These financial statements are the responsibility of the Institute?s Council. Our responsibility is to express an opinion on these financial statements based on our audit. Scope We have conducted our audit in accordance with statements of South African Auditing Standards. Those standards require that we plan and perform the audit to obtain reasonable assurance that the financial statements are free of material misstatement. An audit includes: ?examining, on a test basis, evidence supporting the amounts and disclosures in the financial statements, ?assessing the accounting principles used and significant estimates made by management, and ?evaluating the overall financial statement presentation. Highwall mining is an important coal mining method. Upwards of 60 highwall miners are presently in operation, and they may account for approximately 4% of total U.S. coal production. A review of the Mine Safety and Health Administration (MSHA) accident data over the 20 year period from 1983 to 2002 identified 9 fatalities attributable to auger and highwall mining of which inadequate ground control accounted for 1/3. In the past 5 years, 1 fatality occurred in highwall mining. Estimates of the manpower requirements in highwall mining suggest that its fatality rate is essentially the same as for surface coal mining, thus highwall mining appears to be a very safe modern mining method. Highwall stability is the major ground control related safety concern, and operators are required to develop and follow an appropriate highwall mining ground control plan. The plans usually specify the following geotechnical parameters: hole width, maximum hole depth, maximum overburden depth, seam thickness, web pillar width, barrier pillar width and number of holes between barriers. Calculated web pillar stability factor exceeded 1.3 for most designs evaluated. This study examined records from 5,289 highwail miner holes with a total completed footage of about 2,560,000 feet to understand the reasons for early pull out. Average lost footage is typically about 20% of planned footage. Only 35% of the holes reached planned depth, and 20% were short due to rockfalls. Water and adverse geology accounted for 15% of the losses. Mechanical/electrical problems, guidance, and slope stability problems accounted for the remaining 30%. Web pillar stability factor for these holes also exceeded 1.3 in 95% of cases. Trapped highwall miners present a safety hazard to workers during their recovery. Best practices to avoid trapped highwall miners include: avoid mining in stream valleys, avoid mining near outside corners, careful alignment of each hole and the use of an onboard guidance system, Several issues in highwall mining ground control require further investigation including highwall mining through old auger workings, highwall mining near old underground mines, multiple-seam and multiple lift highwall mining and finally the size and frequency of barrier pillars.

A mine’s inherent capacity is dictated by factors such as equipment, design, people, processes, and environment. Actual production may approach, but never surpass, the inherent capacity limit. The inherent capacity ceiling can only be increased through re-engineering. Autonomous haulage systems have the potential to significantly increase a mine’s inherent and realized capacity. Realized capacity may be maximized through the optimization of soft factors such as the processes, behaviours, and organizational systems, controlling asset utilization, availability, and performance. Truck shovel dispatching systems revolutionized the utilization of mobile assets. High-precision global positioning systems (HP-GPS) are increasingly being adopted to optimize equipment performance. Real-time maintenance management systems have the potential to revolutionize equipment availability and significantly reduce cash costs. This paper explores some of the solutions available to mines to increase their inherent capacity and maximize their realized capacity.

