"We hear all the time about the value of hard work and perseverance, and the part they play in the professional life of successful people. What we do not hear so much about are the many professionals who work hard and persevere, yet never seem to quite reach the level of success they aim for. Some current authors are beginning to give more attention to the part luck/black swans/outliers play in success. This is not a new phenomenon, Machiavelli had a lot to say about it back in the Renaissance. So how can knowledge of this dynamic help in a modern organization? INTRODUCTION Work hard and you will succeed. Go to the right school, major in the right degree, study hard, and get a good job. Look at the lives of successful people, and do what they do. Be all that you can be. You can be anything you want to be. Really? Those who are successful always get attention, people want to know what they did to reach such a high position. Invariably, at least part of their narrative will include hard work, wise insight, taking advantage of opportunities, etc. If they are of a more generous nature, they might also allow that building the right friendships and collaborations also played a role. All of this may be true; what we do not often hear are the stories of those who did all of these very same things, but did not find success. An example from pop culture: The Voice. I have watched this show occasionally the past couple of years. One of the things I like about the show is that the initial screening of contestants is based solely on their singing voice (hence the name of the show), and not on their appearance. At the time of this writing (early November), the judges have just finished elimination rounds for their final teams to go on to the live shows. Here is my point—in almost every case the judges agonized over choosing who would go forward and who would be eliminated. And the reason is because all of them are really talented singers. It is impressive how gifted they are. At the end of each season, only one is declared a winner, out of that gifted pool of singers. When you hear their stories, they are all talented, hard-working individuals sacrificing for their art. And as far as I know, none of them has ever become a star. Hard work, dedication, sacrifice, talent—sometimes it is enough, sometimes not. I will be looking at the part uncertainty/luck/black swans play in our lives, but I want to make it clear at the beginning that I am not saying everything that happens to us is out of our hands or beyond our control. Taleb makes this same point (2007, p. 52): “This problem is chronic: if you tell people that the key to success is not always skills, they think that you are telling them that it is never skills, always luck.”"

One of the major ignition sources of fires and explosions in underground U.S. coal mines is flame cutting and welding. On May 20, 2006, a flame-cutting operation at the Darby Mine No. 1 led to an explosion that resulted in five fatalities. On Jan. 22, 2003, another explosion caused by flame cutting at the McElroy Mine resulted in three fatalities and three serious injuries. On June 22, 1999, a flame-cutting and welding operation at the Loveridge Mine caused a fire that required sealing the mine for an extended period. On March 19, 1992, an explosion initiated by welding at the Blacksville No. 1 Mine resulted in four fatalities. These examples of mine incidents demonstrate the need to conduct research to develop best practices for safely conducting flame cutting and welding in underground coal mines. The National Institute for Occupational Safety and Health (NIOSH) conducted a study on fires and explosions in underground U.S. coal mines that were caused by flame-cutting and welding operations to determine the root causes of these types of incidents. The methodology included interviewing mining personnel who perform flame-cutting or welding operations in underground U.S. coal mines and visiting mines to observe these operations. In addition, MSHA reports of investigations and accidents statistics were analyzed. The findings were used to identify and compare differences between flame-cutting and welding practices and techniques in small and large mines, eastern and western mines, low-seam and high-seam mines, room-and-pillar and longwall mines, and between experienced miners and new miners. As a result of this study, best practices were developed to reduce the number of ?res and explosions caused by flame cutting and welding. This paper provides a summary of this research.

