From many years practical industrial experience the author has drawn together a list of the key points to consider when embarking on a large-scale geometallurgical project. Fundamental aspects which must be considered include (but are not limited to):understanding the variability of the orebody both laterally and with depth;comminution properties and variability;flotation properties, requirements and variability – differing ore zones may have substantially different requirements for optimal performance;the role of texture – the consequences of both grain size and distribution and how the circuit will handle these;realistic expectation in terms of the financial requirements and time frame results are required within; andintegration of, and communication between key personnel within the various departments of the site (mining, geology and processing) is a fundamental aspect of any geometallurgically based processing system. An example of a highly complex nickel sulfide ore will be used as the basis of discussion.

Accidents involving powered haulage and mobile equipment like haul trucks are a major safety concern in the U.S. mining industry. They account for more than 50 percent of surface mine fatalities each year [1]. A recent National Institute for Occupational Safety and Health (NIOSH) analysis of 91 fatal incidents identified loss of vehicle control and situational awareness as key contributors [2]. However, the first-hand perspectives of operators are often missing, emphasizing the need for deeper insights into operators’ decisionmaking, lessons learned, sensory cues and mental strategies used during near-miss events. The findings in this analysis will help inform the development of training and safety solutions to prevent or reduce haul truck accidents.

Augered-Cast-in-Place piles have been used successfully for many deep foundation applications. The four case studies discussed herein describe ACIP pile projects where problems developed during ACIP pile installation requiring changes to the method or sequence of installation. The case studies illustrate the importance of understanding existing subsurface conditions and project constraints when considering the use of this pile type. Ground loosening and over-excavation of soil during the augering process, a significant concern for fine to medium sands and non-plastic silts below the water table, can significantly impact adjacent structures. This risk needs to be evaluated during the pile selection process and generally can be mitigated, but not eliminated, by requiring the use of piling equipment with high torque and slow auger rotation speed to advance the auger with minimum amount of rotation. In some applications where high lateral stresses in the soil are present such as at the base of retaining structures, the use of ACIP piles may not be appropriate if structure movement is unacceptable. On the other hand, ACIP piles have been successfully used to support deep foundations in preconsolidated varved deposits where the new structure was designed to accommodate recompression settlement.

The Waterview Tunnel comprises twin 2.4 km long 13.1 m internal diameter three lane road tunnels in Auckland, New Zealand. The City Rail Link (CRL) is a 3.45 km long, 6.24 m internal diameter twin rail tunnel underneath Auckland’s city center. This paper presents an overview of the tunnel geometry, segmental lining congurations, geometric constraints, performance criteria and functional reuirements for the large diameter Waterview Tunnels and smaller diameter CRL tunnels. Other key aspects, which include sustainability, innovations, construction performance and practical aspects are discussed and compared. Lessons learnt from these mega tunnelling projects are also presented.

The Sobek humidity cell has been in use with minor modification for more than 30 years to determine bulk primary-mineral reaction rates. As a result, it has become a key tool for predicting the bulk rate of sulphide oxidation, the Carbonate Mole Ratio (CMR) of neutralization-potential (NP) consumption to sulphide oxidation, and the rates of metal leaching. Most humidity cells are operated for less than a year, with the resulting rates extrapolated into the future for years or decades. The validity of these extrapolations was tested by examining longer trends from cells that have operated for three to seven years, as found in the International Kinetic Database (IKD). Additionally, large batches of cells for two minesites in the IKD (45 and 46 cells each) were examined to determine if regular, predictable trends were obtained across ranges of geochemistry at a minesite. The long-term cells show that the rates of sulphide oxidation and acid generation, based on sulphate production, have a 50% chance of stabilizing within one year, with the remainder fluctuating significantly throughout their test periods. Despite the fluctuations, the CMR, which is critical for defining site-specific or rock-unit-specific ABA criteria, remains relatively steady in many cases between 1.0 and 2.0 as expected from principles of aqueous geochemistry. However, if the sulphate-production rate falls below 10-30 mg/kg/wk, the CMR value can increase significantly. At and below these low rates, physical factors like flow rates of surface waters and groundwaters will determine the rate of NP consumption, rather than geochemical factors. Also, if weekly rinse pH falls to acidic values, the CMR will decrease below 1 .O, reflecting the exhaustion of fast- neutralizing minerals. The two batches of multiple cells also confirm these trends across arange of geochemistry. The long-term cells show that copper leaching often stabilizes within a narrow range as long as rinse pH remains relatively constant. Also, there is no strong dependence of leaching on the sulphate rate. However, the rate of copper production often increases sharply if rinse pH falls to acidic values. This suggests the measured copper-production rate in some cases is not a true primary rate, but a lower secondary rate after secondary- mineral precipitation within Sobek cells. As with the long-term cells, the batches of multiple cells show that copper production often increases gradually with decreasing rinse pH, with no clear dependence on sulphate production. However, high rates can also occur at near-neutral pH, so secondary-mineral precipitation of copper is not necessarily common.

