The object of this paper is to present for your consideration some of the present-day trends of safety practice in the coal mines in the United States and Canada. Coal mining at its best is a hazardous occupation. Men must be experienced miners in order to appreciate the existing hazards and must remain ever conscious of them if they are to work safely. The question is often asked whether our efforts for increased safety are showing results. So often it appears that strides are made for a period of a few years in the reduction of fatalities and lost-time accidents only to have all ground lost by an unfortunate year, after which progress is commenced again only to have a repetition of the same thing. Table No. 1 is an answer to this question. It shows the fatality rates over the last two decades in the coal mines of the United States and Canada. These are given in divisions of five years. The general trend is noticeably downward, exceptions being due to explosions or disasters which claimed an unusually heavy toll of life. These last are certainly not as common today as they have been in the past. It is only reasonable to suppose that this trend towards a lower fatality rate is the result of well directed safety effort. Reliable statistical organizations have offered figures to the effect that only 15 per cent of all accidents can be eliminated by the use of mechanical safeguards. The state of mind of the man involved, or his attitude towards safety, commonly called 'safety-consciousness,' is the controlling factor in the prevention of 85 per cent of our accidents.

IN THE remarks which I am about to make concern¬ing the safety work of the Bureau of Mines, I want first of all to disengage myself from a disposition, which is frequently in evidence, to give spectacular emphasis to the hazards of mining operations. It is of the' utmost importance to the industrial welfare of our country that the enormous annual requirement of fuel be produced and be made available in our economic activities, with a minimum of distress and hardship. The safety of coal mining is a condition with which. we are all concerned in a fundamental way. And yet we should not rush to accept hastily conceived notions that laxity among employers in respect to safety measures exists to any outstanding degree. In fact in a large number of instances, the precautionary efforts of min¬ing companies, directed to the elimination of accidents are beyond all criticism, albeit the industry suffers in reputation from the carelessness of, its less effi¬cient constituents. The Bureau of Mines has been a leading influence in disseminating the doctrines of safety, in securing the cooperation of the miners themselves, and in stirring up ' the laggards, both among the employers and the employed. It is much to the interest of well-conducted mine operations that the industry, as a whole, observe safety regulations to the utmost.

THE subject assigned me, "Safety in Mining," is a very broad one and only the high spots can be covered in this short paper. As this is a meeting of the Coal Division, these remarks will be directed largely to coal mine safety, although the principles of safety and effective accident prevention methods are relatively the same in every branch of mining or industrial work. The coal mining industry should take pride in the part it has played in the development of the accident prevention movement, and there is ample proof for the statement that the first real organized accident prevention, work, both abroad and in this country, was con- ducted in the coal mining industry.

Safety work in mining at the Andes Copper Mining Company, Potrerillos, Chile, is divided into three parts: (1) accident prevention, (2) fire prevention and protection, and (3) silicosis prevention and control. In the company's operations a steady campaign has been waged to reduce accidents and silicosis, and to prevent fires. In the following pages the safety record for the past twelve years will be examined and the methods explained by which the accident rate was reduced. Table 1 contains yearly data pertaining to the scale of the mining operations and the accident record. Block caving is used for all ore production and the tonnage mined has aver- aged 30,000 short tons per day for the past seven years. Table 2 classifies 44 fatal accidents which have occurred since 1935. The leading causes of accidental death are transportation of ore and use of explosives, followed in order by per- sons falling, falls of ground, and by asphyxiation. In 1973 the percentage of total coal production mined by surface methods in the United States again increased, reflecting a long term trend. Barely three decades ago less than 10 percent of the U.S. coal was produced in this manner. Last year surface coal mines produced 299 million tons, 50.7 percent of our nation's coal. At present there are about 2450 strip mines producing coal in 23 states. While accident rates for surface coal mines are less than underground coal mining rates, surface coal mining is certainly one of the more hazardous occupations. Figure 1 shows the injury rates (fatal and non-fatal accidents) of major U.S. industries. Note that surface coal mining is more hazardous than all other industries except contract construction. Accident rates are about the same as the contract construction industry which uses similar equipment. A limited amount of detailed data was available on elements of surface coal mining jobs and this lack of compiled information made it difficult to assess the significance of the conditions contributing to accidents and to make recommendations that would reduce accident frequency. Therefore as part of the Bureau of Mines continuing research efforts related to the health and safety of the coal miner, a contract was awarded to Theodore Barry and Associates of Los Angeles to conduct an "Industrial Engineering Study of Hazards Associated with Surface Coal Mines."