"Coal bumps, also referred to as dynamic failure events or coal bursts (Lawson, Weakley, and Miller, 2016), continue to be a hazard in underground coal mines. While current design methods have significantly reduced the occurrence of coal bumps, they have not been completely eliminated. This paper investigates a correlation between coal compositional components, coal molecular structure, and probability of coal bumps in an effort to deepen understanding of coal behavior in an effort to reduce hazards in underground coal mines.Samples from the Penn State Office of Coal (PSOC) Department of Energy Coal Samples (DECS) were obtained for Fourier Transform Infrared Spectroscopy (FTIR) analysis using the KBr pellet method. Peaks indicative of the coal molecular structures were obtained using spectral integration software. These data, combined with published elemental data, were compared to historic bump occurrence in order to establish a possible correlation. Finally, results based on multivariate analysis of the elemental and FTIR data with bump status as the dependent variable suggest that elemental sulfur and molecular structure can be used as an indicator of bump susceptibility within the sample set considered.INTRODUCTIONThe study of coal requires consideration of multiple variables: percentages of organic and inorganic components, organic component origins, coal age, pore structure, cleat density, elemental composition, etc. Examining coal in situ increases the number of variables, such as coal depth, seam thickness, surrounding stratigraphy, and so forth. This collection of variables influences the subsequent behavior of coal simultaneously, and weighing their relative impact is a challenge for researchers. “A problem never exists in isolation; it is surrounded by other problems in space and time” (Ackoff, 1956). Having a substance such as coal that we know so much about, and yet so little, we have to ask what can be learned.An experimental study using Fourier Transform Infrared Spectroscopy (FTIR) was undertaken to determine if the chemical structure of coal contains information which may lead to a better understanding of triggers or attributes which may, in turn, lead to an increased probability of a dynamic failure event. While a full analysis of the factors driving dynamic failure are beyond the scope of this study, it is possible that a better understanding of the chemical structure of coal will lead to an increased knowledge of its inherent behavior, including its propensity to burst."

In tunnel condition assessment, the trend is towards comprehensive 3D measurement combined with high quality image texture to document and quantify damages to the tunnel surface and to create a cohesive data base of the structural conditions throughout its lifespan. The faster such measurements can be made on site, the less it impacts traffic conditions of the highly sophisticated subway systems. The Dibit high-speed 3D scanning system which is based on Photogrammetry and LiDAR technology enables data acquisition in tunnels at a speed of up to 60 mph. Given the high rate of speed the system can operate, tunnel shutdowns can be drastically reduced, and tunnel safety increased. The photorealistic texturing of the 3D-model allows the identification and analysis of smallest material damage (e.g., cracks ≥0.3 mm wide). The publication illustrates technical details and 3D results of the innovative system from measurements that were performed in the subway tunnels of the Bay Area Rapid Transit System in San Francisco, CA (total length 66.23 km).

This paper presents preliminary results of a Bureau of Mines study of potential costs of compliance with the federal Resource Conservation and Recovery Act for copper heap and dump leaching. Specifically, estimates are given for the cost of compliance with possible technology standards that would require the use of liners and closure caps regardless of terrain and underlying geology and hydrology. This cost is compared to the co lit of compliance with present state requirements for terrain a typical of those found at existing copper leach sites in the Southwestern U.S.A. Incremental compliance costs are found to widely exceed screening criteria used by the Environmental Protection Agency to denote significant economic impact.

Western mining companies are entering an African critical minerals market where access is harder to secure, risk is being priced more sharply, and execution advantages are increasingly decisive. The continent holds significant reserves and production potential of cobalt, lithium, copper, graphite, manganese and rare earths critical to the energy transition, yet Chinese-aligned capital has built a structural edge in access and execution speed. Simply signing narrow offtake agreements to buy output is no longer sufficient. Western firms win when they convert quality orebodies into bankable projects by reducing aboveground risk faster than competitors through integrated infrastructure access, durable fiscal alignment, and credible local value creation. Western mining companies will break into Africa by pairing capital with partnership, building corridors enabling infrastructure and power, designing credible value addition pathways and operating in a way that is politically durable through transparency, community benefit and fiscal terms that share upside in commodity upcycles. This feature article offers a practical playbook for mining executives and investors. The core argument is straightforward. Winning in Africa requires competing on the full system around the mine. Corridors and power, processing pathways, contract durability, community benefit and regulatory execution are now decisive variables in project valuation and capital access.

The degree of economic, progress of a nation is measured by its possession and use of energy, sources. The future of coal in the West hinges on its ability to participate and grow in the energy market. A source of energy is, in the long run, only as good as the ability of the industry to develop its reserves economically. The economic development of the tremendous coal reserves, literally hundreds of billions of tons of coal in the States of Colorado, Utah, Wyoming, Montana, New Mexico, and Arizona, poses a very real question in the light of the known mining technology of our day. Basically in America today, coal is mined from the earth either by the strip mine or underground mine method; in the West, the large long-term, contracts now in force with utilities are mostly from coal mines utilizing the strip mine method. However, the preponderance of the known coal reserves of the West are located deep underground and will have to be mined by the various; underground mining processes. The problems encountered in the West with regard to underground mining vary from state to state but, on the whole, do not present what might be considered a miner's bonanza. I do not mean to' infer that the underground coal of the West is unmineable, but I do want to emphasize that the tremendous reserve figures do not present a true picture in the light of present day technology.

