Coal mining operations require the expenditure of appreciable start up and working capital in order to produce saleable coal for domestic or export markets. The competitive nature of the coal business restricts margins, and so it is imperative that downside risks, in terms of production outcomes, are minimised. The major risks identified for a coal mining operation, aside from sales volumes and coal price, are generally thought to be confined to the geology of the deposit and the mining process. However, the impact of product quality and yield can be very significant. If product coal is inferior to projections, then it will need to be sold at a discounted price and this can dramatically impact on margins. Equally, if yield is five to ten per cent less than planned, then this will have a huge effect on profitability. This paper will look at issues relating to in situ moisture and density, resource quality evaluation, modelling of washability and dilution, and simulation techniques, and how best practice approaches can be incorporated into mine planning and feasibility studies. Risks are best modelled and understood by adopting a detailed approach that is fully documented and auditable. This is particularly needed for multi-seam and multi-product operations. Once a well defined systematic approach is established the potential risks can be evaluated by testing different scenarios, carrying out sensitivity analyses, variability modelling and examining extreme operating conditions. These have been discussed in the paper, along with some related issues such as the cost of quick fixes and the value of spending a little more in the coal preparation area.

This project will expand sanitary sewer within the City of Columbus. It consists of 2.5 mile long, 72-inch finished diameter pipe grouted inside 10.8-foot diameter tunnel excavation in shale. This paper describes the multi-faceted geotechnical exploration to minimize the underground risks. The tunnel will cross below a buried valley with minimum cover of highly weathered rock. The risk mitigation to cross the valley includes contingencies and cost allowances. Provisions to prevent impact on adjacent groundwater resources will be described. Latest construction updates will be provided including contractor alternate shaft support and pre-excavation probing to mitigate tunneling risk below the valley.

Since 1993, Newmont has been actively growing through the development of three major foreign mining operations with a fourth under construction. The first project, Yanacocha in Peru began operating in October 1993 at a time when political and business risk in that country was considered high. In 1995, Newmont began operating the Muruntau Joint Venture in Uzbekistan, the first of its kind in the CIS. In early 1996, Newmont started operating the Minahasa mine in Indonesia. Now, in early 1997, Newmont is undertaking the construction of a $1.9 billion project in Indonesia and is completing a feasibility study with our partner for the development of a gold operation in Mexico. All of these projects have been considered "risky" by most standards, and Newmont has been very successful in all of them through risk mitigation programs including utilizing strong and successful in-country partners, concentrating on developing a strong national workforce with minimal expatriate employees, utilizing World Bank, EBRD and IFC financing and building and maintaining strong ties with host governments. This paper describes in more detail, some of the risk mitigation programs that Newmont has used to successfully grow the company through development of major foreign mining operations.

Although the outdated idea of calculating risk as the deterministic product of its likelihood and impact is not entirely a thing of the past, a multitude of organizations have adopted the usage of probabilistic methods for risk analysis, as they better reflect the inherent uncertainty of cost and risk estimates with ranges. But are these advancements sufficient in a complex environment such as tunnelling projects, where inadequate modelling of cost and risk can result in estimate discrepancies that run into millions of dollars? This paper explores two advanced probabilistic modelling techniques for tunnelling projects. The first one demonstrates how the cost and schedule uncertainty related to changes in excavation and support classes can be modelled with correlated variables. The second example models the risk related to explosive ordnance clearance on construction sites using a probabilistic event tree.

Risk ranking of projects is one of the importance parts of the complicated risk management procedure. Recognizing the riskiest criteria enable the managers to plan for risk reduction. The first stage for meeting this aim is defining the risky parameters and then working to establish order between them. There are variety methods for risk ranking. The ELECTRE method is one of the best among the multi criteria decision making methods. In this paper with implementing the fuzzy ELECTRE, The risky criteria in the Tehran Subway project are being ranked. The result of this research presents damage and accident of machineries as well as the lack of labor and equipment safety is located in the first order. Collapse factors and local prices are in second rank. Also wrong design, pollution, urban facilities damageable are in third to fifth respectively. Delay and ? are located in the last positions.

