"This paper presents a case study of the use of single axis cement deep soil mixing (CDSM) to deal with static settlement and seismic stability of the foundation soils under a reinforced soil slope (RSS) embankment up to 50 feet high. As part of the widening of Interstate 5 (I-5) to allow High Occupancy Vehicle (HOV) lanes in Tacoma, Washington, a new approach and span over the Puyallup River will be constructed. During the soils investigation and design phase of the project, low plasticity silts (ML) inter-bedded with silty sand layers and organic silt where identified as being potentially liquefiable. CDSM was selected to address the embankment foundation concerns and allow for future trenchless installation of utility lines beneath the embankment. The presence of a confined, artesian aquifer within the design depth of the CDSM led to the installation of a depressurization system to avoid an upward gradient through the wet soil-cement. In addition to addressing the design approach, the paper discusses the challenges associated with quality assurance of CDSM within the time constraints of an ongoing project. The limited recovery rate of conventional triple tube coring in the soil-cement and double tube wet sampling led to the use of sonic coring to provide a near continuous core of the soil-cement.IntroductionCement deep soil mixing (CDSM) was used as a replacement for stone column ground improvement in a corridor which the owner, the Washington State Department of Transportation (WSDOT) wanted to reserve for future trenchless crossings beneath reinforced soil embankments within a freeway interchange. The work was part of HOV improvements to I-5 in Tacoma, Washington. The CDSM construction occurred in late 2010 and early 2011.Approximately 14,500 cubic yards of soil cement columns were placed by CDSM. The soil cement improved ground supports permanent geosynthetic reinforced slopes up to 50 feet high. The ground improvement was designed primarily to provide global stability of the reinforced slopes during seismic loading, but it was also utilized to reduce the time and total amount of settlement following construction of the embankments.This paper presents a discussion of the design approach and challenges associated with quality assurance of a relatively small volume of CDSM within the time constraints of an ongoing project. The challenges associated with attempts at collecting cores of the cured soil-cement mix cured by triple tube coring and pushing a double tube into the wet mix and allowing it to cure insitu are discussed, along with the eventual adoption of sonic coring combined with reliance on laboratory testing of wet mix samples collected daily."

"Mine water generated from underground coal mining operations contains both dissolved and particulate solids. Dissolved solids primarily consist of sodium, calcium, magnesium, potassium, chlorides and sulfates. When discharged to a receiving stream without treatment, these constituents create a potentially toxic environment for aquatic life. A case study is presented to discuss how a centralized treatment plant treats mine water from six locations to meet discharge limitations for chlorides. Located in West Virginia, this facility consistently achieves less than the NPDES permit limitation of 218 mg/L chlorides in the discharge while generating almost zero liquid waste. The dissolved solids concentration in the influent ranges between 5,000 and 10,000 mg/L, with chloride concentrations of 1,000 to 2,000 mg/L and sulfate concentrations of 2,000 to 6,000 mg/L. The mine water is treated using advanced treatment technology to produce clean water for discharge or reuse. The treatment process comprises aeration, softening, filtration, reverse osmosis, RO reject softening, evaporation, crystallization, final effluent remineralization and sludge dewatering. The solid waste generated in the treatment process is landfilled on site. The leachate is sent back to the facility’s thermal treatment process. Because no liquid waste leaves the property, this plant is termed a “Zero Liquid Waste” (ZLW) facility. INTRODUCTION The advanced water treatment system was built to meet a new 218 mg/l regulatory limitation imposed by the West Virginia Department of Environmental Protection for chlorides discharged to surface waters. The system is designed to treat a maximum flow of approximately 5 million gallons per day (795 m3/h) of mine water. The treatment process utilizes reverse osmosis (RO) membrane treatment to achieve the effluent criteria, and evaporation and crystallization technology to manage the RO concentrate from the water treatment process. As a result, the system produces clean water for discharge while generating zero liquid waste. The effluent water can be used for industrial purposes or discharged to a receiving stream. A Water Impact Index (WIIx) study indicated that discharge of this high-quality water into the Monongahela River watershed may improve the water quality for downstream water users. The waste produced in the treatment process, including softening sludge and salt, is disposed of in an on-site landfill."