One of the major concerns of ground control in a coal mine is safety against failure of one of three components (i.e. roof, pillar, or floor) of the mine entry. In order to better design and evaluate stability of the mine entry with a certain confidence, a reliability approach is introduced rather than using the conventional safety factor (SF) method only. Advantages of using the reliability approach is that it not only takes variability of both rock/coal properties and stress distribution into consideration, but also assesses the stability of each component and mine entry as a whole system. Therefore, it is anticipated that, by using this approach, the current ground control technology in coal mine will convey a better sense of confidence. The results of a case example show a significant influence on the current design concept and stability evaluation of mine entry.

A cement, in the broad sense, is any substance which will cause various materials to adhere to each other. Clay was probably the first material to be used by man as a cement or binding agent. Water occurs in or is mixed with the clay to make it plastic, so that it can be formed in place. The sun and air remove the water from the clay and on drying strength is developed. The clays unless fired are not too resistant to water, therefore, their use is best when restricted to dry areas. Gypsum used as a cementitious material is not the subject of this paper, but its use dates back thousands of years. Gypsum mortars were used by the Egyptians in building such massive masonry structures as the pyramids. The Egyptian mortar consisted of a mixture of sand with burned gypsum (of an impure type of 81.5 per cent calcium sulphate and 9.5 per cent calcium carbonate) for the cementitious material. The burning of this type of cementitious material took place at relatively low temperature. These burning reactions will be covered later when the dehydration of gypsum is discussed. The setting of the partially dehydrated gypsum is considered to be crystallization from a supersaturated solution which deposits crystals of dihydrate (gypsum). In low water-content mixes, the interlocking of the needle-like crystals helps to produce higher strengths. Slaked lime made from limestone burned at higher temperatures (1648 F is generally considered as the calcining temperature, although under certain conditions incipient calcination will take place at 1250 F) is another early-period cementitious material. During slaking with water, a colloidal or amorphous hydrated lime is formed. When it is allowed to dry, it hardens and develops low strengths. On exposure to the carbon dioxide of air in the presence of some moisture, carbon dioxide is adsorbed and part of the calcium hydroxide is converted to calcium carbonate. Structures in which a lime mortar is used continue to grow in strength as this reaction proflesses. Hydrated lime has the disadvantage of not setting under water.

The world's largest grinding mill operates at the Cadia Hill Mine in Australia. Svedala Industries supplied the 12.2-m- (40-ft-) diam, 20-MW Cadia gearless, semiautogenous grinding mill (SAG). This was a leap of more than 40% above the largest operating SAG mill. This article reviews the significant items involved in the development, design, manufacture, installation and operation of this mill. Description of the Cadia project The Cadia Hill Mine is located 25 km (15 miles) southwest of Orange, in the Central Tablelands of New South Wales, Australia. The mining lease covers about 3,100 hm2 (7,660 acres) of the Cadia Valley. The Cadia deposit is a large, homogeneous, low-grade, porphyry-type, gold-copper resource. Current reserves stand at 202 Mt (222 million st), grading 0.73 g/t (0.02 oz/st) gold and 0.17% copper. They contain 146.2 t (4.7 million oz) of gold and 347 Kt (382,500 st) of copper. The operation consists of openpit mining and ore processing to produce gold bullion and a gold/copper concentrate. The operation processes 17 Mt/a (19 millon stpy) of ore to produce 91 kt/a (100,300 stpy) of concentrate and 9.1 t/a (293,000 oz/year) of gold. The plant operates two 12-hour shifts a day and had a targeted availability of 94%. The plant facilities include a copper concentrator, o water supply system, an openpit mine, tailings pipeline and dam, and a concentrate pipeline and dewatering facility located in Blayney, about 35 km (22 miles) from the concentrator. Significant features of the process plant include o primary crusher and stockpile, a reclaim conveyor system feeding, the Cadia SAG mill (Figs. 1 and 2) and two 6.7-m- (22-ft-) diam ball mills (Fig. 3). A portion of the gold is extracted in a gravity circuit. The balance is extracted with the copper in the flotation section of the plant. Concentrate is thickened and piped about 35 km (22 miles) to the filter plant and rail loadout facility located in Blayney. Tailings are pumped to a tailings dam and process water is reclaimed from the tailings dam. The water system is designed to control and capture available runoff from local creeks, and to store these waters in dams and weirs. In addition, a major portion of the water for the process plant is treated effluent from the city of Orange and a smaller portion from Blayney Mill selection. The low-grade ore posed a dilemma for Cadia. To achieve a satisfactory economic operation, large-scale processing was required. The selection of the mills, equal to the largest designed and operated at the time, gave a plant configuration consisting of two 11.1- x 5.8-m (36- x 19-ft) SAG mills and three 6.1- x 9.1-m (20 x 30-ft) ball mills. This configuration did not provide the most satisfactory economic model for the project. A review of the single line configuration indicated that a significant reduction in the overall capital cost of the plant could be achieved by increasing the size of the mills and reducing the quantity. New and developing communication systems enable underground miners to not only connect to the surface from an increased communication coverage area but also to be discovered in the event of an emergency. Communication and tracking systems are required by MSHA regulations, aiming to enhance health and safety and routine production in underground operations. Modeling a mine?s potential communication coverage area will increase overall coverage and efficiency of the communication network itself. Modeling the propagation of wireless communications enables ideal broadcast locations to be found when designing a communications network. Strategic placement of broadcast devices results in a reliable communication network that can better withstand disruption in both daily operation and emergencies. This paper will discuss recent regulatory developments in underground coal communication systems, the implementation of these new technologies, and how communication system?s networks can be modeled and analyzed using computer simulations.

