This paper is one of a series dealing with mining problems and summarizing the data contained in the Information Circulars on individual mines that have been prepared by the managers or superintendents and published by the Bureau of Mines. The purpose of this paper is to present the problems encountered in the development of an efficient mine organization and to show the means used by certain mining companies in promoting efficiency. There is also a discussion of the advantages and disadvantages of the various systems of paying and handling labor and a description of their application at the individual mines.

This report describes the study of factors which affect medium voltage (661 - 1000 volts) trailing cable splices used in underground coal mines. Improvements in quality, dependability, and design of splice kits and shop splices are presented in the study results and recommended guidelines and regulations. Recommended research programs are included which describe approaches for development of the recommended guidelines and regulations for medium voltage trailing cable splices. The basis for the conclusions and recommendations presented in this report is included in three appendices which describe a survey, splice examination, and general requirements development prepared during the progress of this study.

"Rio Tinto’s Western Australian expansion, combined with mining within structurally complex geology and increasingly below the water table, presents challenges in effective slope management to ensure safe and economic mining.The Geotechnical Management System (GMS) was developed by Rio Tinto Iron Ore (RTIO) to manage geotechnical risks identified during the design process and implementation, and to ensure feedback based on the ‘as-found’ conditions. The GMS utilizes a risk-based approach to geotechnical risk management and is centred on the geotechnical risk and hazard assessment management system (GRAHAMS). GRAHAMS is used to assess pre- and post-control risk for future potential risks (planned slopes), current risks (as-built slopes), and actual geotechnical hazards (realized risks) identified in the pit. This serves as a core operational risk management tool in identifying and prioritizing key risk sectors and management of critical controls. The system’s database reporting functionality supports effective communication of operational risks to operational personnel, as well as reporting the risk profile across operations to management.A rigorous engagement process between design engineers and sitebased engineers is implemented to ensure that key design assumptions, limitations, risks, and opportunities are understood by the site teams. This information, together with mine plan schedule details, is used to assess the design risk and develop appropriate controls. These controls typically include slope performance monitoring and slope reconciliation. The design feedback loop is closed through sharing of key slope performance and reconciliation data with the design teams.The GMS has been successfully implemented in RTIO pits and is fundamental to successful geotechnical slope management. Improved characterization of design assumptions has allowed for re-assessment of the pit design and improved hazard management in high-risk pits. The GMS, GRAHAMS, and other processes reduce the incidence of unexpected slope instability. Improved understanding of rock mass conditions has allowed for economic optimization through redesign of slopes, allowing for an improved understanding of risk and fewer unexpected conditions (surprises), hence an increased realized value."

This paper presents an assessment of the Robertson (2010a) method within the CPTu inversion parameter framework of Been et al. (1987), using CPTu calibration chamber data presented by Jefferies and Been (2015) and in other literature to assess the validity of the method, with particular focus on applicability of the method to TSFs (i.e. its applicability to loose silts and silty sands). The assessment demonstrates that although the method estimates values of k consistent with calibration chamber data, the method estimates values of m that are generally 1.5 to 2 times greater than calibration chamber data. As a result of this, the method generally overestimates Ψ for values of Ψ less than 0, and underestimates Ψ for values of Ψ greater than 0. This may result in both overly conservative assessment of materials as potentially liquefiable and unconservative predictions of material behaviour for loose materials.