"The bridge across River Mandovi in Goa is a cable stay bridge structure proposed parallel, along existing bridges for regulation and reduction of vehicular traffic; the entire bridge structure is supported on pile foundation socketed in rock. This paper describes drilling related issue encountered during execution of pile where bed rock varies considerably from factual geotechnical investigation, which in turn affected the productivity of bored piles socketed in rock then chiseling is advocated for rock socketing with limited number of blows based on penetration resistance of rock encountered or equivalent energy principle to increase the productivity and also to avoid excess chiseling which otherwise would have fractured the rock mass below and around the pile tip resulting in increased settlement of pile subjected to design load. Contractor decision on socket length have been taken without compromising either quality or productivity; the paper presents various procedures and the equipment involved for installation of pile in rock along with the conclusion related to termination of pile in rock and also the lessons learned in process.INTRODUCTIONPile foundations are generally used to transfer heavy loads in deeper bearing stratum if the soil immediately beneath the base of the foundation does not have sufficient capacity. The bridge across River Mandovi, Goa is in its construction phase as a part of the development plan of Goa State Infrastructure Development Corporation Ltd (GSIDC). The project was awarded to M/s. L&T Construction, Heavy Civil Infrastructure; subsequently the geotechnical investigation was awarded to M/s L&T Geostructures.The client has specified the foundation of the bridge as rock-socketed. Rock socketing is generally done to achieve higher bearing pressure through concrete-rock shear interface which many building codes permit. The design and construction of rock socketed pile foundation entails a detailed analysis and judgement on the superstructure loads, predominant geological conditions as well as ground water levels. The termination of pile is normally based on thumb rule of half to three times the pile diameter into the rock. Quite often the criteria is based on chisel penetration rate (Gandhi,1990) In fact, this is the key factor to consider appropriate and economical depth of embedment of pile into rock stratum which necessitates enormous understanding on the type and quality of rock strata encountered which may significantly influence the selection of suitable rotary rig for pile installation. Such criteria are site specific and also applicable for the set of equipment used for the particular project."

The care necessary to ensure a reliable estimate of average value from the individual values of a series of bore holes sunk in any alluvial deposit containing coarse gold was well exemplified in checking over the mining returns from a previously bored deposit in Central Otago, N.Z. While the working returns are always liable to vary considerably from bore values in any particular sub-section of an area, still, over any considerable area, the two must check within small limits, if profits are to be calculated and capital raised on the average value given by boring.The present figures have probably more value than those from, dredge returns since accurate measurements are difficult to obtain in irregular water-filled dredge paddocks, whereas in the case under review the paddock walls stood almost perpendicular, and, as the measurements were taken and checked in the empty rectangular excavation, the yardage estimate may be taken as substantially exact.NATURE OF DEPOSITThe area consists of a narrow valley deposit of fluvio-glacial origin, the original deposit having been eroded and the gold reconcentrated by post glacial rivers. The wash is between 80 and 90 ft. deep, and is formed in two distinct layers:(i.) The top layer from surface to 70 ft. or 80 ft. consists of about equal quantities of gravel mixed with glacial drift, and a small amount of clay. The gravel is typical discoid glacial stone of a maximum size of about 4 in. diam. by 1 in. thick, and consists of soft micaceous schist and laminated slate. This upper layer of wash contains no coarse gold whatever, and very little fine; indeed, in mining, the top 30 ft. is discarded without being passed over the boxes.(ii.) The lower layer of wash to bedrock is similar to the upper, except iihat it contains about 10% of larger discoid stone up to 12 in. diam. by It in. thick; 90% of the gold is concentrated in this bottom layer.