The bulk of plain carbon steel is produced through conventional Basic Oxygen or Electric Arc Furnace routes where the development of the AC Ladle Furnace has made it possible to also produce alloy steels. Smaller Ladle Furnaces have limitations such as lower efficiency; high electrode consumption and diminutive yields rendering them uneconomical to operate and making the processing of low carbon steel grades difficult due to carbon pick up. The demand for special steels and alloys with strict specifications is increasing while commodity prices remain competitive. The production of special steels and alloys have specific process control requirements and the development of the induction melting furnace (IMF) has made it possible to produce smaller batches economically. Refining and alloying of the melt in the IMF further enhance the efficiency. The INDUREF furnace offers increased productivity and flexibility through increased thermal efficiency and can be utilised for holding molten metal for further processing such as granulating, atomising, nitriding, alloy additions and desulphurisation of Ferro-alloys and steels. The system utilises a specially engineered ladle which is placed in an induction-heating assembly comprised of a stationary water-cooled copper coil and lamination packs. The addition of equipment for vacuum degassing and decarburisation of steels and alloys is also feasible. Benefits are: ? No electrode consumption or carbon pick-up ? Lower installation, operation and maintenance cost. ? No line-flicker facilitates operation on a weak power grid. ? Good power factor and balanced load ? "Environmental desulphurisation is an attractive alternative for the management of acid generating tailings. Non selective froth flotation is the most commonly used method to produce desulphurized tailings, as shown from many previous works (McLaughlin and Stuparyk, 1994; Bussière et al., 1995; 1998; Benzaazoua et al., 2000a,b; 2002). This process can reduce the volume of problematic tailings to manage, by making a sulphide fraction concentrate. The desulphurized tailings can then be used as a moisture retention layer in a cover with capillary barrier effects to prevent acid mine drainage.In this paper, the authors present the results from different laboratory and pilot scale flotation tests which confirm that low sulphide, non-acid generating tailings can be produced by froth flotation. It is also demonstrated that the sulphide concentrates obtained from desulphurisation, when mixed with binders gave good long term results in terms of mechanical behaviour, thus demonstrating that the sulphide fraction can be used for paste backfill material. From previous work (Bussière et al. 1998, 1999) on the evaluation of the operating and capital costs of the desulphurisation process for different ore types, the use of desulphurized tailings to create a dry cover was then studied for several scenarios. The desulphurisation process was compared with other technologies (underwater disposal, engineered soil cover) currently used to prevent acidic drainage (AD) for a new mining operation and the advantages and disadvantages of desulphurisation are presented as well as the costs of this method of AD prevention.INTRODUCTIONThe principal environmental problem of the mining industry today is the generation of acidic drainage (AD) resulting from the natural oxidation of the sulphides in mine tailings. This phenomenon involves the generation of acidity which increase the amounts of toxic metals in the waste water released into the environment by a mine. The environmental impact of the AD has been described in detail in previous work (e.g. Down et Stocks, 1977; Ritcey, 1989; Gray, 1997; Ripley et al., 1996; Aubertin et al., 2002)."

A thermodynamic model and computer program, CuModel, has been developed to model the distribution behaviors of Cu, Fe, S, O, Ni, Co, Sn, Pb, Zn, As, Sb, Bi, Au and Ag, copper loss in the slag, and heat balance in copper smelting process, matte converting process and copper con-tinuous smelting process. It can be predicted for any set of controllable process parameters such as feed composition, smelting temperature, degree of oxygen enrichment and volume of oxy-gen-enriched air. This model accounts for physical entrainment between matte and slag phases. It can be used in Isasmelt process, copper flash smelting process, Mitsubishi process, P-S con-verting process and Noranda process. The predictions by the present computer model are com-pared with the known commercial data from Guixi Smelter in China, PASAR Corporation in Philippines, Horne Smelter in Canada, MIM Limited in Australia and Naoshima Smelter in Ja-pan. The agreements between the computer predictions and the commercial data are excellent. In this paper, the applications of CuModel in the copper metallurgical industries have been pre-sented. CuModel has been used by Daye Smelter and Guixi Smelter in China, and by PASAR in Philippines as well.

Sulphate decomposition was studied in the conditions of a typical copper flash smelting heat recovery boiler. The aim was to determine the kinetics of the copper sulphate decomposition reactions and to find out if they are likely to take place in the process conditions prevailing in an industrial HRB. The information is essential in CFD-modelling of the reactions taking place in the boiler and for optimising the process. Thermal decomposition of sulphates was studied by thermogravimetry. The experiments were conducted with a commercial copper sulphate, a partially sulphated copper oxide and an actual process flue dust. The main variables studied were temperature and gas composition of a N2-O2-SO2-mixture. The samples were characterized with SEM, optical microscope and XRD. Temperature affected the decomposition rate strongly but the gas composition had a minor effect on the kinetics. The decomposition was fastest in the partly sulphated oxide and slowest in commercial copper sulphate. Keywords: Sulphate decomposition, flash smelting, heat recovery boiler, flue dust

The zinc pressure leaching (ZPL) plant at Teck Cominco's Trail Operations was started up in 1981 and represented the world's first commercialization of the technology. From the outset, the process was designed to operate using iron-rich Sullivan Mine zinc concentrate. When the Sullivan Mine reached the end of its life in 2002, the ZPL plant underwent a significant process change with the treatment of 100% Red Dog zinc concentrate. Through careful improvements to existing plant processes, controlled changes to plant operating parameters and a rapid adjustment to the modified process by operations personnel, the plant has been successfully converted to processing a feed of 100% Red Dog zinc concentrate. This paper discusses the challenges faced during the conversion and describes the modifications to plant operating procedures, as well as changes to plant processes.