When originally accepting the assignment, the Client had described the property as vacant land. A prior appraisal was later discovered and revealed that the property included a mineral rights appraisal. Further research disclosed an old dispute with a court ruling based on testimony by AIMA-certified experts. The present appraisal was therefore based on data from findings of the court with a fresh look at discount rate, absorption period, and residual land value. Fortuitously, the property subsequently sold on the market, while meeting all criteria for Fair Market Value. This provided insight into a market-based discount rate. Also, the absorption period could be adjusted in accordance with the current market conditions. The residual land value was minimal (based on comparable sales and projecting trends in market) as the property was affected by a flood zone and the surrounding development was sparse. PROPERTY HISTORY: A GREAT INVESTMENT Briefly, the subject property is located in the floodplain region of the San Jacinto River, a short river in southeastern Texas that flows into Galveston Bay, filled with sand and gravel. This location is on the north side of Houston, north of Beltway 8 on the east side of I-59.

Early construction delays can jeopardize timely completion of subsequent work. As an early construction activity, and due to many unforeseen issues, deep foundation work inherently bears great potential for impacting the critical path of a planned construction schedule. This article describes two construction projects where delays related to deep foundation work forced the Contractor to consider various schedule recovery alternatives to mitigate those delays. Specifically, deep foundations on Vancouver BC's Millennium SkyTrain light rail extension and on a Mill Facility in the United States are discussed. This article cites examples of permitting delays, alleged design changes, differing subsurface conditions and changed construction methodology, the effect each problem had on the project, the mitigation means implemented by the Contractor to minimize adverse impacts of the problem, and Lessons Learned for each scenario.

Accidents involving powered haulage and mobile equipment such as haul trucks often account for the greatest number of fatalities in the mining industry each year. Despite previous analyses that have identified root causes and other contributing factors, there is still a need to better understand the events leading up to these types of accidents, what lessons may be learned, and what strategies can be employed to prevent fatal accidents from occurring. This study examines Naturalistic Decision Making (NDM) using the critical decision method (CDM). The CDM is a retrospective interview approach used to explore timelimited, high-stakes, decision-making. In this study the CDM is used to obtain more information about what happens prior to, during, and after a potentially fatal situation such as a near-miss event, loss of control, or minor accident involving equipment damage. Researchers captured firsthand accounts from 21 haul truck operators involved in near-miss events from mine sites of various sizes and commodities throughout the United States. These accounts provide rich and detailed narratives from the perspective of haul truck operators themselves and reveal insights into what decisions haul truck operators make, what sensory cues they perceive, and what strategies they employ during challenging and non-routine situations. Decision-related themes that emerged from the data are presented and discussed. These results, along with potential solutions offered by study participants, can help to inform future research, raise awareness about hidden hazards, and build more creative interventions and realistic training scenarios for use by the industry to address haul truck safety issues.

"Most, if not all longwall gobs contain explosive gas zones (EGZs), i.e., zones of explosive methane-air mixtures that can cause – and have caused – mine fires and explosions. If the coal is prone to spontaneous combustion, oxygen penetration into the gob must be avoided. This paper summarizes the significant research findings from five years of computational fluid dynamics (CFD) modeling research conducted at the Colorado School of Mines (CSM) under funding from the National Institute for Occupational Safety and Health (NIOSH). CSM Researchers have developed CFD modeling techniques to identify where and under what circumstances EGZs can form in longwall gobs and how EGZ formation and oxygen penetration depend on the ventilation method, face and bleeder ventilation parameters, injection of inert gases and the operation of gob ventilation boreholes. Recognizing these explosion and fire hazards is an important first step in improving the safety of longwall coal mines. CSM modeling research has shown that EGZ formation can be effectively controlled by adjusting ventilation parameters, choosing the proper ventilation pattern and injecting inert gases where necessary. INTRODUCTION In a project sponsored by the NIOSH Office of Mine Safety and Health Research, researchers at the Colorado School of Mines have examined gas compositions and flows in longwall gobs in order to assess explosion and fire hazards. Since gob areas are inaccessible, researchers developed a series of computational fluid dynamics (CFD) numerical modeling tools to characterize these hazards. A variety of CFD models were developed to cover progressively sealed as well as bleeder ventilated gobs for both coal and trona mines. The explosion and fire hazards posed by EGZs in longwall gobs can be summarized as follows.• If the EGZ lies closely behind the longwall face, flames and pressures from an explosion can penetrate the shield supports and reach the active face area, creating blast trauma, burn hazards and toxic, unbreathable atmospheres. • EGZs can be pushed around inside the gob by roof collapses and cave-ins. If they get pushed out into the face area, sudden methane inundations and explosion hazards in face areas can result. • Fresh air flowing into the gob can create an explosion hazard as the gas composition in the gob moves from fuel-rich inert to explosive, creating EGZs. • Ignition sources inside the gob can be frictional ignitions or spontaneous combustion. • Bleeder ventilated gobs are surrounded on all sides by an EGZ that forms along the fringe areas between the fresh-air ventilated bleeder entries and the methane-rich interior of the gob. It is suspected that diffusion flames can propagate rapidly along these EGZ fringes if an ignition occurs"