"This paper presents a case history of drilled pier construction for a new stadium in Atlanta, Georgia. The stadium structure is supported on several different foundation types, with the heaviest ""mega columns"" typically bearing on drilled pier supported mats. The project includes 141 drilled piers with design diameters of 5.5, 6 and 6.5 feet. The piers bear on bedrock at depths of 20 to 110 feet below the ground surface. Many of the piers included rock sockets with design lengths of 5 to 18 feet. A pre-production drilled pier load test program was completed including one Statnamic load test, two Osterberg Cell tests and two lateral load tests. Non-destructive testing (crosshole sonic logging and sonic integrity testing) were also performed during construction. Production pier installation with over 3800 linear feet of earth excavation and over 1350 linear feet of rock excavation was completed in a little over four months. The piers were installed using both the cased hole method and the slurry method. All cased holed required downhole inspection and slurry holes were initially inspected with a mini-SID device and subsequently by sounding. Advanced drilling equipment and tooling were used to meet the production requirements for the project. This paper reviews the highly variable soil and rock conditions encountered at the site, briefly presents the results of the load tests, and discusses pier installation procedures and the equipment used for installation. The paper also presents conclusions related to the installation and lessons learned.INTRODUCTIONWith the opening of AT&T Stadium in Dallas, a new era of stadium construction began in the U.S. The new age stadiums are larger and have more amenities than their predecessors. The new Atlanta Falcons stadium is no exception. Many of the new features, including Atlanta’s innovative retractable roof, result in extraordinary structural loads. The roof structure is supported by a series of mega columns that bear on eight large caps, six of which are drilled pier supported. Each of the pier caps contains between 8 and 16 drilled piers that bear in the underlying bedrock. The design pier diameters are 6 and 6.5 feet. Augercast piles were used to support two of the mega column caps. Drilled piers, augercast piles, aggregate piers and spread footings are used to support the more lightly loaded portions of the structure."

This case study describes the geotechnical aspects of the basement construction for a multi-storey, residential and hotel complex located in the newly developed Elizabeth Quay precinct in Perth, Western Australia. The development included a three-level basement for car parking covering the full site area which was constructed using the top-down construction sequence. Soil conditions generally comprised 2m of fill overlying medium to dense clayey sands that in turn were generally overlying a very stiff clay stratum at approximately 13m depth. Bedrock level was generally encountered at approximately 22m to 35m below ground level. The ground water table was at a depth of approximately 1m below the existing ground surface level. A site boundary basement wall was constructed using a diaphragm wall retention system. The majority of the vertical loads from the building were transferred to the Kings Park Formation (KPF) bedrock through the use of foundation piles constructed by the CFA method. The CFA piles were installed from existing ground level, resulting in approximately 9m sacrificial concrete length from pile platform level to basement 3 (B3) level. The permanent column loads are such that a single CFA pile was not structurally adequate. To allow a full CFA foundation system to be adopted, an innovative design and construction approach was developed for these heavily loaded columns. This paper discusses the site technical details and the design and construction aspects required to meet the challenges offered by this project.

NIOSH researchers collected basic data from two blast rounds at the SSX-Steer Mine as a component of a larger study on controlled blasting, the goal of which is to reduce injuries from falling rocks in underground mines. Drilling and blasting procedures at the mine were observed. Rock mass property data were collected, a program of seismic monitoring and analysis was conducted, and 3-D laser survey scans of the workings before and after each blast were performed. The geologic data showed that the rock mass quality was characterized as poor to very poor. It was essentially uniform within the panels examined. The recorded seismic data indicated that fill material acted as an effective damper to seismic energy. The pre- and post-blast laser scans showed both over-break and under-break conditions, highlighting the problems associated with blasting a weak rock mass. By comparing the achieved results to the intended design, potential improvements in the drilling and blasting practices can be identified. Comparison of these data with similar data from other mines will help define how various combinations of rock and fill behave during blasting. Through studies such as these, data collection procedures and analysis techniques can be refined. An ultimate goal is to develop blast procedures tied to rock mass characteristics that can minimize damage to rock in the perimeter of the opening, limiting the amount of loose rock and improving the safety of miners.

