Computer-related accidents have caused injuries and fatalities in mining as well as other industries. Normal accident theory (NAT) explains that some accidents are inevitable because of system complexity. NAT is a classic argument in organizational sociology although it has been criticized as having imprecise definitions and lacking criteria for quantifying complexity. These limitations are addressed by a unique approach that recasts this organizational theory into an engineering-based methodology to quantify NAT complexities of computer-based systems. In this approach complexity is categorized as external or internal. External complexity is defined by the external behavior of a system, and is quantified by these dependent variables: system predictability, observability, and usability. Dependent variable data contain the perceptions of 32 subjects running simulations of a system. The system?s internal complexity is characterized by modeling system-level requirements with the software cost reduction (SCR) formal method. Model attributes are quantified using 15 graph-theoretical metrics?the independent variables. Five of 15 metrics are correlated with the dependent variables as evidenced by structure correlations exceeding 0.25, with standard errors <0.10 and a 95% confidence interval. The results also show that the system predictability, observability, and usability decreased as NAT complexities increased. This research takes a step forward in operationalizing NAT for computerized systems. The research benefits mining and other industries as well.

Mine financing, or providing adequate capital for developing and bringing a mining property into production, is an essential requirement for a successful operation. Today it presents a different picture than even twenty-five years ago. The day appears to be gone when a mining property could pay its own way from grass roots to production. The country has been quite thoroughly combed for surface showings from which sufficient initial profit could be immediately garnered to pay for further development and production facilities. The four M&apos;s for success in mining-mineral, market, management, and money-are still essential; but doubled or tripled wage costs and steep advances in cost of equipment and supplies throw increased emphasis on adequate capital funds for bringing in a prospect and making it a producer. Consequently, the old picturesque, although seldom rewarding, grub- staking procedure where a single venturesome citizen might advance funds to a prospector on a fifty-fifty basis for whatever discovery he might find, has largely disappeared. Its place, to some degree, has been taken by the small syndicate of venturesome members who band together for furnishing funds to a group of prospectors for the same general purposes. Such a syndicate has a definite organization and is usually assured of sufficient initial funds, or flow of funds, to carry on work for several years. It is headed by an engineer-promoter who has had considerable field experience as well as personal knowledge of his prospecting crews. For the owner of a potential mining property who has ample capital, the problem is to determine (1) justification for the necessary investment in exploration and development, and (2) the economics of the enterprise as to size and degree of integration of the final operating entity. If the owner of such a property has little or no capital, he also faces these same basic problems but, in addition, must be able to present the findings of fact and persuade the prospective investor that the enterprise will produce

Tunnel boring machine (TBM) tunneling is an ever-increasing prospect for underground construction, and, with each new tunnel bored, there are unknown elements. When boring through the earth, even extensive geotechnical baseline reports can miss fault lines, water inflows, squeezing ground, rock bursting and other types of extreme conditions. This article draws on the consider-able field service experience within Robbins to analyze successful methods of dealing with the most challenging conditions encountered. Many tunnel projects are located in areas with relatively poor access along the tunnel alignment and bored under extremely high overburden. These two factors often result in limited geological information. It would be reason-able to state that the deeper the tunnel, the greater the lev-el of uncertainties. When faced with these uncertainties everyone involved with the project including the owner, the contractor and the machine supplier must be prepared to tackle geological surprises. This article describes the problematic geological conditions and associated difficulties faced on three separate projects and the measures that were taken to overcome these difficulties.

"At the start of a new shaft project, a specific number of ground investigations are performed. However, it is not uncommon that the ground conditions during shaft sinking may differ from the designed ones. Therefore, it is extremely important to use tools which minimize discrepancies and to include a monitoring system in the shaft design to collect sufficient data. The paper presents an overview of existing monitoring techniques and discusses future works required to improve, not only the construction during shaft sinking, but also the lining design methodology. INTRODUCTION In most cases, the investigations performed at the start of a new shaft are based on the analysis of rock and soil probes obtained from boreholes. In practice, such analysis performed on a single core often leads to different results when conducted by various experts. The discrepancy in core analysis influences further rock mass assessment and design assumptions. It is not uncommon that the ground conditions during shaft sinking may differ from the designed ones, which affects the estimated stress and strain state of the shaft lining. Therefore, it is extremely important to use tools which minimize such discrepancy and to include into the shaft design a monitoring system to collect sufficient data, which enables control of important phenomena, not only during shaft sinking but also during the life of the shaft. Furthermore, the information collected during shaft sinking, while it is important for proper shaft lining design and updates to the shaft lining due to the local mining and geological conditions, it is also important after shaft sinking. In the whole shaft lifetime, it is not uncommon that the shaft lining is damaged and the liner needs to be renewed (Figure 1) due to various reasons. For this purpose, detailed information collected during construction can be extremely important."

