There has been an unprecedented rise in the fatality rate in the Australian mining industry over the last few years. The factors behind this rise are not clear; however, it is noted that the increase in fatalities has occurred during a fall in commodity prices.This paper examines prevention methods and systems to deal with hazards that have the potential to result in either single or multiple fatalities. Managing the risks associated with mine ventilation is used to illustrate the principals behind the approach.The paper also touches on the broad science of human error and proposes a multifaceted approach to controlling and preventing fatal risks.A summary of the steps involved in developing an effective principal hazard management plan is provided.CITATION:Clough, A J, 2015. An effective maintenance program ensures the adequate availability of production equipment. In turn, this availability enables production to maximize utilization and successfully meet production targets. An overall production strategy is necessary to draw all departments toward this common effort. Only then will production targets be met. From the maintenance view this means that production must comply with PM and repair schedules. Purchasing must supply parts and materials on time. Mine engineering must not overwhelm maintenance with non-vital construction. Accounting must provide correct, timely cost information - - etc. This strategy - - its content, effectiveness and enforcement is the responsibility of the mine manager . . . his alone. This paper describes a successful approach to maintenance within this strategy.

Increasingly over the past couple of decades, the importance of understanding the in situ stress regime in underground hard rock mines has become accepted by all. Providers have responded to this requirement and various techniques for undertaking the measurements have been honed while others have slipped from use. It is a simple, though not inexpensive task to engage a provider to undertake the require measurements, and a comprehensive report will ensue. So, what has actually been obtained? What should the geotechnical engineer do next? What tools are necessary to do so? Regardless of the method of measurement, the report will describe the location of the measurement site and describe the basis of the methodology used in undertaking the measurements and a log of the measurement program. Finally, it will include the results, which should incorporate the normal and shear stress components of the full stress tensor. Ground control management plans (GCMP’s) and stress summary reports and spreadsheets prepared by site geotechnical engineers always include the magnitude, trend and plunge of the principal stresses. Remarkably few site-based summaries include the full stress tensor. This is a significant gap as the full stress tensor is vital for any further data manipulation. However, without the algebraic tools to undertake those manipulations, their value is diminished. This paper seeks to provide a checklist of ideas to be considered. It provides commentary on the options available to dissect the stress measurement report and ensure that the maximum possible value is derived from it. The manipulation and presentation of component stress data is also discussed. The various methods of stress measurement are not the focus of the paper but some of the strengths and weaknesses of them over which the site geotechnical engineer has some control, are highlighted.

A pilot - scale fluidized feeder was developed for continuously feeding up to 1 ton of pulverized coal per hour to a pressure gasifier . Fluidization in the 3 - foot - diameter by 34-1 / 2 - foot - high feeder is effected by two streams of inert gas , a technique called zone fluidization . A stream of inert gas through a drilled plate in the base of a central column near the bottom of the feeder fluidizes the coal . Another stream through a drilled plate in the annulus between the central column and feeder wall provides a source of semifluidized coal for the central column . Fluidized coal from the central column discharges through an extraction funnel and discharge line . Zone fluidization provided good fluidization with moderate requirement for fluidizing gas . Instruments were developed for metering and recording the flow rate of fluidized coal . A system was developed for storing , and weighing pulverized coal , and then delivering it in fluidized form at about 1 to 8 tons per hour .

More than a decade ago, CANMET management recognized the need to make better use of existing technological information in mining and extractive metallurgy-two fields basic to the economic well-being of Canada. There were at that time no indexes or files dedicated to disseminating technical information for the many minerals mined and processed in Canada, including coal. CANMET, with the nation's largest research and library resources in the minerals field, was in a unique position to fill this need. Initial efforts were concentrated on building a mining file, beginning with identification of world sources of published information, development of a special thesaurus of terms for language control and adoption of a manual indexing/retrieval system. By early 1973, this file held 8,300 references, with source, abstract and keywords given for each reference. In mid-1973, operations were computerized. Software for indexing and retrieval by batch mode was written by CANMET staff to utilize the hardware facilities of EMR's Computer Science Centre. The resulting MINTEC file-one of the few files of technological information produced in Canada-is the basis for the national literature search service in mining offered by CANMET. Attention is now focussed on building a sister-file in extractive metallurgy using the system already developed. Published information sources have been identified and a thesaurus of terms is being compiled and tested. The software developed for CANMET's file-building operations has several features, including the selective dissemination of information and production from magnetic tape of photo ready copy for publication, as in a bi-monthly abstracts journal.