This paper focuses on one trona mine’s experience in the development and implementation of a field level risk assessment program to help its entire organization understand and manage risk to an acceptable level. Through a transformational process of ongoing support and communication, Solvay Green River fostered a culture of trust and safety grounded in risk assessment, safety interactions, and hazard correction. The application of consistent risk assessment (RA) tools was critical to create a participatory workforce that not only talks about safety, but actively identifies energies that contribute to hazards and potential incidents. In this paper, reflecting on the mine’s process of RA implementation provides examples of likely barriers that sites may encounter when trying to document and manage risks. As discussed, consistent efforts to establish a participatory risk management program are worth the initial struggle to ultimately reduce incidents. INTRODUCTION Work-related health and safety (H&S) incidents often account for lost days on the job, contributing to organizational/financial and personal/social burdens [1-2]. Accompanying research demonstrates that risk and ambiguity around risk contribute to almost every decision that individuals make throughout the day [3-4]. In response, understanding individual attitudes toward risk has been unquestionably linked to predicting H&S behavior [5]. Although an obvious need exists to identify more comprehensive methods to assess and mitigate potential hazards, some argue that risk management is not given adequate attention in occupational H&S [6]. Additionally, research suggests that a current lack of knowledge, skills, and motivation are primary barriers to mitigating workplace risks [3-6]. Therefore, enhancing knowledge and awareness around risk-based decisions, including individuals’ abilities to understand, measure, and assign levels of risk to determine an appropriate response, is increasingly important in hazardous environments to predict and prevent incidents. This paper focuses on tailored approaches to field level risk assessment (FLRA), measurement, and common barriers to participating in such activities. The authors use a trona mine in Green River, Wyoming, as a case study to illustrate how the organization overcame widespread and site-specific challenges to implement a proactive risk management program. By discussing the mine’s tailored FLRA program, this case study contributes to the literature by first, providing examples of critical components that need to be in place to ensure successful risk management and second, context for continuous quality improvement that can foster a higher level of trust and empowerment on site."

Multiple-seam coal mining is a major issue in underground coal mining that adds additional complexity to the practice of ground control. As reserve exploitation is driven by economics, it is now difficult to mine coal without the existence of previous workings above and/or below the active seam; this is particularly true in the Appalachian coalfields. The frequency in which problematic multiple-seam interactions are now encountered has resulted in significant research regarding the underlying mechanisms of the problems and methods of mitigation. In the last few decades, a great deal has been learned about these interactions and the ability to minimize problems associated with these hazards has greatly advanced, resulting in better economics and improved safety. Minimizing the adverse effects of stress interactions often requires a thorough understanding of site-specific geotechnical parameters and previous mining geometry. This is especially evident in bump-prone ground, where pillar behavior is highly dependent on local conditions. This paper presents a case study in a deep cover, Central Appalachian coal mine where three bump events occurred. Initially, empirical design software was used in an effort to analyze the situation and prevent further instability. The empirical study proved ineffective. A detailed investigation of the bump events using the boundary element software LaModel 3.0 was then performed. This study resulted in a site-specific calibration standard that will be applied in numerical pillar design for the remaining reserve.

TBM (TBM) mining in urban areas particularly under low overburden is a big challenge to design and excavate. During excavation of the Lucknow Metro project, in Uttar Pradesh, India the metro authorities encountered a design stretch where the TBM had to cross a sewer canal crossing of 15 m length under overburden of less than 1 m. Not only the stability of TBM due to uplift was of concern but also the stability of both banks. Most structures on the canal banks were non-engineered structures whose stability due to vibration of TBM was also of concern. Due to these risks, a method was proposed that met stability requirements for both ground and superstructures. The design was practical and cost effective to construct, and a collaborative exercise between the client, contractor and designer that took approximately 18 months to implement.

This case study provides an example of how social marketing was applied to meet the needs of business customers. Federal regulations require that all companies develop a written program to track hazardous chemicals at their sites. The HazCom Writer, created by the National Institute for Occupational Safety and Health (NIOSH), is a software tool designed to help companies prepare their required Hazard Communications (HazCom) written plan and list all hazardous chemicals at the company's worksite. The social marketing goal was to provide a product for small- to medium-sized companies to use for federal HazCom rule compliance. The HazCom Wm'ter is a direct NIOSH response to customer and stakeholder needs (including the Mine Safety and Health Administration, the mining community, and Occupational Safety and Health Administration- regulated companies).