"The Little Lake Harris (LLH) Bridge was constructed in 1951 to replace the eastwest County Road 48 Bridge connecting the towns of Howey-in-the-Hills and Astatula in Lake County, Florida, United States. The 78-span bridge carries two lanes of traffic, is 3,130 feet long, and 36.1 feet wide. The concrete deck is supported by steel I-girders carried by one row of 18-inch square precast concrete driven pile. In the late 1980's, slight settlements were observed in several bents. In 1990, Florida Department of Transportation (FDOT) initiated annual survey of deck elevations during bridge inspections to monitor settlement. Due to the slow nature and magnitude of settlement, substructure instability was not a major concern to the department. However, public concern over the settlement and rough ride of the bridge continued to rise. FDOT contracted Kisinger Campo & Associates Corp. (KCA) to provide the needed design to correct the dips in the riding surface and minimize future settlement. KCA's initial design was based on the concept of ""helper bents"" at the settled bents in order to restore the bridge to original elevation. Estimated construction cost for seven helper bents was about $3.3M. FDOT requested evaluating other alternatives to reduce the cost of the bridge restoration. KCA involved the geotechnical consultant, Terracon Consultants, Inc. (Terracon), to perform a peer review and provide an economical alternative solution. Jacking and shimming the bridge was determined to be both technically viable and economical with a total cost of less than $300K. The design was completed in March 2010 and construction was completed in February 2011. This paper provides an overview of the LLH Bridge history, summarizes the evolution of the design methodology including the findings of the geotechnical study and presents lessons learned during the design and construction of the project.INTRODUCTIONThe Safety and Preservation of the State Highway System is the top priority of the FDOT. Therefore, FDOT initiated a bridge management program that starts with conducting biennial safety inspections performed by Certified Bridge Inspectors (CBI). The inspection reports together with the CBI’s recommendations are reviewed by a registered Professional Engineer (PE) and work orders are issued with priority ranking depending on the severity of the bridge deficiencies. If the scope of work is considered beyond the in-house crews’ capability, the work is then included in a contract for and bid. LLH Bridge was a little different. It was initiated in direct response to number of complaints the FDOT District 5 Structures Maintenance Office (D5SMO) received regarding settlement of the Little Lake Harris Bridge."

"Once a health based guidance limits for exposure to diesel exhaust have been established, and as part of a multi-disciplinary team, the program for exposure reduction should incorporate the plan-do-check-adjust cycle. Setting a suitably protective (health based) occupational exposure limit (OEL) for diesel particulate matter (DPM) and nitrogen dioxide is a key driver to reduce exposure. It is important to go beyond regulations and reduce exposure to diesel exhaust, DPM, as low as reasonably achievable (ALARA) such as that prescribed under the Australia, Queensland Mining and Quarrying Safety and Health Regulation 2017 where under section 135 Limiting workers exposure, the workers exposure must not exceed the exposure limit applying to the worker for the hazard; and is ALARA. It appears that previously acceptable OELs are no longer safe. Therefore it is important that the internal responsibility system be implemented and the precautionary principle approach be followed, especially as it may be some time before a suitably protective OEL specifically for DPM is in place. One leading multi-national mining company BHP Billiton has acknowledged a significant lag that exists between regulatory action and the level of science that informs health risk. This company is managing exposure to diesel exhaust to as low as technically feasible. This same company that has set an interim target for managing diesel exhaust to 0.03 mg/m3 is achieving these levels which paves the way for other mining companies and general industry including construction. The Occupational Cancer Research Centre 2017 recommends reducing personal exposure to within 0.02 mg/m3 (EC) for underground mining and 0.005 mg/m3 for other workplaces. Controlling DPM is important to prevent cancer however it is also important that that nitrogen dioxide (NO2) be reduced (ALARA) and that careful selection of control technology be considered such as the proper selection of a diesel oxidation catalyst so that the level of NO2 does not in fact increase. The example from BHP Billiton provides a leading practice including using a multi-disciplinary approach and the importance of controlling emission at source and having an auditable diesel emissions based monitoring program in place."

A resource document, prepared for the Royal Commission on the Health and Safety of Workers in Mines, which outlines aspects of occupational health and safety in the Canadian metallurgical industry has been expanded to provide examples of some practical solutions. General process descriptions are provided in conjunction with some of the major potential hazards associated with each step. Emissions to the working environment, particularly around converters, are discussed. Newer metallurgical processes, the role of automation and other advances in the metallurgical industry are described with respect to their impact on the work place. The role of staff and unit employees in providing safe working conditions in the complex metallurgical plant of today is outlined and the need for close cooperation within the human sector emphasized. The technology is available and is being applied to integrate the complex process requirements with realistic regulations to control emissions and provide a safe, healthy environment in metallurgical plants.

This volume is the second of a four-volume final report on research carried out under a contract from the U.S. Bureau of Mines on the development of a computer simulation model for surface mine reclamation planning. The other volumes are titled as follows: Volume I: Ore Reserve Estimation, Volume III: Mine Coordinate Generator System, and Volume IV: User's Manual for OPMHS/2. The dragline model described in this report simulates the movement of the dragline bucket across the digging face, the swing load, the dump, and swing back. It is programmed to handle chopdown, key cut, main cut, and rehandling operations. Provisions have been made for simulating equipment and mining delays. Any mining method can be specified in terms of numbered blocks, the dragline positions for the mining block and the dumping location for the block. Accounting procedures permit the calculation of production rates, rehandle volume, dig and delay times. The model can be used to study the effects of varying mining conditions, cut width, and spoiling sequence with special reference to toxic spoil, overburden and bench removal procedures. The basic input data for the model include machine parameters, the geology and material characteristics, the mining parameter, and the delay characteristics of the equipment.