Tunnels are frequently specified by owners to be designed to a structural code, often a bridge code. In the USA, AASHTO have created a tunnel code, based on structural principles, the AASHTO LRFD Tunnel Design Specification. The Load Resistance Factor Design method (LRFD) described in the AASHTO title is an approximation to the probabilistic design of structures, based on the independence of load and resistance. This methodology is well known to be problematic in geotechnical design, but it is widely used. The paper considers the difficulties of this approach and describes alternative approaches that would result in more appropriate designs for tunnels.

"Drilling and blasting are fundamental operations in the mining cycle and constitute an important component of the mining costs. Problems associated with improper drilling and blasting practices can create losses of several millions of dollars per year. For this reason, a Blasting Technology program was initiated in 1984 at the Noranda Technology Centre (NTC). In this paper, advances made at NTC in drill and blast technology are reviewed. Impacts of drilling and basting for Noranda are assessed in terms of dilution and ore losses. Future challenges are addressed. IntroductionUp to 1997, Noranda was a diversified natural resources company that operated in three sectors: mining and metallurgy, forest products, and oil and gas. The company employs more than 33 000 people in operations and sales offices around the world. Today, Noranda has returned to its roots in mining and metallurgy. "The availability of fresh water to meet present and future needs in Ontario is of concern to an increasing number of people. Rain and snow are the source of our potable water and provide, on the average, some 180,-000 billion gallons of water annually. This amount is reduced to 94 billion gallons a day as a result of the evaporation and transpiration of water and our inability to construct the necessary reservoirs for capturing surplus stream flow.The water used for domestic, irrigation, public supply and industrial purposes amounts to some 7 billion gallons daily at present and will reach 21 billion gallons daily by the year 2000; however, only about one-fifth of the water used is actually consumed, and this, along with the possibilities of water re-use, improves the water supply picture still further.The need for adequate pollution control has accompanied industrial and urban expansion and is an important factor in the extent to which our surplus water is used. In the 10-year period since the formation of the Ontario Water Resources Commission, the cost of water and sewage works constructed in the Province has exceeded one billion dollars. A special effort is now being made to control the industrial pollution situation.Numerous federal and provincial agencies are engaged in hydrologie studies in the Province, providing in-formation of benefit to those involved with water resources management."

San Antonio has experienced sustained growth for decades, putting increased pressure on existing wastewater infrastructure. In June 2013, San Antonio Water System (SAWS) reached a Consent Decree agreement that requires significant improvements to the wastewater system over a 10-year period to reduce Sanitary Sewer Overflows. One of the critical projects was the W-6 Upper Segment Tunnel project to reduce sanitary sewer overflows on an existing sewer main running through Lackland Airforce Base. The replacement project consisted of a new tunnel nearly 29,000 LF of primarily 104" FRP. The project also includes nine shafts and multiple trenchless crossings to make near surface connections to the new works.

This report describes research carried out by the Bureau of Mines to develop new technology for electrorefining metals at much higher current densities than those normally used in commercial practice, preliminary studies using rotating-ring and cylinder electrodes to increase the mass transport of copper are discussed. Following this work, single- and multichannel cells were developed that utilized a high electrolyte flow in the channels formed by the anode-cathode pairs. Using these cells, copper was successfully refined at current densities in the range of 70 to 300 amp/ft2. The effects of flow rate, electrolyte composition, and grain-refining and leveling agents on the morphology and purity of electrodeposited copper were determined.

The effect of High Pressure Grinding Rolls (HPGR), as a preceding stage to a conventional ball mill circuit at PT Freeport Indonesia?s C1/C2 concentrators, has been studied in terms of downstream grinding efficiency and circuit performance. When milling the product from the HPGR, mill capacity increased by 21% for the same product grind size, when compared to processing similar ore from a conventional crushing circuit. This achievement is a result of several reasons, among them: reduction in feed size (F80), fines generation beyond the proportional reduction in F80, reduction in ore hardness induced by HPGR comminution mechanism, optimization of the recycle system of the HPGR and ball string optimization in the ball mill circuit given the new feed to the mill.