This paper outlines the evolution of managing high consequence risk scenarios in BHP Coal, with particular focus on managing the stability of an endwall that is adjacent to a public infrastructure corridor running through a Central Queensland coalmine. BHP Coal developed a Geotechnical Consequence Register after industry failures identified shortcomings in traditional risk assessment practices. Traditional risk assessments consider the consequence and likelihood of a geotechnical event occurring under normal operational conditions. As such, the risk rating for a high consequence, low likelihood failure may fall within the acceptable range, even where a lack of meaningful geological data exists due to the area being off lease or outside of mining reserves. The Geotechnical Consequence Register lists all geotechnical failure scenarios of high consequence, identifies if current controls are adequate, and highlights if specific Geotechnical Management Plans are required. This case study reviews a failure scenario that was identified through the geotechnical consequence register. The scenario had previously been considered low likelihood and subsequently driving a low risk; however, the likelihood ranking was impacted by a lack of geological data outside of the mining lease and predominately informed by an acceptable historical slope behaviour. Additional exploration identified previously unmapped faulting which increased the risk rating. The requirement to develop a specific management plan resulted in an increase in the required factor of safety, design changes, the implementation of additional monitoring and a corresponding monitoring Trigger Action Response Plans (TARP). In the case study area, the initiation of a geotechnical instability was identified early through the increased monitoring program. The instability was successfully managed and any impact to the infrastructure corridor off lease was prevented. BHP Coal has used the Geotechnical Consequence Register for several years to identify and assess high consequence geotechnical failure scenarios. In some areas this has justified additional exploration, more stringent design acceptance criteria and an increased level of geotechnical monitoring. The process has been successful in ensuring that geotechnical risk is controlled in areas that often receive less attention than active production areas.

Mine to Mill optimisation is not a recently developed concept. It is one which is finally gaining greater acceptance in the Australian Mining Industry, possibly as a consequence of the downturn in markets and certainly made possible by the development of tools and techniques to measure and predict performance of mining equipment and milling plants. Fragment size after blasting has a major bearing on the performance of all aspects of mining and a major influence on the economics of the mine and mill. The process to optimise fragmentation consists of identifying the key factors, measuring their sensitivity through a modelling and trialing program and conducting economic evaluations on the changes on both the mine and mill. Management of the Ernest Henry copper-gold mine in NorthernQueensland, Australia, has espoused the Mine-to-Mill concept and isactively pursuing excellence in blasting to drive down the total cost of mining. Ernest Henry has extremely hard, wet and heavily jointed geological conditions, and had high powder factors and a large percentage of oversize material reporting to the crusher or ROMstockpile. A blasting improvement program was undertaken betweenErnest Henry Mining, Thiess Contractors and Orica Explosives tooptimise the ore size distribution, particularly the oversize component, as a step to reducing production costs. Measures of success included bringing the rock breaker back from double shifts to part-time operation and increasing direct dumping into the crusher from 70 per cent to 92 per cent. Tools and technologies to predict and measure the performance ofindividual blasts are needed before it is possible to optimisefragmentation or the downstream processes. These include suchspecialised Orica Explosives tools as SABREX, POWERSIEVE, andPOWERLINE and commercially available drill monitors. As importantare the more common still and video camera, borehole loggers,measuring tapes, surveying equipment such as face profilers, and ahigh-speed camera if possible. There are many tools which can be applied however it is rarely necessary or possible to use all of them due to time constraints and a point of diminishing returns. The critical point is knowing what the true parameters of the blast were compared to design - only then can the predictive tools be effectively utilised. Stage one of the program has seen gradual improvement as theunderstanding grew and practices have been implemented resulting inreduced bridging in the crusher, geological mapping with respective blast designs and the oversize component under control. This paper is about a process approach which links tools and technology to understand the fragmentation drivers and then to optimising the size distribution.The continuous improvement program continues...

Silicon Valley Clean Water signed a progressive-design-build (PDB) contract with Barnard Bessac Joint Venture (BBJV) to design and construct the Gravity Pipeline tunnel in Redwood City, CA. Owner and Design-Builder teamed with designer Arup to provide a final product: 3.3 miles of 182 in. O.D. precast concrete segmentally lined tunnel with 10 ft. and 11 ft. I.D. Fiberglass Reinforced Polymer Mortar (FRPM) carrier pipe, complete with two FRPM Drop Structures. BBJV and Arup collaborated with SVCW to evaluate alternative design concepts, negotiate an agreeable price and reduce contingencies through a transparent estimating process, and deliver the project ahead of schedule.