Regulation of mineral property valuation methodology by governments and others is due to frauds, condemnation and takings, banking industry problems, and litigation. Definitions of "reserves," "resources," and "qualified person" continue to evolve, along with guidelines to the data and disclosure required to support valuations. A less recognized but vital issue relates to the regulatory acceptability of alternatives to the comparable sales method. Comparable sales generally work well for surface real estate, and sometimes for precious metal deposits, but not at all for most industrial mineral properties. Alternative methods, such as net present value, or a geoscience rating system, often provide better valuations. Yet US regulations dictate comparable sales as the preferred method. The task of convincing regulators of the inapplicability of comparable sales to mineral properties, particularly those lacking reserves, may be as important as the valuation itself.

To maintain flexibility and ductility of tunnels crossing active faults, various mitigation measures are being utilized depending on fault offset movements and seismic design criteria. Examples of those approaches are simple bell and spigot joint, articulated joint, and complex deformable longitudinal bolt and spring joint system with seismic isolation washer. This paper presents a summary of tunnel fault crossing mitigation approaches and design considerations using previous relevant experiences in California as well as case histories from Japan, Taiwan, Turkey, Greece, and Italy found in literatures. Comparisons are made between large and small diameter tunnels, transportation, and utility tunnels, as well as bored, mined and cut-and-cover tunnels. Recent innovations, such as Japanese and Italian techniques, for tunnels crossing active faults are also discussed.

Previous researchers have put forward two different theories as to the origin of air overpressure from quarry blasting. In 1980, Siskind et al postulated that the initial face movement gave rise to the displacement of air and that this resulted in the air overpressure pulse. However in 1990 McKenzie et al carried out research that indicated that the air overpressure pulse came into being as a consequence of the shock wave created by the detention of the explosive charge in a blast hole. In an attempt to evaluate these hypotheses, an investigation was undertaken over a 4-year period with the aim of determining the precise origin of air overpressure. The research was carried out at a chalk quarry in the North of England.

From September 1978 to December 1983, ESD Corporation (formerly a Division of FMC Corporation) undertook the development and testing of a bidirectional auger under contract with the U.S. Department of Energy. Underground testing began in August 1981 at Peabody Coal Company&apos;s Marissa Mine near Marissa, Illinois. The auger was developed to increase mine production and coal recovery without increasing land subsidence. It bored 4-foot-diameter by 10-foot-deep holes into pillars on 7-foot centers (each hole recovering 5 tons). All operating during this test was performed by maintenance personnel with the goal of finding ways to increase the machine potential. The maximum excavation rate achieved was 1.4 tons per minute. It was concluded that the concept of drilling short holes to extract coal from underground pillars without surface subsidence was practicable with a wheel mounted auger, but that more power was desirable for this mine, and that the hydraulically driven augering system that provided experimental versatility was not sufficiently reliable for production use.

THIS REPORT describes the occurrence, mining, and use of refractory clay in Colorado within Region III of the Federal Bureau of Mines, which includes the States of North arid South Dakota, Nebraska, Wyoming, Colorado, Utah, New Mexico, and Arizona. Field work by the Bureau of Mines, United States Department of the Interior, included examination of active and abandoned refractory-clay mines as well as a reconnaissance survey and sampling of numerous clay-bearing formations exposed in Colorado. Information from the survey and the sampling will help guide private industry in its search for virgin sources of refractory clay. Library research of published and unpublished reports from various Federal, State, and private sources was conducted during the early phase of the investigation. Most of the refractory clay mined in the Denver-Golden district of Colorado is not used for refractory purposes but is blended with other clays in the manufacture of structural clay products. The predominantly flint-clay de-posits in the Pueblo and Fremont County districts are mined and utilized chiefly in fabricating refractory products.