Having seen significant improvements to accident rates in the last 40 years, companies in the Swedish mining industry now show a greater focus on the development of safety cultures throughout their organisations and workplaces. However, there is a lack of research examining the different safety initiatives and strategies practiced in the industry today. This study explores the potential influence and consequences such initiatives may have on the development of safety cultures in the Swedish mining industry. Twelve interviews with experts on safety initiatives from four different Swedish mining organisations were conducted and analysed in a process based on qualitative thematic analysis to identify notable connections to safety culture development. The results of these interviews highlight proclivities in the implementation and use of safety initiatives such as subjects of focus, methods and desired effects. This enables the interpretation of the conceptualisation and methods for the development of safety culture in these organisations based on their approaches to safety development. We believe the results of this study can serve as support for discussions on safety culture development in the Swedish mining industry, and be of interest for international mining industries, in addition to approaches to research in this field. However, we also believe it is important to emphasise the opportunities to approach safety culture in mining from different perspectives than those common today.

The time-worn adage "an ounce of prevention is worth a pound of cure" was never more applicable than when used in reference to coal mine safety. Many individuals have dedicated many hours to developing effective accident prevention programs, and much has been accomplished. Each production escalation, however, creates new hazards and safety problems which need special study and research. It is a never ending cyle. To study the progress of coal-mine safety is to raise memories of many individuals who have been dedicated to removing the hazards that have developed with increased productivity and deeper penetration into the earth for coal. Some vivid examples of this dedication are the works of the men who pioneered the use of the safety lamp and the use of rock dust in mining.

Safety First and zero accidents are the targets most mines in Australia are aiming for, but manual processes and procedures limit the capabilities and response time in emergency situations. Even with a high safety focus, accidents and incidents still happen, too often with tragic outcomes. Dangerous areas, narrow roads and winding curves are all daily obstacles that threaten workforce safety. Every unforeseen event, such as traffic collisions and other accidents, puts lives in danger and weakens production. Safety must be guaranteed.

Safety Leadership The management and leadership of a mining operation is a challenge at all times. The pressure to make production, safety, and environmental goals is intense. At some point mining cycles will put every operator into periods of time where economic issues seem to over-ride all other concerns. This is when leadership is most needed.

Improvements in mechanisation should not be considered independently of safety and of environmental hazards. The need to keep gas contents within statutory limits may make the time needed to partially drain a gassy seam the effective control on face development times. Dust levels may provide an effective control on coal extraction rates on any one face. The combination of gas and dust on high output longwall faces may make it necessary for each one to be treated as a separate mine in terms of ventilation, and the faceworkers may need special protection. Mechanisation and safety can be mutually beneficial when mechanisation combined with systematic safety assessment reduces accidents, and when a thorough study of how to reduce accidents and health risks indicates the best method to improve mechanisation.

The problem of spontaneous heating/fire is very common in coal mines all over the world. The coal industry faces many problems day by day due to the occurrence of spontaneous heating in the open pit coal mines. Presently, about 70 per cent of total coal production in Indian mines is being carried out by open pit mining. Fire in coal, particularly in open pit mines not only causes irreparable loss of natural wealth but also damages the surface structures and pollutes the environment. It is well known that 75 per cent of the fires occur due to the spontaneous combustion of coal. Spontaneous heating is a very common occurrence in coal mines and is a major factor contributing to mine fire. The liability of coal to spontaneous heating causes many serious problems before, during and after mining. In opencast mines, coal immediately oxidises and catches fire due to the intrinsic characteristics of coal, such as low rank, high moisture, high volatile matter, presence of sulfur in the form of pyrites, low crossing point temperature (CPT) and ignition point temperature (IPT) value and less incubation period. In opencast mines, when the coal benches are left idle for a longer time, heat accumulation takes place in favourable conditions and sometimes leads to fire. The purpose of this paper is to present the different successful case studies regarding the safety management of open pit coal mines from occurrences of spontaneous heating/fire.