An evaluation was recently performed on a district recirculation system at a trona mine in Granger, Wyoming. A 1.52m (5 ft) diameter, 37.3kw (50 hp) vane axial fan was installed in a crosscut between the intake and return airways and recirculated approximately 31.4 m'/sec (66,578 ft'/min) of air back into the intake. This amounted to a recirculation factor of approximately 20 pct of the return air. This air was part of the primary ventilation to three different continuous miner sections at operating distances of between 6l0m (2,000 ft) and 1,829m (6,000 ft) from the recirculation fan. The mine environment was constantly monitored for respirable dust, methane, and carbon monoxide in the operating sections and in the main intake and return entries. In addition, air samples were taken with and without the recirculation system during the evaluation and analyzed using gas chromatography. The results of this study showed only small increases in dust, while all other mine contaminants remained at such low levels that it was impossible to determine any relative changes from the recirculation system. Because of this, a brief SF. tracer gas analysis study was subsequently performed to more precisely determine an increase in a mine contaminant with the recirculation system.

This paper highlights salient features of an excavation 38 m (Length) by 10 m (Width) by 24 m (Depth) carried out in Central Kolkata. Secant pile wall (SPW) formed using a combination of alternating 1200 mm & 800 mm diameter piles was used. Six layers of struts and waler beams, including bottom slab were utilized as lateral restraints to piles. The geotechnical profile for excavation zone was predominantly silty sand to sandy silt. The profile is characteristic of river channel deposit usually encountered in Kolkata. Temporary stage analysis (including seepage analysis) and design were carried out in FEM softwares. Design checks for clay bursting, piping, bottom heave, etc. were also carried out. Extensive instrumentation and monitoring were carried out during the excavation period to check parameters such as ground movement, pile movement, piezometric heads, etc. as per review levels. During excavation, ground improvement using Tube-a-manchette grouting technique was also required to be carried out as per field observations. The paper discusses some of the major challenges faced as well as measures taken to counter them, during deep excavation carried out in silty soils in a very congested Urban setting in the heart of a Megacity.

The rock mass to be excavated is classified using data obtained from the initial site investigation, and the design of excavation and support is then made based on the rock class. Rock classification provides an empirical design with guidelines for tunnel support compatible with rock mass quality and the method of excavation. Although most clients provide standard patterns corresponding to each rock class, the patterns of drilling and blasting have only been used in estimating the excavation cost and not used in the construction stage. The purpose of the study presented in this paper is to establish a standard blast pattern to yield optimum blasting performance along with an attempt to reduce a difference between design and construction. A series of test blasts were carried out at two tunneling sites of Central Inland Highway crossing the country. Long hole blasting was tested in rock mass whose RMR value is over 60. The blasting results were evaluated by measuring degree of fragmentation, advance rate, muckpile distribution, overbreak, etc. Based on the results of test blast, several patterns were proposed for a twolane highway tunnel.

Seismic risk in the deep gold mines of South Africa has been studied for many decades. A clear understanding of the effect of geotechnical conditions on the seismic hazard nevertheless remains elusive. Certain reef types seem to be associated with a higher risk of rockbursts. The stability and deformation behaviour of excavations on the different reef horizons are affected by the rock types and the varying strength properties. The seismic response to mining is therefore also expected to differ according to the geotechnical conditions. As a case study of this behaviour, the seismicity at Mponeng mine was investigated. On the VCR (Ventersdorp Contact Reef) horizon, two areas can be delineated. On the eastern side of the mine, the footwall is shale, and on the western side it is a strong brittle quartzite. More large-magnitude events occur in the area with the shale footwall than the area with the quartzite footwall. Moment tensor analyses indicated that the majority of the large-magnitude events are not related to geological structures, but are face-related, implying that shear failure of intact rock is occurring ahead of the mining front. Preliminary modelling indicated that the closure volume for the shale footwall may be higher than that for the quartzite footwall, providing a possible explanation for the observed difference in seismic response.
