The launch of a Tunnel Boring Machine (TBM) is one of the most challenging activity parts of the construction of a tunnel. Challenges are coming from limited space available, learning curve, concurrence of other activities and in some cases from the geological conditions. Launching a TBM is an extraordinary and complicated operation requiring a great level of planning. The Earth Pressure Balance (EPB) TBM named “Chris” was successfully launched for the Northeast Boundary Tunnel, the largest component of the Clean Rivers Project in Washington, DC. Due to space and time constraints, the 8-meter diameter TBM was launched from a single 20-meter diameter shaft, with a 30-meter-long tail tunnel by means of umbilical connections. This paper presents the steps followed during the TBM assembly and launching to complete the excavation of the initial drive of the tunnel maintaining high level of efficiency and quality despite the space constraints. Specifically, it focuses on the methods and phases used to extract the excavated material (muck) from the TBM and from the tunnel, to supply consumables, precast rings and backfill grout. The paper presents results in terms of productivity, lesson learned and possible alternatives to be considered in similar conditions.

The Beverley uranium mine û now in production in the remote north of South Australia at1000 tonnes per annum û was established to meet a projected, but little-recognised shortfallin primary supply of uranium for the nuclear industry in the early part of the 21 st century. Inthe context of AustraliaÆs lack of bi-partisan support for uranium mining and the nuclearindustry, a four to five year window of opportunity existed from 1996 to establish the mine.During this period, Beverley achieved a number of firsts including: the first ISL mine inAustralia; the largest ISL mine in the world and the first to produce more than 500 tonnes ofuranium in its first year; the first to be established under a new Native Title regime in SouthAustralia; and the first to be established under a revised South Australian Mining Act.Beverley was brought into production against the background of: hostile opposition fromanti-nuclear activists; a suspicious and at-times hostile media; a regulatory and politicalcommunity that was unfamiliar with ISL mining; commercially unsophisticated Aboriginalgroups; an inexperienced engineering industry; and the vagaries of development in a remote,semi-arid location with neither population nor infrastructure. The lessons that can be learnedfrom Beverley showed that û like much in life û flexibility and timing are crucial.

On the night of June 8, 1917, the flame of a carbide lamp accidentally set fire to the uncovered and frayed insulation of an armored power cable near the 2,400-foot level of the North Butte Mining Co.'s Granite Mountain shaft. How this insulation accidentally became exposed is explained on page 15. The highly flammable oiled fabric set fire to the shaft timbers, and as this is a downcast shaft the fire spread with great rapidity, soon filling the mine workings with smoke and gas. At the time 410 men were working underground, 247 of whom escaped by various means, but most of the 163 remaining were probably overcome soon after the lire started, and perished. Only two men were actually burned. The cause of the disaster was purely accidental. The immediate effect was the unexpected reversal of normal conditions; the ultimate effect was great loss of life and the destruction of the main hoisting shaft, putting it temporarily out of service. The work of rescue and fire fighting continued eight days. The following pages present an attempt to draw some practical conclusions and suggestions from this disaster. The property of the North Butte Mining Co. consists of about 220 acres in the northeastern part of the Butte district, Montana. Next to the Anaconda Copper Mining Co. it is the largest pro¬ducer of the district. In 1916, the year before the disaster, there were 1,160 men employed. The output was 560,947 tons of ore, which yielded 24,498,181 pounds of copper, 1,047,063 ounces of silver, 1,712 ounces of gold, and 412,953 pounds of zinc. The profit was $2,479,595. The total development work for that year was 21,694 feet. During the first half of 1917, the year of the accident, the daily tonnage of ore averaged 2,000, an increase of 400 tons over that of the year 1916.

On June 14, 2023 Jay Dee Contractors’ Tunnel Boring Machine (TBM) “Phoenix” holed through and completed mining the second drive of the I-75 Segment 3 Storage and Drainage Tunnel. Completion of the tunnel came only after overcoming several challenges, including a groundwater intrusion event which deposited thousands of cubic yards of wet ground material into the tunnel. This paper will discuss the events which delayed completion of the tunnel by over nine months, the risk management and construction approaches assumed by the Design-Build team during bid, several challenges faced by Design-Build teams when building stormwater resilience tunnels using traditional Department of Transportation standards, and recommendations for improving risk allocation and proposal documents for future similar Design-Build tunnel projects.