"INTRODUCTION Since the introduction, several decades ago, of the first concepts of tunnel boring machines (TBMs), the unremitting increase of mechanized and automated tasks and the reduction of manual tasks carried out by human operators have gone hand in hand with the continuous improvement of these machines. In the present state of TBMs, the replacement of worn cutting tools is a critical, tedious, and dangerous manual operation especially for earth pressure balanced (EPB) and slurry TBMs. For such pressurized TBMs, hyperbaric interventions are used. They consist in replacing the pressurized soil/mud inside the chamber with compressed air that balances the face pressure. The operators can then ""dive"" into the chamber; however, because the human body cannot naturally undergo pressure changes, they have to follow strict procedures that have been developed for subsea divers, based on decompression stops. Indeed such interventions are actually quite often performed by professional deep-sea divers. The pressurized TBMS are consequently equipped with a manlock that allows for the required compression and decompression cycles. Hyperbaric interventions increase risk to the operators and require long decompression procedures that increase the TBM idle time and reduce its efficiency. At higher pressures, over 3.5 bar (ITA Report No 10, 2011), specific equipment and special gas mixes and/or saturation techniques are recommended (Lamont, 2012). Such procedures are even more expensive and time consuming although the duration of work in the pressurized area can be very limited for health and safety reasons. Contractors are therefore increasingly interested in automating these operations to reduce the risks to human workers and improve the TBMs performance. Moreover, the demand for new, ambitious and challenging tunneling projects (very deep, high pressure boring) is ever increasing, but the supply of suitable TBMs is limited and the risk from working in the hyperbaric environments becomes increasingly unacceptable. Impact of hyperbaric interventions The UK Health & Safety Executive has analyzed the consequences of working in intermediate pressure compressed air. The retrospective study of decompression illness (DCI) indicated that at certain pressures its incidence was around 2% of all exposures. More worryingly it showed that over 20% of the exposures commonly undertaken by shift workers in tunneling (those over 1 bar pressure for 4 hours or more) resulted in DCI. It was concluded that on some contracts up to 50% of shift production workers experienced DCI at some time during the contract, a situation which was considered to be wholly unacceptable (Lamont, 2006)."

New technology projects, particularly those involving first-of-a-kind metallurgical facilities, have a poor track record of achieving their overall financial targets, as they incorporate technological risks in addition to normal project risks. Traditional project management tools focus on risk from concept development through implementation, but typically fail to address the additional risks associated with a new technology project including slow start-up, slow ramp-up, and failure to achieve design performance (quality or throughput). These additional risks affect future cash flows produced by the project and hence its value, typically measured as net present value (NPV) or internal rate of return (IRR). Common accounting practice is to reduce the present value of future cash flows by applying a chosen discount rate, often reflecting the corporation's cost of capital. This discount rate does not contain an allowance for technological risk, which is often underestimated or misunderstood by both the financial and engineering project managers. This paper will focus on the topic of project risk and new technology, and how these play a role in the probability of achieving the predicted future cash flows. By properly addressing technological risk in the form of risk-weighted returns, marginal projects can be prevented from proceeding, while sound projects can be given the additional time and resources required to achieve the optimal level of front-end-loading (the level which returns the maximum risk-weighted net present value). A simple set of mathematical tools should enable engineers and financial professionals to establish a common level of understanding, thereby leading to a more accurate assessment whether a new technology project is ready to pass through its next level of approvals.