This glossary is publi~hed- by the Bureau of Mines as a contribution' to the mining literature in the belief that it will fill a long-felt need. It contains about 20,000 terms; these include both technical and purely local terms related to metal mining, coal mining, quarrying, petroleum, and natural gas, and metallurgical works; names of useful, important, and common minerals and rocks; an<lgeological terms. It presents in one comprehensive volume the available standard, technical terms relating to the mining and mineral industry, as well as provincialisms that have been or are now in use in English-speaking countries. The glossary also includes many terms relating to ceramics and the clay industry, glass making, foundry practice, railway and building construction, electrical installation and power-plant equipment, and chemical terms relating to metallurgical practice. Complete lists of terms for' each of these allied industries are beyond the scope of this glossary. Paleontological terms, although closely associated with geology, are far removed from mining and metallurgical operations, and for this reason have been omitted. In a compilation of this magnitude, it is difficult, within a reasonable time to verify all definitions as to the latest usage. Much verifying was done and it is hoped that the best and latest definitions have been used. Reference to the publications cited will enable the reader to determine approximately the period when the definition was used. Definitions in use by engineers of high national or international reputation are given first preference. When definitions from different sources are the same, credit is given to the earlier author as being the original or nearest to the original source. Immediately following each term the name of the locality wherein the word is presumed to have originated or is widely used is given, where such information is available. The name of the author or source from which the definition was obtained follows the definition, and serves as a key to the publications listed. "INTRODUCTION There is no doubt about the time and effort companies put in establishing and maintaining their equipment. However, the question about how these companies can map the benefits comes up. Neither do they have the tools to quantify the benefit of their action, nor do they have the tools to know the actual quality of their current maintenance system. The resources used in the process seem futile to observers. It seems impossible to trace improvements and to get information about the impact of actions in retrospect. Improvements in maintenance performance cannot be recognised by management or shareholders. Thus the necessity for measuring the performance of maintenance systems is given. In this paper ConMoto is systematically showing the strengths and weaknesses of many companies' maintenance management and is providing field-tested proven design elements for measuring maintenance systems and implementing Value Oriented Maintenance. Considering only business assets in production facilities and related machinery out of all investment goods in Europe, you reach a value of around 9.7 trillion euros in 2008. In Germany alone the equipment replacement value amounted to about 1.9 trillion euros [8,10,11,13]. If you calculate a cross sector maintenance costs rate of 4.8 % (corresponding to the average value identified in this study), direct maintenance costs in European production locations add up to over 450 billion euros per year [3]. These figures speak for themselves. Yet maintenance is not only of enormous significance for European enterprises. Our experiences show that maintenance and asset management, particularly sustainability issues, are also increasingly moving to the forefront in the BRIC states, in the Middle East and in many emerging countries. First of all, in order to meet the resulting high quality and cost related requirements of maintenance systems, a comprehensive overview of the issue is needed. This overview is provided by a detailed analysis of 83 companies and production plants from various sectors – with amazing results. SIGNIFICANCE OF MAINTENANCE Current studies show the high financial significance of maintenance and asset management. According to the market study in the Asset Manager [9], in industrialised countries about 10 to 15 per cent of the labour force are employed in maintenance and facility management tasks. In Europe about 40 to50 million people are directly and indirectly involved in retaining the value of machinery, plants, buildings, transport systems and infrastructure facilities. A glance at the costs side also demonstrates the great relevance of maintenance. EU-wide annual direct maintenance costs amount to a total of more than 1,500 billion euros. In Germany alone they are about 250 billion euros. This impressive total is considerably exceeded by indirect maintenance costs caused by technical failures. Indirect costs are estimated to be five times higher (Figure 1). [7,12,14]"