Microtunneling is rarely a straight forward “business as usual” endeavor. More often than not, these projects take twists and turns that require on the fly, out of the box solutions to get the project completed. This paper is a case history of a 63-inch diameter, Permalok, microtunneling project, extending nearly 700 linear feet under a hill forming the left-hand abutment to an earth embankment tailings dam. The job had three major and unexpected challenges that required unorthodox solutions. Challenge #1— Face Configuration: The Santiago Formation, a cemented sand formation with widely varying strengths, presented a challenge to configuring the face with regards to the number and length of pics and cutters. Subtle differences in type and size of the tooling had a big impact on the performance of the machine. Challenge #2—Eccentric Loading: Issues at the launch shaft with the Permalok pipe adaptors caused eccentric loading on the casing pipe. This issue resulted in overstressing and breaking of several pipe joints that ultimately led to seizure of the tunnel. Challenge #3—Back-tunnel Excavation and MTBM Recovery: There were many detailed considerations into excavating a recovery tunnel to retrieve the MTBM, especially being under a high hazard earth embankment dam.

This case history for three major additions to a tire manufacturing plant in north Georgia discusses the impact of highly variable Karst (sinkhole) geology on both foundation design and construction. For Phase 3, alternatives were considered for a heavily loaded mixing tower with up to 2,000 kip column loads. A non-traditional approach of using auger cast piles was adopted. To satisfy a risk-averse Owner, a pile load testing program including strain gauges was conducted. One test pile was cored full depth to demonstrate effective bearing on steeply inclined rock. For Phase 4, similar methodology was used at another critical mixing tower structure associated with that phase and the installation procedure was refined to reduce grout take. In Phase 5 construction, another mixing tower and the equipment pits were supported on auger cast piles. After pile installation, a sinkhole opened up between two equipment pits. Remedial grouting was performed. Development also required construction of a R&D building over a 96 inch diameter RCP storm sewer. Auger cast piles were used to support the building to avoid overstressing the existing pipe and risk separating joints. This case history outlines lessons learned while constructing auger cast piles in Karst and proving quality was built into the foundations.

"Thousands of years ago large glaciers carved much of the landscape in Puget Sound, depositing abrasive and granular soils including cobbles and boulders along with layers of clay and sand. Tunneling in this geology – common throughout much of the northern and Midwestern U.S. – is challenging and can be unpredictable. The use of pressurized face tunneling, most commonly with EPBs and slurry type machines, has made excavation in this geology more efficient, but difficulties still remain. Through analysis of recent projects in Seattle, Washington, USA, the authors draw conclusions as to the best TBM designs and operational parameters for this variable, abrasive and often difficult ground. Performance data is analyzed with an eye towards advancements that could further improve TBM tunneling in glacially deposited soils. INTRODUCTION Over the past several ice ages, extensive glaciers reaching across much of North America transported large amounts of soil and rock as they moved across the land. The substantial weight of the ice sheets mixed together a wide range of soil types along with a variety of rocks and boulders of all sizes. Many rock formations were reduced to pebbles or particles containing highly abrasive characteristics. Any boulders able to survive were generally comprised of harder materials which were scattered throughout the soil. This highly variable mixture of abrasive soils and high strength boulders can be found in many areas across the Puget Sound and other regions of the country. Wherever they are encountered, they often create a difficult and unpredictable mining environment for both equipment design and operation. Modern TBMs are optimized early in the design process for specific types of geology, and unexpected conditions can require extensive modifications. In addition, several operational parameters need to be adjusted to balance ground characteristics, and advance rates can be significantly affected in mixed and rapidly changing soil conditions. Both EPB and slurry machines can be used successfully in these conditions, provided their respective operating criteria are met. Ideally, an EPB is operated with a relatively thick muck consistency, which accommodates moderate changes in soil permeability and maintains a reliable pressure differential through the screw conveyor. However, this also causes higher wear on rotating components and requires more energy at the cutterhead. Alternatively, slurry machines typically operate best with a more fluid muck consistency that can be easily pumped through the extensive piping system. This tends to reduce the wear on the cutterhead and the power requirements of the cutterhead drive system, but also makes the machine more sensitive to changing soil conditions. Generally, the slurry piping system is also more capable of controlling high face pressures, although the screw conveyor of an EPB can also be designed to accommodate most high-pressure conditions."