A physical model based on similarity principles was constructed for simulating the RH degasser of CST (Companhia Siderurgica Tubarao, Vitoria, ES, Brazil) to evaluate the circulation rate of the metal which essentially defines the degree of exposure of the metal to vacuum in the chamber and thus of the refining rate, The circulation rate on the other hand will influence the decarburization rate and removal of dissolved gases like nitrogen and hence it is essential to optimise this parameter to get maximum refining efficiency with minimum refining times, thus improving the productivity of the secondary refining process. Mixing characteristics in the ladle is evaluated by pulse tracer addition using conductivity-meters located at the down leg as well as at several locations in the ladle. The influence of gas injection through nozzles in the upleg with simultaneous bottom injection of gas as well as nozzle blockage on the circulation rates were investigated. Also simulation experiments of kinetics of decarburization were carried out using CO2 adsorption-desorption process in sodium hydroxide solution as circulating fluid and the effect of, bottom gas flow rate in this case was also evaluated. The results were carried over to prototype and to optimize the process.

Operational excellence is generally known as a management system integrated across critical functions that is geared towards continually improving operational performance. Functional areas such as health, environment and safety, quality and human resources are the focus of many corporate-wide operational excellence programs. Operational excellence also focuses on improving areas such as customer orientation, employee empowerment as well as process, and systems optimization. It is widely viewed as critical to sustaining business performance improvement. Although it is difficult to find a universally accepted definition, or even common foundation in many respects, operational excellence remains a popular term in the business world. Many organizations have developed their own version of organization excellence, influenced by such individual factors, other similar institutions or even third-party service providers. A mining organization is no different from other organizations in that adopting and implementing effective operational excellence programs can dramatically improve business execution and performance across regional or global interests. This paper brings together various perspectives of operational excellence and reconciles them for a useful composite for the mining industry. While most agree that operational excellence should include normal functions in a manufacturing operation such as production, maintenance and engineering and processes such as quality, environmental and continuous improvement, many other factors for operational excellence seem to vary from perspective to perspective. Thus, programs that manufacturers engage in may seem the same, but may, in fact, be different in key areas. This may affect organizational performance in tangible ways. Management must buy into the company?s definition of operational excellence and be dedicated to achieving it. A review of many of the existing definitions of operational excellence, explicit or embedded as part of a program, seems to yield the following common factors: ? It should help organizations better execute their processes and services. ? It should be geared towards improving quality ? It requires that the various operations should be aligned with the overall enterprise strategy. ? It should be geared towards continuous improvement. A composite of operational excellence definitions suggest that it is: ? Geared towards helping the organization achieve sustained profitability; ? tied to strategic alignment and business objectives; ? tied to solid investment strategies; ? integrated as part of the culture, process, time plan and performance; ? a relevant performance measurement system; ? extended to include all aspects of the supply chain (including external suppliers); ? integrated with health, safety and environmental (HSE) aspects. And, while the pursuit of operational excellence intuitively suggests that businesses should improve, it is very difficult to achieve given the difficult challenges facing companies today. Mining businesses face numerous pressures to perform in a global marketplace such as demand, energy cost, variable cost and pricing fluctuations as well as infrastructure challenges to further confound the environment. After considerable input from the mining industry, the Canadian Securities Administrators (CSA) finalised National Instrument 43-101: Standards of Disclosure for Mineral Projects. This Instrument, which became law across Canada on 1 February 2001, applies to all technical public disclosure on mineral projects and requires all technical disclosure to be based on the work of a æqualified personÆ (QP). While the QP is quite similar to the Competent Person in Australia there are important differences when it comes to all disclosure. This paper will summarise the main areas in which the Instrument introduces major changes to Canadian disclosure standards on mineral projects including the role and responsibility of the qualified person; the increased requirements to file technical reports; the incorporation by reference of the October 2000 CIM definitions of resources and reserves and additional rules governing written disclosure of scientific and technical information. In October 2001, eight months after the implementation of NI 43-101, the CSA compiled a list of frequently asked questions (FAQs) and CSA responses to assist mining industry participants and their advisors in understanding and applying NI-43-101. This document has been well received by industry. The CSA plans to update the FAQs when further guidance is requested. Two industry committees have been established to provide the CSA with technical advice on the application of NI 43-101. As recommended by the Ontario Securities Commission/Toronto Stock Exchange Mining Standards Task Force, the CSA has established an external industry advisory committee û the Mining Technical Advisory and Monitoring Committee (MTAMC) û to monitor the application of NI 43-101 and advise the CSA on industry and professional developments. The role of the CSA-Canadian Institute of Mining, Metallurgy and Petroleum (CIM) Working Committee on Reserves and Resources is to ensure that the future changes and updates to the CIM Standards can be efficiently and effectively incorporated in NI 43-101. During this first year, the Canadian securities regulators have seen an improvement in the quality of disclosure on mineral projects, which will increase investor confidence in the industry.