The total number of tailings accidents has reduced from over 50/decade in the 1960s-1980s to less than 30/decade in the 2000s to 2010s. However, the rate of significant tailings dam failures has not been reduced. The significant tailings dam failures the mining industry experienced in the recent history include: Merriespruit, South Africa, 1994; Los Frails, Spain, 1998; Kolontár, Hungry, 2010; Mount Polley, Canada, 2014; and Samarco, Brazil, 2015. The conventional Factor of Safety approach itself may not be enough to further reduce the rate of significant tailings failures due to its limitations such as uncertainty. For example, the same factor of safety could have a vast range of annual probability of failure for different tailings dams, and insufficient consideration of failure consequence. This paper presents a risk based approach for design with the objective being to minimise tailings dam failures, especially those significant tailings failures with high consequence and low probability. This approach includes risk based design and risk management using bowtie tools. In the risk based design, quantitative analysis will be used to estimate the annual probability of failure and the tolerable failure criteria is linked to the consequence category. Bowtie analyses will be used to understand hazards and risks and identify and select independent controls that are effective in preventing and mitigating catastrophic failures. The Guillermo Gaviria Echeverri tunnel and its access roads project in Antioquia, Colombia is a mega infrastructure project that includes a road network with the longest tunnel in Latin America and several short tunnels, the latter being the focus of this study. The design of the project required careful consideration of several special factors, such as the tunnels’ steep slopes of up to 5%, high fire load of 100 MW, bidirectional operation, and short lengths that make it difficult to place ventilation equipment. The combination of all these parameters cause a challenging design problem for the ventilation systems and smoke control of the short tunnels. To address this challenge, a risk assessment study was conducted to support the design of a new and innovative ventilation strategy that deviates from relevant guidelines and standards, like the Austrian RVS [1][5][6] or the German EABT [8] in order to provide a customized solution for this particular project. This strategy resulted a cost-effective solution, that allowed a reduction in the number of required jet fans and at the same time reaches or reduces the risk level compared to traditional ventilation strategies.

The safety of persons from accidental explosions and fires during manufacture, storage, transport etc. of explosives, pyrotechnics, propellants, firework and ammunition is most commonly assessed according to prescriptive rule-based approach: Safety is given when all requirements which are listed in numerous regulations and directives (such as e.g. safety distances, limited quantities, protective installations etc.) are complied with. Such a deterministic prescriptive rule-based approach is quite easy to apply but has got disadvantages, however: The actual state of safety in a specific case is not explicitly and quantitatively expressed, and it is not distinguished between the endangered individuals and the whole group. As a result, the effects of the safety measures cannot be measured. This is why uneconomical and in the worst case useless safety precautions cannot be avoided. In addition, waivers are difficult to negotiate, meaningful and sound arguments for pro and con being lacking.