The intent of this manual is to provide guidelines for inspectors of drilled shaft construction otherwise referred to as drilled piers, caissons or bored piles. It should also be helpful to train beginning geotechnical engineers with limited first hand experience or for geotechnical engineers who are training inspectors. It is expected that the general guidelines and procedures described here will be supplemented by local geotechnical engineers working in a particular region based on local practice and experience. Since drilled shaft construction procedures vary widely based on local geology and size of project, an all-inclusive manual is not considered practical. This manual focuses on traditional topside inspection for routine drilled shaft construction. Other inspection and evaluation methods that rely on electronic equipment are available, but these methods require extensive training and experience in the interpretation of results. These more sophisticated methods of drilled shaft inspection and evaluation are common in construction, but they are usually provided by specialty subconsultants. The subconsultant will typically submit a report for each shaft, which will be reviewed by the geotechnical engineer. Other manuals are available to describe the electronic methods of drilled shaft inspection.

In the construction field and to a lesser degree in mining, flagman are often necessary to move traffic and or pedestrians through work zones. Everyone readily recognizes that they are essential for the safety of the general public and the employees on the job. What is not often considered is the safety of the flagmen themselves. Actually, let’s call these employees “flaggers” recognizing the fact that there are as many women as men doing this job. The first problem we need to overcome is the misperception that flagging is a low skill function that can be done by any untrained employee. The reality is that the flagger works in a very hazardous position with serious responsibility for the safety of his fellow employees and the public at large. And to do this job properly re q u i re s that the flagger receive sufficient training before being sent out to the highway. It is also necessary to convince the flagger of the value and importance of their job in order to motivate them to do a professional job.testing, documentation of both, and accurate re c o rdkeeping are key aspects of HM126F. Of course, training and documentation requirements will vary depending on the type of operation involved, and there may be some differences in the application of requirements depending on your jursdiction. Nonetheless, failure to train and properly document such training can result in heavy fines and/or penalties.

Whatever system is devised to control noise and ground vibration from blasting operations must present an effective compromise between the rights of the individual to be undisturbed by the actions of others, and the necessity for society, the whole of the population individually and collectively, to have the services and the products of commerce, industry and transportation along with the relaxation of recreational and cultural pursuits, all at a price within the means of the same society. One of the first jurisdictions in North America to attempt to quantify acceptable environmental limits for specific blasting operations was Ontario, Canada’s largest and most populous province. A process for development of guidelines applicable only to quarries and strip-mining operations was begun in 1974 and culminated with their inclusion in a “Model By-Law” published by the Ontario Ministry of the Environment in August 1978. This paper examines the process of development of the final guideline document and its application over the last 25 years. While acknowledging that the published guidelines for air blast and ground vibration control have provided a useful and generally workable framework for the purposes of land use planning and complaint or damage claim reduction, a number of flaws, weaknesses and misinterpretations connected with their application are identified. Recommendations for amendment of the controversial items are submitted with a view to making these and potentially similar guidelines in other jurisdictions, more acceptable to both government and industry.

The Meikle Mine is a mature underground gold mine utilizing primary and secondary longhole open stoping method with delayed backfill. Combined production with the newly developed Rodeo Mine is 4700 tons per day (4265 tonnes per day) of ore. The vast majority of Meikle Mine production comes from secondary and remnant stope blocks. Remnant stopes are ore blocks that were left behind in the initial mining sequence due to various reasons. Mining of these remnant blocks has created many challenging blasting situations. The application of electronic initiation to blast remnant stopes has proven extremely beneficial. Electronic initiation has provided the blasting engineer with a high degree of flexibility with respect to timing to successfully extract these difficult stopes. Post blast assessment of the remnant stopes using the mine’s cavity monitoring system (CMS) demonstrates that the mine is maximizing ore recovery and minimizing waste dilution. The largest remnant stope blast to date required 54 holes and produced 6700 tons (6075 tonnes) of ore. Another area where the application of electronic detonators has proved successful is in the extraction of slots in which ground conditions are extremely poor. Taking the slot blasts in one shot with double-decked holes as opposed to two single-decked shots has eliminated the need to clean and or re-drill holes. The ability to open the slots in one blast has helped reduce the total mining cost. This concept has recently been expanded to blast complete stopes in one shot. This reduces the stope cycle time considerably and allows the mine to bring ore forward. Current plans for 2004 are to excavate 50% of mine wide stope production using electronic initiation technology.

