Introduction A brief account of the East Malartic fire is essential if this paper is to serve the purpose implied by its title. The fire originated in the timbers of No. 4 shaft at the 10th, or 1,570-foot, level; this was the top or collar of the underground sinking operations. At the time, sinking was in progress and the shaft bottom was 242 feet below the 12th station horizon, or 618 feet 'below the l0th level. Above the l0th level, the shaft was not in use as the surface plant was not yet completed. The shaft crews descended from surface in No. 3 vertical shaft to the l0th level, where they transferred to No. 4 shaft, a distance of 1,450 feet. Of the sixteen shaftmen mucking at the shaft bottom, four were able to reach safety. One shaftman was overcome by fumes midway between the two shafts on the l0th level, while attempting his escape. The remaining eleven men climbed as far as the 11th station and there succumbed to carbon monoxide. No. 4 shaft, inclined at 61 degrees, bad been raised and timbered from the 10th level to surface; this bad been followed by sinking operations below the l0th. Two air hoists had been set up in the footwall of the shaft, 110 feet above the 10th level, and directly behind compartments No. 2 and No. 4. The waste from the shaft bottom was hoisted in 28-cubicfoot buckets on mono rails and dumped over standard shaft sinking doors into waste chutes built into No. 1 and No. 3 compartments. The five-compartment shaft, 7 ft. 8 in. by 34 ft. 6 in. overall size, was heavily timbered. In addition to the standard shaft timbers of 8 in. by 8 in. and 8 in. by 10 in. B.C. fir, the hanging-wall and ends of the shaft were laced with 4-inch spruce lagging. This represented approximately 300 board feet of timber in placee for every foot of shaft. The cause of the fire bas never been determined, despite every effort to do so. The inquest held for those who lost their lives in the fire was an extremely thorough one, but only the location of the origin, and not the cause, was established.

High Speed 2 (HS2) mandated that Building Information Modelling (BIM) processes be implemented during design and construction. Gall Zeidler Consultants (GZ) was responsible for developing the BIM Models for the portals, tunnels, and cross passages of the of the 16 km-long Chiltern twin-bore Tunnel. In addition, GZ developed the GBR for the Chiltern Tunnels and incorporated the relevant information into the Project’s BIM framework. This paper presents the implemented BIM framework, describes the modelling approach, and discusses the advantages and challenges of implementing BIM, particularly how BIM improved interface coordination and how BIM was used in feasibility assessments of different construction options.