This study considers realistic data for a planned open pit iron ore mine, but is applicable to any open pit situation. By interpolating drill hole data, grades are generated for a rectangular block model. Each blockÆs grade designation vector has components for each analyte (chemical element or compound) influencing ore value. Interpolation by means of geostatistical kriging leads to each block being assigned its expected value, and thus underestimates the overall grade variability. Alternatively, interpolation by means of conditional simulation is a method that implements random sampling from an infinite population of solutions. Each conditional simulation has appropriate overall grade variability, but estimating any blockÆs mean and variance requires sampling from multiple conditional simulations. For a block model, an ore/waste selection criterion maximises the expected tonnage at a target grade. This criterion is a linear composite of the grade components, with positive coefficients for the beneficial analyte (iron) and negative coefficients for the deleterious analytes (such as silica, alumina and phosphorus).Although conditional simulation gives the same expected grade as kriging for each block, the expected maximum tonnage of ore selectable at a target grade may differ from that obtainable from the kriged solution. We apply the linear composite selection criterion to each of 25 conditional simulations, as well as to the kriged block model. The resulting distribution of product tonnage is confirmed to have an expected tonnage that is over 20 per cent greater than that of the kriged model. The method also enables a selection probability to be computed for each block, and thus a probabilistic pit boundary distribution to be identified and used in mine planning. Proposed extensions will consider risk-based scheduling of the multiple selection solutions through minimisation of a derived æstress factorÆ and treating the mining process as an iterative system with actual or artificial ædepletionsÆ modelled in line with the mine plan, using the updated state (with new information) to re-evaluate the mine plan for subsequent periods.CITATION:Everett, J E and Grobler, F, 2013. Fiji is a developing nation that relies on economic opportunities and recently experienced increased interest in mining and mineral exploration that could potentially benefit the country. In order to understand the lack of, or failure of, past rehabilitation activity, this study applied risk-based approaches to identify the significance of activities or actions applicable to rehabilitation of post-mined land in Fiji. This involved the use of qualitative, semi-quantitative and quantitative risk methodologies, which allow assessment and evaluation of mechanisms of mining administration and practices, as well as rehabilitation methods and approaches that have been applied successfully to post-mined land in other countries with similar environmental and climatic conditions and social and regulatory environments to Fiji. The semi-quantitative risk-based technique followed an earlier approach applied for mine closure by Laurence (2006) and provided a systematic way to assess and evaluate rehabilitation issues that were identified on-site by assessors, who were able to estimate through expert judgement the likelihood and consequences based on conditions, situations observed and circumstances on-site. It demonstrated that experienced professionals could reproducibly score likelihood and consequence of selected items relevant to mine site rehabilitation, despite the wide-ranging issues identified during post mined land rehabilitation. The quantitative risk-based approach was adopted as a method to determine environmental bonds for mining projects using the assessment of consequences and likelihood results from the semi-quantitative approach, then incorporating unit cost of materials or activities for local conditions required to fully rehabilitate each issue at net present value. The Tunnel of Guillermo Gaviria Echeverri, spanning 6.05 miles, will be Latin America’s longest road tunnel, is currently under construction in Colombia’s Antioquia Department with a single bidirectional tube connected to a parallel rescue tube through multiple cross-passages. The study faces challenges in implementing the prescriptive semitransversal ventilation system due to design restrictions. Different ventilation concepts are eplored to nd the most suitable system for regular operation and re scenarios. A quantitative ris assessment evaluates each alternative system and identies the longitudinal ventilation system, accompanied by ris mitigation measures, as the safest choice, since it surpasses the prescribed semitransversal ventilation concept.

Australia has an enviable global reputation as a leader in health and safety outcomes in the hazardous industry of mining, in part due to an increased application of risk management regarding operational planning and mining practice. In 2010, the Australian Coal Association Research Program (ACARP) initiated a major project, RISKGATE, to build, capture and deliver a national body of knowledge to assist companies with the management of risk in coal mining. The online system contains information for tyres, collisions, fires, isolation, strata underground, ground control open cut, explosions, explosives underground, explosives open cut, manual tasks, slips/trips/falls, outbursts, coal bumps and bursts, human–machine interface, tailings dams, occupational hygiene and inrush. In 2014, RISKGATE is developing content for fitness for work (fatigue, drugs, alcohol, physical well-being, psychosocial well-being).This paper analyses underground mining fatalities in Australia and the United States since 2002 and introduces RISKGATE with a focus on the knowledge base for management of hazards in underground operations. Through capturing coal industry expert operational knowledge, RISKGATE provides a cumulative corporate memory at a time of high personnel turnover within the mining industry. Controlling unwanted ground movement is critical in mining as well as many other highly hazardous environments, such as construction and tunnelling. Sharing knowledge about effective hazard management is paramount to improving health and safety and operational outcomes across industries, and some industry sectors have embraced this openness as part of the way that they do business. In 2010, the Australian Coal Association Research Program funded the RISKGATE project to build a body of knowledge for managing a broad range of hazards across mine sites, lease areas and mine infrastructure within a mine-life or product life cycle perspective (eg from exploration through to decommissioning, or design through to operation). The RISKGATE body of knowledge was assembled from Australian coal industry experts through a semi-structured bowtie analysis-based action research workshop cycle. By capturing operational knowledge from industry experts, RISKGATE serves to provide a cumulative corporate memory at a time of high personnel turnover in the coal industry.This paper presents an overview of RISKGATE topics within the geological and geotechnical domain (strata underground, ground control open cut, outburst, coal bumps and bursts, tailings dams and inrush) with a summary of individual topic structures, as well as showing the contrasts and inter-relationships between these ground management topics. Fires are a common and recurrent unwanted event in the operation of open pit mines. In most cases, the causes of events such as fires are already æknownÆ but are either overlooked or not fully addressed in planning. Ongoing rapid growth of the global mining industry is creating a scarcity of skilled personnel, and high turnover within operations. This contributes to loss of corporate memory in risk management activities. RISKGATE« is an online risk management system designed to support and assist in the analysis of priority unwanted events in Australian coal mining operations. Funded by the Australian Coal Association Research Program (ACARP), RISKGATE« is a knowledge management program that delivers best-practice controls for unwanted events and assists operations in staying abreast of emergent risks and threats. RISKGATE« content has been primarily developed from an association of industry experts, and enriched with input from additional academic and technological resources. In mining operations, RISKGATE« provides a continuum of knowledge and best practice for risk identification, assessment and management. RISKGATE« will assist companies in improving safety performance, operational efficiency, operating practice and training. South Africa is one of the leading producers and exporters of coal globally. A significant amount of the country’s production is obtained from previously mined underground bord and pillar workings. This coal is in the form of pillars and remnants on the roof and floor of the old workings. The good quality coal pillars were left behind as primary support during underground bord and pillar mining operations. Due to the depletion of virgin coal reserves, the pillars and remnant coal are now removed using opencast mining rather than underground methods. However, the secondary extraction of pillars and remnant coal from the old workings using opencast methods entails some serious challenges that have a negative impact on the safety and productivity of the operations, affecting both personnel and machinery. If these risk factors and challenges are managed properly, then the opencast mining operations could remove the pillars safely at recoveries competitive with those of virgin coal operations. In this study we review the recurring challenges affecting opencast pillar mining by means of field investigations and consultations with experts at five opencast pillar mining operations, and evaluate the best practices used to combat these challenges. It was found that each mine has its own unique conditions and challenges.