It is always necessary to expand existing buildings, so that they could meet the growing societal needs. Compared with demolishing them and then constructing new ones, the expansion of existing buildings could not only reduce project cost, but also save the use of energy and raw materials in projects. Therefore, it is widely adopted in most under-developed countries. However, the expansion of existing buildings is not an easy task. It has been involved with a lot of issues in defining the scope of work, planning landscape, managing construction activities, etc. This paper investigates the potential challenges in expanding existing buildings. The project of expanding the Al-Agoza Hospital, one of the biggest and oldest hospitals in Cairo, Egypt, is selected as a case study. In the case study, the project documents have been carefully reviewed. The project engineers and the health-care professionals working in the hospital have been interviewed. This way, the point of views from the owner, the architect/engineer, and the contractor have been taken into account. Several lessons are learned through the discussion with these people. The results from this paper are expected to provide pieces of useful advice and suggestion for the expansion of existing buildings in the future.

From many years practical industrial experience the author has drawn together a list of the key points to consider when embarking on a large-scale geometallurgical project. Fundamental aspects which must be considered include (but are not limited to):understanding the variability of the orebody both laterally and with depth;comminution properties and variability;flotation properties, requirements and variability – differing ore zones may have substantially different requirements for optimal performance;the role of texture – the consequences of both grain size and distribution and how the circuit will handle these;realistic expectation in terms of the financial requirements and time frame results are required within; andintegration of, and communication between key personnel within the various departments of the site (mining, geology and processing) is a fundamental aspect of any geometallurgically based processing system. An example of a highly complex nickel sulfide ore will be used as the basis of discussion.

The Waterview Tunnel comprises twin 2.4 km long 13.1 m internal diameter three lane road tunnels in Auckland, New Zealand. The City Rail Link (CRL) is a 3.45 km long, 6.24 m internal diameter twin rail tunnel underneath Auckland’s city center. This paper presents an overview of the tunnel geometry, segmental lining congurations, geometric constraints, performance criteria and functional reuirements for the large diameter Waterview Tunnels and smaller diameter CRL tunnels. Other key aspects, which include sustainability, innovations, construction performance and practical aspects are discussed and compared. Lessons learnt from these mega tunnelling projects are also presented.

Accidents involving powered haulage and mobile equipment such as haul trucks often account for the greatest number of fatalities in the mining industry each year. Despite previous analyses that have identified root causes and other contributing factors, there is still a need to better understand the events leading up to these types of accidents, what lessons may be learned, and what strategies can be employed to prevent fatal accidents from occurring. This study examines Naturalistic Decision Making (NDM) using the critical decision method (CDM). The CDM is a retrospective interview approach used to explore timelimited, high-stakes, decision-making. In this study the CDM is used to obtain more information about what happens prior to, during, and after a potentially fatal situation such as a near-miss event, loss of control, or minor accident involving equipment damage. Researchers captured firsthand accounts from 21 haul truck operators involved in near-miss events from mine sites of various sizes and commodities throughout the United States. These accounts provide rich and detailed narratives from the perspective of haul truck operators themselves and reveal insights into what decisions haul truck operators make, what sensory cues they perceive, and what strategies they employ during challenging and non-routine situations. Decision-related themes that emerged from the data are presented and discussed. These results, along with potential solutions offered by study participants, can help to inform future research, raise awareness about hidden hazards, and build more creative interventions and realistic training scenarios for use by the industry to address haul truck safety issues.