"INTRODUCTION While the United States coal mining uses prescriptive regulation under the Title 30 Code of Federal Regulation enforced by the Mine Safety and Health Administration (MSHA), Australia uses a risk based approach. Australian regulation is individual to each state. In Queensland, Australia this risk based approach to address principal hazards is underwritten by the Queensland Coal Mining Safety and Health Act of 1999 and the Coal Mining Safety and Health Regulation of 2001. Each Queensland Mine is required to have an overarching Health Safety and Environment Management System, of which Safety Management Plans are an element. “Safety Management plans (SMP’s) are intended to formalise the process by which mines address principal hazards and other related matters to ensure the safety of mine personnel.”7 THE QUEENSLAND CONTEXT In Queensland, the basic elements of a coal mine’s safety and health management system must provide for risk identification and assessment, hazard analysis, hazard management and control, and the reporting and recording of relevant safety and health information and data.5 (Part 2 Division 1 Section 6(a-d)) The initial guidance for writing Safety Management Plans comes from the Approved Standard for Mine Safety Management Plans and Guidance Note QGN09- Reviewing the Effectiveness of Safety and Health Management Systems. The Guidance Note and Approved Standard are supplemental to the Act and Regulation, and put in place to provide more clarity around the topic. The definitions in this section come from the Approved Standard and are included to provide context for those who may not be familiar with Australian or Queensland law and regulation. A hazard is defined as a source of potential harm or a situation with a potential to cause loss, while a principal hazard is a hazard with a potential to cause multiple fatalities. A risk is the chance of something happening that will have an impact upon objectives which is measured in terms of consequences and likelihoods. It is important to be able to make the distinction between a hazard and a risk. For instance, a lion is a hazard, while the risk associated with a lion is getting eaten. As defined by the Queensland Approved Standard for Mine Safety Management Plans, a Safety Management Plan (SMP) requires a systematic definition of all the actions necessary to ensure that mining operations are carried out safely. A SMP includes, but is not limited to organisational structures, planning, activities, responsibilities, practices, risk identification, audits and reviews.7 The goal of a SMP management plans is to mitigate the risks to bring them to an As Low As Reasonably Achievable (ALARA) level."

The Mining Division of Utah Power and Light Company is located in the coal fields of southeastern Utah -main office in Huntington, Utah. The Division will produce 6.5 million tons of low sulfur steam coal in 1988, making UP&L the largest underground mining operation west of the Mississippi. Coal is mined from three underground mines utilizing continuous miner and longwall mining methods. All coal produced by the Mining Division is utilized at UP&L's two largest steam-electric power plants, the Hunter and Huntington plants with a combined capability of 1,995,000 Kw. At UP&L we believe that to achieve success a company must have clearly defined goals which are understood, communicated and shared at each level of operation. We have also found from experience that a safe operation is generally an efficient and productive operation. As a Division, we have established a goal to achieve the safest, most efficient operation in the country. This is an ambitious goal. To achieve such a goal requires real commitment and a safety/training program capable of achieving needed results. The following is an overview of the program we have established at UP&L which has enabled us to reduce our accident frequency rate from over 40 in 1980 to 8.00 at present, as illustrated in Figure 1.

ABOUT 1929 an engineer was engaged to organize a Safety Department, lay out an educational program, and achieve a reduction in accident frequency. Due to the large number of employees, about 12,000, and the distances between the various properties, it was a big undertaking. Most of the workmen are from the Sierra and for generations have been agricultural people, with little conception of modern tools and machinery, and to a large degree are illiterate. However, gratifying results have been obtained, as evidenced by the accident frequency curve during the sixteen-year period. Safety offices have been established in each of the six major properties, staffed by men who devote their entire efforts to inspections of working conditions and equipment, instructions as to safe methods, and investigations of accidents. The Corporation is a member of the Inter-American Safety Council, receiving their publications and literature, which serve to keep us up to date and informed on modern methods. Dissemination of this material is accomplished chiefly through word-of-mouth and bulletin boards. When it is found that the accident rate is increasing in a certain occupation, as for instance underground haulage or mine hoisting, the operators so engaged are assembled and given renewed instructions on the safety code pertaining to their work. Inter-camp competitions in accident avoidance are being conducted, with annual awards for that unit having the lowest severity rate for the year.

ALTHOUGH most accidents occur through the A carelessness or misfortune of the workmen; that is no reason why we should not take all physical precautions practicable. The best way to approach the problem of mine safety is to acquire a personal feeling of responsibility for the accident and then plan how it might have been prevented had we known that it was to occur. A statistical analysis of the physical conditions contributory to accidents will give an indication of some of the methods or mechanisms that should be revised or safeguarded. The handling of rock or ore is a common source of minor injuries. Freedom from accident when mucking largely depends on the skill and care of the workman; this also is true, to a certain degree, to loading cars from chutes, but much can be done in the layout of chutes and their equipment to provide equal safety with less skill. Permanent stagings, or rigid units that may be easily assembled, should be provided so that the men handling the chute boards or gates and barring the jammed chunks will have a convenient safe footing for their operations. A stance to the side of the chute mouth is to one in front, because a man will not be liable to be struck by flying lumps or his bar, should it he caught in the mass, yet in many mines the workmen face the chute when loading a car. Fig. 1 shows the operation of a plank chute having special latches which support the chute boards in such manner that they may be conveniently dropped into place to stop the flow of ore.