To make a thorough survey of a coal mine in order to eliminate hazards, the competent safety engineer must be familiar with mere than one branch of engineering. It is essential that he know the properties of mine gases and the methods of detecting them, as well as hew to prevent dangerous as accumulations. It is also essential that he understand the action of overlying strata when the coal is removed, and the proper methods of protection by timbering or other suitable means of support. He must know the value of protective clothing in reducing injuries and mast be informed as to the correct handling and use of explosives to avoid premature blasts, misfires, and ether failures in blasting practice. He must understand hew to operate locomotives and cutting, loading, and various other types of machines to be able to recommend effective safeguards for protecting the men who use them in addition to being informed as &apos;to these and many ether activities connected with the mining of coal, he must net overlook electrical engineering if he wishes to combat the fire, shook, and ignition hazards attendant upon the use of electrical machinery and circuits in and around the mine intelligently. There are many points relating to safely in the selection, maintenance, and use of electrical circuits and equipment that are perfectly obvious to the electrical engineer but perhaps are not se evident to those not trained in this particular branch of engineering. Engineers of the U.S. Bureau of Mines who have made a special laboratory study of electricity as related to safety in minus haw e also made inspections of electrical installations at a large number of bituminous mines. These inspections have revealed practices and conditions that not only do not contribute to safety but that add nothing to efficiency and economy in the operation of the mine.

Placer mining is the mining and treatment of alluvial deposits for the recovery, of their valuable minerals. The method has been used principally for mining gold, but a large proportion of the world&apos;s production of tin, platinum, and diamonds and other gem stones and minor quantities of other heavy minerals have been won in this manner. In the United States, as in the world at large, gold has been the principal mineral obtained by placer mining. Minor quantities of metals of the platinum group are recovered with the gold in some localities. Important quantities of sapphires have been produced at placer mines in Montana, and tungsten minerals have been obtained on a commercial scale from placer deposits in California and Colorado. Other heavy minerals or gem stones, however, have not been mined to any important extent by this method in the United States. The search for placer gold and the working of the deposits when found have had much to do with the early development of the West. Placer mining has been gradually overtaken and surpassed in importance by lode-gold mining, until in 1932 less than a quarter of the country&apos;s total gold production was from placers or about an eighth, excluding Alaska. In 1932 about 76 percent of the placer gold produced in the United States was recovered by dredging. Although other forms of placer mining still are important, they have been declining for many years, as the richest and most readily mined deposits of gravel along the stream courses have been exhausted. During 1931 and 1932 there was a revival of small-scale mining, but few new deposits were discovered.

The primary objective of this paper is to discuss the hazards and furnish a list of recommendations for the safe operation of quarries and related works. They have been compiled from Bureau of Mines publications covering the safety experience of engineers before and during the year in the mineral industry,5 from a publication of the National Safety Council, Inc.,6 from explosives recommendations made by various explosives manufacturers,7 and from suggestions obtained from quarries visited. The second objective is to describe and illustrate safety devices, practices, and quarrying methods. The quarrying and related industries of the United States had a lower accident-frequency rate than either the metal- or coal-mining industry as reported for 1942.8 However, the quarry industry ranked eleventh in 15 major industries during 1942 in lost-time accident fre¬quency; metal mining ranked fourteenth and coal mining fifteenth.

Introduction 1. Factors on Which the Design of the Roaster Plant was Based The roasting operation at Lake Shore, it was realized from the outset, would be profitable only if costs and losses could be reduced to a low figure. It was absolutely imperative that the operation should be self roasting over the whole range of tonnages and sulphur grades which might obtain during the life of the mine. The roasting operation could not bear the cost in fuel and labour of even a partially fired roast. To meet this requirement, it was necessary to resort to additional insulation in the construction of the roaster and to provide a more flexible means of rabble cooling than the conventional water jacket (see p. 31). A second basic consideration was that of handling a wet, putty-like concentrate. The operating costs of a drier ruled out the possibility of drying the charge before feeding it to the roaster. Moreover, the dusting which would result from charging a dry product virtually all -40 micron (325 mesh) in size would seriously handicap the use of a dryer. Dust lubrication (p. 3S) due to the back-mixing action of the Edwards rabbles, and the use of a Sullivan slapper ( p. 27) on the concentrate filter, completely solved the problem of handling the roast feed. The third consideration was the reduction of dusting to a sufficiently low point that a Cottrell or other dust collecting system was unnecessary. The Edwards roaster when properly supervised, it is believed, can be operated to give the minimum possible dusting. At Lake Shore, the evidence has been that less than 1 per cent dusting occurs at any time and that the greater part of the dust produced is recovered either in the spray tower or in the dust chambers.