The complexities of mining in current economic conditions, where ore extraction schedules are varied on a weekly basis so as to ensure constant supply to the process plant, can result in a number of scenarios that pose geotechnical challenges. This paper presents a case study where alterations in the mining schedule of a particular mine resulted in the necessity to mine under a large uncompacted embankment, ie integrated tailings and mine waste landform. To effectively ensure that ore recovery was maximised, a well-designed slope monitoring system to evaluate design performance and to provide timely warning of impending failures was imperative. In order for an appropriate slope monitoring strategy to be developed, a thorough understanding of the potential slope failure mechanisms, capabilities and limitations of available monitoring techniques is required. Communication of the monitoring results and response strategy to alarms generated by monitoring systems must also be carefully considered to maximise effectiveness. The results of preliminary stability modelling can be used to guide the design of a monitoring system, which in turn can then generate data which can be used to iteratively refine the geotechnical model. Open pit anthracite mining at the Canmore Creek Mines, Canmore, Alberta, south of Banff National Park, resulted in several pit lakes by the early 1970’s. To enable future mining opportunities in the Rocky Mountains, operators decided to reclaim the site into a non-polluting landscape which would provide a memorable outdoor experience for residents and visitors alike, and would preserve wildlife habitat. Reclamation began with the stabilization and revegetation of spoil piles adjacent to Canmore Creek, and the backfilling and revegetation of Canmore Creek Mines No. 1 and 2 to create “Salamander Pond.” The company dammed the outlet of an open pit at Canmore Creek Mine No. 3 to raise the water level and create Quarry Lake. Reclamation concluded in 1980 when the Town of Canmore took ownership. Quarry Lake is now the centerpiece of 293-acre Quarry Lake Heritage Park, a popular destination for swimming, hiking, and fishing, visited by about 400 people on summer days. The Alberta Environment Conservation Authority described this as “The best mine reclamation in Western Canada.” Quarry Lake is an exceptional example of walk-away, pit lake closure, where the mining company created a valued post-mining asset and was released from future liabilities. INTRODUCTION One of the most important environmental issues associated with contemporary open pit mining is the closure of mine pit lakes, and the perpetual legacy these lakes can potentially generate. Ideally, successful pit lake closure includes the following: (i) the water quality within the pit lake, plus surface water and groundwater discharging from the pit lake, meets pre-mining, or other acceptable water quality conditions of the receiving environment, (ii) the termination of all active treatment processes, (iii) the return of the pit lake to the government for long-term care and maintenance, (iv) the release of the mining company from all future liabilities associated with the pit lake, (v) the return the environmental bond to the mining company, and (vi) the reuse of the pit lake for some post-closure, beneficial purpose. The achievement of these goals is known as “walk away, pit lake closure.” Peer-reviewed, publications of successful walk away, pit lake closure provide mining companies the ability to address public concerns regarding the safety of pit lakes which commonly occur during the permitting, operations, and closure phases of mining. To date, most published work on walk away, pit lake closure work has occurred in the coal mining sector in Germany and Australia (McCullough et al., 2009; Jones and McCullough, 2011; Kumar et al., 2013; Schultz et al., 2013). However, in North America, examples of walk away, pit lake closure are not widely discussed in the literature despite there being excellent examples from the coal mining sector in Alberta, Canada (e.g. Pleasure Island, Black Diamond, East Pit, Sphynx, Robb, Lovette, Siltstone, and Sterling), and from the iron mining sector in Minnesota in the United States (e.g. Portsmouth Lake, see MNDNR, 2018). Although seldom included in discussions of pit lakes from the metal and coal mining sectors, the aggregate and fabricated-stone mining sectors also produce pit lakes, and there are numerous examples throughout North America where these pit lakes have been successfully closed and transformed into post-mining resources. The difficulty of finding these good examples in the literature makes it challenging for companies to respond to public and regulatory questions regarding pit lake closure and reuse. As such, there is a pressing need document the motivations and approaches used in examples of successful, walk away, pit lake closure in North America so that the mining industry at large can learn from these examples and can better address public concerns.