Introduction A brief account of the East Malartic fire is essential if this paper is to serve the purpose implied by its title. The fire originated in the timbers of No. 4 shaft at the 10th, or 1,570-foot, level; this was the top or collar of the underground sinking operations. At the time, sinking was in progress and the shaft bottom was 242 feet below the 12th station horizon, or 618 feet 'below the l0th level. Above the l0th level, the shaft was not in use as the surface plant was not yet completed. The shaft crews descended from surface in No. 3 vertical shaft to the l0th level, where they transferred to No. 4 shaft, a distance of 1,450 feet. Of the sixteen shaftmen mucking at the shaft bottom, four were able to reach safety. One shaftman was overcome by fumes midway between the two shafts on the l0th level, while attempting his escape. The remaining eleven men climbed as far as the 11th station and there succumbed to carbon monoxide. No. 4 shaft, inclined at 61 degrees, bad been raised and timbered from the 10th level to surface; this bad been followed by sinking operations below the l0th. Two air hoists had been set up in the footwall of the shaft, 110 feet above the 10th level, and directly behind compartments No. 2 and No. 4. The waste from the shaft bottom was hoisted in 28-cubicfoot buckets on mono rails and dumped over standard shaft sinking doors into waste chutes built into No. 1 and No. 3 compartments. The five-compartment shaft, 7 ft. 8 in. by 34 ft. 6 in. overall size, was heavily timbered. In addition to the standard shaft timbers of 8 in. by 8 in. and 8 in. by 10 in. B.C. fir, the hanging-wall and ends of the shaft were laced with 4-inch spruce lagging. This represented approximately 300 board feet of timber in placee for every foot of shaft. The cause of the fire bas never been determined, despite every effort to do so. The inquest held for those who lost their lives in the fire was an extremely thorough one, but only the location of the origin, and not the cause, was established.

During the period May 15 ? 17, 2006 four fatalities occurred at #1 Waste Dump, at Teck Cominco?s Sullivan mine located at the city boundary of Kimberley, British Columbia, Canada. The fatalities occurred in the sump of a water monitoring station. The Chief Inspector of Mines and his staff immediately responded to the incident. The team worked collaboratively with the Teck Ltd. Trail Hazmat team, a consultant group from Rescan Ltd. and other Teck operation and technical staff. Sampling in and around the water monitoring station in the days following the fatalities showed that exposure to a depleted oxygen environment was the immediate cause of the fatalities. The sump located at the base of the shed was hydraulically connected to a French (coarse rock) drain, located at the toe of the covered waste dump. This presentation will briefly review the immediate cause of the accident, the investigation, a brief introduction to the work of the Sullivan Technical panel and the British Columbia government?s response to the incident and the recommendations of the Coroner?s inquest.

High Speed 2 (HS2) mandated that Building Information Modelling (BIM) processes be implemented during design and construction. Gall Zeidler Consultants (GZ) was responsible for developing the BIM Models for the portals, tunnels, and cross passages of the of the 16 km-long Chiltern twin-bore Tunnel. In addition, GZ developed the GBR for the Chiltern Tunnels and incorporated the relevant information into the Project’s BIM framework. This paper presents the implemented BIM framework, describes the modelling approach, and discusses the advantages and challenges of implementing BIM, particularly how BIM improved interface coordination and how BIM was used in feasibility assessments of different construction options.

