting simulator (ARCCS), simulating the action of the continuous miner in underground coal mining. During the tests under a particular set of in-sit and operating parameters a number of other para¬meters such as 1) penetration of bit into coal, 2) penetration resistance (thrust and cutting pressures), 3) rotating velocity of cutting head, and 4) acoustic emission activity in the coal block were monitored. After each cutting cycle the fractured surfaces were photographed and a velocity survey was conducted by using a sonic technique. At the end of each experiment, the coal fragments were collected in bags using a vacuum cleaner. These fragments were then sieved for size distribution analysis.INTRODUCTIONThe Federal Coal Mine Health and Safety Act of 1969 was enacted to emphasize to coal operators that the airborne respirable dust should not exceed 2 mg/m3, thus reducing the incidence of coal workers pneumoconiosis.Dust control techniques such as conventional water sprays and dust collectors are only partially effective and require additional equipment expenditures. A more authentic approach would be to reduce respirable dust at the source the continuous mining machine cutting head, where the fragmentation process occurs. Improving the fragmentation process by characterizing the coal breakage will not only reduce respirable dust at the face, but it will also decrease the amount of respirable dust that is liberated during the secondary handling.Fragmentation of coal can be expressed as a function of three major groups of parameters; namely, 1) coal properties, 2) in-situ conditions and 3) operating parameters. This paper deals with the characterization of coal breakage along both face/butt cleat direction as a function of machine operating parameters such as rate of advance, angle of attack, bit configuration, bit lacing (spacing and pattern) and bit speed at various equivalent in-situ pressures."

"Part IV.- The Resistance Caused by Right-Angle Bends. IntroductionThis paper deals with the fourth phase of an extensive research on coal-mine ventilation factors. A comprehensive report of the entire work is in course of preparation, but abstracts of the various chapters are being prepared in order to make the major results public as rapidly as possible. Parts I, II and III were issued as Serials Nos. 2621 (July 1924), 2647 (October 1904) and 2671 (March 1925) and dealt with the resistance of unobstructed entries, the resistance due to mine cars and, the resistance of timbered entries respectively.Description of the Tests With the last cutthrough between the two butt entries of the test section in the Experimental Mine open, the air flow was deflected, around two right-angle bends in the same direction, the butt entries being 50 feet center to center and approximately 6 by 9 feet in cross section. Each bend had a dead end opening off of it; that at the first bend was in the direction of flow and that at the second bond was at right angles to the flow. For a primary test condition the inner corner of each bend was trimmed square and the dead ends were closed off with board brattices set flush with the line of the cut-through.The two corners of the first band were, then modified both independently and in conjunction. Various modifications of both bends in conjunction were also tested with rather surprising results as to the effect of conditions at the first bend, upon the pressure losses at the second. To confirm the theory developed to fit these results a series of tests were made to give a direct comparison of the -pressure losses as between two bends with the air flow deflected in the some direction at each and with the air flow deflected in opposite directions. The former is designated ""double"" and the latter a ""reversed"" right-angle bend. This series of tests was conducted, however, on the bends at one of the intermediate cutthroughs at which the flow could be deflected us either direction at the second, bend. Here also the average cross-section was 6 by 9 feet and the bends were 50 feet apart."