The main reasons for the development of higher heat levels in German coal mines are the increasing depth of working with the con- sequent higher rock temperatures, increased use of electrical equipment, and increasing coal output due to concentration and mechanisation of workings. At the turn of the century the keys to industrial success were coal and iron. At that time, Pennsylvania was the ranking mineral producer among the States and close to 90% of the Nations energy output was generated by coal. The growth of this Common-wealth into the greatest industrial State in the nation has been due to the huge resources of easily accessable coal found by the early settlers. It was the development of these deposits that provided the United States with a new source of fuel energy. For nearly a century, the Commonwealth was the chief powerhouse of our nation and the chief source of its industrial blood and bone. In 1928 it was truly said that coal was king in Pennsylvania. Pennsylvania that year mined a billion dollars worth of coal. A single train carrying this output would reach more than twice around the world at the latitude of Pennsylvania. Coal formed nearly 67% of the freight of Pennsylvania and was responsible for its four billion dollar iron and steel industry. Coal was the real reason why Pennsylvania although 32nd in size was second and not tenth or fifteenth in wealth, population and industry. Pennsylvania was slow to take-up her heritage as the greatest coal producing State in the union. But the knowledge that coal was present was not slow in accumulating. In fact the Indians were aware of its existence long before the white man arrived. They knew that many of the hills were underlain with black stones that would burn. It is reasonably certain that Arnold Viele, the first white man to cross the mountain in this State saw coal in them in 1692. But awareness was not enough. The population in these days was confined to southeastern corner of the State and the population at this time had little need or interest in the resources further inland. Pennsylvania anthracite was discovered in 1762 but was first regarded only as a curiosity. Commercial productions started in 1820. This stimulated the development of many native industries and extensive canal system and afterwards an elaborate net-work of railroads were constructed at great costs to bring the products of the hard

The feasibility of maintaining equipment location, personnel location and vital statistics in surface and underground mining operations has increased with advances in radio frequency identification (RFID) technology. The paper addresses the use of this technology, relatively new in the mining industry, to track surface equipment in mine pits, loading points, and processing facilities. Specific applications are discussed including a simplified and a complex truck tracking system and an automatic pit ticket system. The paper concludes with a discussion of the future possibilities of RFID technology in mining, such as monitoring biofeedback of personnel (heart and respiration rate, body temperature, exertion level, etc.) repetitious movements to study work habits, and logging air quality via personnel sensors.

In June 1998, the world's largest grinding mill, the 40 ft (12.2 m) diameter 20 MW Cadia gearless SAG mill, was commissioned. This was a leap of over40% above the largest operating SAG mill. A significant saving in capital cost gave the incentive necessary for a single mill line at Cadia, resulting in the selection of the 40 ft (12.2 m) diameter mill. Now, after 18 months of operation, this paper will review the significant items involved in the development, design, manufacture, installation, and operation of this mill.