The Washington Metropolitan Area Transit Authority, WMATA, has constructed over 92 revenue miles of rapid transit facilities including some 14 miles of rock tunnel and 12 miles of earth tunnel. The system map is shown in Figure 1. During construction it is not unusual for contractor to encounter ground conditions resulting in major problems and extra cost. Problems such as major rockfalls and a "stuck" boring machine in squeezing rock are not unusual. In recent years, faced with additional tunnel construction projects, the Authority has sought newer tunneling methods which might be less costly. One new method which has been tried is NATM, the New Austrian Tunnel¬ing Method. NATM, which is applicable to both soft ground, mixed face and rock tunneling, relies on miners and comparatively small machines to excavate in conventional ways, but it is unique in its methods of obtaining "stand-up" time at the excavated face and its method of using shotcrete as an initial lining, frequently with a drainage system to reduce hydro-static pressure on the final lining. Lower costs result from use of a cast-in-place concrete liner, and from a reduction in the number of work operations at the mine heading which provides better daily production. WMATA has recently completed three twin bore tunnels constructed by NATM technology. The first was the Wheaton Tunnel and Section, followed by the tail track tunnel to total some 4200 linear feet cut through jointed rock up to 270 feet below ground surface, and the last was in soft ground at Fort Totten. A gold mine in north-eastern Africa has expanded throughput from 5 to 10 Mt/y in recent years. The original carbon regeneration kiln was incapable of achieving required throughput or meeting process conditions to reactivate the carbon due to heavy fouling by high molecular weight frothers and collectors. A new high temperature kiln was required.The design criteria for the carbon reactivation kiln and associated equipment were revised, then implemented in close cooperation with a kiln vender who had previously supplied kilns operating successfully on highly fouled carbon. The result is a whole facility, holistically designed to achieve the technical criteria.The new carbon reactivation facility has been operating since 1 July 2014. What have we learned? What has performed to expectations? What could be done better?This paper presents lessons learnt regarding the design, implementation, operation and maintenance of the Ausenco Carbon Reactivation Kiln. ‘Lessons’ are associated with threats and opportunities (risks) that have that have been realised. The basis of this paper is a compilation of work in the design and construction of copper heap leach facilities, and interviews with various heap leach operations personnel in Chile and Peru. The purpose is not to highlight specific mines and their performance, but rather to discuss geotechnical/civil features of copper heap leach facilities that function well, as well as features that do not. During the 1990s several copper heap leach facilities were brought on-line in South America. Many of these facilities have now been operational for a number of years during which time the operators have been able to monitor the performance of their heaps. This paper serves as a compilation of operators? experiences, and provides a discussion of the lessons learnt over the past decade. Several key geotechnical issues are examined, focusing upon the integrity of the ore, drainage material and pipe network, along with the success of inter-lift liners. This conference provides a timely update of the original paper (Burkhalter et al, 2002).

During the 1980s and early 1990s, the South African coal industry experienced a series of methane explosions in which well over 200 coal miners lost their lives. In addition, a fire and an explosion in the gold mining sector claimed a further 240 lives. These tragic incidents resulted in the establishment of a research facility at Kloppersbos dedicated to explosion and fires research and the funding of an extensive research program, and almost certainly influenced the government of the day in its decision to establish, in 1993, the Leon Commission of Enquiry into Mine Health and Safety. The results of this focused research program, the wide-ranging recommendations of the Leon Commission and the determination of the coal mining industry to improve its safety record all combined to result in the past 20 years without a major explosion.This paper lists some of the research outcomes and, through three case studies, illustrates what can be learnt from investigations following explosions. The paper concludes with an evaluation of some of the findings of the Royal Commission on the Pike River coal tragedy. Many of the Commission’s conclusions are pertinent and relevant to the coal mining industry worldwide and their implementation could significantly improve coalmine safety; however, a careful study of the available facts indicates there could well be an alternative explanation of the source of the methane, the location of the ignition and the size of the explosion at the Pike River coalmine.CITATION:Phillips, H R, 2015. Six recent case studies were selected based on the eight main causes of mine disasters (Table 1; Quinlan, 2014) and where credible information sources, such as Royal Commission Reports, Coronial Investigations and textbooks, were available. Upstream embankment raises often represent the lowest cost option for increasing the capacity of tailings storage facilities (TSF), but also pose the highest stability risk due to constructing on top of previously deposited tailings.This paper presents an overview of key aspects in the geotechnical investigation and design of upstream raises and some recent lessons learnt from experience across a number of TSFs in Australia. A summary of recommended field investigation and laboratory testing methods is provided with a focus on collecting quality information for design purposes. A strong focus is given in developing an accurate assessment of pore water pressures, which, if not correctly understood can have disastrous consequences on tailings strength parameter interpretation and wall stability assessment in static and post-seismic conditions.The assessment of the contractive or dilative shearing behaviour of the tailings material and rigorous interpretation of design characteristic values are presented.