FROM 1870 to date, the coal production of Utah has been somewhat less than 85,000,000 tons. There have been at least three major disasters with total fatalities about 380; or approximately. 4.4 persons killed by mine explosions per million tons produced, a record that is decidedly bad. Even during the periods when there have been no disasters, such as the years 1914 to 1920, inclusive, the fatalities .in ordinary working ran about 5.4 per million tons produced, against 3.7 per, million tons. for the whole United States for the same years. The five-year period following the war, 1918 to 1922 inclusive, showed 4.7 fatalities per million tons, hence the fatality record in the coal mines of Utah has uniformly been in excess of the average of the coal mines of the entire country. Notwithstanding this poor record, Utah has given to the coal-mining world many excellent safety methods and practices and the Utah. coal mines in general take greater measures toward safe mining than do the coal mines of any other State; if practices and methods allowed and pursued in coal mines of other States were tried in Utah coal mines, there would be a succession of disasters. Utah's principal producing coal mines, operated by about eighteen companies and numbering less than forty, have comparatively thick seams. Very little coal under 5 ft. thick is being worked and practically none under 4 ft.; much, if not most, of the coal comes from seams in excess of 8 ft., some running as high as 30 ft. In thick seams in which coal is left on floor and roof, any dust found or made is entirely combustible. In addition, the thick seams induce the use of large capacity cars, handling which in rooms and other workings on fairly heavy grades (up to 10 or .12 per cent.) tends to cause haulage accidents. The thick seams give high ribs and roof, in which it is difficult to prevent slabs of coal or rock from dropping on workers and also difficult to keep free of gas and of coal dust. The coal is high volatile, running usually over 40 per cent and in some instances nearly to 50 per cent; hence is highly flammable. Most of the coals have very high resin content, the resin usually being in dust form, hence almost as highly flammable as black powder. Best practice standards generally recommend assessment of the environmental and societal risks posed by tailings storage facilities, but they rarely specify how the analysis should be made. As a case study, this paper describes a safety assessment of the Zelazny Most tailings pond operated by the Hydrotechnical Unit of KGHM Polska Miedz SA in Poland. The complete assessment includes a failure mode effect criticality analysis (FMECA) that considers all conceivable failure modes, but this paper focuses on one key mode, the loss of stability during the planned raising of the crest above its current level. Reliability against static dam failure considers uncertainty on the safety factor in a limit equilibrium approach. The threat of dynamic (earthquake induced) instability is assessed by developing a model of future seismicity as a function of the mining plan and considering uncertainty on the induced permanent displacements along critical sliding surfaces. We quantify the additional level of protection against outflow provided by the beach using an event tree with expert-assigned probabilities. Continuous monitoring of the dam for premonitory signs of instability and a set of loss-prevention plans in the case of a spill provide additional layers of protection; their effectiveness is quantified by estimating the fraction of the population that could be evacuated as a function of warning time. The probabilistic analysis indicates that the safety targets are met and produces practical recommendations on how to maintain and improve the safety level while the dam crest is raised above its current level.CITATION:Van Dyck, J, Pytel, W, Veneziano, D, Snow, R and Zanoli, O, 2015. Electronic detonators have been in commercial use for a decade, with an excellent safety record. This paper lists known incidents involving electronic detonators. Typical standards required by regulatory bodies for static electricity and electromagnetic fields are reviewed. The performance of some detonators is compared to these standards; in general the minimum standards are easily exceeded. The general resistance of electronic detonators to extraneous electrical energy that can be derived from the body of test results is compared to danger levels for exposure of humans to these energies; it is shown that electronic detonators can safely tolerate higher electrical energies than people can. This comparison includes comparable data for electric detonators, which are shown to be more vulnerable to extraneous electrical energy than people are. Electronic detonators also bring significant safety benefits in blasting due to their testability, two-way communications, reliability, programmability and precision. The links between these attributes and enhanced safety are discussed with examples from actual blasting. Although electronic detonators are usually more vulnerable to extraneous electricity than non-electric initiation systems, the paper shows that the net safety benefit in handling and blasting is in favor of electronic detonators, which are the safest initiation system that has ever been offered to the mining industry.