In 1948 the United States became a net importer of oil. Coupled with an unprecedented domestic production of 5,900,000 barrels daily, imports approximating 500,000 barrels made available a total supply of 6,400,000 barrels a day. Thus, by a herculean effort, the oil industry was able to meet an extraordinary demand at home and, in addition, to ship abroad almost 460,000 barrels daily of petroleum and products. The country’s oil position now differs greatly from that prevailing at the beginning of World War II. Then, actual oil production averaged about 4 million barrels a day, and productive capacity probably was greater than 5 million barrels a day. As the demand of the military and industrial machine grew with the rising tempo of the drive toward victory, the United States not only met its own needs but produced about two-thirds of the total oil requirements of the Allied Nations, excluding Russia. As a people, we can be deeply grateful that the Nation’s petroleum resources were adequate and sufficiently developed to support such a tremendous effort. Even in those critical hours when immediate needs were the paramount issues, a prudent Congress paused to contemplate the importance of an assured long-term supply of liquid fuels. From these deliberations came the Synthetic Liquid Fuels Act of 1944, which authorized research and development work on new sources of oil – coal, oil shale, and agricultural and forestry products. Since World War II, a civilian demand for 6 million barrels of petroleum products has become the order of the day. By drilling as many as 39,000 new wells a year, the production rate has increased enough to meet domestic requirements by a bare margin. Some imports were necessary, however, to fill both domestic and foreign demands, and these doubtless will increase. Is this policy of exploration within the United States and the development of oil in foreign countries adequate insurance for the fulfillment of our future national needs? This question has two aspects – one purely civilian and economic in character and the other significant from the standpoint of our national defense. There are now approximately 4,000 producing oil fields in this country. At first thought, it would appear that production could decline in a considerable number of these fields without materially affecting the overall daily output. However, 48 percent of the oil comes from only 111 major fields which contain 60 percent of the total proved domestic reserves. It is apparent, then, that the flow from large fields – having proved reserves of 100 million barrels or more each – must be sustained if daily production is to be kept at the present levels. Essentially, this means that new major fields must be discovered about as rapidly, or perhaps more rapidly, than the older ones lose productive capacity. From 1940 through 1947, only eight major fields with estimated reserves of more than 100 million barrels each were discovered and proved – an average of one such field a year. The total estimated reserve of these eight fields was less than 1 year’s supply at the present rate of use. As drilling continues, some existing fields ultimately will prove reserves larger than those now known. Without a pronounced increase in the number of major discoveries, however, the prospects for increasing our present productive level are not encouraging. Any gap that appears between domestic productive capacity and demand could be filled with supplies from proved oil resources abroad. This circular presents Part III-A of a series reviewing the literature on effects of breathing dusts with special reference to silicosis; it deals with the economic and legal aspects of dust diseases in industry; Part III-B will contain data on the status and cost of compensation for silicosis, Parts I and II of the series have been issued as Information Circulars 6835, 6840, and 6848; Part I defines and classifies dusts, indicates sources of exposure, and describes the physiological effects of breaching dusts; Part II-A discusses the principal dust factors producing pulmonary pathology and methods of determination of dust in air; and Part II-B presents data on the engineering and medical control in the prevention of dust diseases and general recommendations for their control in industry. A summary and general conclusions will complete the series.

The U.S. Bureau of Mines contracted a study to review the air and water monitoring requirements of five Federal environmental acts (the Federal Water Pollution Control Act, the Safe Drinking Water Act, the Clean Air Act, the Surface Mining Control and Reclamation Act, and the Resource Conservation and Recovery Act) and to compile information on the currently available monitoring instruments. A literature review of the acts and their associated regulations was conducted to determine applicability to the mining industry as well as any air and water monitoring requirements. In addition, an industry search and literature review was performed to assess the instrumentation available to comply with the monitoring regulations. Based on this research, the instruments are classified by general applicability to air, surface water, or ground water monitoring, then catalogued by monitoring parameters. A discussion is provided on recommended and innovative monitoring techniques.