"The Bureau of Mines evaluated the long-term cost and availability of primary gold production from 135 mines and deposits worldwide. Collectively, the evaluated countries represent 93 pct of world gold production. Total recoverable gold available (as of January 1984) from a subset of 111 significant producing mines and developing deposits in 13 market economy countries (MEC's) is estimated at 810 million tr oz. The Republic of South Africa is estimated to account for 87 pct of total recoverable gold. The United States and Canada account for 4 and 4.5 pct of the total, respectively. Eighty-three percent of total recoverable gold is available at a constant 1984 break-even price of $400/tr oz, and 70 pct is available at $300/tr oz. South Africa accounts for 90 pct of the gold available at $400/tr oz or less.Major conclusions are that (1) South Africa should remain the largest world producer through the year 2000, (2) annual MEC output in 2000 should not be significantly different than current output given constant 1984 gold prices of more than $300/tr oz, and (3) there may be a significant decline in production after 2010 in the absence of major new discoveries and continued development of new mines to offset the depletion of the South African mines. INTRODUCTIONSince the late 1970's the world gold mining industry has been characterized by dynamic growth in production and volatility in price. World production in 1983 was almost 15 pct greater than in 1980, and the short-term trend continues upward. Annual dollar-based price changes since the early 1970's have been unprecedented in both magnitude and volatility. This commodity, which sold for $35/tr oz3 for over a third of a century, reached an annual average $612/tr oz in 1980. Until 1980, the future price of gold seemed set on an upward trend. Since that year, however, world gold prices have fallen by over 50 pct to slightly over $300/tr oz at the end of 1984.Against this background, this study was undertaken to determine (1) the level of world demonstrated gold resources, (2) total and annual availability of gold mine production, (3) long-run production costs per operation and by national aggregates, (4) the economic and technical factors that impact upon cost and availability, and (5) probable future mine production and cost trends."

This report presents a description of the Prototype Demonstration Vehicle and Safety Guidelines developed under U.S. Bureau of Mines Contract No. JO 366003, ?Improving Man Transit Safety. This vehicle was constructed to assess the feasibility of a set of equipment related guidelines which were drafted in Phase I of the same contract. State-of-the-Art improvements are embodied in such areas as collision survivability, occupant comfort, and multiple mode (on and off rail) operation. The electric battery powered vehicle carries a twelve man section crew at speeds up to 12 mph. The description of the demonstration vehicle Is presented In Volume I. Safety Guidelines are presented in Volume II.

This report describes two goniometers that, by simple manipulation, can be used to ascertain the attitude of a joint, vein, or fracture plane observed on the planar projection of a borehole photograph taken with an NX borehole camera of the type developed by the U.S. Army Corps of Engineers. One goniometer was designed to interpret photographs taken in vertical drill holes and the other for photographs taken in inclined drill holes. In practice, several measurements are made on each photograph with the goniometer. These data are then substituted into mathematical equations, which are derived in this report and the attitudes of fracture planes are determined.

Primary crushing has received comparatively little attention in the technical literature in spite of the fact that the design of the whole crushing plant should be correlated to the performance of the primary crusher.

The petrographic microscope plays an important role in the mineral dressing of nonmetallic materials. Petrographers and mineral technologists have evinced an interest in the technique developed, specifically, for mineral-dressing problems. The Bureau of Mines investigated techniques for dewatering slurries from talc-processing operations to improve water reclamation and pro-duce dewatered solids suitable for disposal. Various flocculating reagents were evaluated, and a nonionic polyacrylamide was found to be most effective in a specialized flocculation dewatering technique. The flocculation dewatering method was applied in laboratory batch tests and in continuous tests using a static screen, a belt-roller device, and a clarifier-thickener laboratory unit. In continuous tests using the unit, wastes of 3.5 pct solids were dewatered to 56.9 pct solids using 0.03 Ib of flocculant per ton of dry solids treated.

The effects of impurity elements silicon , aluminum, copper , nitrogen , and manganese , at 50 to 300 ppm, on properties of the zirconium alloy Zircaloy 2 were investigated . Variations in microstructure , mechanical properties , and corrosion behavior in some inorganic acid and salt solutions were observed with variations in level of impurity element . Corrodents were solutions of sulfuric acid , phosphoric acid , copper chloride , and iron chloride . Silicon was found to modify annealed microstructures by spheroidizing and dispersion of secondphase material throughout grains . Silicon or manganese improved ultimate and yield strength of the alloy without loss in ductility . All five elements affected corrosion rates of the alloy , the effect being generally different for each element .