Processing of ores to recover metals usually generates large amounts of waste materials, such as rejects from concentrating operations, tailings from leaching and slags from smelting. These wastes often contain substantial amounts of iron and other metals, recovery of which are often considered to be of commercial value. Earlier versions of this paper investigated a number of processes built to recover iron from a variety of copper processing residues. This paper will extend that coverage to residues from nickel/cobalt, aluminum, asbestos and oil shale processing and coal plant fly ash. Products of interest are iron, ferroalloys, magnesium and aluminum. While none of these processes achieved long-term commercial success, their efforts cover the gamut of problems that must be addressed in treating dirty and difficult materials to recover generally low-value products. An Adept is an alchemist who can create a Philosopher's Stone, which could transmute base metals into gold. In 1701 in Berlin one Johann-Freidrich Böttger, a nineteen year old apothecary's apprentice, told King Frederick I that he could discover how to make the Stone. He failed and fled Frederick's wrath to Poland. He failed again, and King Augustus II threw him in jail. Then, in 1709, he discovered instead the secret of making Dresden china, certainly a more practical search. His failures in alchemy led to the nickname Adeptus Ineptus.

Good Morning. Thank you for your kind welcome, Lyle and thank you for having me here today. I spoke at the fall meeting of the Utah state RMCMI in Price and it's nice to be invited back. I can see by the program that you have two great days of interesting and informative meetings, including the mine rescue team exhibition and the safety recognition award presentations. As you all know, I am here standing in for Dave Lauriski, who is in China with Secretary of Labor Chao. As you also know, Utah is near and dear to his heart - and so is the Rocky Mountain Coal Mining Institute. I want to assure you Dave would be here if it were at all possible. But it is my good fortune that I could come in his place. We have made great progress as an Agency thanks to the hard work and dedication of MSHA's employees. But we are always looking for new ways to solve problems and continue to send more miners home safe and healthy every day. And we have been working on some great ways to do that, including thinking strategically. During my presentation today I'm going to talk about where the Agency is in terms of our transformation to the MSHA of the 21st century, a little bit about how we got to be where we are, and then describe our roadmap to the future state. And I'm going to focus on the significance of our plan and the planning process to demonstrate the implication for the mining industry. Former President Dwight Eisenhower once said, "In preparing for battle I have always found that plans are useless, but planning is indispensable." Now I do like Ike, but I would have to take exception to the first part of the sentence. I have found plans and the planning process to be indispensable - the plan to give me a roadmap to my goal, and the process to focus me on the steps I must take to get there. We all know that having a strategic plan makes the difference between success and failure. We've seen it time and time again, in business, in our lives - and in government: Having a strategic plan is crucial to success. And the process of making the strategic plan is crucial to its success. Putting together a workable strategic plan - a good roadmap - that will successfully lead you to your goals requires a thorough, careful process that takes into account each factor that you may encounter along your way to success, as well as those with whom you will travel. In order to talk about our current plan I want to take you back to three years ago, when Dave Lauriski went to Washington to lead MSHA. He knew that change was in the air. The "Pile Prediction Event" at the ASCE GeoInstitute's Deep Foundation Conference in February 2002 involved prediction of drivability and capacity of three 14m long, 324mm diameter, closed-toe pipe piles driven into loose to compact fine sand to slightly silty sand. Predictors were provided the results of two soil borings and three cone penetrometer tests, as well as type and model of hammer to be used for the pile driving. A total of 33 persons submitted predictions. As to penetration resistance distribution, many predictions were far below actually observed values. As to capacity in compression, the predicted values ranged from 270KN through 2,340KN with a mean of 1,070KN. As to capacity in tension, the predicted values ranged from 100KN through 1,650KN. The maximum load applied to the compression pile in the static loading test was 1,25OKN, at which no more reaction load was available. The reaction system for the tension test failed early in the test. CAPWAP analysis on End-of-Driving blows showed capacities at the end of the driving for the push and pull piles of 670KN, and 710KN, respectively. No restrike tests were made. Additional CPT soundings indicate that the pile driving had densified the soil. Static analysis and calculations from the CPT indicated that capacities ranged from 1,000KN through 2,000KN. The paper presents the data and discusses the various methods of determining pile capacity and addresses influence of set-up Lessons useful for future similar events are indicated.