During the 1980s and early 1990s, the South African coal industry experienced a series of methane explosions in which well over 200 coal miners lost their lives. In addition, a fire and an explosion in the gold mining sector claimed a further 240 lives. These tragic incidents resulted in the establishment of a research facility at Kloppersbos dedicated to explosion and fires research and the funding of an extensive research program, and almost certainly influenced the government of the day in its decision to establish, in 1993, the Leon Commission of Enquiry into Mine Health and Safety. The results of this focused research program, the wide-ranging recommendations of the Leon Commission and the determination of the coal mining industry to improve its safety record all combined to result in the past 20 years without a major explosion.This paper lists some of the research outcomes and, through three case studies, illustrates what can be learnt from investigations following explosions. The paper concludes with an evaluation of some of the findings of the Royal Commission on the Pike River coal tragedy. Many of the Commission’s conclusions are pertinent and relevant to the coal mining industry worldwide and their implementation could significantly improve coalmine safety; however, a careful study of the available facts indicates there could well be an alternative explanation of the source of the methane, the location of the ignition and the size of the explosion at the Pike River coalmine.CITATION:Phillips, H R, 2015. Upstream embankment raises often represent the lowest cost option for increasing the capacity of tailings storage facilities (TSF), but also pose the highest stability risk due to constructing on top of previously deposited tailings.This paper presents an overview of key aspects in the geotechnical investigation and design of upstream raises and some recent lessons learnt from experience across a number of TSFs in Australia. A summary of recommended field investigation and laboratory testing methods is provided with a focus on collecting quality information for design purposes. A strong focus is given in developing an accurate assessment of pore water pressures, which, if not correctly understood can have disastrous consequences on tailings strength parameter interpretation and wall stability assessment in static and post-seismic conditions.The assessment of the contractive or dilative shearing behaviour of the tailings material and rigorous interpretation of design characteristic values are presented. In a situation where big government is trying to have an effect on even small details of a work activity, otherwise simple situations become complex. Regulations are written to such detail that constant compliance with strict interpretations can be a nightmare-not to mention off the target of the intended purpose for the regulation. One soon recognizes the onerous characteristics of federal statutes. Another admonition the Lord gave those lawyers and pharisees back then was that they tended to strain at a gnat and swallow a camel! He meant that the pharisees were given to devoting themselves to the strict interpretation of law by writing hundreds of subrules or interpretation to law. In doing so they eventually lost sight of the original purpose of the law. I think they were being told, keep the law-but don't neglect to humanize it Keep it simple Recognize the purpose of why the law. "Mix in" a common sense approach. Someone once said, "Common sense is instinct -enough of it is genius." Today, as 2000 years ago, we need more "genius" displayed by legislators in considering the weightier matters of law. First of all true legislature genius calls for a declarative statement about the purpose of the statute and a fair, objective analysis of the ground rules by which it will be implemented. In the Health & Safety Act of 1969, the "findings and purpose" as set forth in Section 2 do define its purpose. I believe most of us affected can be in accord with the reasons advanced for the legislation. However, beyond those well-intentioned few paragraphs, we very quickly run into trouble. Congress complicated the simplicity of the statement of purpose by mandating government intervention into areas that cannot be effectively legislated, nor even should be legislated. The Act provided for broad gaged regulation which is unnecessary, inappropriate, and tremendously costly. Frankly, it detracts from a constructive, cooperative effort by the parties involved. These are strong negatives which characterize inappropriate legislation. Appropriate legislation and subsequent regulation, which is realistically conceived and practically enforced must be born of an honest desire for improvement, based on a working knowledge of common problems which are thoughtfully analyzed. Then the legislation must be written in clear concise language so that there can be little or no doubt as to its literal meaning and intent. To be practically enforced, it must be understood by all who will enforce it and by all who are subject to it. That is to say, an effective law or standard cannot be fixed or inflexible so that it cannot be tempered in human judgment. There must be an elasticity in the statute to provide for consideration of all of the relevant facts of a given situation. Such elasticity makes the compliance effort as meaningful as it can be for the time and dollars invested. Meaningful compliance requires room for common sense. Today, we in business along with our associates in government and labor, urgently need to pause, and assess our current situation. The problems arising from the 1969 Act have been intensified through the Mine Safety & Health Act of 1977. After reading the "findings and purpose" of this legislation, and by process of elimination, we should all clearly see what is and is not the purpose of the Act. 1) It is not to turn the management of private industry over to government. 2) It is not to impair the efficient operation of business. 3) It is not to reduce productivity and profits. 4) It is not to reduce national unemployment. 5) It is not to perpetuate the bureaucracy. 6) It is not to increase the tax burden on an already overburdened people. 7) It is not to increase our balance of payments deficit. 9) It is not to replace management/labor relations in working together to solve problems.

The Occupational Safety and Health Act of 1970, although providing no precise definitions of safe and healthful working conditions, specifies mandatory duties, and methods and procedures -- "to assure so far as possible every working man and woman in the nation safe and healthful working conditions." The dynamics of the workplace ad the dynamics of this coextensive Act both spring from the same source. Both require at any given time an application of the then con- temporaneous highest state of the art. Both are flexible, ongoing, adaptive, changing; both advance by a blending of experience and research.

I chose the title "Let's Clear the Air" for this afternoon's presentation because it has two meanings, the literal and the figurative. Most of you know what MSHA is. Most of you know what MSHA does. And, for the most part, most of you know why. What I hope to accomplish today is to "clear the air" around our health standards, to clarify for you how MSHA views its congressional mandate under Title I Section 101(a)(6), (7), and (9) of the Mine Safety and Health Act. Asbestos, silica, radiation, noise, welding fumes, carbon monoxide, lead, hydrogen sulfide, nitrogen dioxide - the list of physical and chemical hazards encountered in the metal and nonmetal mining industry is as diversified as the industry itself, possibly even more so. In addition, miners are often exposed to several of these health hazards simultaneously.