Introduction A brief account of the East Malartic fire is essential if this paper is to serve the purpose implied by its title. The fire originated in the timbers of No. 4 shaft at the 10th, or 1,570-foot, level; this was the top or collar of the underground sinking operations. At the time, sinking was in progress and the shaft bottom was 242 feet below the 12th station horizon, or 618 feet 'below the l0th level. Above the l0th level, the shaft was not in use as the surface plant was not yet completed. The shaft crews descended from surface in No. 3 vertical shaft to the l0th level, where they transferred to No. 4 shaft, a distance of 1,450 feet. Of the sixteen shaftmen mucking at the shaft bottom, four were able to reach safety. One shaftman was overcome by fumes midway between the two shafts on the l0th level, while attempting his escape. The remaining eleven men climbed as far as the 11th station and there succumbed to carbon monoxide. No. 4 shaft, inclined at 61 degrees, bad been raised and timbered from the 10th level to surface; this bad been followed by sinking operations below the l0th. Two air hoists had been set up in the footwall of the shaft, 110 feet above the 10th level, and directly behind compartments No. 2 and No. 4. The waste from the shaft bottom was hoisted in 28-cubicfoot buckets on mono rails and dumped over standard shaft sinking doors into waste chutes built into No. 1 and No. 3 compartments. The five-compartment shaft, 7 ft. 8 in. by 34 ft. 6 in. overall size, was heavily timbered. In addition to the standard shaft timbers of 8 in. by 8 in. and 8 in. by 10 in. B.C. fir, the hanging-wall and ends of the shaft were laced with 4-inch spruce lagging. This represented approximately 300 board feet of timber in placee for every foot of shaft. The cause of the fire bas never been determined, despite every effort to do so. The inquest held for those who lost their lives in the fire was an extremely thorough one, but only the location of the origin, and not the cause, was established.

High Speed 2 (HS2) mandated that Building Information Modelling (BIM) processes be implemented during design and construction. Gall Zeidler Consultants (GZ) was responsible for developing the BIM Models for the portals, tunnels, and cross passages of the of the 16 km-long Chiltern twin-bore Tunnel. In addition, GZ developed the GBR for the Chiltern Tunnels and incorporated the relevant information into the Project’s BIM framework. This paper presents the implemented BIM framework, describes the modelling approach, and discusses the advantages and challenges of implementing BIM, particularly how BIM improved interface coordination and how BIM was used in feasibility assessments of different construction options.

Upstream embankment raises often represent the lowest cost option for increasing the capacity of tailings storage facilities (TSF), but also pose the highest stability risk due to constructing on top of previously deposited tailings.This paper presents an overview of key aspects in the geotechnical investigation and design of upstream raises and some recent lessons learnt from experience across a number of TSFs in Australia. A summary of recommended field investigation and laboratory testing methods is provided with a focus on collecting quality information for design purposes. A strong focus is given in developing an accurate assessment of pore water pressures, which, if not correctly understood can have disastrous consequences on tailings strength parameter interpretation and wall stability assessment in static and post-seismic conditions.The assessment of the contractive or dilative shearing behaviour of the tailings material and rigorous interpretation of design characteristic values are presented. Today, I would like to talk about ways to maximize the efficiency of surface coal operations. I doubt I will present any startling new ideas, but my comments may, hopefully, stimulate some conversation today and later back at your own operations. Strip mining is a very old technique used by ancient societies. Only with the coming of the machine age in the early twentieth century did we attempt to increase the magnitude of our operations. The industry, as we know it today, really only got started in the early 1900s with the advent of steam shovels. In the first portion of this presentation, I will attempt to show that we have, in many cases, not changed the basics of strip mining, only the scale. In doing so, quite often we have complicated the operations to the point of being unmanageable and unprofitable.

In this position he leads a multi-disciplinary team and conducts individual research directed towards fundamental ground control safety issues affecting miners. Dr. Mark is recognized as one of the country?s leading authorities on coal mine ground control. Through his research, he has made numerous important contributions to mine safety that have been widely adopted into mining practice. He is a highly original researcher, and has often developed new and innovative methods to tackle extremely complex problems in his field. In recent years Dr. Mark has also achieved wide international recognition. Five research projects in Canada, Australia, and South Africa have been based largely on his work.