Marine mining on the continental shelf was first attempted in 1900 for gold off the beaches of Nome, Alaska. Attempts continued until 1990 with cutter-suction, airlift and bucket-ladder dredges. Offshore Thailand profitable production of tin resulted from clamshell, bucket-ladder and trailing suction hopper dredges. Today in Indonesia numerous bucket-ladder dredges continue to mine tin. Off the coast of Namibia and South Africa nearly a dozen large and medium sized vessels are engaged in diamond production. The obvious risk results from the working environment. Vessels and lives have been lost in the past. At best the availability of mining equipment is reduced. Leaving aside the marketing and financing aspects, otherwise the most important risks are technical. They manifest themselves as diamond production short-falls with obvious financial consequences. Bias and deficiencies in sampling equipment and procedures may result in misleading grade estimates, even serious under-estimation. Inadequate sampling in terms of sample support and density may result in over-optimistic grade determinations, especially if under-sized mining blocks are outlined. Quantitative measurement of the geotechnical characteristics of the sediments to be mined is an essential stage in a feasibility study. Without a full understanding of the bedrock profile and the overlying sediments the ease, completeness and speed of excavation during the mining process cannot be properly estimated. Both historically and at present the greatest negative discrepancies from production estimates result, not from grade shortfalls, but from significantly lower mining rates than thought possible by the operators. The learning curve in marine, production mining is steeper and longer than is generally recognised by newcomers to the industry. Examples are provided from tin, gold and diamond mining experiences.

Abstract -The Riviera tungsten occurrence in the Cape Province of South Africa was discovered in October 1979 as a result of stream-sediment panned concentrate sampling. The Cape age granite and late Precambrian Malmesbury Group carbonates and chlorite schists which it intrudes and which host the mineralization are covered by between 10 m and 30 m of Tertiary/Recent alluvium and clay. The scheelite/powellite and molybdenite mineralization occurs in a greisenized granite cupola with an overprint of porphyry-type alteration as well as in minor skarns. The ore reserve is estimated to be 46 million tonnes at an average grade of 0.2160Jo W03 and 0.020% Mo. The deposit is uneconomic at current tungsten prices.