Bateman Engineering managed the establishment of a second diamond treatment plant at the Letseng Mine in the Mokhotlong District of northern Lesotho, about 100 km from the town of Buthe Buthe. Letseng Diamonds Proprietary Limited, is owned 70 % by Gem Diamonds and 30 % by the Government of the Kingdom of Lesotho. The second plant, which commenced construction in late 2006, doubled Letseng Mines' hard rock processing capacity from 2.6 million t/yr to 5.2 million t/yr, making it the worlds' seventh largest diamond mine by throughput. Bateman Engineering provided the engineering design, procurement and construction management of the new plant.

I can think of no better candidate than Daniel that is deserving of this award ? an award to recognize technical excellence in advancing ground control technologies [and] approaches. Daniel?s education and career spans over three decades. Having graduated from National Cheng Kung University in Taiwan in 1972 with a B.S. degree in Mining and Petroleum Engineering, he spent the next four years in the Taiwanese military and mining industry, before moving to the United States, where he enrolled in West Virginia University?s Mining Engineering Department. He received his M.S degree in Mining Engineering from WVU in 1978; then in 1982 became the first person to ever receive a Ph.D. in Mining from WVU. His thesis was titled, ?Development of Ultrasonic Techniques for the Measurement of In Situ Stresses?.

Through his research, he has made numerous important contributions to mine safety that have been widely adopted into mining practice. He is a highly original researcher, and has often developed new and innovative methods to tackle extremely complex problems in his field. In recent years Dr. Mark has also achieved wide international recognition. Five research projects in Canada, Australia, and South Africa have been based largely on his work.

A proposta de aproveitamento de fibras vegetais para utilização em desenvolvimento de compósitos de matriz polimérica reforçados pelas mesmas é a motivação da pesquisa visando obter um compósito com melhores propriedades mecânicas e químicas do que sua matéria prima. Por virtude da importância do desenvolvimento desses materiais, a caracterização mecânica da fibra, da matriz e, finalmente, do compósito através de ensaios destrutivos e não destrutivos é fundamental para estudo de sua aplicação. Após a obtenção dos dados, percebeu-se que a adição das fibras melhorou as propriedades mecânicas do compósito resultante. Mostrando, portanto, resultados promissores para o desenvolvimento desses materiais, promovendo assim um material mais barato e de melhor propriedade mecânica, além de uma alta leveza. Em suma, os resultados estão de acordo com o esperado, impulsionando assim um melhor estudo desses materiais, por exemplo, realizando outros ensaios, como ensaios de impacto.

"This paper discusses the unique design and construction circumstances of the cantilevered dual sheet pile walls and the Accordion Sheet Pile Wall for the repair and upgrade of the East Garden Grove-Wintersburg Channel in Huntington Beach, California that was concluded in 2014. It also describes the 2008 Emergency Sheet Pile Project for the same channel. Both projects utilized the Press-in Piling Method for its low noise and almost no vibration advantages.BackgroundThe East Garden Grove-Wintersburg Channel is part of the Orange County Flood Control District (OCFCD) and is operated and maintained by OC (Orange County) Public Works (see Figure 1 for its location). The channel, referred to as OCFCD facility # C05 (C05), had suffered significant toe undercutting and slope erosion of its levee due to a series of storms. These began after the facility was constructed in the 1960’s and culminated in 2005. Consequently, the levee was deemed significantly deficient and was eventually declared as a local emergency in 2007. C05 was an incised channel and its levee was constructed primarily from native materials. Operation and Maintenance (O&M) personnel notified the design staff at OC Public Works to be aware of major consequences associated with the dilapidation problem in the most downstream reach of the channel in the vicinity of the Bolsa Chica Wetlands Restoration Project. In 2007, the Orange County Board of Supervisors declared a local emergency to fix the levees with sheet piles to be driven by the Press-in Piling Method, hydraulic pushing piles into ground by holding onto previously installed piles. The project that ensued represented the first in-tandem use on the West Coast of the United States of the Press-in pile driver with its GRB System (Press-in pile driver, a support crane and pile transporter travel on top of already-driven sheet piles). This is further discussed in this paper in the sections titled “Press-in Piling Method” and “GRB System”, respectively. The project repaired approximately 3,800 feet of erosion on the north levee of C05 with 45-foot long cantilevered sheet piles. The sheet piles design constraints are discussed in this paper in the section titled “C05 2008 Emergency Sheet Pile Project (Phase 1)”. Between 2012 and 2014, another mile’s worth of levee reach was improved with the Soil-Mix-Sandwich (SMS) Method. This concept consisted of deep soil cement mix (DSCM) columns sandwiched within the dual rows of sheet piles. Additional details on the SMS, its DSCM construction, and its hybrid geo-structure design concepts are well beyond the scope of this discussion, which is primarily focused on sheet piles. Instead, the interested reader is referred to two papers on the SMS and its DSCM metrics (Fayad, 2015; and Arora, et al., 2015). These were presented and published in the Proceedings of the Deep Foundations Institute (DFI) 2015 Conference on Deep Mixing. Hence, in the section of this paper titled “C05 SMS Dual Sheet Piles Project (Phase 2)” only the sheet pile design constraints of the SMS are covered. Finally, in the area where the project traversed the Alquist-Priolo zone of the Newport-Inglewood tectonic fault, an expandable and deflectable sheet pile wall was used to isolate the flood channel from the Bolsa Chica wetlands. Its details are addressed in the section titled “C05 Accordion Sheet Pile Wall (Phase 2)” of this paper."