John Marks said in his acceptance of the Hartman Award in 2008, “I guess that without the occasional complaint, your mine is probably over-ventilated” (Marks, 2008). Miners are seldom in this situation and spend much of the time trying to find enough air to keep an operation running. With examination, there are usually changes that can be made to increase the efficiency of ventilation systems. Mine ventilation air is a costly commodity, especially by the time it has been heated or cooled and moved to the bottom of the shaft and through the mine airways. Modifications that result in surplus mine ventilation air could bring financial and environmental benefits. Some issues mine operators have needed to address recently are increased airflow requirements to dilute diesel particulates in metal and nonmetal mines, to ventilate reserves that are deeper, hotter, and further away from fans and shafts, and to meet demands from increased production. This paper discusses successful methods to find additional usable air when there is no extra ventilation capacity. Most of the options presented in this paper are useful for metal/ nonmetal operations but some may be applicable to coal as well.

The Otjikoto Gold Mine in the Republic of Namibia is located approximately 300 kilometers north of the capital city of Windhoek. The project features surface mining with a modern gravity/carbon-in-pulp mill for ore treatment and recovery of gold. Initial mill design capacity was 2.5 million tonne per annum but then expanded after 9 months of operation to 3.1 million tonne per annum. The technical steps to establish the Otjikoto mill design from process selection in the due diligence stage, through flowsheet development in the feasibility study followed by detailed engineering design are reviewed in this paper along with mill performance during start-up and in 2019.

Accurate evaluation of stress and geological conditions is critical to ground control design in underground openings. For a slope entry, the problem is more difficult, because a slope will transverse through many types of strata. Over the last few years, Keystone Mining Services LLC, an affiliate company of the Jennmar Corporation, has developed a new methodology designated the “Stress, geologic and support design system (SGS).” The method analyzes the stress, geology and support design for a mine slope. The method has been successfully applied in various new slopes across the United States, and ground control plans developed using SGS methodology has helped coal companies successfully pass review and/or hearings by state and federal regulatory agencies. With a case application in the U.S. Midwest, this paper details and demonstrates the main concepts of the methodology, including (1) analysis of stress and geology and identification of strong, fair and weak zones along the slope; (2) primary and supplemental support design; (3) long-term support design in the form of steel square sets or arches and (4) validation of the support design utilizing advanced finite element computer modeling.

The PC computer program PCVENT, version 3.3, for the ventilation network analysis now incorporates a real-time fire simulator. Position of the fire or a source of contamination and the character of the event are input from the keyboard. There are several options of type of fire: heat only, contaminant only, or both with a constant burning rate or with oxygen as the only limiting factor. Airway resistances or fan characteristics can be modified during a fire simulation. This allows testing of various fire suppression strategies. Program application in the analysis of a real mine fire is presented.

Among underground coal miners, roof bolter operators are generally considered to have some of the highest risks for hazardous respirable dust exposure. This is because bolting requires drilling into roof strata that can often be a source of silica and silicate dust, which are associated with occupational lung diseases. However, little is known about the variability of dust characteristics (e.g., mineralogy constituents, particle size) in the vicinity of the bolter—or when specific dust controls are applied. As part of a prior NIOSH study, respirable dust samples were collected during several different events in standardized locations around an active roof bolter, and personal samples were also collected from the operator during the cleanout of the bolter’s dust collection system when it was equipped with a novel wet dust box versus a traditional dry box. Those samples were made available for follow-up analysis by scanning electron microscopy with energy-dispersive X-ray spectroscopy (SEM–EDX), as well as direct-on-filter Fourier transform infrared spectroscopy (FTIR). Results showed variability in dust constituents and particle sizes at locations around the roof bolter, and indicated some event-to-event differences in dust sources. Additionally, compared to the dry dust box, the wet dust box appeared to reduce (by 41–82%) the relative silica and silicate content in the respirable dust to which the operator was exposed during cleanout. Furthermore, an inter-laboratory comparison demonstrated the reproducibility of a standardized direct-on-filter FTIR method for estimating quartz mass (i.e., the predominant form of crystalline silica) in respirable coal mine dust samples. However, for constituent analysis by SEM–EDX, differences observed between results from two independent labs indicate that standardization of the analytical protocol is necessary to enable comparability of results.