My bets are on you, every time! But who is right? In the "cobalt issue" of our favorite magazine, January 1951, you stated: "By far the best immediate United States prospect for, large amounts of cobalt is the Blackbird mine of the Calera Mining Co., which is operating for Howe Sound Co., in Leinhi County, Idaho." In the March 1 issue of Saturday Evening Post, Ernest F. Mechlin, Jr., chief cobalt section, National Production Authority, states: "The Bethlehem Steel Co.'s Cornwall, Pa. ore bed is the principal domestic source." Maybe he should have said: "was the principal domestic source", or perhaps Howe Sound's Idaho cobalt is still in the prospect stage in Mr. Mechlin's opinion, perhaps in yours too. KATHARINE W. The engineering personnel shortage, present and impending, so well brought out in the article in Mining Engineering in the August, 1951 number, both in the editorial lead and the article by William B. Plank and Henry J. Petrie, has left untapped and has ignored a potential source of supply that would fill the need ably and with complete satisfaction. This source is comprised of the women engineers- and geologists, both presently, trained and able to handle work at all levels, and the many who would make splendid engineers and geologists if they could hope for even a modest taste of the square deal that should be theirs, but, as we all know, is not. I write from experience. It is not pleasant or sensible to have to urge borderline cases among men students to consider engineering and geology as careers to fill the need, when I have far more able, interested and suited young women students whom I must divert into other channels. It does not make sense to scrape the bottom and sides of the barrel to induce potentially second or third rate male engineers and geologists to enter the profession when we have superior material among the women, eager to do good work in these fields. It does not make sense for these men to become second or third rate in these fields when they might be first rate salesmen, musicians, advertising copywriters or policemen. Nor does it make any more sense to force potentially first rate female geologists and engineers to become second or third rate and very frustrated typists, kindergarten teachers, dress sales-women or clerks. At this point I am willing to admit that where superstitious -miners object to women in underground workings, men are available for the jobs. Moreover, it is not necessary for women to be sent into the most difficult country, if there is reluctance on either side of the contract. There may be fewer of them eager for the roughest assignments, in proportion to the men, but women are so accustomed to discrimination far more complete than that against any racial or religious group, that a reasonably tough assignment would seem like a heaven-descended joy. In my own experience, I have found that my girl students majoring in geology, could keep up with me in the field in the roughest terrain, but. the men, all but the best students and the most physically fit young men, complained that they could not keep up with me and that I always "walked their-legs off. The women, a few men and I always returned with bags heavy with well chosen specimens and proper notes. Most of the men carried no specimens and-but poor scraps of notes on loose bits of paper. Yet the men were urged to prepare for a career in engineering or geology if their examination marks were reasonably good. When we find that we still have a shortage of engineers and geologists, after putting all the qualified women to work, then we may begin to worry. "The Level Crossing Removal Project – Package 1 is part of the Victorian Governments commitment to remove 50 of the most dangerous and congested level crossings across Melbourne. This involved removing three crossings on the Frankston Line in the south east suburbs of Melbourne. The works involved replacing approximately 2.5km of at grade track with three rail cuttings up to 8 m deep at each of the bridges that replace the level crossings. The cuttings are retained by piled retaining walls and during the main construction occupation approximately 250,000 cubic metres of soil was removed. The project has presented significant geotechnical challenges due to the location in residential suburbs and tight rail corridor and rail occupation timeframe. These challenges included temporary and permanent wall stability, foundations, earthworks and dewatering. The primary earthworks challenge for the project was moving the 250,000 cubic metres of soil in a 10 day window, however, from a geotechnical viewpoint, obtaining a stable subgrade on which to operate the necessary construction plant in a short time frame with constrained site access and time to import materials was the key. Whilst closely watching the performance of the dewatering system, a range of contingency plans were developed to cope with any difficulties."