"Solids concentration (% solids) profiles and the clear liquid/slurry interface level (mudline) were determined from conductivity measurements in thickeners at Falconbridge' s Strathcona mill (Ni concentrate and non-mag thickeners) and Kidd Creek Division (Cu, Zn concentrates and final tails in the concentrator and thickeners in the Zn plant).The conductivity measurement system consisted of a probe 1.5 m in length with 24 electrode cells, a 24-channel relay, a conductivity meter and a computer. A conductivity profile is collected which locates the mud-line by the sharp change in conductivity. The conductivity measurements are readily converted to solids concentration profiles without need for calibration.The results from the conductivity measurements were compared with those from pressure profiles and direct measurements on local samples obtained using a modified Noranda sampler.It is concluded that the conductivity technique is applicable to locate the mud-line and to estimate the solids concentration profiles in thickeners. This on-line information could improve the operational efficiency of industrial thickeners.INTRODUCTIONAn automated, on-line technique for detecting the clear liquid/slurry interface (or mudline) and the variation in solids concentration with depth would enhance thickener operation and control. In some processes such enhanced control offers significant benefits. For example, in hydrometallurgical operations solid/liquid separation is often the process limiting step. Water treatment plants are increasingly expected to operate remotely and reliable sensing is vital.Current attempts to detect the mud-line include reflection of some signal (e.g. sonar), floats (Dahlstrom, 1985) and measurement of pressure (Rosenblum, 1991), and in one case, natural radioactivity of the solids (Johnston and Simic, 1991). A recent commercial development uses reflection from a vertical array of optical transmitters/sensors, which also gives local solids concentration (BTG Inc., 1991)."

"The Level Crossing Removal Project – Package 1 is part of the Victorian Governments commitment to remove 50 of the most dangerous and congested level crossings across Melbourne. This involved removing three crossings on the Frankston Line in the south east suburbs of Melbourne. The works involved replacing approximately 2.5km of at grade track with three rail cuttings up to 8 m deep at each of the bridges that replace the level crossings. The cuttings are retained by piled retaining walls and during the main construction occupation approximately 250,000 cubic metres of soil was removed. The project has presented significant geotechnical challenges due to the location in residential suburbs and tight rail corridor and rail occupation timeframe. These challenges included temporary and permanent wall stability, foundations, earthworks and dewatering. Whilst closely watching the performance of the dewatering system, a range of contingency plans were developed to cope with any difficulties."

Simultaneous mass-balancing of a mineral processing flow sheet results in adjustments to each data point that reflects the accuracy and precision across the full set of data. In this way, all the good work that goes into designing the sampling and analysis protocol for each sample point is leveraged in order to produce a æbetterÆ set of balanced numbers for every sample point in the circuit. Few operating plants employ whole of circuit balancing techniques to produce reported operating results. This situation is changing with the application of mass balancing technology which utilises redundant data for production accounting and metal reconciliation from product to ore source across multiple phases and multiple time periods. Mine and plant data can now generate credible information to improve productivity. Mine and plant personnel can now use consistent data sets for review and planning. Decisions based on balanced data can be made quicker and with increased confidence.