Polymer Filtration (PF) Is an innovative, selecthle metal removal technology. Chelating, water-soluble polymers are used to selectively bind the desired metal ions and ultrafiltration is used to concentrate the polymer-metal complex producing a permeate with low levels of the targeted metal ion. When applied to the treatment of, Industrial metal-bearing aqueous process streams, the permeate water can often be reused within the process and the metal ions reclaimed. This technology is applicable to many types of industrial aqueous streams with widely varying chemistries. Application of PF to 'aqueous streams from nuclear materials processing and electroplating operations will be described.

The performance requirements for modern steelmaking facilities are ever more stringent, as customers and markets are continuously pushing for technologies which allow having high quality steel produced in an economic and sustainable way. In the details, this paper has the aim to present the excellent results obtained by integrated steel plant where an EAF is fed with high quality hot ENERGIRON DRI. The recent technology developments, combined with the affirmed basic process design, contributed to further maximize the energy of the DRI, reaching the EAF at temperatures above 600°C with a carbon content up to 3.5%, as confirmed by plant data. These improved input material characteristics resulted in reduced electrical energy consumption and increased liquid steel production.

Reservoir properties including porosity and permeability distributions in longwall coal mine gobs have direct application to modeling the flow patterns and gas distributions in the gobs. Proper assessment of these distributions is required to assess gob gas hazards such as spontaneous combustion and explosive methane mixtures and to optimize gob gas venthole operation and nitrogen inertization. Determining the reservoir properties is difficult given that the gob is inaccessible and direct measurements cannot be easily taken. Several techniques had been used by previous researchers and will be discussed. This paper will present the methodology used to model the gob compaction by simulating the sequential extraction of coal. Two Western US coal mines were used for case studies. By determining volumetric strain, the porosity and permeability distributions in the gob can be calculated. The geo-mechanical model predicts an axisymmetric volumetric strain distribution with a steeper slope near the start-up room and a shallower slope near the active face.

A joint federal-provincial approach in gathering basic information from industry on mine production, concentrator operations and ore reserves, initiated during 1978, is a major step toward the goal of achieving better government-industry communication through a common data base. Information from questionnaires sent out by provincial governments is shared with the federal government; correct interpretation is ensured by mine visits conducted jointly by provincial and federal representatives. Initial focus is on nonferrous metal mines. The federal Department of Energy, Mines and Resources (EMR) is responsible for a national analysis that is aimed at the early detection of worrisome developments-and their reasons-in the flow from resources to reserves and from reserves to supply of these metals. The heart of the national analysis is the annual construction of a "monitoring curve", showing each metal's aggregated mine production as it would decrease over time if no new reserves and production capacity were to be added from the year of analysis. For a given metal, a comparison of monitoring curves from year to year can reveal early signs of supply stagnation. In anyone year, a monitoring curve in conjunction with foreseen needs for Canadian mine output will show the production gap that would have to be filled from reserves and production capacity yet to be developed. The production gaps for copper, nickel, zinc and lead, the four leading base metals mined in Canada, are shown for the 20-year period ahead, in the context of production achieved during the past 20 years. It is shown that reserve levels commensurate with likely future production levels would require that average annual reserve additions during 1978-98 exceed the average annual additions during the preceding 20 years. Reserve levels of all four metals have declined in recent years. A review of significant discoveries in the last five years suggests that this fall in reserves is not the result of a lack of exploration successes, but of stagnation in development for various economic reasons.

MicroEDM process is mainly used for producing miniaturized features like microholes on different types of materials that are hard to cut and difficult to machine. One typical application of microEDM process is microhole drilling operation. Choice and utilization of optimum pulse parameters is of utmost importance for achieving superior surface quality and higher machining rates. However, the process is characterized by low machining rates and considerable overcut as compared to conventional EDM drilling. An attempt is made to investigate the effect of pulse parameters on maraging steel 300 alloy during micro EDM drilling. Parameters like pulse on time, pulse off time, voltage and current are explored through 09 experiments replicated thrice using Taguchi method. Machining characteristics like material removal rate and overcut are studied while drilling micro holes of 500 µm diameter using brass electrode. Evaluation of microhardness after microdrilling is also performed. It is found that puls on time and current influenced the machining characteristics. Minimal change is observed int he microhardness of the alloy after microdrilling.