In 1960, the Secretary of the Interior, acting under the provisions of Public Law 454, 80th Congress, appointed a Lignite Advisory Committee to aid in programming research on coal (lignite) of the north-central area. The chairman of this committee is the Secretary of the Interior or his delegated representative, and in all meetings held during 1960 and 1961, the Director of the Bureau of Mines, Marling J. The project implementation phase, FEL4, is generally a complex undertaking. The project has been approved for construction and the capital has been allocated. At this point the project team comes under significant pressure to meet the safety, schedule, cost, and start-up committments. For the design team the principal tasks are to finalize detailed design, complete procurement, and issue construction contracts. Design activities must be closely coordinated with equipment and material procurement. A clear engineering plan should be based on well-defined work packages to meet construction, commissioning, and startup requirements. A range of review processes by discipline, area, and third parties assure quality. Constructability, safety, and hazard reviews are performed throughout the design process. Three-dimensional models that incorporate extensive databases help to maximize efficiency and minimize errors. Well-managed projects have a clearly accountable design team, rigorous change control processes, and well-defined workflows. Key individuals from the design team participate in commissioning, operator training, and startup.

"Downhole Seismic Testing (DST) is an important geotechnical testing technique for site characterization as it provides low strain (<10-5) in-situ interval compression (Vp) and shear (Vs) wave velocity estimates. These velocities are determined by obtaining relative arrival times of source waves as they travel through the stratigraphy and are recorded by one or more vertically offset seismic sensors. A challenging aspect of this process is to characterize the seismic data sets to determine the analysis method that will result in the most accurate interval velocity values. This paper introduces an assessment technique which utilizes linearity estimates (i.e., hodograms fitting straight lines) from the polarization analysis in conjunction with crosscorrelation coefficient calculations of the full waveforms as well as the deviation of the source wave frequency spectrum from a desirable bell-shaped curve. The paper will provide an overview of the technique and include some examples of actual data sets where the technique was applied to analyze those data sets.1. INTRODUCTIONIn general terms, Downhole Seismic Testing (DST) such as Seismic Cone Penetration Testing (SCPT) is a geotechnical technique for measuring in-situ shear and compression wave velocities (VS and VP respectively). The main goal in DST is to obtain arrival times as the source wave travels through the soil profile of interest, and from these arrival times the velocities are then calculated. Figure 1 shows a schematic of the typical DST configuration: a seismic source is used to generate a seismic wave train at the ground surface. One or more downhole seismic receivers are used to record the seismic wave train at predefined depth increments. The downhole receiver(s) may be positioned at selected test depths in a borehole or advanced as part of an instrumentation package as in the case of SCPT. When triggered by the seismic source a data recording system records the response of the downhole receiver(s)."

A class of gravity separation unit operations in ore-dressing relies for its operation on the production of a stratified bed. Heavier material gravitates downwards, and separation is achieved by splitting the ore horizontally, with the recovery of a dense stream and a lighter stream. The important ore-dressing unit operations that fall in this class are the jig, the Reichert cone and the pinched sluice. A new quantitative model based on Mayer&apos;s potential theory is developed here. This model is developed completely for the simple case of binary mono-size particulate materials, but the extension to units operating with feed material distributed over particle size and density is discussed in principle. The model can be used to predict the performance of these units and is very suitable for simulation. The model is tested successfully against experimental data obtained from a batch jig and a Reichert cone.

The two preceding, Bulletin 149 and Bulletin 165, were for the years 1915 and 1916, respectively. In arranging the references the same general scheme has been followed, but as in those bulletins, several changes and additions have been made in the classifica.tion, the more important being in the classes " Development and production " and "Refining and refineries." German periodicals for 1917 were not procurable and references to articles in those and in other foreign journals that were not available have been credited to the sources from which they were procured.