Ensuring effective management of contractual processes is essential for maintaining profitability and operational efficiency in mining. However, due to limited capacity and resources, sites often struggle to manage these processes effectively, leading to oversights and financial losses. Recent advancements in Artificial Intelligence (AI) offer promising solutions to these challenges. Large Language Models (LLMs), like those powering ChatGPT, now enable machines to understand unstructured data like contracts. These can therefore be applied to address these challenges by taking some of the heavy lifting off of humans. In this paper, we analyze a real-world case study of the application of LLM tools for preventing a $400k contractual issue on an underground gold mine in Western Australia. We provide a gap analysis of the failure modes in existing processes that led to the issue. We then describe the application of LLM tools to flag it early and facilitate a collaborative solution between the operator and the supplier. This includes a comparative analysis of base case vs LLM case to evaluate the impact on project cost and time. Finally, we discuss how the integration of AI can foster improved relationships between clients and contractors across a broad range of potential contractual applications, allowing the time required for collaborative and proactive problem solving.

As air quality sensor networks become increasingly popular in underground coal mines, it is important to generate paradigms for the application of the collected data. To date, air quality sensing has been primarily used as an early warning system for hazardous air conditions. Using data collected from a network of sensors in a US underground coal mine, we have created multiple visualization methods to show the interactions of carbon monoxide evolution with ventilation airflow. These visualization methods can allow for further analysis of the source of contaminant, as well as better data resolution across the area of concern within the mine. By utilizing univariate spatial interpolations, we present methods for identifying the movement of carbon monoxide at one-minute intervals. The resulting visualizations display the evolution of carbon monoxide concentration across a sizeable study area over a period of 14 minutes.

Building Information Modeling (BIM) is an increasingly popular method for generating and managing facility information during the life cycle of a building, ranging from facility conceptualization, through design, construction and its operational life. Organizations involved in Facility Management (FM) have the opportunity to use BIM as a knowledge repository to document evolving facility information and to support decisions made by the facility managers during the operational life of a facility. This paper demonstrates the potential of using BIM to develop algorithms that automate decision making for FM applications. The potential of utilizing BIM as an analysis tool is demonstrated through the scenario of HVAC (Heating, Ventilation, and Air Conditioning) system failure in an operating facility. In case of a typical HVAC malfunction today, facility occupants record complaints in a ticketing database maintained by the FM organization. Upon receiving notification of HVAC system failure, facility inspectors visit the location to confirm the reported failure. Upon confirmation, facility managers review building plans and specifications to develop a detailed plan of action to repair any HVAC components suspected of damage. Based on the plan of action, inspectors visit the facility to inspect and, in case of damage, repair the appropriate HVAC system components. These FM practices ? as currently implemented across the industry ? are labor intensive, time consuming, and often rely on unreliable and outdated information. To address these shortcomings, the authors propose an alternative methodology of HVAC fault detection in operational buildings. The authors implement an algorithm that leverages complaint ticket data and automates BIM to determine potentially damaged HVAC system components. Based on the list of HVAC components suspected of damage, the algorithm develops a plan of action for the facility inspectors. Finally, the authors discuss the advantages of the proposed method as well as the challenges of implementing automated BIM-enabled decision making processes in the FM industry.

Each year, hundreds of mine workers are involved in machinery-related accidents. Many of these accidents involve inadequate or improper use of lockout/tagout (LOTO) procedures. To mitigate the occurrence of these accidents, new safety methods are needed to monitor access to hazardous areas around operating machinery, improve documentation/monitoring of maintenance that requires shutdown of the machinery, and prevent unexpected startup or movement during machine maintenance activities. The U.S. National Institute for Occupational Safety and Health (NIOSH) is currently researching the application of Internet of Things (IoT) technologies to provide intelligent machine monitoring as part of a comprehensive LOTO program. This paper introduces NIOSH’s two-phase implementation of an IoT-based intelligent machine monitoring system.