Electronic detonators have been commercially available to the mining industry worldwide for over ten years. It is estimated that total cumulative global sales will have reached 25 million units by the time of the Conference in February 2004. This is below 0.2% of all detonator types sold during this period. Perhaps 20 million will have been used underground in South African gold, diamond and platinum mines. These will have been produced by South African companies, in South Africa for South African customers. Here efforts have met with greater success and in 2003 fully 5 million, or 3% of local detonators sold, could be of an electronic type. However, it is estimated that about US$7.00 (R50.00) has been expended on R & D, manufacturing facilities and marketing, for each unit so far sold. Large sums of money continue to be spent. Despite the apparent success in South Africa, is it justified? Electronic initiation systems have not produced significant technical benefits to the end-user underground nor corporate profits to the supplier. Some doubt they ever will (even from the large surface operations where the price per unit is much higher). A conclusion might be that they have failed. In 1998 Smit and Pistorius wrote a paper titled “Implications of the Dominant Design in Electronic Initiation Systems in the South African Mining Industry”. This ISEE paper seeks to summarise and review the conclusions of the earlier work. These were: that a dominant design had not yet emerged; the type of connector and the way in which detonators are connected and programmed would be a strong determinant influencing the development of a “winning” product; technical aspects rarely solely determine which becomes the dominant design (marketing, social and economic factors often have a strong influence); electronic initiation systems were an emerging technology for the underground gold mining industry in South Africa and because it was under severe cost pressure the mines would look to them to raise productivity and hence profits; finally, as a dominant design begins to emerge, one could expect a number of players to exit from the industry.

Regardless of the care exercised by a blaster, seismic and acoustic effects produced by blasting can be disruptive and alarming to many people. With the general trend toward larger blasts, increased population, and urban sprawl, as well as sensational accounts of explosives - related incidents by the news media, fear and concern often leave residents searching for damage to their property. As a result, many blasting companies today find themselves at the center of litigation at an accelerating rate. Obviously, the best strategy for handling vibration and airblast complaints is to operate in a manner that does not create grounds for them in the first place. During the bidding process, ask questions. See the actual site to perf o rm an “exposure analysis.” Collect as many facts as possible before blasting including geological issues, contractual obligations, zoning, historical use of site, disruption of daily life to local residents, demographics of the surrounding area, as well as any unfavorable public opinions or prejudices connected with local construction projects or mining operations. A consultation with local pre-blast firms may assist in tracking down previous problems in the area.

Adopting environmentally acceptable practices in New Zealand is a crucial requirement for mineral wealth development. Recent planned expansions in the coal industry have increasingly placed mining in the media spotlight. A lack of appropriate national environmental guidelines has resulted in inconsistent responses and assessments of current and prospective mine sites. Such inconsistencies have lead to delays in decisions on developing economic mineral deposits that may have only minor short-term impacts, and have also allowed unacceptable environmental management practices to continue elsewhere.   A research collaboration between CRL Energy, Landcare Research, University of Canterbury and University of Otago aims to provide regulator groups and the mining industry with a unified approach to the requirements of the Resource Management Act 1991, which is the legislation that predominantly drives the nationÆs environmental policies and guidelines. The tangible outcome of this research programme will be guidelines for an integrated decision-making framework based upon robust multidisciplinary science. The result of this unified process will facilitate the development of viable mineral deposits and prevent excessive environmental impacts.   The framework will be derived from a synthesis of four objectives. Objective 1 identifies potential water quality impact. This objective is based on a targeted geochemical study of the lithology and waterways of the West Coast and Southland. Objective 2 categorises the degree of impacts on the ecology of receiving aquatic ecosystems and determines the processes that drive aquatic ecosystem recovery. Objective 3 identifies best strategies to sufficiently manage impacted ecosystems, and to prevent future unacceptable impacts. Objective 4 combines the findings of Objectives 1 - 3 into a standard decision-making framework. The development of this framework development will include wide consultation with representatives of the mining industry and regulator groups. It is anticipated that this multidisciplinary approach will provide a robust and coherent, standardised system that will assist in achieving industry and environmental targets.

Those employed in the explosives industry must have a keen familiarity of the explosives products they handle, the safety pro c e d u res they must follow, and the increasing number of laws and regulations governing blasting operations in order to perform their duties satisfactorily. For this reason, companies must make certain that their employees have the necessary knowledge and full understanding of their blasting and non-blasting responsibilities. Consequently, training is one of the most important elements in any company’s safety program.

Dofasco was the first company to adopt oxygen steelmaking in North America in 1954, two years after the first LD vessel installed and operated successfully in Linz, Austria. Since then, Dofasco has investigated and implemented several strategies to maximize hot metal usage and minimize overall cost. Some key strategies -low hot metal silicon, combined blowing conversion, vacuum degassing tank upgrade and BOF slag carryover control - are discussed. Post combustion, scrap preheat, carbon injection trials along with current operating philosophies are described.
